A suture-free, shape self-adaptive and bioactive PEG-Lysozyme implant for Corneal stroma defect repair and rapid vision restoration

limitation of donor tissue shortage clinically. In addition, suturing-needed transplantation potentially causes postoperative complications. Herein, we design a PEG-Lysozyme injective hydrogel as a suture-free, shape self-adaptive, bioactive implant for corneal stroma defect repair. This implant experiences a sol-gel phase transition via an in situ amidation reaction between 4-arm-PEG-NHS and lysozyme. The physicochemical properties of PEG-Lysozyme can be tuned by the components ratio, which confers the implant mimetic corneal modulus and provides tissue adhesion to endure increased intraocular pressure. In vitro tests prove that the implant is beneficial to Human corneal epithelial cells growth and migration due to the bioactivity of lysozyme. Rabbit lamellar keratoplasty experiment demonstrates that the hydrogel can be filled into defect to form a shape-adaptive implant adhered to native stroma. The implant promotes epithelialization and stroma integrity, recovering the topology of injured cornea to normal. A newly established animal forging behavior test prove a rapid visual restoration of rabbits when use implant in a suture free manner. In general, this work provides a promising preclinical practice by applicating a self-curing, shape self-adaptive and bioactive PEG-Lysozyme implant for suture-free stroma repair.

Preoperative Characteristics of Ocular Biometry in Children with Unilateral Congenital Cataracts

The ocular biometry characteristics are clinically significant for children with unilateral congenital cataracts,but there is a lack of data analysis concerning the preoperative *** axial length(AL),mean keratometry(Km),cor-neal astigmatism(CA),and the anterior chamber depth(ACD)from both eyes before cataract surgery were obtained from 205 patients(410 eyes,3-15 years of age)with unilateral congenital *** the congenital cataract eyes,shorter AL(22.44±1.52 mm vs.22.57±1.04 mm,p=0.036)and higher CA(-1.89±0.91 D vs.-1.24±0.67 D,peye from the congenital cataract eye)that less than *** preoperative ocular biometry of shorter AL,larger Km,higher CA,and shallower ACD should be considered in females with unilateral congenital *** age and the binocular axial differences had a statistically significant correlation(r=-0.192,p=0.006).Therefore,changes in the binocular axial differences associated with aging may enhance the guidelines for intraocular lens selection and the management of congenital cataracts.

The East London glaucoma prediction score: web-based validation of glaucoma risk screening tool

AIM:It is difficult for Optometrists and General Practitioners to know which patients are at risk. The East London glaucoma prediction score (ELGPS) is a web based risk calculator that has been developed to determine Glaucoma risk at the time of screening. Multiple risk factors that are available in a low tech environment are assessed to provide a risk assessment. This is extremely useful in settings where access to specialist care is difficult. Use of the calculator is educational. It is a free web based service. Data capture is user specific. METHODS:The scoring system is a web based questionnaire that captures and subsequently calculates the relative risk for the presence of Glaucoma at the time of screening. Three categories of patient are described:Unlikely to have Glaucoma; Glaucoma Suspect and Glaucoma. A case review methodology of patients with known diagnosis is employed to validate the calculator risk assessment. RESULTS:Data from the patient records of 400 patients with an established diagnosis has been captured and used to validate the screening tool. The website reports that the calculated diagnosis correlates with the actual diagnosis 82% of the time. Biostatistics analysis showed:Sensitivity = 88% ; Positive predictive value = 97%; Specificity = 75%. CONCLUSION:Analysis of the first 400 patients validates the web based screening tool as being a good method of screening for the at risk population. The validation is ongoing. The web based format will allow a more widespread recruitment for different geographic, population and personnel variables.

Scleral ultrastructure and biomechanical changes in rabbits after negative lens application

AIM： To address the microstructure and biomechanical changes of the sclera of rabbits after negative lens application by spectacle frame apparatus. METHODS： Five New Zealand rabbits of seven weeks post-natal were treated with -8 D lens monocularly over the course of two weeks. Refractive errors and axial length （AXI.） were measured at the 1st, 7th and 14th days of the induction period. Ultrastructure of sclera was determined with electron microscopy. Biomechanical properties were tested by an Instron 5565 universal testing machine. RESULTS： ***-induced （1.1） eyes elongated more rapidly compared with fellow eyes with AXI. values of 15.56±0.14 and 15.21±0.14 mm （P〈0.01）. Fibril diameter was significantly smaller in the I.I eyes compared with control ones in the inner, middle, and outer layers （inner layer, 63.533 vs 76.467 nm; middle layer, 92.647 vs 123.984 nm; outer layer, 86.999 vs 134.257 nm, P〈0.01, respectively）. In comparison with control eyes, macrophage-like cells that engulfed fibroblasts, dilated endoplasmic reticulum, and vacuoles in fibroblasts were observed in the inner and middle stroma in the/U eyes. Ultimate stress and Young＇s modulus were lower in the I.I eyes compared with those in the control eyes. CONCLUSION： Negative lens application alters eye growth, and results in axial elongation with changes in scleral ultrastructural and mechanical properties.

Posterior capsule opacification(PCO)remains the predominant complication following cataract surgery,significantly impairing visual function *** this study,we developed a PCO model that closely mimics the anatomical structure of the crystalline lens capsule *** model incorporated a threaded structure for accurate positioning and observation,allowing for opening and *** 3D printing technology,a stable external support system was created using resin material consisting of a rigid,hollow base and *** replicate the lens capsule structure,a thin hydrogel coating was applied to the resin *** biocompatibility and impact on cellular functionality of various hydrogel compositions were assessed through an array of staining techniques,including calcein-AM/PI staining,rhodamine staining,BODIPY-C11 staining and EdU staining in conjunction with transwell ***,the PCO model was utilized to investigate the effects of eight drugs with anti-inflammatory and anti-proliferative properties,including 5-aminoimidazole-4-carboxamide ribonucleotide(AICAR),THZ1,sorbinil,4-octyl itaconate(4-OI),xanthohumol,zebularine,rapamycin and caffeic acid phenethyl ester,on human lens epithelial cells(HLECs).Confocal microscopy facilitated comprehensive imaging of the PCO *** results demonstrated that the GelMA 605%þPLMA 2%composite hydrogel exhibited superior biocompatibility and minimal lipid peroxidation levels among the tested ***,compared to using hydrogel as the material for 3D printing the entire model,applying surface hydrogel spin coating with parameters of 2000 rpm�2 on the resin-based 3D printed base yielded a more uniform cell distribution and reduced ***,rapamycin,4-OI and AICAR demonstrated potent antiproliferative effects in the drug intervention *** microscopy imaging revealed a uniform distribution of HLECs along the anatomical structure of the crystalline lens capsule with

Photopic pupil size change in myopic orthokeratology and its influence on axial length elongation

AIM:To explore the photopic pupil size behavior in myopic children undergoing overnight orthokeratology(ortho-k)over 1-year period and its effects on the axial ***:A total of 202 Chinese myopic children were enrolled in this prospective clinical *** subjects in ortho-k group and eighty-eight subjects in spectacle group completed the 1-year *** length(AL)was measured before enrollment and every 6mo after the start of *** photopic pupil diameter(PPD)was determined using the Pentacam AXL and measured in an examination room with lighting of 300-310 *** multiple linear regression analysis was used to identify variables contribution to axial ***:Compared with spectacle group,the average 1-year axial elongation was significantly slower in the ortho-k group(0.25±0.27 vs 0.44±0.23 mm,P0.05,Bonferroni correction).The 4.81 mm PPD may be a possible cutoff point in the ortho-k *** with PPD below or equal to 4.81 mm tended to have smaller axial elongation compared to subjects with PPD above 4.81 mm after 1-year period(t=-3.09,P=0.003).In ortho-k group,univariate analyses indicated that those with older age,greater degree of myopia,longer AL,smaller baseline PPD(PPDbaseline)experienced a smaller change in *** multivariate analyses,older age,greater AL and smaller PPDbaseline were associated with smaller increases in *** spectacle group,PPD tended to be stable(P>0.05,Bonferroni correction)and did not affect axial ***:PPDs experience significantly decreases at 1-month and 3-month ortho-k *** with smaller PPD tend to experience slower axial elongation and may benefit more from ortho-k.

Comparison of conventional and Hang-back methods of inferior oblique recession in V-pattern strabismus with inferior oblique overaction

AIM:To compare outcomes of conventional and Hangback methods of inferior oblique(IO)recession in V-pattern strabismus with inferior oblique overaction(IOOA).METHODS:Comparative randomized study was conducted consisting of 50 patients,age 6 to 35 y having V-pattern strabismus[>15 prism diopter(PD)]with *** were divided equally in two groups and underwent IO recession,group A by conventional method and group B by Hang-back *** evaluated were reduction in V-pattern and IOOA,shift in torsion,improvement in binocular *** was a residual V-pattern of less than 10 PD at 1 y of follow ***:The mean V-pattern preoperatively and postoperatively at 1 y was 24±5.4 PD and 4.2±3.51 PD in group A and 23.44±6.44 PD and 5.76±3.8 PD in group B *** mean reduction in V-pattern was 20±6.78 PD in group A and 18.2±5.48 PD in group *** mean correction of IOOA was 18.48±3.13 PD in group A and 16±2.93 PD in group *** shift in extorsion was 3.08±1.8 degree in group A and 3.72±2.14 degree in group ***:Both the procedures achieve a significant and comparable reduction in V-pattern and ***-back recession being a landmark free surgery eliminates the need for intrascleral suturing thereby reducing the risk of scleral perforation with possible postoperative adjustment of *** may be considered as a good alternative for IO recession in patients of V-pattern strabismus with mild to moderate amount of IOOA.
