Revealing and predicting the long-term biomechanical response of orthokeratology by developing a patient-specific computational model

Orthokeratology(OK) is widely used for effective myopia correction and control. However, the incomplete understanding of its biomechanical mechanisms makes OK lens fitting rely heavily on clinician judgment, complicating accurate predictions of treatment outcomes. In this paper, we performed clinical experiments and numerical analysis to study corneal deformation modes and long-term changes in central corneal thickness. Clinical experiments were conducted on 194 Chinese myopic patients under OK treatment for 3 months. Based on the experimental data, a patient-specific computational biomechanical model for OK was established and validated. Specifically, the anisotropic mechanical properties of the cornea were incorporated into the model to describe the significant difference between its shear modulus(29.5 kPa) and tensile modulus(768.4 kPa). Additionally, a viscohyperelastic material model with a prolonged corneal relaxation time of 5.6 h was developed to capture the long-term deformation response. The results show that corneal thickness reduction in OK is primarily due to out-of-plane shear deformation,influenced by the cornea's low shear resistance. Modeling the extended corneal relaxation time is crucial for predicting long-term biomechanical responses. The computational model effectively captures long-term changes in central corneal thickness, potentially improving OK lens fitting accuracy.

Episcleral macular buckling for posterior retinal detachment in silicone oil filled eyes associated with myopic macular hole

AIM: To evaluate anatomical and visual outcomes of episcleral macular buckling (EMB) for posterior retinal detachment in silicone oil filled eyes associated with myopic macular ***: Five cases of EMB for initial failure of retinal reattachment after internal limiting membrane (ILM) peeling and silicone oil tamponade caused by myopic macular hole were retrospectively reviewed. A silicone sponge sutured directly across the macular region was performed on the silicone oil filled eyes. Silicone oil was removed no sooner than 1 month post- EMB. The duration of follow -up time after removal of silicone oil was more than 3 ***: Retinas of five eyes were all reattached at the last follow-up. The postoperative vision ranged from counting fingers to ***: Anatomical results improved after EBM for posterior retinal detachment in silicone oil filled eyes associated with myopic macular hole, which was not evident for visual outcome.

Ocular dominance(OD) is referred to the superiority of one eye over the other when the visual sense is produced[1]. This condition can be considered as a preference for the particular laterality of the eye during a mono-visual task or the relative contributions of visual signal transduction between eyes[2]. Empirically, OD tendencies are exhibited in habitual and subconscious behavior.

A randomized study of network-based perception learning in the treatment of amblyopia children

AIM:To compare the effectiveness of network-based perception learning(NBPL) and traditional training in the treatment of amblyopia ***:This randomized controlled clinical trial recruited 56 participants aged 4-12 y with anisometropic and/or strabismic amblyopia. Participants were randomly divided into two groups:the NBPL group(n=28) who received patching and NBPL for 3 mo, and the control group(n=28) who got 3 mo of patching and traditional training. Best-corrected visual acuity(BCVA) in the amblyopic eye and stereoacuity were measured and compared at baseline, 1, 2, and 3 mo ***:There were no significant differences in age, gender ratio, and BCVA between the two groups at baseline. At 3 mo, most patients gained lines(2 log MAR lines on average) of BCVA in both groups except one 11-year-old girl in the control group(Peye between the two groups was found(P=0.725), and amblyopia resolved(BCVA of 0.1 log MAR or better or within 1 log MAR line of the fellow eye) for 13(46.4%) participants in both groups. The number of patients with improvement of stereoacuity was 25 and 13 in the NBPL group and control group(P=0.041), respectively, and a significant difference exists in the distribution of stereopsis at 3 mo between the two groups(P=0.015). Besides, in patients with measurable stereopsis improvement degree and space for improvement in the two groups, the NBPL group also achieved better stereoscopic improvement than the control group(10/11 vs 4/11, P<0.05).CONCLUSION:The NBPL system has a significant effect on the improvement of BCVA and stereoacuity of amblyopia children and is better than traditional training in terms of stereoacuity improvement. Perceptual learning visual training may play a more important role in the treatment of amblyopia in the future.

Decentration and tilt of plate-haptic multifocal intraocular lenses in myopic eyes

Background:To investigate the decentration and tilt of plate-haptic multifocal intraocular lenses(MfIOLs)in myopic ***:Myopic(axial length[AXL]>24.5 mm)and non-myopic(21.0 mmeyes were enrolled in this prospective study and randomly assigned to receive implantation of Zeiss AT LISA tri 839MP lenses(Group A)or Tecnis ZMB00 lenses(Group B).In total,122 eyes of 122 patients were available for *** and tilt of MfIOLs,high-order aberrations(HOAs),and modulation transfer functions(MTFs)were evaluated using the OPD-Scan III aberrometer 3 months *** symptoms were assessed with a Quality of Vision ***:Near and distance visual acuities,tilt and horizontal decentration did not differ between the two groups,***,myopic eyes of Group B showed greater vertical decentration than those of Group A(−0.17±0.14 mm vs.-0.03±0.09 mm,respectively),particularly when the MfIOLs were placed horizontally or *** decentration of myopic eyes was greater in Group B than in Group A(0.41±0.15 mm vs.0.16±0.10 mm,respectively).In Group B,AXL was negatively correlated with vertical decentration and positively correlated with overall *** such correlations were found in Group *** total HOAs,coma,trefoil and spherical aberrations were lower in Group A than in Group B for a 6.0 mm pupil among myopic ***,Group A had better MTFs and fewer subjective symptoms than Group B among myopic ***:Plate-haptic design of MfIOLs may be a suggested option for myopic cataract eyes due to the less inferior decentration and better visual quality postoperatively.

Placement of dual capsular tension rings for the combined management of traumatic cyclodialysis cleft and zonular dialysis

Background:To evaluate the efficacy and safety of placing dual capsular tension rings for the combined management of traumatic cyclodialysis cleft and zonular ***:A modified capsular tension ring was inserted into the ciliary sulcus and a capsular tension ring or modified capsular tension ring was inserted into the capsular bag in 20 eyes in 20 consecutive patients showing signs of ocular hypotony and ectopia *** measures included intraocular pressure,best-corrected visual acuity,and postoperative ***:Dual capsular tension ring placement was performed in 20 patients with a mean age of 48.7 *** cyclodialysis cleft extended over 2.9 clock hours(range 0.5-6.5).The modified capsular tension ring was successfully inserted into the ciliary sulcus and a capsular tension ring or modified capsular tension ring in the capsular bag in all *** the last follow-up,the cyclodialysis cleft was closed in 16/20(80.0%)*** intraocular lens was stable in all patients ***-corrected visual acuity,in terms of the logarithm of the minimal angle of resolution,improved from 1.3±0.8 before surgery to 0.4±0.3 after surgery(P<0.001).Intraocular pressure increased significantly from 10.6±3.2 mmHg before surgery to 13.0±4.8 mmHg after surgery(P=0.040).Postoperative complications included a painful reversible intraocular pressure spike in four patients(20.0%).Logistic regression revealed no significant factors associated with successful cleft closure and a stable final intraocular pressure of≥10 ***:The placement of two capsular tension rings into the ciliary sulcus and the capsular bag is a safe,successful procedure combined for repairing a traumatic cyclodialysis cleft and managing zonular dialysis.

Cap morphology after small-incision lenticule extraction and its effects on intraocular scattering

AIM： To investigate cap morphology after small-incision lenticule extraction （SMILE） and its effects on intraocular scattering. METHODS： Sixty-five eyes of 33 patients undergoing SMILE were enrolled. In addition to regular evaluation, Fourier-domain optical coherence tomography was used to investigate cap thickness at ld, lwk, 1 and 3mo postoperatively. The optical quality including modulation transfer function cutoff frequency, Strehl ratio, Optical Quality Analysis System （OQAS） values, and objective scattering index （OSI）, were evaluated using OQASTM. RESULTS： Cap thickness decreased from ld to lwk （P〈0.001）, but remained higher than intended thickness of 120 μm after 3mo （P〈0.001）. Cap thickness in central area was thinner than that of in the paracentral and peripheral areas （P〈0.0001）. Total number of microdistortions decreased from ld to 3mo （P〈0.0001）. Pearson analysis revealed a weak correlation between OSI and standard deviation of cap thickness at ld and lmo, as well as between range of cap thickness and OSI at 1mo. No correlation was found between microdistortion and OSI, but a negative correlation existed between microdistortion and range at ld and lmoafter surgery. CONCLUSION： The corneal cap tends to be more accurate and regular with time lapse. Better cap morphology tends to contribute less intraocular scattering in the eyes undergoing SMILE.
