Relationship between climatic factors and the flea index of two plague hosts in Xilingol League, Inner Mongolia Autonomous Region

Climatic factors are closely associated with the occurrence of vector-borne diseases, and they also influence the distribution of vectors. The occurrence of plague is closely related to the population dynamics of fleas and their host animals, as well as climatic conditions. This study focused on Xilingol League, utilizing climatic and flea index data from 2012 to 2021. Spearman correlation and "Boruta" importance analysis were conducted to screen for climatic variables. A generalized additive model (GAM) was employed to investigate the influence of climatic factors and rodent density on the flea index. GAM analysis revealed distinct trends in flea index among different rodent hosts. ForMeriones unguiculatus, the flea index declined with increased density and with higher humidity, yet rose with greater lagged sunshine duration. ForSpermophilus dauricus, an initial increase in flea index with density was observed, followed by a decrease, and a rise in the index was noted when ground temperatures were low. This study reveals the nonlinear interactions and lag effects among climatic factors, density, and flea index. Climatic factors and density variably influence the flea index of twoYersinia pestis hosts. This research advances the prediction and early warning efforts for plague control, providing a theoretical basis for rodent and flea eradication strategies.

Transforming growth factor-beta 1(TGF-β1)has been extensively studied for its pleiotropic effects on central nervous system *** neuroprotective or neurotoxic effects of TGF-β1 in specific brain areas may depend on the pathological process and cell types ***-gated sodium channels(VGSCs)are essential ion channels for the generation of action potentials in neurons,and are involved in various neuroexcitation-related ***,the effects of TGF-β1 on the functional properties of VGSCs and firing properties in cortical neurons remain *** this study,we investigated the effects of TGF-β1 on VGSC function and firing properties in primary cortical neurons from *** found that TGF-β1 increased VGSC current density in a dose-and time-dependent manner,which was attributable to the upregulation of Nav1.3 *** VGSC current density and Nav1.3 expression were significantly abolished by preincubation with inhibitors of mitogen-activated protein kinase kinase(PD98059),p38 mitogen-activated protein kinase(SB203580),and Jun NH2-terminal kinase 1/2 inhibitor(SP600125).Interestingly,TGF-β1 significantly increased the firing threshold of action potentials but did not change their firing rate in cortical *** findings suggest that TGF-β1 can increase Nav1.3 expression through activation of the ERK1/2-JNK-MAPK pathway,which leads to a decrease in the firing threshold of action potentials in cortical neurons under pathological ***,this contributes to the occurrence and progression of neuroexcitatory-related diseases of the central nervous system.

Severe Allergic Reaction Following Administration of Antivenom Serum for Snakebite: A Case Report from Togo

Background: Ophidian envenomation is common in Sub-Saharan Africa, and its management is hampered by the lack of access to healthcare services in rural areas, in particular the availability and appropriate use of antivenom. Rare cases of serious side effects following the administration of antivenom have been reported. This is the case for a young farmer from the central region of Togo, who experienced a second snake bite within four years, and in whom antivenom serotherapy led to severe allergic manifestations. Case presentation: This 24-year-old patient, with a history of antivenom and tetanus serotherapy, was admitted to the centre Hospitalier Régional (CHR) of Sokodé for a snakebite that occurred 45 minutes earlier while working in the field. Clinical assessment on admission revealed grade 1 envenomation, characterized by local pain in the right upper limb, with no sign of complication. He received an intravenous infusion of antivenom serotherapy, which rapidly relieved the pain, allowing him to be discharged after 24 hours of hospital monitoring. However, he was readmitted five days later for a skin rash associated with generalized pruritus and edema of the face and the neck, which prompted his evacuation to the Sylvanus Olympio University Hospital. He was diagnosed with a hypersensitivity-type allergic reaction to antivenom serum. Symptomatic treatment with antihistamines resulted in a favourable outcome after five days in the hospital. Conclusion: This young farmer developed a severe allergic reaction following a second course of antivenom serotherapy for low-grade ophidian envenomation. Although the efficacy of antivenom serum is undeniable in the management of snakebites, its use should be guided by a sound clinical assessment and framed by rigorous monitoring, especially in people sensitized to antivenom or antitoxin serotherapy. BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

Dear Editor,Severe fever with thrombocytopenia syndrome(SFTS)is an emerging tick-borne infectious disease caused by a novel bunyavirus called SFTS virus(SFTSV).It was initially identified in China in 2009(Yu et al.,2011).Since then,the number of reported SFTS cases has rapidly increased in China,South Korea,and Japan(Li,et al.,2018;Takahashi et al.,2014;Kim et al.,2018).Sporadic SFTS cases have also been identified in several other Asian countries,such as Vietnam,Pakistan,Myanmar,and Thailand(Takahashi et al.,2014;Tran et al.,2019;Li et al.,2021).This disease is recognized as a highly lethal viral hemorrhagic fever with a mortality rate ranging from 12%to 50%(Yu et al.,2011;Li et al.,2018;Takahashi et al.,2014;Kim et al.,2013).SFTS primarily spreads to humans through bites from ticks infected with SFTSV,with the Haemaphysalis longicornis tick acting as the predominant vector for SFTSV(Zhuang et al.,2018).

Fulminant myocarditis induced by SARS-CoV-2 infection without severe lung involvement:insights into COvID-19 pathogenesis

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection often leads to pulmonary *** sequelae,including myocarditis and heart failure,have also been ***,the study presents two fulminant myocarditis cases infected by SARS-CoV-2 exhibiting remarkable elevation of cardiac biomarkers without significant pulmonary injury,as determined by imaging *** staining reveals the viral antigen within cardiomyocytes,indicating that SARSCoV-2 could directly infect the *** full viral genomes from respiratory,anal,and myocardial specimens are obtained via next-generation *** analyses of the whole genome and spike gene indicate that viruses in the myocardium/pericardial effusion and anal swabs are closely related and cluster together yet diverge from those in the respiratory *** addition,unique mutations are found in the anal/myocardial strains compared to the respiratory strains,suggesting tissue-specific virus mutation and *** findings indicate genetically distinct SARS-CoV-2 variants have infiltrated and disseminated within myocardial tissues,independent of pulmonary injury,and point to different infection routes between the myocardium and respiratory tract,with myocardial infections potentially arising from intestinal *** findings highlight the potential for systemic SARS-CoV-2 infection and the importance of a thorough multi-organ assessment in patients for a comprehensive understanding of thepathogenesisof COVID-19.
