Impact of COVID-19-related lifestyle changes on diabetic macular edema

AIM:To assess diabetic macular edema(DME)progression during the early phases of the COVID-19 pandemic,when severe societal restrictions raised the concern of possible deterioration of health in patients with systemic conditions,particularly those requiring frequent office ***:This is a multicenter retrospective chart review of 370 patients(724 eyes)with an established diagnosis of DME seen on 3 separate visits between January 2019 and July *** 1 was January 2019 to February 2020(considered pre-COVID-19),period 2 was March 2020 to December 2020(considered the height of the pandemic;highest level of pandemic-related clinical and societal regulations)and period 3 was January 2021 to July 2021(re-adjustment to the new“pandemic norms”).Main outcome measures included visual acuity,body mass index(BMI),blood pressure(BP),hemoglobin A1c(HbA1c),macular thickness,patient adherence to scheduled ophthalmology visits,and DME treatment(s)received at each *** facilitate measurement of macular thickness,each macula was divided into 9 Early Treatment Diabetic Retinopathy Study(ETDRS)-defined macular sectors as measured by OCT ***:There was no change of BMI,systolic BP,and diastolic BP between any of the time ***1c showed a very small increase from period 1(7.6%)to period 2(7.8%,P=0.015)and decreased back to 7.6%at period 3(P=0.12).Macular thickness decreased for 100%of macular *** central macular thickness decreased across all 3 periods from 329.5 to 316.6μm(P=0.0045).After analysis of multiple variables including HbA1c,BMI,adherence to scheduled appointments,different clinic centers,and treatment interventions,there was no easily identifiable subgroup of patients that experienced the increase in ***:DME doesn’t worsen during the COVID-19 pandemic,instead sustaining a very small but statistically significant *** identifying a mechanism behind our findings is beyond the scope of this study,potential explanat

AIM： To investigate the variation in the central lamina cribrosa thickness（cLCT）, and the central anterior lamina cribrosa surface depth（cALCSD）, as well as the central prelaminar tissue thickness（cPLTT） related to age in healthy Chinese ***： A total of 96 eyes from 96 Chinese healthy subjects were recruited. According to age, the 96 cases were divided into three groups： the young group（YG, 18-39 y）, middle-age group（MG, 40-59 y） and older-age group（OG, 60 y and above）. Lamina cribrosa images were obtained from all participants using radial linear protocol by enhanced depth imaging spectral-domain optical coherence tomography. The cLCT, cALCSD and cPLTT were calculated from the average value of the lamina cribrosa thickness, anterior lamina cribrosa surface depth and prelaminar tissue thickness in the optic nerve head（ONH） centre point and paracentral points（150 μm from the centre point in the horizontal and vertical directions）. RESULTS： For the total subjects, the mean cLCT, c ALCSD and cPLTT were 235.18±41.27, 358.02±93.80 and 182.02±92.11 μm, respectively. No statistically significant differences in cLCT, cALCSD or cPLTT were found between gender and different eyes（P=0.27-0.92）. The cLCT of the OG was the thickest among the three groups, while the c PLTT of the YG was the thickest among the three groups（P〈0.05）. Age was positively correlated with cLCT（r=0.42, P〈0.001）, and negatively correlated with cPLTT（r=-0.24, P=0.02）. No significant correlation was found between the age and cALCSD（r=-0.06, P=0.55）. And no correlation has been found between axial length and cLCT, cALCSD and c PLTT（P=0.11-0.81）.CONCLUSION： The impact of age on the cLCT and the cPLLTT should be taken into account when analysing glaucoma and other diseases related to lamina cribrosa.

Potential therapeutic effects of pigment epithelium-derived factor for treatment of diabetic retinopathy

Diabetic retinopathy (DR), a major micro-vascular complication of diabetes, has emerged as a leading cause of visual impairment and blindness among working adults in the worldwide. The pathobiology of DR involves multiple molecular pathways and is characterized chronic neurovascular degeneration. Current approaches to prevent or to treat DR are still far from satisfactory. Therefore, it is important to develop new therapeutic strategies for the prevention and treatment to DR. Pigment epithelium-derived factor (PEDF), a 50-kDa secreted glycoprotein, has been described as a multi-functional protein. Some emerging evidences indicate that PEDF are able to target multiple pathways exerting neurotropic, neuroprotective, anti-angiogenic, antivasopermeability, anti-inflammation, anti-thrombogenic and anti-oxidative effects in DR. In this review, we addressed the functions of PEDF in different pathways, which could lead to potential therapeutics on the treatment to DR.

Visual quality assessment after presbyopic laser in-situ keratomileusis

AIM： To assess visual quality after presbyopia correction using an aspheric ablation profile and a micro-monovision protocol. METHODS： This is a retrospective interventional study. Fifty-four eyes of 27 patients （mean age, 50.2±7.5y） who underwent presbyopia correction with an aspheric micro-monovision protocol were enrolled. The values of modulation transfer function （MTF） cutoff frequency, Strehl ratio, objective scattering index （OSI） and accommodation range were quantitatively assessed using the HD analyzer. Preoperative and postoperative contrast sensitivity （CS） at far （2.5 m） and near （40 cm） distance and higher-order aberrations （HOAs） were analyzed. Subjective visual satisfaction was evaluated by self-reported questionnaire regarding optical visual symptoms. RESULTS： One year after presbyopia correction, no significant differences were found in the MTF cutoff frequency, Strehl ratio and OSI, however, the lID analyzer accommodation range significantly differed postoperatively （P=0.004）. Postoperative CS at 12 and 18 cpd at near showed statistically significant improvement （P=0.020 and 0.008, respectively）. Visual performance by self-reported questionnaire revealed satisfactory results in terms of subjective visual quality improvement. CONCLUSION： Objective optical quality parameters show good visual outcomes. Subjective visual quality assessed by self-reported questionnaire in the presbyopia correction group show satisfactory results.

Accuracy of segmented measurement of axial length in ultra-high myopia filled with silicone oil using immersion B-scan ultrasonography

AIM:To evaluate the accuracy of segmented measurement of axial length(AL)in high myopia filled with silicone oil by immersion B-scan ultrasonography(immersion B-scan).METHODS:From June 2016 to June 2020,a total of 67 ultra-high myopia inpatients(67 eyes)who underwent silicone oil removal combined with cataract extraction and intraocular lens(IOL)implantation were retrospectively *** preoperative axial length(AL)of 31 patients with severe cataract were segmented measured using immersion B-scan(B-scan group)and another 36 patients with mild or moderate cataract were measured using IOLMaster 500(IOLMaster group).The post-operative ALs in two groups were both measured using IOLMaster *** IOL power was calculated with Haigis *** differences in ALs between pre-and post-surgery,as well as the postoperative refractive spherical equivalent,absolute refractive error,the prediction deviation of postoperative refraction and best corrected visual acuity(BCVA)were ***:The pre-and post-operative ALs were 30.46±1.63 mm(range 28.09-33.51 mm)and 30.42±1.70 mm(range 28.03-33.90 mm)in B-scan group(t=0.644,P=0.542)and 30.51±1.21 mm(range 28.03-33.90 mm)and 30.43±1.27 mm(range 28.54-33.50 mm)in IOLMaster group(t=1.843,P=0.074),*** months after surgery,BCVA were 0.45±0.13(range 0.3-0.9)and 0.44±0.20(range 0.2-1.0)in B-scan and IOLMaster group respectively(t=0.086,P=0.932).There was no significant difference of the postoperative spherical equivalent(-3.11±0.65 D vs-3.21±0.51 D,t=0.671,P=0.505)and the absolute refractive error(0.589±0.340 vs 0.470±0.245 D,t=1.615,P=0.112)between two *** B-scan group,absolute refractive error within±0.50 D was found in 18 eyes(58.1%),within±1.00 D in 26 eyes(83.9%),and within±1.50 D in 31 eyes(100%).In IOLMaster group,absolute refractive error within±0.50 D was found in 23 eyes(63.9%),within±1.00 D in 34 eyes(94.4%),and within±1.50 D in 36 eyes(Z=0.757,P=0.449).CONCLUSION:The segmented measurement of ALs

Evaluation of regional bulbar redness using an imagebased objective method

AIM: To develop an image-based objective method to precisely evaluate regional ocular bulbar injection. · METHODS: Six healthy adult volunteers were photographed in four orientations(superior,inferior,nasal and temporal sides) with and without stimulating eye drops. Six line segments(covering 30°) were drawn 4mm away from the limbus on each image using ImageJ software. The graph peaks,which were derived from the areas under the line segments and corresponded to the cross-sectional grey-level of the vessels,were analyzed to obtain peak area,peak height/width(PH/PW),and peak numbers. Different-sized areas were selected to calculate the pixels based on the edge-detection algorithm. Also,conjunctival and superficial scleral vessels were analyzed separately. ·RESULTS: This method had a smaller coefficient of variation,especially for PH/PW,in all four orientations. Hyperaemia parameters changed the least after challenging in the superior region. Moreover,95% of the PH/PW ratios were greater than 0.87 in conjunctival vessels and less than 1.00 in superficial scleral vessels. PH/PW significantly increased in conjunctival vessels and changed less in superficial scleral vessels. ·CONCLUSION: A new method of objectively assessing bulbar injection based on ocular surface images was developed. This method can be used to quantify ocular regional injection and to distinguish the superficial scleral and conjunctival vessels.

Bacterial spectrum and resistance patterns in corneal infections at a Tertiary Eye Care center in South China

AIM： To investigate the spectrum and antibiotic susceptibility of bacteria isolated from patients with suspected corneal infections in Zhongshan Ophthalmic center in South China over the past four years retrospectively. METHODS： Totally 1943 corneal scrapes from patients with corneal infections from 2010 to 2013 were cultured and processed using standard microbiological procedures to identify bacterial isolates. Simultaneously, the bacterial isolates were tested for antibiotic susceptibility to 8 antibiotics （ceftazidime, cefuroxim, cefazolin, levofloxacin, ofloxacin, neomycin, tobramycin, chloramphenicol） using the Kirby-Bauer disc diffusion technique. RESULTS： Of the total 1943 scrapes, 397 （20.43%） were culture-positive, of which 294 （74.06%） were gram- positive （GP） and 103 （25.94%） were gram-negative （GN） bacteria. Of the GP organisms, the most prevalent genera were Staphylococcus spp. （56.17%, n=223）, Kocuria spp. （5.29%, n=21） and Micrococcus spp. （1.26%, n=5）. On the other hand, the most prevalent genera were Pseudomonas spp. （12.85%, n=51）, Burkholderia spp. （2.02%, n=8） and Acinetobacter spp. （1.51%, n =6） for the GN organisms. Among five antibiotics that have eye drop products, the resistant to neomycin of GP （7.82%, 95% Ch 4.72%-10.92%） and GN isolates （9.71%, 95% Ch 4.01%-15.41% ） was lowest, while the resistant to chloramphenicol was highest （GP： 34.35%, 95% Ch 28.92%-39.78%; GN： 60.19%, 95% Ch 50.74%-69.64%）. CONCLUSION： Staphylococcus spp. was the most common bacterial pathogens isolated from patients with corneal infections in this setting. High percentages of GP and GN bacteria were mostly susceptible to neomycin and highly resistant to chloramphenicol.

Collagen crosslinking with photoactivated riboflavin in advanced infectious keratitis with corneal melting: Electrophysiological Study

AIM: To assess the effect of photoactivated chromophore for keratitis crosslinking(PACK-CXL) in case of severe keratitis with melting on the electrophysiological function of the retina and the optic ***: The study included 32 eyes of 32 patients with smear positive severe infectious keratitis with corneal melting. The patients were randomly divided into two groups. Group I(control group) included 16 eyes received systemic and topical antimicrobial drugs guarded by culture and sensitivity test. Group II underwent CXL and then continued their antimicrobial treatment. Full field electroretinogram(ERG) and flash visual evoked potential(VEP) were done for each patient in both groups basically and then 1 wk, 1 and 3 mo post-treatment to assess the changes in the electrophysiological function of the retina and optic ***: Healing of 10 eyes in group I in comparison to 14 eyes in group II was recorded. The mean duration of healing was 36.56±5.21 d in group I vs 20.2±4.4 d in group II(P<0.005). In group II, ERG showed an insignificant reduction of all parameters of ERG and VEP after CXL. The amplitude of scotopic rod response, oscillatory potential amplitude, flicker amplitude and photopic cone response were insignificantly decreased(P=0.4, 0.8, 0.1, and 0.3 respectively). There were insignificant prolongation of latencies of scotopic rod, oscillatory potential, flicker and photopic cone response(P=0.2, 0.7, 0.5 and 0.1). There was slight delay in latency of VEP without a significant reduction in ***: CXL is an effective technique in treatment of severe infectious keratitis with melting as it halts the melting process with acceptable safety on the retinal and optic nerve function.

Intravitreal anti-VEGF agents, oral glucocorticoids, and laser photocoagulation combination therapy for macular edema secondary to retinal vein occlusion: preliminary report

AIM： To evaluate the efficacy and safety of combined anti-vascular endothelial growth factor （VEGF） agents, oral glucocorticoid, and laser photocoagulation therapy for macular edema （ME） secondary to retinal vein occlusion （RVO）. METHODS： This study included 16 eyes of 16 patients with RVO-associated ME. Patients were initially treated with oral prednisone and an intravitreal anti-VEGF agent. Two weeks later, patients underwent standard laser photocoagulation. Best-corrected visual acuity （BCVA）, central retinal thickness （CRT）, and retinal vessel oxygenation were examined over 12mo. RESULTS： Patients received 1.43＋0.81 anti-VEGF injections. Mean baseline and 12-month IogMAR BCVA were 0.96±0.51 （20/178） and 0.31±0.88 （20/40）, respectively, in eyes with central retinal vein occlusion （CRVO）（P〈0.00）, and 1.02±0.45 （201209） and 0.60±0.49 （20/80）, respectively, in eyes with branch retinal vein occlusion （BRVO）（P〈0.00）. At 12mo, CRT had significantly decreased in eyes with CRVO （P〈0.00） and BRVO （P〈0.00）. Venous oxygen saturation had significantly increased in eyes with CRVO （P〈0.00） and BRVO （P〈0.00）. No examined parameters were significantly different between the 2 RVO groups. No serious adverse effects occurred. CONCLUSION： Anti-VEGF, glucocorticoid, and photocoagulation combination therapy improves visual outcome, prolongs therapeutic effect, and reduces the number of intravitreal injections in eyes with RVO- associated ME.

Effect of lens constants optimization on the accuracy of intraocular lens power calculation formulas for highly myopic eyes

AIM: To evaluate the effect of different lens constant optimization methods on the accuracy of intraocular lens(IOL) power calculation formulas for highly myopic ***: This study comprised 108 eyes of 94 consecutive patients with axial length(AL) over 26 mm undergoing phacoemulsification and implantation of a Rayner(Hove, UK) 920H IOL. Formulas were evaluated using the following lens constants: manufacturer’s lens constant, User Group for Laser Interference Biometry(ULIB) constant, and optimized constant for long eyes. Results were compared with Barrett Universal II formula, original Wang-Koch AL adjustment method, and modified Wang-Koch AL adjustment method. The outcomes assessed were mean absolute error(MAE) and percentage of eyes with IOL prediction errors within ±0.25, ±0.50, and ±1.0 diopter(D). The nonparametric method, Friedman test, was used to compare MAE performance among ***: Optimized constants could significantly reduce the MAE of SRK/T, Hoffer Q, and Holladay 1 formulas compared with manufacturer’s lens constant, whereas the percentage of eyes with IOL prediction errors within ±0.25, ±0.50, and ±1.0 D had no statistically significant differences. Optimized lens constant for long eyes alone showed non-significant refractive advantages over the ULIB constant. Barrett Universal II formula and formulas with AL adjustment showed significantly higher accuracy in highly myopic eyes(P<0.001). CONCLUSION: Lens constant optimization for the subset of long eyes reduces the refractive error only to a limited extent for highly myopic eyes.
