Status Quo and Analysis of the Cardiovascular Clinical Practice Guidelines/Expert Consensuses of Chinese and Integrative medicine:A Systematic

Objective:To describe and analyze the status quo of cardiovascular clinical practice guidelines or expert consensuses including both Chinese medicine(CM)and integrative medicine,through systematic literatures searching and quality ***:Data bases including Chinese Biomedical Literature Database,the China National Knowledge Infrastructure,Wanfang Data,China Science and Technology Journal Database were searched for published CM or integrative cardiovascular clinical practice guidelines or expert *** website *** was also retrieved as *** clinical practice evaluation tool AGREEⅡwas used to assess the quality of included guidelines or ***:A total of 31 relevant clinical practice guidelines or expert consensuses were included,covering diagnosis,treatment,Chinese patent and patient *** cardiovascular diseases like coronary heart diseases,heart failure and arrhythmia were also *** analysis it was found that both the quantity and quality of included guidelines have been improved year by year.A total of 4 evidence-based clinical practice guideline has been found,one of which was a guideline project *** that,the remaining 27 reports were all consensusbased *** scores of each field,from highest to lowest,were clarity of presentation(58%),scope and purpose(54%),stakeholder involvement(28%),rigor of development(21%),applicability(13%)and editorial independence(8%).Conclusions:Although clinical practice guidelines in cardiovascular domain of Chinese have gained increasing concern,with both quantity and quality improved,there is still huge gap in methodology and reporting standards between CM guidelines and international *** the one hand,it is essential to improve and standardize the methodology of developing CM *** the other hands,the evaluation system of evidence and recommendation with CM characters should be developed urgently.

Background:Eye-acupuncture(EA)is a fine-needle acupuncture therapy for systemic *** bibliometric analysis aims to provide a comprehensive review of the characteristics of EA for ischemic or hemorrhagic apoplexy based on clinical ***:A total of 195 clinical studies from six databases such as CNKI,VIP,Wanfang Data,SinoMed,PubMed and the Cochrane Library from inception to December 31,2017 were *** information mainly including study type,participant characteristics,intervention and comparison details,and outcomes were *** was analyzed descriptively using SPSSsoftware to determine their distribution(range,interquartile range)and central tendency(median).Results:The studies covered 108 RCTs,36 controlled clinical trials,45 case series and 6 case reports on EA for treatment of ischemic or hemorrhagic apoplexy,involved 15466 stroke *** was used at different stages of ischemic or hemorrhagic apoplexy:acute stage(n Z 38),recovery(n Z 32),sequela(n Z 13)and unclear(n Z 112).The most frequently used EA acupoints were in the upper-jiao and lower-jiao *** commonly used comparisons were EA alone versus traditional acupuncture,or EA plus traditional acupuncture versus traditional *** deficit scale,Modified Barthel Index,and activities of daily living were the most frequently used outcome ***:EA as an adjuvant therapy is effective and safe in treating stroke at acute and recovery stages,but high-quality RCTs are lacking.

Characteristics and quality of clinical practice guidelines for diabetic kidney disease:a systematic review

Pathogenic changes of community-acquired pneumonia in a children’s hospital in Beijing,China before and after COVID-19 onset:a retrospective study

Background This study aimed to analyze the pathogenic characteristics of community-acquired pneumonia(CAP)in a children’s hospital before and after the coronavirus disease 2019(COVID-19)pandemic and to provide testimony for preventing CAP in the *** A retrospective analysis was *** information was collected from the electronic medical record system of the hospital.A total of 2739 children were included from February 1,2019,to January 31,*** Among these 2739 patients were 1507(55.02%)males and 1232(44.98%)females;the median age was 3.84 *** were 2364 cases during the pre-COVID-19 period and 375 cases during the post-COVID-19 *** number of hospitalized children after the pandemic was 84.14%*** median age after the onset was 1.5 years younger than that before the onset(4.08 years old)(Z=−7.885,Pbacteria(50.42%)>viruses(29.57%)>fungi(3.43%).During the post-COVID-19 period,the pathogen proportions were bacteria(56.53%)>viruses(53.60%)>MP(23.47%)>fungi(3.73%).Conclusions There was a significant decrease in the number of children with CAP hospitalized after the pandemic,especially among school-age children,and the pathogen proportions of CAP with MP and IV were significantly *** inferred that CAP was effectively prevented in school-age children because of the strong mitigation measures.

Hospital-based health technology assessment(HB-HTA)is highly valuable in the application of hospital medical technology,cost control,improvement of medical quality,and protection of medical safety,and its use is increasing daily ***,in China,HB-HTA is still in its infancy,and it is yet to be adopted in traditional Chinese medicine(TCM).Therefore,this article introduces the application and development of HB-HTA,as well as discusses the current situation of TCM and related future development strategies to provide ideas and references for the development of HB-HTA in *** conclude and recommend that all major stakeholders,including the state,health departments,and hospital leaders,support and promote the exploration and development of HB-HTA in TCM hospitals and integrative medicine *** also suggest the utilization of existing talents and introduction of other necessary talents to promote better and faster organizational development,as well as suggest the formation of a national and internationally recognized HB-HTA guidance manual and toolkit for TCM hospitals based on both the national HTA guidelines and the evaluation of TCM hospitals pilot ***,we recommend that HB-HTA pilot study should be executed with Chinese patent medicine as the starting point to form the implementation process and framework in the TCM arena.

Traditional Chinese medicine(TCM) is one of the oldest systems of medicine. More and more attention has been paid to TCM application, but the variable quality of clinical trials with TCM impedes its widespread acceptance. The Standard Protocol Items: Recommendations for Interventional Trials(SPIRIT) 2013 Statement has established guidelines for designing clinical trials to ensure that the trial results are accurate and reliable. However, there are difficulties when applying SPIRIT 2013 Statement to trials with TCM, due to the unique theory and the characteristic of TCM intervention. An Extension to the original SPIRIT was developed to ensure the quality of trial design with TCM. As Chinese herbal formulae, acupuncture and moxibustion are common and representative interventions in TCM practice, the executive working group determined that the SPIRIT-TCM Extension focus on these three interventions. Extension was developed through initiation, 3 rounds of Delphi consensus survey, and finalizing expert meeting. Seven items from the SPIRIT 2013 Statement were modified, namely, "title", "background and rationale", "objectives", "eligibility criteria", "interventions", "outcomes", and "data collection methods". The Extension includes the introduction of the concept of TCM pattern and 3 major TCM interventions, with examples and explanations. The SPIRIT-TCM Extension 2018 provides suggestion for investigators in designing high quality TCM clinical trials. It is expected that wide dissemination and application of this extension ensure continuous improvement of TCM trial quality throughout the world.

Electroacupuncture for treatment of diabetic peripheral neuropathy:A systematic review of randomized controlled trials

Objective:To assess the effectiveness and safety of electroacupuncture for the treatment of diabetic peripheral neuropathy(DPN).Clinical studies in China have shown the beneficial effect of electroacupuncture compared with conventional ***,the effectiveness of electroacupuncture has not been well acknowledged ***:We searched the following databases from their inception through February 2016:MEDLINE,the Cochrane Central Register of Controlled Trials(April,2015),SinoMed,China National Knowledge Infrastructure,VIP,Wanfang Database,Chinese Important Conference Papers Database,and the Chinese Dissertation *** controlled trials(RCTs)were included if they compared electroacupuncture to conventional medicine,placebo,or no treatment on DPN *** 5.3 software was used for data analysis,with effect estimate presented as relative risk(RR)and mean difference(MD)with a 95%confidence interval(CI).Results:Eleven RCTs involving 837 participants were *** methodological quality of included RCTs was generally poor in terms of sequence generation,allocation concealment,blinding,incomplete outcome data,and selective outcome ***-analysis showed that electroacupuncture had a better effect on global symptom improvement than methylcobalamin(RR Z 1.29;95%CI:1.14e1.46)and vitamin B(RR Z 1.60;95%CI:1.33e1.94).Only two RCTs reported adverse events.
