At the onset of the coronavirus disease 2019(COVID-19)pandemic,pediatric emergency department(ED)visits decreased worldwide by an estimated 30%-89%[1].In Canada,there was a 58%reduction in pediatric ED visits in March and April 2020 relative to the same months in previous years[2].These reductions have been attributed to a decrease in non-severe acute respiratory syndrome corona-virus 2(SARS-CoV-2)infections in children resulting from confinement and public health measures,as well as a shift toward virtual care[3,4].

AIM: To investigate the conformity of management practices of gastrointestinal hemorrhage in cirrhotic patients with relevant guidelines. METHODS: A questionnaire on the management of digestive bleeding was completed for all consecutive cirrhotic patients admitted to 31 French hospitals. RESULTS: One hundred and twenty-six bleeding events were recorded. It was the first bleeding episode in 79 patients (63%), of whom 40 (51%) had a prior diagnosis of cirrhosis and 25 (32%) had previously undergone an endoscopy. The bleeding episode was a recurrence in 46 patients (37%). The median time between onset and admission was 4 h, but exceeded 12 h in 42% of cases. There was an agreement between centers forearly vasoactive drug administration (87% of cases), association with ligation (42%) more often than sclerosis (21%) at initial endoscopy, and antibiotic prophylaxis (64%). By contrast, prescription of beta-blockade alone or in combination (0 to 100%, P = 0.003) for secondary prophylaxis and lactulose (26% to 86%, P = 0.04), differed among centers. CONCLUSION: In French hospitals, management of bleeding related to portal hypertension in cirrhotic patients is generally in keeping with the consensus. Broad variability still remains concerning beta-blockade use for secondary prophylaxis. Screening for esophageal varices, the use of antibiotic prophylaxis and patients information need to be improved.

Background: Surgical treatment of upper mesocolic organs is improved by preoperative diagnosis of anatomical variants of celiac trunk. According to the literature, these anatomical variants are little known in sub-Saharan Africa. Purpose: To evaluate the prevalence of anatomical variants of celiac trunk in relation to its branching. Materials and methods: This was a cross-sectional study of descriptive type. It retrospectively evaluated 160 abdominal contrast enhanced CT-scan, from patients attending Yalgado OUEDRAOGO teaching hospital, from 1 January 2015 to 30 September 2016. Patients with a history of heavy abdominal surgery were excluded. Images obtained by 64-row CT-scan were analyzed for anatomical variants of the celiac trunk. Results: One hundred and twenty-eight patients (80%) had a classic anatomical configuration of celiac trunk, while thirty-two (20%) had at least one anatomical variant. Two anatomical variants were found in fifteen patients (9.4%) while five other patients (3.1%) had more than two variants. The most frequent anatomical variant was the hepato-splenic bifurcation, found in fourteen patients (8, 8%). It was followed by common celiac and mesenteric trunk, and then collateral arteries, in particular left hepatic artery and right lower diaphragmatic artery, each with three patients (1.9%). Conclusion: Anatomical variants related to celiac trunk branching, are as frequent in our study as in the literature. However, the two most common anatomical variants were hepato-splenic bifurcation and common celiac and mesenteric trunk.

BACKGROUND Probiotics are a promising solution for managing irritable bowel syndrome(IBS).Saccharomyces cerevisiae(***)I-3856 has already demonstrated beneficial effects in IBS subjects,particularly in IBS with predominant constipation(IBS-C).AIM To confirm the efficacy of *** I-3856 in the management of gastrointestinal symptoms in *** A randomized,double-blind,placebo-controlled clinical study was performed in a total of 456 *** a run-in period,subjects were randomly assigned to the group receiving *** I-3856(8×109 CFU daily)or the placebo for 8 wk,and they performed daily self-evaluations of gastrointestinal *** primary objective was to assess the effect of the probiotic on abdominal *** secondary objectives were the evaluation of other gastrointestinal symptoms,bowel movement frequency and consistency,and quality of life(QOL).RESULTS A significantly higher proportion of abdominal pain responders was reported in the Probiotic group(45.1%vs 33.9%,P=0.017).A nonsignificant difference in the area under the curve for abdominal pain over the second month of supplementation was observed in subjects receiving probiotic vs placebo[P=0.073,95%CI:-0.59(-1.23;0.05)].No statistically significant differences were reported in the evolution of bowel movement frequency and stool consistency between the *** 8 wk of supplementation,the overall QOL score was significantly higher in the Probiotic group than in the Placebo group[P=0.047,95%CI:3.86(0.52;7.20)].Furthermore,exploratory analyses showed statistically significant and clinically relevant improvements in QOL scores in abdominal pain responders vs *** The results of this clinical study confirmed the abdominal pain alleviation properties of *** I-3856 in *** pain relief was associated with improved *** identifier:NCT03150212.

We describe a case of very late transcatheter heart valve(THV) thrombosis of a first-generation SAPIEN prosthesis(Edwards Lifesciences, Irvine, CA) implanted in a 64-yearold woman with severe symptomatic aortic stenosis. More than 54 mo after implantation, she presented with severe symptomatic prosthesis dysfunction(stenosis) which was successfully treated with oral anticoagulation. To our knowledge, this is the tardiest case of THV thrombosis ever reported. This case should increase clinical awareness for THV thrombosis even beyond the first two-year period following implantation.

Prevalence and Predictive Factors of Severe Coronary Lesions in Algerian Patients Undergoing Coronary Angiography

Background: The severity of coronary lesions in patients with coronary artery disease (CAD) has important prognostic and therapeutic consequences. Clinical symptoms not always reflect the disease severity and this study aimed to evaluate coronary lesions in the Algerian population where these data are scarce. Methods: It was a prospective cross-sectional study conducted in consecutive patients with an age ≥20 years who underwent clinically-indicated coronary angiography in our center. The objective of the study was to determine the prevalence of severe coronary lesions assessed with the Gensini score. The predictive factors of severity were evaluated in a multivariate analysis. Results: A total of 507 patients were included (male, 79.7%;mean age, 58.8 years). Coronary angiography was prescribed most frequently for ST-segment elevation myocardial infarction (STMI) (46.9%) and non-STMI (38.1%). The prevalence of severe coronary lesions was 69.6% (95% CI 65.5 - 73.5). In multivariate analysis, the independent predictive factors of severe coronary lesions were male sex (odds ratio [OR] 2.00;p = 0.0141), diabetes (OR 1.92;p = 0.0070), left ventricular dysfunction (OR 1.81;p = 0.0059), age (OR 1.72;p = 0.0297) and no lipid-lowering treatment (OR, 0.47;p = 0.0388). Conclusions: Severe coronary lesions were present in two out three patients in this cohort of Algerian patients undergoing coronary angiography. Independent predictive factors of severe coronary lesions were male sex, diabetes, age, left ventricular dysfunction and no lipid-lowering treatment. It is important to identify these at-risk patients, as they should be explored at an asymptomatic stage before a cardiovascular event and receive prompt treatment with angioplasty or surgery.

Knowledge, Attitudes and Practices of Health Care Staff about Glaucoma in Lomé

Introduction: Glaucoma is a major cause of irreversible blindness worldwide. Early detection and awareness are essential to reduce the impact on eye health. The aim of this study is to assess the knowledge, attitudes and practices of health care staff about glaucoma in Lomé. Materials and Methods: This was a prospective cross-sectional and descriptive study including health care staff in Lomé, from October 1st to december 31st, 2020, for a period of 3 months. After their agreement, the officers responded to questions based on a survey sheet. Results: A total of 169 agents from 3 centers in Lomé, i.e. the Sylvanus Olympio University Teaching Hospital, the Campus University teaching Hospital and the Bè Hospital were involved. 50.9% of the agents were male versus 49.1% female, i.e. a ratio of 1.04. The average age was 33.66 ± 10.45 years. 46.10% were doctors versus 53.90% paramedics. 58.60% had at most 5 years of experience against 41.40% who had more than 5 years of experience. 53.80% defined glaucoma as ocular hypertension while 17.20% defined it as optic nerve disease. 46.20% did not know that there were many types of glaucomas compared to 53.20% who knew that there are many types of glaucomas. 92.30% knew that glaucoma can lead to cause visual loss. Regarding attitudes and practices about glaucoma, 91.70% declared that the follow-up should be done by the ophthalmologist, 81.10% thought that it would be necessary to be checked by the doctor only every 6 months. Finally, 65.08% declared that they had to use eye drops for life for the treatment. Conclusion: Steps should be taken to educate staff more about glaucoma attitudes and practices and also plan for action in the population.

dear Editor, The cytosolic protein Tau is naturally present in human neurons, where it has a pivotal role in controlling microtubule stability. Hyperphosphorylation of Tau （observed during neurodegenerative diseases, such as Alzheimer＇s disease） impairs the protein＇s ability to bind microtubules. This results in microtubule disassembly and the formation of Tau aggregates, Tau protein is also widely expressed in peripheral tissues. In the male reproductive system, screening for Tau has focused solely on the rodent and bovine testis. In the present study, we used immunofluorescence and immunoenzymatic techniques （with a Tau-specific antibody） to investigate the presence of Tau protein in human ejaculated sperm and testicular tissue.

Background: Large papillary excavations constitute a true differential diagnosis of glaucoma in our environments. Aim: To present a clinical case of large papillary excavation simulating glaucomatous optic neuropathy in a young subject. Case Presentation: The ophthalmologic examination made it possible to note a visual acuity of LogMar 0.00, ocular hypertonia and a large papillary excavation in both eyes. The visual field noted bilateral perimetric involvement suggesting optic neuropathy. Optical coherence tomography (OCT) of the ganglion complex revealed an absence of involvement of the ganglion complex and the ganglion ridge essential to evoke optic neuropathy. An ocular hypotonizing treatment made it possible to normalize the intraocular pressure. Conclusion: The particularity of this observation lies in the fact that the perimeter involvement that preceded any involvement can simulate a glaucomatous involvement. Functional damage to the visual field must be correlated with structural damage to suggest glaucomatous damage in our ophthalmic patients.
