assessment of Satisfaction in the Reception and Emergency Department of a health center

Introduction The main objective of any healthcare establishment must be to ensure the quality of patient care and customer satisfaction. It is necessary to regularly assess patient satisfaction. The aim of this study was to assess the level of satisfaction of customers aged over 18 years attending the emergency department of the health center. Methodology This was a descriptive and analytical cross-sectional study of patients aged 18 years and over, who attended the Samu Municipal emergency department between 02 and 30 May 2023. The satisfaction index was determined using the adapted 2009 SAPHORA-MCO questionnaire and the Likert satisfaction scale. Results A total of 400 patients were surveyed. The average age was 35 years, with a standard deviation of 14.7. Of those surveyed, 51% were women, 87% were educated, 50% lived in Grand Yoff and 59.5% were unemployed. Satisfaction levels linked to perception of the cost of care (72%), waiting time (64.3%), information given to patients (69.1%) and pain management (74 .5%) are fair. On the other hand, the levels of satisfaction linked to administrative procedures (82.5%), staff attitudes towards patients (84%), staff availability (86.4%), patient privacy (89.2%), general atmosphere (87.2%), staff competence (87.3%), and the effectiveness of care (89.4%) were satisfactory. The average waiting time was 38 minutes. However, 32% of patients waited less than 30 minutes and 92% less than an hour. The satisfaction index linked to administration and reception was 72.9% and 79.85%, respectively. The satisfaction index linked to the administration and technical quality of care is equal to 85.8% and 83.7%, respectively. The overall satisfaction index is equal to 80.6%;the level of satisfaction of users of the health structure is satisfactory. Conclusion Patient satisfaction is an essential part of quality care. Patient satisfaction must be based on effective communication from the healthcare team and the creation of a patient-caregiver relationship.

Clean air actions （CAAs） in China have been linked to considerable benefits in public health.However,whether the beneficial effects of CAAs are equally distributed geographically is unknown.Using high-resolution maps of the distributions of major air pollutants (fine particulate matter[PM2.5]and ozone[O3]) and population,we aimed to track spatiotemporal changes in health impacts from,and geographic inequality embedded in,the reduced exposures to PM2.5and O3from 2013 to 2020.We used a method established by the Global Burden of Diseases Study.By analyzing the changes in loss of life expectancy（LLE） attributable to PM2.5and O3,we calculated the gain of life expectancy （GLE） to quantify the health benefits of the air-quality improvement.Finally,we assessed the geographic inequality embedded in the GLE using the Gini index （GI）.Based on risk assessments of PM2.5and O3,during the first stage of CAAs（2013 to 2017）,the mean GLE was 1.87 months.Half of the sum of the GLE was disproportionally distributed in about one quarter of the population exposed （GI 0.44）.During the second stage of CAAs（2017 to 2020）,the mean GLE increased to 3.94 months and geographic inequality decreased （GI 0.18）.According to our assessments,CAAs were enhanced,from the first to second stages,in terms of not only preventing premature mortality but also ameliorating health inequalities.The enhancements were related to increased sensitivity to the health effects of air pollution and synergic control of PM2.5and O3levels.Our findings will contribute to optimizing future CAAs.

Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in China.Methods:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at baseline.An interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on quartiles.Cox regression was used to analyze the association between PA and CKD risk.Results:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)cases.Total PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest quartile.Similar results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN risk.Increased nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,respectively.Conclusion:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.

Social Network Analysis of COVID-19 Research and the Changing International Collaboration Structure

Research in Information Science and interdisciplinary areas suggested the formation of a growing network of international research collaboration.The massive transmission of COVID-19 worldwide especially after the identification of the Omicron variant could fundamentally alter the factors shaping the network's development.This study employs network analysis methods to analyze the structure of the COVID-19 research collaboration from 2020 to 2022,using two major academic publication databases and the VOSviewer software.A novel temporal view is added by examining the dynamic changes of the network,and a fractional counting method is adopted as methodological improvements to previous research.Analysis reveals that the COVID-19 research network structure has undergone substantial changes over time,as collaborating countries and regions form and re-form new clusters.Transformations in the network can be partly explained by key developments in the pandemic and other social-political events.China as one of the largest pivots in the network formed a relatively distinct cluster,with potential to develop a larger Asia-Pacific collaboration cluster based on its research impact.

Evaluating the performance and influencing factors of three portable black carbon monitors for field measurement

Black carbon(BC)is associated with adverse human health and climate change.Mapping BC spatial distribution imperatively requires low-cost and portable devices.Several portable BC monitors are commercially available,but their accuracy and reliability are not always satisfactory during continuous field observation.This study evaluated three models of portable black carbon monitors,C12,MA350 and DST,and investigates the factors that affect their performance.The monitors were tested in urban Beijing,where portable devices running for one month alongside a regular-size reference aethalometer AE33.The study considers several factors that could influence the monitors'performance,including ambient weather,aerosol composition,loading artifacts,and built-in algorithms.The results show that MA350and DST present considerable discrepancies to the reference instrument,mainly occurring at lower concentrations(0-500 ng/m^(3))and higher concentrations(2500-8000 ng/m^(3)),respectively.These discrepancies were likely caused by the anomalous noise of MA350 and the loading artifacts of DST.The study also suggests that the ambient environment has limited influence on the monitors'performance,but loading artifacts and accompanying compensation algorithms can result in unrealistic data.Based on the evaluation,the study suggests that C12 is the best choice for unsupervised field measurement,DST should be used in scenarios where frequent maintenance is available,and MA350 is suitable for research purposes with post-processing applicable.The study highlights the importance of assigning portable BC monitors to appropriate applications and the need for optimized real-time compensation algorithms.

Development and validation of a novel non-invasive test for diagnosing nonalcoholic fatty liver disease in Chinese children

With the exploding prevalence of obesity,many children are at risk of developing nonalcoholic fatty liver disease.Using anthropometric and laboratory parameters,our study aimed to develop a model to quantitatively evaluate liver fat content(LFC)in children with obesity.Methods A well-characterized cohort of 181 children between 5 and 16 years of age were recruited to the study in the Endocrinology Department as the derivation cohort.The external validation cohort comprised 77 children.The assessment of liver fat content was performed using proton magnetic resonance spectroscopy.Anthropometry and laboratory metrics were measured in all subjects.B-ultrasound examination was carried out in the external validation cohort.The Kruskal-Wallis test,Spearman bivariate correlation analyses,univariable linear regressions and multivariable linear regression were used to build the optimal predictive model.Results The model was based on indicators including alanine aminotransferase,homeostasis model assessment of insulin resistance,triglycerides,waist circumference and Tanner stage.The adjusted R^(2)of the model was 0.589,which presented high sensitivity and specificity both in internal[sensitivity of 0.824,specificity of 0.900,area under curve(AUC)of 0.900 with a 95%confidence interval:0.783–1.000]and external validation(sensitivity of 0.918 and specificity of 0.821,AUC of 0.901 with a 95%confidence interval:0.818–0.984).Conclusions Our model based on five clinical indicators was simple,non-invasive,and inexpensive;it had high sensitivity and specificity in predicting LFC in children.Thus,it may be useful for identifying children with obesity who are at risk for developing nonalcoholic fatty liver disease.

Environmental health research aims to identify environmental conditions suitable for the healthy living and reproduction of human beings.Through the interdisciplinary research bridging environmental sciences and health/medical sciences,the impacts of physical,chemical,and biological environmental factors on human health are investigated.This includes identifying environmental factors detrimental to human health,evaluating human exposure characteristics to environmental factors,clarifying causal relationships between environmental exposure and health effects,analyzing the underlying biochemical mechanisms,linking environmental factors to the onset and progression of diseases,establishing exposure-response relationships,and determining effect thresholds.Ultimately,the results of environmental health research can serve as a scientific basis for formulating environmental management strategies and guiding prevention and intervention measures at both the public and individual levels.This paper summarizes the recent advances and future perspectives of environmental health research in China,as reported by a group of Chinese scientists who recently attended a workshop in Hainan,China.While it is not intended to provide a comprehensive review of this expansive field,it offers a glimpse into the significant progress made in understanding the health impacts of environmental factors over the past decade.Looking ahead,it is imperative not only to sustain efforts in studying the health effects of traditional environmental pollution,but also to prioritize research on the health impacts of emerging pollutants and climate change.

Time flies,in the blink of an eye,it is already 2023.The journal Biomedical and Environmental Sciences(BES)has been established for thirty-five years.Today,I am honored to participate in the BES’s 35th anniversary symposium,organized by the Chinese center for Disease Control and Prevention(China CDC).Seeing Director Hongbing Shen,Deputy Director Xiaoming Shi,along colleagues from the center’s affiliated units and departments,as well as fellow researchers,we have come together for this symposium under the theme of“Looking back and looking forward”,I am not only deeply appreciative and thankful,but also envisioning a bright future for BES in the next 35 years.I would like to share my thoughts and hopes under the theme of“Review and Development”.

The underappreciated role of fugitive VOCs in ozone formation and health risk assessment emitted from seven typical industries in China

Fugitive emission from industrial sources may result in ozone formation and health risk,while the exact contribution of this source remains incompletely understood.In this study,emission characteristics,ozone formation potential(OFP)and health risk of fugitive VOCs in7 representative industries were investigated.Chemical material industry was the dominant contributor to VOCs of fugitive emission in comparison with other industries.The OFP of VOCs from fugitive emission was in the range of 1.45×10^(3)-3.98×10^(5)μg/m^(3),with a higher value than that of organized emission in seven industries except for the coking industry and the chemical material industry,suggesting that fugitive VOCs should be taken into account while developing control strategies.Acetaldehyde,m,p-xylene,n-nonane,ethylene,vinyl chloridethe and other high OFP-contributing species were the major reactive species that should be targeted.health risk assessment investigated non-cancer and cancer risks of fugitive VOCs in 7 industries were all above safe level(HR>1 and LCR>1×10^(-4)),posing remarkable health threats to human health.OVOCs were the main contributor to non-cancer risk,while halohydrocarbons and aromatics contributed most to cancer risks,posing remarkable health threat on human health.Our findings highlighted the contribution of fugitive VOCs on ozone formation and health risk was underestimated,indicating which should be considered in emission control strategies of industrial sources.

Laboratory and field evaluation of a low-cost optical particle sizer

Low-cost sensors are widely used to collect high-spatial-resolution particulate matter data that traditional reference monitoring devices cannot.In addition to the mass concentration,the number concentration and size distribution are also fundamental in determining the origin and hazard level of particulate pollution.Therefore,low-cost optical sensors have been improved to establish optical particle sizers(OPSs).In this study,a low-cost OPS,the Nova SDS029,is introduced,and it is evaluated in comparison to two reference instruments—the GRIMM 11-D and the TSI 3330.We first tested the sizing accuracy using polystyrene latex spheres.Then,we assessed the mass and number size distribution accuracy in three application scenarios:indoor smoking,ambient air quality,and mobile monitoring.The evaluations suggest that the low-cost SDS029 rivals research-grade optical sizers in many aspects.For example,(1)the particle diameters obtained with the SDS029 are close to the reference instruments(usually<10%)in the 0.3-5μm range;(2)the number of particles and mass concentration are highly correlated(r≥0.99)with the values obtained with the reference instruments;and(3)the SDS029 slightly underestimates the number concentration,but the derived PM_(2.5)values are closer to monitoring station than the reference instruments.The successful application of the SDS029 in multiple scenarios suggests that a plausible particle size distribution can be obtained in an easy and cost-efficient way.We believe that low-cost OPSs will increasingly be used to map the sources and risk levels of particles at the city scale.
