[Background] Medical staff have many occasions to be exposed to anticancer drugs (ACDs) in the process of performing cancer chemotherapy. Although guidelines for the safe handling of ACDs have been published, actual measures against exposure to ACDs differ between hospitals, and no surveillance data regarding the conditions of each hospital in relation to performing chemotherapy in Japan are available. [Methods] To understand current measures against exposure to ACDs and problems related to cancer chemotherapy at outpatient departments, we conducted a questionnaire survey on doctors, pharmacists, and nurses in 10 leading hospitals for cancer chemotherapy in Japan. [Results] Responses were received from all 10 institutions. The hospital pharmacists prepared all ACDs for outpatients in each institution. All hospitals took basic measures against ACD exposure, such as using personal protective equipment and penetration-resistant waste containers. Conditions against exposure to ACDs generally improved between 2012 and 2015, especially in terms of conveyance of ACDs and use of a closed-system drug transfer device. However, no measures linking ACDs with administration routes or injection sites were commonly taken in any of the hospitals in 2015. [Conclusions] Conditions against ACD exposure improved between 2012 and 2015, possibly because new guidelines were issued. To improve measures for ACD exposure in Japan, preparing more appropriate manuals and offering more opportunities to medical staff for continuing education are considered important.

以岩性相近但气候和土地利用迥异的两个具有高分辨率水文水化学自动监测数据的岩溶水系统——湿润亚热带以土质坡地为主的岩溶水系统（贵州普定后寨岩溶水系统）和高寒冰雪覆盖下以石质坡地为主的岩溶水系统（瑞士阿尔卑斯山区的Tsanfleuron冰川岩溶水系统）作为研究对象，进行岩溶作用碳汇强度对比研究。结果发现，后寨岩溶水系统的[HCO_3]年均值为222 mg/L，而Tsanfleuron冰川岩溶水系统的[HCO_3]年均值仅为131 mg/L，表明湿润亚热带岩溶水系统具有较高的[HCO_3]特征。然而，湿润亚热带岩溶水系统的碳汇强度（60.82 t/ (km~2·a)）与Tsanfleuron冰川岩溶水系统的碳汇强度（59.06 t/ (km~2·a)）相当。冰川岩溶水系统同样具有较高岩溶作用碳汇强度的原因主要是在全球变暖的背景下，冰川退缩加剧，冰川固体水库水量释放增多，致使冰川岩溶水系统径流量显著增加效应超出因稀释作用而产生的[HCO3-]降低效应，进而使得山岳冰川岩溶水系统的碳汇强度增大。这也暗示着随着某些冰川融化，径流量逐渐减小将使岩溶作用碳汇强度降低。

Objective: To find whether plasma and follicular prorenin concentrations have any effect on the uterine arterial blood flow in women with polycystic ovarian syndrome (PCOS) compared to those with normal menstrual cycles (NMC). Study design: Controlled prospective clinical study involved 55 women undergoing in vitro fertilization: 24 with PCOS and 31with NMC. In both groups transvaginal colour Doppler assessment of uterine arterial blood flow was analysed on day 22 of the cycle, on the day of human chorionic gonadotrophin (HCG) administration and 36 h after HCG. Plasma and follicular (in the dominant follicle containing mature oocyte, and in the pooled follicles) prorenin and active renin, and serum estradiol and androstenedione concentrations were measured at these timepoints. The Student’s t-test and Pearson correlation were used for the statistical analysis. Results: The resistance index (RI) in the NMC group decreased from 0.84 ±0.05 on the day of HCG to 0.78±0.08 36 h after HCG (P < 0.05); in the PCOS group the RI did not decrease. Follicular prorenin concentrations in the dominant follicle and in the pooled follicles were lower in the PCOS than in the NMC group (20,210 ±10,831 μU/l, 16,753 ±8634 μU/l versus 42,637 ±35,400 μU/l, 33,067 ±26,200 μU/l; P < 0.05). Conclusions: Plasma prorenin concentrations do not affect vascular impedance to the uterine artery, but follicular prorenin do by newly formed low resistant vessels in the follicles.

During the operation of complex process, such as oil production or refining, abnormal situations may occur, leading to an alarm flooding. Alarm flooding is the signalling of a large number of alarms in a few minutes, in such a way that it is impossible for the operator to attend to all alarms. On these occasions, it is usual that the operator leaves the alarm summary list and gets an analysis of the plant through the screens of the DCS (digital control system), seeking to understand the situation. The alarm summary list ceases to be a useful tool. In such cases, the operator might have the aid of a filter that would present the highest priority alarms and other information associated with them, enabling him to gain a better knowledge of the situation. This paper describes the interface of a system aimed to help the operator to have a more comprehensive knowledge of the process (a better situational awareness) during process upsets that cause alarm flooding, recovering the utility of the alarm layer to the safety of industrial processes.

2016年,世界卫生组织(WHO)在其发布的关于世界烟草使用情况的报告中指出,每年大约有600万人死于吸烟诱发的疾病,危害居民身心健康的同时,也给社会造成了严重的经济负担。作为社区居民健康的"守门人",全科医生在推动社会控烟事业进步的过程中应扮演重要的角色。本文基于全科医生职业使命的伦理考量,提出全科医生在社会控烟事业中应承担的伦理责任:第一,全科医生应该在行为上树立榜样,自身坚决杜绝烟草;第二,全科医生应尽力规劝其患者戒烟,同时在社区中倡导建立无烟环境;第三,全科医生应在整个医疗体系内积极宣传无烟化的理念,将戒烟辅助纳入常规诊疗服务流程。此外,全科医生还应该致力于促进政府对烟草控制框架公约的履行,推动全社会控烟事业的进步。

Management Practices for Lentil as Relay Crop in the Rice Based Cropping system

The field experiment was conducted at Pulses Research centre of Bangladesh Agricultural Institute (BARI), Ishurdi, Pabna, Bangladesh during rabi season of 2009 to find out the suitable management package for better plant establishment and higher yield of lentil under relay cropping. The experiment was based on three genotypes-Rajbari local, X95S-167(4) and X95S-136; two types of rice straw height-10 cm and 30 cm; three seed rates (kg/ha)-e. g., 30, 50 and 60, and three levels of fertilizer-control, 30-60-20 kg/ha of N-P 2 O 5 -K 2 O and 30-60-30 kg/ha of N-P 2 O 5 -K 2 O. The experiment was laid out in a split-split-split-plot design with three replications. Genotype was placed in the main-plot, rice straw height was in the sub-plot, seed rate was in the sub-sub-plot and fertilizer level was in the sub-sub-sub-plot. From this study, it was observed that lentil genotype X95S-136X planted with a rice straw height under 30 cm, at a seed rate of 50 kg/ha, and with a fertilizer level at 30-60-30 kg/ha of N-P 2 O 5 -K 2 O produced the highest lentil seed yield of 2,070 kg/ha and the lowest seed yield (550 kg/ha ) was obtained from genotype Rajbari local, planted with a rice straw height under 10 cm, at a seed rate of 30 kg/ha and no fertilizer. Similar result regarding seed and straw yield was also found in the farmer"s field. The accession X95S-136 along with improved production technologies produced the highest grain and straw yield, and net return but the lowest grain and straw yield as well as net return were obtained by Rajbari local in the demonstration field. It was also identified that, by the inclusion of lentil in the transplanted monsoon rice field, lands could be brought under lentil cultivation to enhance lentil production, provide human nutrition and also to ensure soil health improvement for sustainable production system.
