Spatio-temporal variability and dynamics in Sahelian agro-pastoral zones make each local situation a special case. These specificities must be considered to guide the dissemination of agricultural options with a view to sustainable development. The territorial scale of municipalities is not sufficient for this necessary contextualization;the scale of the “village terroir” seems to be a better option. This is the hypothesis we put forward in the framework of the Global Collaboration for Resilient Food Systems program (CRFS), i.e. local context is spatially defined by village terroir. The study is based on data collected through participatory mapping and surveys in “village terroirs” in three regions of Niger (Maradi, dosso and Tillabéri). Then the links between farm managers and their cultivated land, as well as the spatio-temporal dynamics of local context are analyzed. This study provides evidence of the existence and functional usefulness of the village terroir for farmers, their land management and their activities. It demonstrates the usefulness of contextualizing agricultural options at this scale. Their analysis elucidates the links between “terroirs village” and the specific functioning of the agrosocio-ecosystems acting on each of them, thus laying the systemic and geographical foundations for a model of the spatio- temporal dynamics of “village terroirs”. This initial work has opened up new perspectives in modeling and sustainable development.

YIKI is an antimalarial phytomedicine used by a traditional healer to treat malaria in Bobo-dioulasso. However, there is no scientific evidence to support its use by local populations. The aim of this study was to identify the medicinal practices of the healer holder of YIKI and assess the clinical evidence of its phytomedicine in the uncomplicated malaria treatment. ethnomedical survey based on a semi-structured and open questionnaire was conducted from October to december 2019 with the healer. Malaria knowledge and diagnosis methods, patient treatment and monitoring, and recipe formulation steps were surveyed. Moreover, thick and thin blood smears were taken, haemoglobin levels and temperature of consenting patients were measured before treatment, mid-treatment and at the end of treatment. The survey revealed that the healer has a good knowledge of malaria symptoms and his diagnosis is based on observation and physical examination of patients. The healer’s malaria diagnosis was rudimentary and had accuracy problems, with only 62.79% of malaria cases confirmed by microscopy. The formulation of YIKI and its use to treat malaria follow a standard process for plant harvesting, powder quantities and posology, but do not use any reproducible parameters for dose adjustment. Forty-three patients diagnosed and treated by the healer participated in the study. Laboratory results revealed 27 Plasmodium falciparum infection cases, including 2 with parasitaemia ≥ 200,000 p/µl blood. 25 patients were selected for therapeutic evidence assessment. There was a 48% elimination of parasites, a 28% parasitaemia decrease without complete cure, with gametocytogenesis in some patients, and a 24% parasitaemia increase. Haemoglobin and temperature results suggested that YIKI was not cytotoxic and reduced fever. encouraging preliminary results have been obtained, but in view of the low number of patients, further YIKI efficacy and toxicity studies will be necessary for patient safety.

When contributing to participatory research, farmers usually appreciate the performance of cowpea varieties using qualitative scores. The score they attribute to each variety are based on local knowledge. The specific criteria they individually use to attribute a score are not well described. The objectives of this work were to: 1) identify and describe exhaustively the local criteria used by farmers to measure the agronomic performance of cowpea;2) assess the variability and statistical structure of these farmer criteria across local contexts;3) and analyze the association between these farmer criteria and the classical agronomic measurement. To achieve these objectives, an augmented block design was implemented across fifteen locations in the regions of Maradi, dosso and Tillabéri, representing a diversity of local contexts. From a set of 36 cowpea varieties, fifteen varieties were sown per location, including five varieties (controls) common to all locations. In each location, two replicates were sown in randomized Fisher’s blocks. After agronomic measurement and participatory evaluation (scoring of varieties by farmers), a group survey (focus group) was conducted in each location to identify the criteria considered by farmers to found their discretional scoring of varieties during the participatory evaluation. The analysis of the data identified, across locations, thirteen criteria defined by farmers to characterize the agronomic performance of cowpea. Some of these criteria were different according to location. Farmers ranked the three varieties with the best performance for each agronomical trait (Top 3 varieties). A comparison of the farmer ranking with the ranking based on agronomic measurements revealed similarity and complementary between both methods. This study highlighted the importance of considering both local and scientific knowledge in local varietal evaluations.

Introduction: The epidemiology of both hepatitis B virus (HBV) and hepatitis C virus (HCV) infections among drug users (dUs) is little known in West Africa. The study aimed to assess the prevalence of hepatitis B and C viruses among drug users in Burkina Faso. Methodology: This was a cross-sectional biological and behavioral survey conducted between June and August 2022, among drug users in Ouagadougou and Bobo dioulasso, the two main cities of Burkina Faso. A respondent-driven sampling (RdS) was used to recruit drug users. Hepatitis B surface antigen was determined using lateral flow rapid test kits and antibodies to hepatitis C virus in serum determined using an enzyme-Linked Immunosorbent Assay. data were entered and analyzed using Stata 17 software. Weighted binary logistic regression was used to identify the associated factors of hepatitis B and C infections and a p-value Results: A total of 323 drug users were recruited with 97.5% males. The mean age was 32.7 years old. The inhaled or smoked mode was the most used by drug users. The adjusted hepatitis B and hepatitis C prevalence among study participants were 11.1% and 2.3% respectively. The marital status (p = 0.001), and the nationality (p = 0.011) were significantly associated with hepatitis B infection. The type of drug used was not significantly associated with hepatitis B infection or hepatitis C infection. Conclusion: The prevalence of HBsAg and anti-HCV antibodies among dUs are comparable to those reported in the general population in Burkina Faso. This result suggests that the main routes of contamination by HBV and HCV among dUs are similar to those in the population, and could be explained by the low use of the injectable route by dUs in Burkina Faso.

The antiretroviral treatment (ART) has significantly reduced the number of new HIV/AIdS infections and related deaths. However, cases of immunovirological discordance (IVd) are found in various locations. The objective of this study was to determine the profile of People living with HIV (PLHIV) with IVd and to identify associated factors. We conducted a cross-sectional study based on the records of PLHIV under ART for at least 6 months, followed at Hpital de Jour donka from 2015 to 2017, and having both viral load (CV) and Cd4 T-cell count. Prevalence of IVd was 34.57%, with 23.87% for immunological discordance (Id) and 10.7% for virological discordance (Vd). Females were predominant (66.26%), and male gender influenced IVd with a statistically significant difference (p = 0.006) and was associated with Vd (p = 0.007). The average age was 38.77 11.30 years. PLHIV were classified at WHO stages 3 and 4 (86.01%). The median initial haemoglobin level was 11.5 g/L [3.2 - 12]. The mean initial Cd4 T-cell count was 272.84 cells/mm3 201.6. The median initial viral load (VL) was 147,337 copies/mL [1092 - 31,675,000]. The initial Cd4 T-cell count 3 was associated with IVd with a statistically significant difference (p = 0.0009) and correlated with Id (p = 0.000). Prurigo was associated with IVd with a statistically significant difference (p = 0.003). Cerebral toxoplasmosis was not associated with IVd but was associated with Id (p = 0.04). This study allowed us to describe the profile of PLHIV with IVd. The main associated factors were male gender, initial Cd4 T-cell count 3, toxoplasmosis, prurigo, and herpes zoster.

Introduction: In serodiscordant couples, sharing serostatus is a crucial step in the HIV risk management process. The aim of this study was to determine the prevalence and factors associated with serostatus sharing among HIV serodiscordant couples in the city of Parakou. Setting and methods: This study was carried out in the city of Parakou, capital of northern Benin. It was a descriptive, analytical cross-sectional study with non-probability sampling for convenience. Socio-demographic data, as well as data on antecedents, behaviours, social and marital life, were collected using a questionnaire gave to the participants. Clinical and therapeutic data were collected from patients’ medical records and follow-up charts. data were analyzed and processed using R 4.3.1 software. The significance level was set at 5%, and logistic regression was used to identify potential predictors of shared serostatus among serodiscordant couples in the city of Parakou in 2022. Results: A total of 299 participants in long-term serodiscordant relationships were included in our study. The average age of the subjects was 37.55 ± 10.25 years. Women were the infected partner in 80.27% of cases. The most common level of education was secondary (33.45%). Most respondents (230 subjects, 76.92%) were married. The average duration of the couple’s relationship was 121.68 months ± 96.32. The most frequent screening circumstances were: Prevention of mother-to-child transmission (PMTCT) (50.84%) and hospital screening (26.09%). The infected partner shared his or her serostatus with his or her spouse in 47.83% of cases. Potential predictors of serostatus sharing within the couple were: marital status (p Conclusion: The sharing of serostatus within serodiscordant couples needs to be managed in a way that takes into account the social and marital characteristics of the couple.
