Introduction: Childbirth on a scarred uterus is a major issue for health centers, especially peripheral, due to the major obstetric risks it presents. The objectives were to evaluate the frequency, route of delivery and maternal-fetal prognosis of this type of delivery at csref of Kolondiéba. Materials and Methods: This was a retrospective cross-sectional study for one year (1 January 2023-31 December 2023). All patients admitted to the maternity ward of the center with at least one uterine scar and treated in the center were included. We extracted data from partograms, OR records, birth records and obstetric records. Input was done on Excel 2010 and analysis on SPSS.23. Results: The frequency of scarring uterus was 16.8% (217/1285 births). The average age was 27. Pauciparous were most represented (59%). Patients were received from community health centers (44.7%). Prenatal consultation sessions (1 - 3 sessions) were performed at (64.9%). Uterine scars were obstetric in (99%). The cesarean section was performed immediately in (59.4%), it was prophylactic in 17%. The uterine test was attempted in (25.34%) with (69%) success. We recorded 3.6% uterine ruptures, 8.7% postoperative complications, 5.5% stillbirths and one maternal death (0.46%). Conclusion: Births on a scarred uterus are frequent and associated with a high rate of complications.

The deliberate engineering of the d-band center of metal site represents an effective strategy to boost the intrinsic electrocatalytic performance toward the oxygen reduction reaction(ORR).Herein,following a heterointerfaceinduced orbital coupling rationale,we report a judicious design of an efficient ORR electrocatalyst consisting of Fe_(3)O_(4)/CeO_(2)hetero-nanoparticles in-situ encased into N-doped carbon nanofibers(abbreviated as Fe_(3)O_(4)/CeO_(2)@N-CNFs hereafter).The theoretic calculations uncover that the Fe_(3)O_(4)/CeO_(2)heterointerface-triggered orbital coupling can cause the down shift of the d-band center positions of Fe sites,which leads to the weakened chemisorption of oxygenated groups and lowered energy barrier for the potential-determining step,ultimately dramatically boosting the ORR intrinsic *** a consequence,the well-designed Fe_(3)O_(4)/CeO_(2)@N-CNFs display admirable ORR activity with a half-wave potential of 0.84 V and outstanding structural/electrochemical stability in an alkaline electrolyte,surpassing the commercial Pt/C benchmark and a majority of recently reported Fe_(3)O_(4)-based *** encouragingly,the Fe_(3)O_(4)/CeO_(2)@N-CNFsincorporated Zn-air battery outperforms the Pt/C-assembled counterpart with higher power density,larger energy density,and excellent cycling stability,serving as a competent candidate for ORR-involved renewable energy *** study offers an innovative approach for the rational manipulation of the d-band center and interfacial electron behavior of active sites toward the optimization of electrocatalytic performance.

The Prevalence, Patterns, and Trend of Antimicrobial Resistance among Patients at Kumi Orthopaedic center: A Retrospective Study

Background: Antimicrobial Resistance (AMR) has been recognized as a global public health problem of utmost importance that needs to be tackled. Low-income countries such as Uganda have the greatest burden of severe and life-threatening infections and are most likely to suffer more from the spread of untreatable resistant bacteria. Despite the generally increasing trends in AMR, there is hardly any documented data on AMR in orthopedic care. Methods: We analyzed data from the pathogenic isolates from pus and tissue from the laboratory from 2019 to 2023. We calculated the proportions of isolates resistant to commonly used antimicrobial classes. We used the chi-square test for trends to evaluate changes in AMR across the study period. Results: Out of the 286 isolates, 104 (43.5%) were from pus, 112 (46.9%) were from tissue samples and the remaining 9.6% were from different fluid aspirates like synovial fluid. Most of the isolates were from males (64.2%) and aged between 19 and 45 years (46.2%). The distribution of whether the bacteria were gram-positive or negative was relatively equal with 114 (45.2%) being gram-negative and 120 (47.6%) gram-positive. Generally, there was increased AMR across all antibiotics from 2019 to 2021 e.g. for Imipinem, it went from 0% in 2019 to 70.8% in 2021 and for Amoxyclav, it went from 10% in 2019 to 93.2% in 2021. There was a general decline in AMR noted from 2021 to 2023. However, currently, the highest resistance is noted in ceftriaxone (80.0%) and the lowest in Imipinem (11.76%). Conclusion: There is a general decreasing trend in AMR, most probably due to increasing policies governing the use of antibiotics. However, there is still high resistance to commonly used and affordable antibiotics. Continuous monitoring of AMR is still recommended to reduce the AMR problem in Uganda at large through public health policy and planning.

The technology of solid-state lighting has developed for decades in various ***,as an element part,determines the application domain of lighting *** instance,blue and redemitting phosphors are required in the process of plant supplementing light,arrow-band emitting phosphors are applied to backlight displays,*** this work,a Bi3+-activated blue phosphor was obtained in a symmetrical and co mpact crystal structure of Gd3Sb07（GSO）.Then,the co-doping strategy of alkali metal ions（Li+,Na+,and K+） was used to optimize the *** result shows that the photoluminescence intensity is increased by 2.1 times and 1.3 times respectively by introducing Li+and K+*** only that,it also achieves narrow-band emitting with the full width of half-maximum（FWHM） reaching 42 nm through Na+doping,and its excitation peak position also shifts from 322 to 375 nm,which can be well excited by near-ultraviolet（NUV） light emitting diode（LED） chips（365 nm）.Meanwhile,the electroluminescence spectrum of GSO:0.6 mol%Bi3+,3 wt%Na+matches up to 93.39% of the blue part of the absorption spectrum of chlorophyll *** summary,the Bi3+-activated blue phosphor reported in this work can synchronously meet the requirements of plant light replenishment and field emission displays.

To the Editor:We read with great interest the recent article by Balci et al.[1].In the study, the authors mentioned that their mixed reality (MR)technology has already successfully been applied in open and laparoscopic living donor hepatectomy and in complex laparoscopic liver resections. During such operations, the MR models of the liver are pulled out and superimposed onto the actual organ. Surgeons canverifytherelationshipbetweentumorsandcrucialareasin ordertoenhancemarginclearanceandR0resectionandreduce the risk of injuring major blood vessels. By applying MR (including augmented reality) technology, blood vessels and tumor structures in the liver can be displayed during surgery, which permits precise navigation during complex surgical procedures. Also MR was used in surgical education, and most learners believed that 3D models were useful in planning safer surgical paths, reducing intraoperative complications and training of young surgeons [2].

BACKGROUND Endoscopic retrograde cholangiopancreatography is a challenging procedure involving bile duct *** the development of several cannulation devices,none have effectively facilitated the *** To evaluate the efficacy of a recently developed catheter for bile duct *** We retrospectively examined 342 patients who underwent initial *** compared the success rate of bile duct cannulation and the incidence of complications between the groups using existing and novel *** The overall success rates of bile duct cannulation were 98.3%and 99.1%in the existing and novel catheter groups,respectively(P=0.47).The bile duct cannulation rate using the standard technique was 73.0%and 82.1%in the existing and novel catheter groups,respectively(P=0.042).Furthermore,when catheterization was performed by expert physicians,the bile duct cannulation rate was significantly higher in the novel catheter group(81.3%)than in the existing catheter group(65.2%)(P=0.017).The incidence of difficult cannulation was also significantly lower in the novel catheter group(17.4%)than in the existing catheter group(33.0%)(P=0.019).CONCLUSION The novel catheter improved the bile duct cannulation rate using the standard technique and reduced the frequency of difficult cannulation cases,valuable tool in endoscopic retrograde cholangiopancreatography procedures performed by experts.

Recent estimates indicate that one-fifth of botanical species worldwide are considered at risk of becoming extinct in the wild. One available strategy for conserving many rare plant species is reintroduction, which holds much promise especially when carefully planned by following guidelines and when monitored long-term. We review the center for Plant Conservation Best Reintroduction Practice Guidelines and highlight important components for planning plant reintroductions. Before attempting reintroductions practitioners should justify them, should consider alternative conservation strategies, understand threats, and ensure that these threats are absent from any recipient site. Planning a reintroduction requires considering legal and logistic parameters as well as target species and recipient site *** selecting the genetic composition of founders, founder population size, and recipient site will influence establishment and population growth. Whenever possible practitioners should conduct reintroductions as experiments and publish results. To document whether populations are sustainable will require long-term monitoring for decades, therefore planning an appropriate monitoring technique for the taxon must consider current and future needs. Botanical gardens can play a leading role in developing the science and practice of plant reintroduction.

Prognostic Factors for Operated Traumatic Brain Injury at the Yaounde Emergency center

Introduction: Head injuries constitute a public health problem in Cameroon and everywhere else in the world. They represent 23% of admissions to the Yaounde emergency center (CURY), which is a center exclusively dedicated, since 2014, to emergency care in Yaounde. In the management of trauma brain injuries at CURY, several are operated on. However, to date, no evaluation of these operated patients has yet been made. Goals: The objective of this study was to highlight the prognostic factors in patients operated for TBI at CURY. Methodology: We conducted a descriptive study whose data collection was done retrospectively over 2 years (01 January 2021 to 31 December 2022) at CURY. Data was collected from the registers of operative reports. Results: We enrolled 105 medical reports of patients who were victims of TBI operated on. The male gender predominated with a sex ratio of 3/1. The average age of the patients was 37.5 ± 18.83 years. Public road accidents were the leading cause of TBI in 75.2% of cases. The means of transport of the victims were mostly non-medical 97.1%. 45.7% of patients were admitted in less than 6 hours following injury. The initial clinical evaluation found 45.8% of patients with a Glasgow Coma Score (GCS) between [14, 15], and 13.2% of patients had a GCS 8. The indications for surgery were extradural hematoma (30%), followed by acute subdural hematoma (24%). The major complication was postoperative infection (25%). The mortality rate of the series was 7.9%. Poor prognostic factors were the depth of the coma on admission, advanced age and postoperative complications. Conclusion: The results of this study suggest that most patients operated on for TBI at CURY had a favorable outcome. The poor prognostic factors were the depth of the coma on admission, advanced age, postoperative complications and comorbidities.

A comparative study of totally laparoscopic distal gastrectomy versus laparoscopic-assisted distal gastrectomy in gastric cancer patients: Short-term operative outcomes at a high-volume center

Objective： Laparoscopic gastrectomy has been established as a standard treatment for early gastric cancer, and its use is increasing recently. Compared with the conventional laparoscopy-assisted distal gastrectomy （LADG）, totally laparoscopic distal gastrectomy （TLDG） involves intracorporeal reconstruction, which can avoid the additional incision, resulting in pain reduction and early recovery. This study aimed to compare the short-term postoperative outcomes of TLDG vs. LADG in gastric cancer in a high-volume ***： A retrospective cohort study was conducted on 1,322 patients who underwent laparoscopic distal gastrectomy from June 2012 to June 2017 at the National Cancer center, Korea. LAD G was performed in the early period before July 2015, and TLDG was applied in the later period. Postoperative short-term outcomes were compared in terms of complication and clinical course between the two groups. Pain score was measured by rating the pain intensity from 0 to 10 points on postoperative day （POD） 1 and 3. Results： A total of 667 patients underwent LADG and 655 patients underwent TLDG. Clinieopathologic characteristics were not different in both groups. Intraoperative estimated blood loss （EBL） was significantly lower in the TLDG group （P〈0.001）. Postoperative pain scores were significantly lower in the TLDG group than in the LADG group on POD 1 （5.1±1.5 vs. 4.8±1.4, P=0.015）. First flatus passage after operation was significantly earlier in the TLDG group （3.4±0.8 d vs. 3.2±0.6 d, P〈0.001）. There were no differences in postoperative complications and hospital stay between the two groups. Conclusions： Based on the reported short-term postoperative outcomes, TLDG is safe and feasible as well as LADG. Moreover, compared with LADG, TLDG can reduce intraoperative EBL and postoperative pain and enhance the bowel motility in gastric cancer surgery.
