Cold injury refers to local or systemic injury caused by a rapid, massive loss of body heat in a cold environment. The incidence of cold injury is high. However, the current situation regarding the diagnosis and treatment of cold injury in our country is not ideal. To standardize and improve the level of clinical diagnosis and treatment of cold injury in China, it is necessary to make a consensus that is practical and adapted to the conditions in China. We used the latest population-level epidemiological and clinical research data, combined with relevant literature from China and foreign countries. The consensus was developed by a joint committee of multidisciplinary experts. This expert consensus addresses the epidemiology, diagnosis, on-site emergency procedures, in-hospital treatment, and prevention of cold injury.

Computed tomography coronary angiography after excluding myocardial infarction:high-sensitivity troponin versus risk score-guided approach

BACKGROUND:Patients with suspected acute coronary syndrome (ACS) in whom myocardial infarction has been ruled out are still at risk of having obstructive coronary artery disease (CAD).This rate is higher among patients with intermediate high-sensitivity troponin I (hsTnI) concentrations (5 ng/L to 99th percentile) than low concentrations (emergency department with suspected ACS,433 patients without elevated hsTnI who also underwent CTCA were selected and *** performances of hsTnI concentration and HEART score were compared using sensitivity,specificity,positive predictive value (PPV),and negative predictive value (NPV).RESULTS:Overall,120 (27.7%) patients had obstructive *** with intermediate hsTnI concentrations were more likely to have obstructive CAD than those with low hsTnI concentrations(40.0%vs.18.1%);patients with non-low-risk HEART scores (≥4 points) were also more likely to have obstructive CAD than those with low-risk scores (0 to 3 points)(41.0%vs.7.6%).The HEART score had higher sensitivity and NPV for detecting obstructive CAD in each classification than hsTnI concentration (sensitivity:89.2%vs.63.3%;NPV:92.4%vs.81.9%,respectively).CONCLUSION:After excluding myocardial infarction in patients with suspected ACS,adding the HEART score for selecting candidates for CTCA could improve patient risk stratification more accurately than relying on hsTnI concentration.

Hepatitis B is an infectious disease with serious health im-plications caused by hepatitis B virus. With blood, iatro-genic, sexual and mother-to-fetus transmission as its main transmission routes, hepatitis B virus causes diseases in-cluding acute or chronic hepatitis, liver cirrhosis and liver cancer. Most adults infected with hepatitis B virus can be healed, while infected infants may typically live with a chronic and persistent infection. According to the national survey of hepatitis B epidemics conducted in 1979, among all surveyed people, the mean carrier rate of hepatitis B surface antigen （HBsAg） was 9.76% and that children may reach or approximate the peak value when they are only four years old.

Performance of the EDACS-ADP incorporating high-sensitivity troponin assay:Do components of major adverse cardiac events matter?

Hematopoiesis is a finely tuned process that generates all blood cell types through self-renewal and differentiation, which is crucial for maintaining homeostasis. Acute infections can prompt a hematopoietic response known as emergency myelopoiesis. In this study, using a Candida albicans (C. albicans) infection model, we demonstrated for the first time that disruption of Fhl2 led to increased fungal burden,heightened inflammatory response and reduced survival rates. Impaired myeloid hematopoiesis and immune cell production were evident, as proved by the decreased numbers of hematopoietic stem and progenitor cells (HSPCs) and granulocytes in the bone marrow of Fhl2-deficient mice. In conclusion, FHL2 regulated emergency myelopoiesis in response to C. albicans, affecting the host’s defense against pathogens.

The emergency Department Crash Cart: A systematic review and suggested contents

BACKGROUND: As the field of emergency Medicine grows worldwide, the importance of an emergency Department Crash Cart(EDCC) has long been recognized. Yet, there is paucity of relevant peer-reviewed literature specifically discussing EDCCs or proposing detailed features for an EDCC suitable for both adult and pediatric patients. METHODS: The authors performed a systematic review of EDCC-specific literature indexed in Pubmed and Embase on December 20, 2016. In addition, the authors reviewed the 2015 American Heart Association(AHA) guidelines for cardiopulmonary resuscitation and emergency cardiovascular care, the 2015 European Resuscitation Council(ERC) guidelines for resuscitation, and the 2013 American College of Surgeons(ACS) Advanced Trauma Life Support(ATLS) 9 th ***: There were a total of 277 results, with 192 unique results and 85 duplicates. After careful review by two independent reviewers, all but four references were excluded. None of the four included articles described comprehensive contents of equipment and medications for both the adult and pediatric populations. This article describes in detail the final four articles specific to EDCC, and proposes a set of suggested contents for the EDCC. CONCLUSION: Our systematic review shows the striking paucity of such a high impact indispensable item in the ED. We hope that our EDCC content suggestions help enhance the level of response of EDs in the resuscitation of adult and pediatric populations, and encourage the implementation of and adherence to the latest evidence-based resuscitation guidelines.

Comparison of laboratory parameters,clinical symptoms and clinical outcomes of COVID-19 and influenza in pediatric patients:A systematic review and meta-analysis

Uplink transmission design for crowded correlated cell-free massive MIMO-OFDM systems

In cell-free massive multiple-input multiple-output(MIMO) orthogonal frequency division multiplexing(OFDM) systems, user equipments(UEs) are served by many distributed access points(APs),where channels are correlated owing to finite angle-delay spread in realistic outdoor wireless propagation environments. Meanwhile, the number of UEs is growing rapidly for future fully networked society. In this paper, we focus on the uplink transmission design in crowded correlated cell-free massive MIMO-OFDM systems with a limited number of orthogonal pilots. For the pilot transmission phase, we identify active UEs based on non-orthogonal pilot phase shift hopping patterns and non-orthogonal adjustable phase shift pilots(APSP). We derive a closed-form expression of mean square error of channel estimation(MSE-CE) and obtain an optimal condition for minimizing MSE-CE. According to this condition, the APSP set allocation scheme is proposed. Furthermore, for the data transmission, the max-min power control algorithm is devised to maximize the minimum spectral efficiency(SE) lower bound among active UEs. Simulation results indicate significant performance gains in terms of MSE-CE for the proposed APSP set allocation scheme. The proposed power control scheme can further improve the minimum SE among active UEs. Hence, they are crucial for crowded correlated cell-free massive MIMO-OFDM systems.
