Attitudes towards pain management in hospitalized cancer patients and their influencing factors

Objective： To investigate patients＇ attitudes towards cancer pain management and analyze the factors influencing these ***： The self-developed Demographic and Disease-Related Information Questionnaires, Pain Management Barriers Questionnaire-Taiwan form（BQT）, and Pain Knowledge Questionnaire were administered to 363 pairs of hospitalized cancer patients and their caregivers from the oncology departments of 7 hospitals in Beijing, ***： The average patient score for attitudes towards pain management was 2.96±0.49. The dimension scores indicated good attitudes in three areas（scores 〈2.5）, ＂Desire to be good＂（2.22±1.04）, ＂Fatalism＂（2.08±0.81） and＂Religious fatalism＂（1.86±1.00）, and poor attitudes in six areas（scores ≥2.5）, ＂Tolerance＂（3.83±0.96）, ＂Use of analgesics as needed（p.r.n.）＂（3.73±1.01）, ＂Addiction＂（3.44±1.05）, ＂Disease progression＂（3.28±1.26）, ＂Distraction of physicians＂（3.16±1.07） and ＂Side effects＂（2.99±0.68）. Two factors were entered into the regression equation：the caregivers＇ attitudes towards cancer pain management and the patients＇ pain knowledge. These two factors explained 23.2% of the total variance in the patients＇ average scores for their attitudes towards cancer pain ***： The patients＇ attitudes towards cancer pain management were poor and could be influenced by the caregivers＇ attitudes and the patients＇ pain knowledge, and thus need to be improved.

Skin integrity breakdown and pressure ulcer formation on the heel are predictable and preventable healthcare occurrences. By minimizing surface contact pressures that can cause heel capillary occlusion, heel off-loading devices help mitigate the risk of decubitus ulceration. The purpose of this study is to investigate the efficacy of six off-loading methods commonly used in clinical settings to decrease heel contact pressure. Pressure mapping is used to evaluate contact pressure at the heel for underweight, normal weight, and overweight simulations. Based upon averages across four trials, it is shown that all devices reduce heel contact pressure when compared to no off-load- ing. The PRUventor™ boot exhibits significantly lower average pressure values than other off-load- ing strategies tested, suggesting that the PRUventor™ is the most effective off-loading device. From the analysis of the experimental data and comparison to similar data for respective off-loading alternatives, it is concluded that the PRUventor™ boot is an effective heel off-loading device for use in clinical settings.

BACKGROUND Prior studies indicate that doing breathing exercises improves physical performance and quality of life (QoL) in heart failure patients. However, these effects remain unclear and contradictory. AIM To determine the effects of machine-assisted and non-machine-assisted respiratory training on physical performance and QoL in heart failure patients. METHODS This was a systematic review and network meta-analysis study. A literature search of electronic databases was conducted for randomized controlled trials (RCTs) on heart failure. Respiratory training interventions were grouped as seven categories: IMT_Pn (inspiratory muscle training without pressure or < 10% maximal inspiratory pressure, MIP), IMT_Pl (inspiratory muscle training with low pressure, 10%-15% MIP), IMT_Pm (inspiratory muscle training with medium pressure, 30%-40% MIP), IMT_Ph (inspiratory muscle training with high pressure, 60% MIP or MIP plus aerobics), Aerobics (aerobic exercise or weight training), Qi_Ex (tai chi, yoga, and breathing exercise), and none. The four outcomes were heart rate, peak oxygen uptake (VO2 peak), 6-min walking distance test (6MWT), and Minnesota Living with Heart Failure QoL. The random-effects model, side-splitting model, and the surface under the cumulative ranking curve (SUCRA) were used to test and analyze the data. RESULTS A total of 1499 subjects from 31 RCT studies were included. IMT_Ph had the highest effect sizes for VO2 peak and 6MWT, IMT_Pm highest for QoL, and Qi_Ex highest for heart rate. Aerobics had the second highest for VO2 peak, Qi_Ex second highest for 6MWT, and IMT_Ph second highest for heart rate and *** This study supports that high- and medium-intensity machine-assisted training improves exercise capacity and QoL in hospital-based heart failure patients. After hospital discharge, non-machine-assisted training continuously improves cardiac function.

Purpose: The present study aimed to investigate the reliability of the non-exhaustive double effort(NEDE) test in running exercise and its associations with the ventilatory thresholds(VT_1 and VT_2) and the maximal lactate steady state(MLSS).Methods: Ten healthy male adults(age: 23 ± 4 years, height: 176.6 ± 6.4 cm, body mass: 76.6 ± 10.7 kg) performed 4 procedures:(1) a ramp test for VT_1 and VT_2 determinations measured by ratio of expired ventilation to O_2 uptake(VE/VO_2) and expired ventilation to CO_2 output(VE/VCO_2) equivalents, respectively;(2) the NEDE test measured by blood lactate concentration(NEDE_(LAC)) and heart rate responses(NEDE_(HR));(3) a retest of NEDE for reliability analysis; and(4) continuous efforts to determine the MLSS intensity. The NEDE test consisted of4 sessions at different running intensities. Each session was characterized by double efforts at the same running velocity(E1 and E2, 180 s), separated by a passive recovery period(90 s rest). LAC and HR values after E1 and E2(in 4 sessions) were used to estimate the intensity equivalent to"null delta" by linear fit. This parameter represents, theoretically, the intensity equivalent to maximal aerobic ***: The intraclass correlation coefficient indicated significant reliability for NEDE_(LAC)(0.93) and NEDE_(HR)(0.79)(both p < 0.05). There were significant correlations, no differences, and strong agreement with the intensities predicted by NEDE_(LAC)(10.1 ± 1.9 km/h) and NEDE_(HR)(9.8 ± 2.0 km/h) to VT_1(10.2 ± 1.1 km/h). In addition, despite significantly lower MLSS intensity(12.2 ± 1.2 km/h), NEDE_(LAC) and NEDE_(HR) intensities were highly correlated with this parameter(0.90 and 0.88, respectively).Conclusion: The NEDE test applied to running exercise is reliable and estimates the VT_1 intensity. Additionally, NEDE intensities were lower but still correlated with VT_2 and MLSS.

Effectiveness of Auricular Acupressure for Acute Postoperative Pain after Surgery: A Systematic Review and Meta-Analysis

Objective: To identify the effectiveness of auricular acupressure(AA) in patients with acute postoperative pain after surgery by systematic review. Methods: A search of randomized controlled trials was conducted in 5 English medical electronic databases and 4 Chinese databases. Two reviewers independently retrieved related studies, assessed the methodological quality, and extracted data with a standardized data form. Meta-analyses were performed using all time-points meta-analysis. Results: A total of 26 studies with 1,682 participants were included. Results showed that compared with conventional therapy, AA significantly improved the total effective rate [risk ratio=1.25, 95% confidence interval(CI), 1.13 to 1.37, P<0.0001; heterogeneity: P<0.0001, I^2=85%]. In the subgroup analysis, the results changed in different follow-up time and surgery categories. The pain relief in the AA group might be the most significant at 72 h after surgery(mean difference=–0.85, 95% CI, –1.20 to –0.50, P<0.0001) and in abdominal surgery(mean difference=–1.15, 95% CI, –1.41 to –0.90, P<0.0001). Sensitivity analysis demonstrated that the results of this meta-analysis were stable. No serious adverse effects were recorded. Conclusions: It was recommended to provide AA to patients with acute postoperative pain. However, a more accurate estimate of the effect requires further rigorously designed large-scale and high-quality RCTs for improving acute postoperative pain after surgery.

Background Evaluation of psychiatric disorders in children is essential in timely *** individual studies,there is no information on the exact status of psychiatric disorders in *** present study was conducted to determine the prevalence of depression among children with type 1 and type 2 *** This meta-analysis is registered with PROSPERO(CRD42021231491).Several databases(Web of Science,Scopus,and PubMed)were searched from January 1,2000 to December 15,*** key words included:depression,anxiety,children,and *** steps of search,comprising screening,risk of bias,and extraction of study data,were performed separately by two *** It was found that a total of 109 studies had been conducted involving 52,493 children with type 1 and type 2 *** studies were included in the present review *** prevalence of depression,anxiety among children with type 1 diabetes were 22.2%[95%confidence interval(CI)19.2–25.2],and 17.7%(95%CI 15.2–20.1),*** prevalence of depression was higher among girls(29.7%)than boys(19.7%).The prevalence of depression was higher in lower-middle-income countries at 29.3%(95%CI 18.6–40.0).The prevalence of depression among children with type 2 diabetes was 22.7%(95%CI 17.3–28.0).Conclusion The results of the present study indicate the importance of paying attention to extensive periodic screening and appropriate activities to reduce pediatric depression.

BACKGROUND: The aim of the study is to evaluate the theoretical knowledge and psychomotor skill acquisition of basic life support(BLS) by a group of secondary school students in Nigeria pre and post BLS training. METHODS: This was quasi-experimental study design with 210 participants. They were taught on adult BLS and all the participants practiced BLS on a Resusci-Anne manikin immediately after the training. Data were collected using American Heart Association(AHA) structured questionnaire and psychomotor skills checklist for BLS at baseline, and post BLS ***: The pre-training knowledge score was 1.9±1.4. This increased after the BLS training to 11.4±2.4, and the mean difference between the pre and post BLS training scores was 9.48571. Pre-training psychomotor skill score was 0.00±0.00, this increased to 8.9±1.9 after the training, and the mean difference was 8.90000. The knowledge and psychomotor skill difference between pre and post BLS training was signifi cant(Pschool students were not knowledgeable about BLS. Therefore, there is need for the creation of more awareness among the students.
