Clinicopathological and molecular features of HR+/HER2- breast cancer patients with distinct endocrine resistance patterns

Objective: Recurrence continues to be a pivotal challenge among hormone receptor-positive（HR+）/human epidermal growth factor receptor 2-negative（HER2-） breast cancers. In the international consensus guidelines,HR+/HER2-breast cancer relapse patterns are divided into three distinct types: primary resistant, secondary resistant, and endocrine sensitive. However, owing to the lack of cohorts with treatment and follow-up data, the heterogeneity among different recurrence patterns remains uncharted. Current treatments still lack ***: This analysis included data from a large-scale multiomics study of a HR+/HER2-breast cancer cohort（n=314）. Through the analysis of transcriptomics（n=312）, proteomics（n=124）, whole-exome sequencing（n=290）,metabolomics（n=217）, and digital pathology（n=228） data, we explored distinctive molecular features and identified putative therapeutic targets for patients experiencing ***: We explored distinct clinicopathological characteristics, biological heterogeneity, and potential therapeutic strategies for recurrence. Based on a shared relapse signature, we stratified patients into high-and lowrecurrence-risk groups. Patients with different relapse patterns presented unique molecular features in primary tumors. Specifically, receptor tyrosine kinase（RTK） pathway activation in the primary resistant group suggested the utility of RTK inhibitors, whereas mammalian target of rapamycin（mTOR） and cell cycle pathway activation in the secondary resistant group highlighted the potential of mTOR and CDK4/6 inhibitors. The latest data from the NATALEE trial showed the absolute 3-year invasive disease-free survival benefit was 4.9% between the experimental and control groups. That is to say, in the intermediate-risk hormone receptor positive/human epidermal growth factor receptor-2 negative subgroup, there are also some patients with primary resistance to ribociclib. These patients benefit less from ribociclib, and they are unable to gain significant benefit even with the intensive adjuvant therapy of ribociclib. Considering the drug toxicity and health economic benefits, a 3-year course of ribociclib may not be appropriate for all intermediate-risk populations. Therefore, how to screen out the prime population for intensive adjuvant therapy of ribociclib needs to worth explored. In this paper, we discussed that the adaptive neoadjuvant endocrine therapy can screen out the prime population for intensive adjuvant therapy of ribociclib.

BACKGROUND Rectal cancer is prevalent and associated with substantial morbidity and *** To develop a nomogram prediction model for overall survival(OS)in patients with rectal cancer by leveraging a comprehensive analysis of demographic,clinicopathological,haematological,and follow-up data to identify independent prognostic *** We conducted a prospective cohort study in China involving rectal cancer patients and applied Cox regression and least absolute shrinkage and selection operator regression to assess the significance of various variables as independent prognostic factors for *** identified factors were integrated into a nomogram model,which was evaluated for predictive accuracy via the C-index,area under the curve(AUC),calibration curve,and decision curve analysis(DCA).RESULTS Multivariate analysis revealed independent predictors of OS,including the Karnofsky performance status,age,sex,TNM stage,chemotherapy,surgery,targeted therapy,β2-microglobulin,lactate dehydrogenase,and the neutrophil-to-lymphocyte *** nomogram demonstrated a C-index of 0.80 for the training and validation cohorts,with AUC values indicating high predictive accuracy for 1-year,3-year,and 5-year *** calibration curves confirmed the model's excellent agreement with the observed survival rates,and DCA revealed the superior clinical utility of the nomogram over the TNM staging *** In this study,a novel prognostic model that accurately predicts the OS of rectal cancer patients was *** model exhibited excellent discriminatory and calibration capabilities,thus offering a reliable tool for health care professionals to estimate patient survival.

It is of great significance to find safe and effective radiosensitizers. A primary investigation has been made on fisetin's modification of radiation effect, but its radiosensitization and related mechanisms still need to be deeply clarified. Furthermore, fisetin with high hydrophobicity is difficult to dissolve in water,severely limiting its research and application. In this study, we fabricated a safe and soluble radiosensitizer fisetin micelle for precisely enhancing radiotherapy by inhibiting platelet-derived growth factor receptor-β(PDGFRβ)/signal transducer and activator of transcription 1(STAT1)/signal transducer and activator of transcription 3(STAT3)/B cell lymphoma 2(Bcl-2) signaling pathway in the tumor. Systematic and detailed studies were performed to verify its radiosensitization effect in vitro and in vivo. On the cellular level, fisetin micelles selectively increased the radiosensitivity of tumor cells(CT26 and 4T1 cells)and had little effect on the sensitivity of normal mouse cells(L929 cells) to radiation. In the mouse models of colon and breast cancers, fisetin micelles showed an efficient radiosensitization capacity without apparent toxicity. Additionally, we first found that fisetin micelles played a radiotherapy sensitization role by inhibiting the PDGFRβ/STAT1/STAT3/Bcl-2 pathway activity. In general, this work not only confirmed that fisetin micelles precisely exhibit a radiosensitization effect in vitro and in vivo, but also profoundly explored its mechanisms underlying, to provide a theoretical and experimental basis for the clinical application of fisetin micelles.

Tumor suppressor p53-mediated cell cycle arrest and DNA damage repair may exert cytoprotective effects against cancer therapies,including WEE1 *** that p53 activation can also lead to multiple types of cell death,the role of this tumor suppressor in WEE1 inhibitor-based therapies remains *** this study,we reported that nucleolar stress-mediated p53 activation enhanced the WEE1 inhibitor AZD1775-induced ferroptosis to suppress lung cancer *** findings showed that AZD1775 promoted ferroptosis by blocking cystine uptake,an action similar to that of ***,inhibition of WEE1 by the WEE1 inhibitors or siRNAs induced compensatory upregulation of SLC7A11,which conferred resistance to ***,AZD1775 prevented the enrichment of H3K9me3,a histone marker of transcriptional repression,on the SLC7A11 promoter by repressing the expression of the histone methyltransferase SETDB1,thereby enhancing NRF2-mediated SLC7A11 *** finding was also validated using the H3K9me3 inhibitor ***,we found that the nucleolar stress-inducing agent Actinomycin D(Act.D)inhibited SLC7A11 expression by activating p53,thus augmenting AZD1775-induced ***,the combination of AZD1775 and Act.D synergistically suppressed wild-type p53-harboring lung cancer cell growth both in vitro and in ***,our study demonstrates that AZD1775 promotes ferroptosis by targeting cystine uptake but also mediates the adaptive activation of SLC7A11 through the WEE1-SETDB1 cascade and NRF2-induced transcription,and inhibition of SLC7A11 by Act.D boosts the anti-tumor efficacy of AZD1775 by enhancing ferroptosis in cancers with wild-type p53.
