Endoscopic and non-endoscopic approaches for the management of radiation-induced rectal bleeding

Pelvic radiation is a commonly utilized treatment for malignancy of the genitourinary and lower gastrointestinal tract. Radiation proctitis and the resultant clinical picture varies from asymptomatic to potentially life threatening. Similarly, treatment options also vary greatly, from medical therapy to surgical intervention. Commonly utilized medical therapy includes sucralfate enemas, antibiotics, 5-aminosalicylic acid derivatives, probiotics, antioxidants, short-chain fatty acids, formalin instillation and fractionated hyperbaric oxygen. More invasive treatments include endoscopic-based, focally ablative interventions such as dilation, heater and bipolar cautery, neodymium/yttrium aluminum garnet argon laser, radiofrequency ablation or argon plasma coagulation. Despite its relatively common frequency, there is a dearth of existing literature reporting headto-head comparisons of the various treatment options via a randomized controlled approach. The purpose of our review was to present the reader a consolidation of the existing evidence-based literature with the goal of highlighting the comparative effectiveness and risks of the various treatment approaches. Finally, we outline a pragmatic approach to the treatment of radiation proctitis. In light of the lack of randomized data, our goal is to pursue as least invasive an approach as possible, with escalation of care tailored to the severity of the patient's symptoms. For those cases that are clinicallyasymptomatic or only mildly symptomatic, observation or medical management can be considered. Once a patient fails such management or symptoms become more severe, invasive procedures such as endoscopically based focal ablation or surgical intervention can be considered. Although not all recommendations are supported by level I evidence, reported case series and single-institutional studies in the literature suggest that successful treatment with cessation of symptoms can be obtained in the majority of cases.

AIM:To examine whether visceral adipose tissue(VAT)serves as a risk factor for colorectal adenoma-early colorectal cancer(CRC)***:A retrospective case-control study was conducted with 153 patients with stageⅠCRC,age/sex-matched 554 patients with colorectal adenoma and557 normal *** subjects underwent various laboratory tests,abdominal fat computed tomography(CT),and *** was defined as an intraabdominal adipose tissue area measured by CT *** tissue area was measured at the level of the umbilicus from CT *** used the lowest quartile of VAT and subcutaneous adipose tissue area as a reference ***:The body mass index(BMI),total cholesterol,fasting glucose and VAT areas were significantly different among normal,adenoma and CRC *** VAT area was 120.6±49.0 cm2in normal controls,130.6±58.4 cm2in adenoma group and 117.6±51.6cm2in CRC group(P=0.002).In univariate analysis,increased BMI was a risk factor for CRC compared to control(P=0.025).However,VAT area was not a risk factor for CRC compared to *** multivariate analysis that adjusted for smoking,alcohol consumption and subcutaneous adipose tissue area,VAT area was inversely related to CRC,compared to the adenoma(OR=0.53,95%CI:0.31-0.92,highest quartile vs lowest quartile).CONCLUSION:Our study shows that visceral obesity is not a risk factor for early *** obesity might influence the normal-adenoma sequence but not the adenoma-early carcinoma sequence.

In the fields of tissue engineering and controlled drug release, electro-spun fibers are often required to have structural characteristics such as high porosity and large specific surface area. The traditional scanning electron microscope can observe the microscopic appearance of the sample clearly, but it damage to the polymer electro-spun fiber, and the detection takes a long time. In view of this, we have tested a polarization method to quickly distinguish different morphological features of the samples, such as smooth surface, microporous, and beaded microspheres using the depolarization parameter MMDD-△,which is obtained by the Mueller matrix polar decomposition. The preliminary results show that this method is simple, fast, and potentially capable of non-destructive evaluation of the microstructure properties of the object surface.

Investigation of thermostability will lead the groundbreaking of unraveling the mechanism of influence of ion-doping on the properties of calcium *** this work,octacalcium phosphate(OCP),a metastable precursor of biological apatite,was used as a stability model for doping ions(Fe^(3+)and Sr^(2+))with different ionic charges and *** treated under hot air at different temperatures(110-200◦C),the phase,morphology,structure,physicochemical properties,protein affinity,ions release,and cytological responses of the ion-doped OCPs were investigated *** results showed that the collapse of OCP crystals gradually occurred,accompanying with the dehydration of hydrated layers and the disintegration of plate-like crystals as the temperature *** collapsed crystals still retained the typical properties of OCP and the potential of conversion into *** to the undoped OCP,Fe-OCP,and Sr-OCP had lower and higher thermostability respectively,leading to different material surface properties and ions *** adjusted thermostability of Fe-OCP and Sr-OCP significantly enhanced the adsorption of proteins(BSA and LSZ)and the cytological behavior(adhesion,spreading,proliferation,and osteogenic differentiation)of bone marrow mesenchymal stem cells to a varying extent under the synergistic effects of corresponding surface characteristics and early active ions *** work paves the way for understanding the modification mechanism of calcium phosphates utilizing ion doping strategy and developing bioactive OCP-based materials for tissue repair.
