Current therapeutic strategies for ischemic stroke fall short of the desired objective of neurological functional ***,there is an urgent need to develop new methods for the treatment of this *** are natural cell-derived vesicles that mediate signal transduction between cells under physiological and pathological *** have low immunogenicity,good stability,high delivery efficiency,and the ability to cross the blood–brain *** physiological properties of exosomes have the potential to lead to new breakthroughs in the treatment of ischemic *** rapid development of nanotechnology has advanced the application of engineered exosomes,which can effectively improve targeting ability,enhance therapeutic efficacy,and minimize the dosages *** in technology have also driven clinical translational research on *** this review,we describe the therapeutic effects of exosomes and their positive roles in current treatment strategies for ischemic stroke,including their antiinflammation,anti-apoptosis,autophagy-regulation,angiogenesis,neurogenesis,and glial scar formation reduction ***,it is worth noting that,despite their significant therapeutic potential,there remains a dearth of standardized characterization methods and efficient isolation techniques capable of producing highly purified *** optimization strategies should prioritize the exploration of suitable isolation techniques and the establishment of unified workflows to effectively harness exosomes for diagnostic or therapeutic applications in ischemic ***,our review aims to summarize our understanding of exosome-based treatment prospects in ischemic stroke and foster innovative ideas for the development of exosome-based therapies.

In this paper,we extend the deterministic learning theory to sampled-data nonlinear *** on the Euler approximate model,the adaptive neural network identifier with a normalized learning algorithm is *** is proven that by properly setting the sampling period,the overall system can be guaranteed to be stable and partial neural network weights can exponentially converge to their optimal values under the satisfaction of the partial persistent excitation(PE)***,locally accurate learning of the nonlinear dynamics can be achieved,and the knowledge can be represented by using constant-weight neural ***,we present a performance analysis for the learning algorithm by developing explicit bounds on the learning rate and *** factors that influence learning,including the PE level,the learning gain,and the sampling period,are *** studies are included to demonstrate the efectiveness of the approach.

Target tracking using distributed sensor network is in general a challenging problem because it always needs to deal with real-time processing of noisy information. In this paper the problem of using nonlinear sensors such as distance and direction sensors for estimating a moving target is studied. The problem is formulated as a prudent design of nonlinear filters for a linear system subject to noisy nonlinear measurements and partially unknown input, which is generated by an exogenous system. In the worst case where the input is completely unknown, the exogenous dynamics is reduced to the random walk model. It can be shown that the nonlinear filter will have optimal convergence if the number of the sensors are large enough and the convergence rate will be highly improved if the sensors are deployed appropriately. This actually raises an interesting issue on active sensing： how to optimally move the sensors if they are considered as mobile multi-agent systems？ Finally, a simulation example is given to illustrate and validate the construction of our filter.

Designing and completing clinical intervention trials can be challenging. Many aspects must be considered to ensure that patients who fulfill the inclusion criteria for the intervention are identified and recruited effectively. The aim of this paper was to disseminate the results of a cancelled trial and present unpredictable barriers met underway, so future researchers can learn from these. The trial examined perioperative analgesic and anxiolytic effects of melatonin. It was registered at https://***/ (NCT02386319) and a study protocol was published a priori. Participants were recruited from the plastic surgery ward of a Danish private hospital. The intended sample size of the trial was 72 patients based on power calculations of the outcome measures. During the six-month recruitment period, six patients were included, with only three completing the trial. Unpredictable barriers were poor communication between investigators and facility staff, lack of access to booking and operation schedules at the recruitment facility, the patient group being unwilling to participate, and the timing of recruitment conversations being unsuited as patients often did not have time to talk to the investigators. Too few data were collected to make any meaningful statistical analyses. Our trial was cancelled prematurely because of unpredictable barriers after commencing recruitment. Considering these barriers when designing a clinical trial may help future researchers avoid cancelling trials. Transparency of research is important and even prematurely cancelled trials should publish their findings.

AIM To determine morbidity and mortality in this specific patient group and also to assess for any independent associations between Do Not Resuscitate(DNR) status and increased post-operative morbidity and *** We conducted a propensity score matched retrospective analysis using de-identified data from the American College of Surgeons' National Surgical Quality Improvement Project(ACS NSQIP) for all patients undergoing hip fracture surgery over a 7 year period in hospitals across the United States enrolled in ACS NSQIP with and without Do Not Resuscitate Status. We measured patient demographics including DNR status, co-morbidities, frailty and functional baseline, surgical and anaestheticprocedure data, post-operative morbidity/complications, length of stay, discharge destination and *** Of 9218 patients meeting the inclusion criteria, 13.6% had a DNR status, 86.4% did not. Mortality was higher in the DNR status compared to the non-DNR group, at 15.3% vs 8.1% and propensity score matched multivariable analysis demonstrated that DNR status was independently associated with mortality(OR = 2.04, 95%CI: 1.46-2.86, P < 0.001). Additionally, analysis of the propensity score matched cohort demonstrated that DNR status was associated with a significant, but very small increased likelihood of post-operative complications(0.53 vs 0.43 complications per episode; OR = 1.21; 95%CI: 1.04-1.41, P = 0.004). Cardiopulmonary resuscitation and unplanned reintubation were significantly less likely in patients with DNR *** Whilst DNR status patients had higher rates of postoperative complications and mortality, DNR status itself was not otherwise associated with increased morbidity. DNR status appears to increase 30-d mortality via ceilings of care in keeping with a DNR status, including withholding reintubation and cardiopulmonary resuscitation.

A series of novel acylide derivatives have been synthesized from clarithromyc｜n A via a tacile procedure. The C-3 modifications involved replacing the natural C-3 cladinosyl group in clarithromycin core with different aryl-piperzine sidechain via chemical synthesis, Meanwhile a distinctive intermediate with 10,11-epoxy moiety was obtained, The structure and stereochemistry of this novel structure were confirmed via NMR and X-ray crystallography. Potential anti-bacterial activities against both Grampositive and Gram-negative bacteria were reported. Because of existence of C10,11-epoxide, these derivatives can be used as intermediates for further structural modification.

Multi-messenger gravitational wave(GW)observation for binary neutron star merger events could provide a rather useful tool to explore the evolution of the *** particular,for the third-generation GW detectors,*** Einstein Telescope(ET)and the Cosmic Explorer(CE),proposed to be built in Europe and the U.S.,respectively,lots of GW standard sirens with known redshifts could be obtained,which would exert great impacts on the cosmological parameter *** total neutrino mass could be measured by cosmological observations,but such a measurement is model-dependent and currently only gives an upper *** this work,we wish to investigate whether the GW standard sirens observed by ET and CE could help improve the constraint on the neutrino mass,in particular in the interacting dark energy(IDE)*** find that the GW standard siren observations from ET and CE can only slightly improve the constraint on the neutrino mass in the IDE models,compared to the current *** improvements in the IDE models are weaker than those in the standard cosmological *** the limit on neutrino mass can only be slightly updated,the constraints on other cosmological parameters can be significantly improved by using the GW observations.

In the coming decades,the space-based gravitational-wave(GW)detectors such as Taiji,TianQin,and LISA are expected to form a network capable of detecting millihertz GWs emitted by the mergers of massive black hole binaries(MBHBs).In this work,we investigate the potential of GW standard sirens from the Taiji-TianQin-LISA network in constraining cosmological *** the optimistic scenario in which electromagnetic(EM)counterparts can be detected,we predict the number of detectable bright sirens based on three different MBHB population models,i.e.,popⅢ,Q3d,and *** results show that the TaijiTianQin-LISA network alone could achieve a constraint precision of 0.9%for the Hubble constant,meeting the standard of precision ***,the Taiji-TianQin-LISA network could effectively break the cosmological parameter degeneracies generated by the CMB data,particularly in the dynamical dark energy *** combined with the CMB data,the joint CMB+Taiji-TianQin-LISA data offerσ(w)=0.036 in the wCDM model,which is close to the latest constraint result obtained from the CMB+SN *** also consider a conservative scenario in which EM counterparts are not *** to the precise sky localizations of MBHBs by the Taiji-TianQin-LISA network,the constraint precision of the Hubble constant is expected to reach 1.2%.In conclusion,the GW standard sirens from the Taiji-TianQin-LISA network will play a critical role in helping solve the Hubble tension and shedding light on the nature of dark energy.
