Necessity and proper way of intermediate kissing balloon dilation for culotte stenting: further insights from bench testing

Objective: The aim of this study was to investigate whether intermediate kissing balloon dilation(IKBD) is necessary during mini-culotte stenting(MCS) and how it can be properly conducted. Methods: MCS was emulated in a bifurcation model with a branch diameter difference(BDD) in three-step sizes of 0.50, 0.75 and 1.00 mm, and with intermediate balloon dilation(IBD) in three treatments of routine intermediate solo balloon dilation(rISBD), concurrent IKBD(cIKBD) or sequential IKBD. Microcomputed tomography was performed to assess stent under-expansion(SUE) around the polygon of confluence(POC), residual ostial stenosis(ROS) at the ostial side-branch(SB) and main-branch(MB) and stent cell distortion(SCD) in the bifurcation segments. Results: There were both main and interactive effects of IBD and BDD on ROS at the ostial SB and SCD in the ostial SB, but there were only main effects of IBD or BDD on SUE around the POC, ROS at the ostial MB and SCD in the ostial MB. Analysis of the main effects showed that SUE around the POC or ROS at the ostial SB was significantly different between sI KBD and rISBD and between cI KBD and rISBD. There was also a significant difference in SCD in the ostial SB between sI KBD and rI SBD and between sI KBD and cIKBD. Analysis of the interactive effects showed that ROS at the ostial SB or SCD in the ostial SB was affected by all IBD treatments in all BDD step-sizes. Moreover, increasing the BDD step-sizes significantly increased ROS at the ostial SB as treated by rISBD and SCD in the ostial SB as treated by rISBD or cIKBD. Conclusions: SIKBD was shown to be essential and superior to rISBD or cIKBD, resulting in better bifurcated stent expansion and coverage when using MCS.

Objective To evaluate the long-term prognosis of patients with ST-segment elevation myocardial infarction(STEMI)treated with different reperfusion strategies in Chinese county-level hospitals Methods A total of 2,514 patients with STEMI from 32 hospitals participated in the China Acute Myocardial Infarction registry between January 2013 and September *** success of fibrinolysis was assessed according to indirect measures of vascular *** primary outcome was 2-year *** Reperfusion therapy was used in 1,080 patients(42.9%):fibrinolysis(n=664,61.5%)and primary percutaneous coronary intervention(PCI)(n=416,38.5%).The most common reason for missing reperfusion therapy was a prehospital delay>12 h(43%).Fibrinolysis[14.5%,hazard ratio(HR):0.59,95%confidence interval(CI)0.44–0.80]and primary PCI(6.8%,HR=0.32,95%CI:0.22–0.48)were associated with lower 2-year mortality than those with no reperfusion(28.5%).Among fibrinolysistreated patients,510(76.8%)achieved successful clinical reperfusion;only 17.0%of those with failed fibrinolysis underwent rescue *** was no difference in 2-year mortality between successful fibrinolysis and primary PCI(8.8%vs.6.8%,HR=1.53,95%CI:0.85–2.73).Failed fibrinolysis predicted a similar mortality(33.1%)to no reperfusion(33.1%vs.28.5%,HR=1.30,95%CI:0.93–1.81).Conclusion In Chinese county-level hospitals,only approximately 2/5 of patients with STEMI underwent reperfusion therapy,largely due to prehospital *** 30%of patients with failed fibrinolysis and no reperfusion therapy did not survive at 2 *** improvement initiatives are warranted,especially in public health education and fast referral for mechanical revascularization in cases of failed fibrinolysis.

BACKGROUND Epidemiologic studies have explored the association between a single cardiovascular risk factor(CVRF)and resting heart rate(RHR),but the research on the relation of multiple risk factors with RHR remains *** study aimed to explore the associations between CVRFs clustering and the risk of elevated *** In this cross-sectional study,adults aged 35-75 years from 31 provinces were recruited by the China PEACE Million Persons Projects from September 2015 to August *** focused on seven risk factors:hypertension,diabetes mellitus,dyslipidemia,obesity,smoking,alcohol use,and low physical *** logistic regression was used to calculate odds ratios(OR)for elevated RHR(>80 beats/min).RESULTS Among 1,045,405 participants,the mean age was 55.67±9.86 years,and 60.4%of participants were *** OR(95%CI)for elevated RHR for the groups with 1,2,3,4 and≥5 risk factor were 1.11(1.08-1.13),1.36(1.33-1.39),1.68(1.64-1.72),2.01(1.96-2.07)and 2.58(2.50-2.67),respectively(Ptrend<0.001).The association between the CVRFs clustering number and elevated RHR was much more pronounced in young males than in other age-sex *** comprising more metabolic risk factors were associated with a higher risk of elevated RHR than those comprising more behavioral risk *** There was a significant positive association between the CVRFs clustering number and the risk of elevated RHR,particularly in young *** clusters comprising more behavioral risk factors,clusters comprising more metabolic risk factors were associated with a higher risk of elevated *** may serve as an indicator of the cumulative effect of multiple risk factors.

Objective To assess whether serum Apelin correlates with spontaneous reperfusion(SR)of the infarct-related artery(IRA)in STEMI *** Blood samples were obtained on admission and before p PCI in 280 consecutive STEMI patients,who were hospitalized in China-Japan Friendship Hospital between January 2016 and December *** chemistry,Apelin level were *** arteriography and echocardiography were performed after *** were divided into SR(TIMI 2-3,n=57)and non-SR(TIMI 0-1,n=223)groups according to TIMI *** regression analysis was performed to define the independent predictors of *** Prevalence of smoking and pathoglycemia(including diabetes,impaired glucose tolerance and impaired fasting glucose)was significantly higher in the non-SR group than in the SR group(both P<0.05).Peak values of plasma troponin-I and percent of the patients with Killip 2 ~ 3 were significantly higher in non-SR group than in SR group(P<0.05).Left ventricular systolic and diastolic functions were remarkably improved in SR group compared with non-SR *** scores were lower in the SR-group than the non-SR group(P=0.02).Serum Apelin was significantly higher in patients with SR compared to patients with non-SR([0.82±0.34] ng/ml vs [0.35±0.22] ng/ml,P=0.04).A cut off Apelin value of 0.75 ng/ml had a predictive value of 77% to identify patients with SR(sensitivity=81%).Logistic regression analysis showed that serum Apelin level is an independent predictor of SR(OR=2.18,95% CI:1.10-4.33,P=0.01)for STEMI *** Patients with SR have better cardiac systolic function and diastolic *** increased serum Apelin level is an independent and novel predictor of SR for STEMI *** of serum Apelin level is helpful to predict SR in STEMI patients.

To the Editor： A 73-year-old female was referred to our hospital for evaluation of unstable angina pectoris. She had a greater than 10-year history of hypertension. coronary angiography （CAG） was performed in another hospital in March 2017, which showed diffuse calcific stenosis （about 90%） in the proximal and mid-left anterior descending （LAD） artery, 70% stenosis in the ostial obtuse marginal branch, and no apparent stenosis in the right coronary artery. The lesion in the LAD could not be intervened because the 2.0 mm×15.0 mm Sprinter balloon （Medtronic, USA） could not be expanded and the 2.5 mm×15.0 mm NC Sprinter balloon （Medtronic, USA） could not pass through the lesion.

Background Numerous studies have developed a ＂severity score＂ or ＂risk index＂ for short-term mortality associated with coronary artery bypass grafting （CABG）. Due to the different distribution of disease types, the number of valve surgeries in the US and Europe is relatively small. Thus, a risk-scoring system for valve surgeries was developed later and used less than that for the CABG surgery. We retrospectively reviewed 5128 cases of heart valve replacement, to quantitatively assess the risk factors for hospital mortality, and establish risk models for the hospital mortality of cardiac valve replacement *** A total of 1549 cases of aortic valve replacement, 2460 cases of mitral valve replacement, and 1119 cases of combined aortic valve and mitral valve replacement that were recorded from January 2005 to December 2009 in the cardiac surgery database at Beijing Anzhen Hospital were selected for this study. The cases were randomly assigned to a indicators were selected as possible influencing factors for hospital mortality. Single-factor analysis was performed to screen these factors, and then multi-factor analysis was used to determine the risk factors for hospital mortality in the three surgeries and to establish risk *** In the multi-factor analysis, age, body surface area, etiology, cardiopulmonary bypass time, preoperative cardiothoracic ratio, cardiac functional classification, and preoperative creatinine were risk factors for aortic valve replacement. Etiology, preoperative history of heart failure, cardiopulmonary bypass time, preoperative cardiothoracic ratio, and preoperative left ventricular end systolic diameter were risk factors for mitral valve replacement. Age, body mass index, cardiopulmonary bypass time, and cardiac function classification were risk factors for combined aortic valve and mitral valve replacement. Appropriate heart rate in a failing pulmonary ventricle remains unknown, particularly in congenital heart diseasewith unique hemodynamics. A 71-year-old male with repaired tetralogy of Fallot and a pacemaker for a sinusnode dysfunction suffered from heart failure symptoms with preserved left ventricular function. Simply changingthe pacemaker’s lower rate from 60 to 75 bpm, New York heart Association classification improved from III to II,and hemodynamic parameters drastically improved. We regarded this case as informative. Appropriate heart ratecould be higher in congenital patients with failing right and non-failing left ventricles than in adults with malfunctioningLV.
