The Drivers of Child Mortality During the 2012–2016 Drought in La Guajira, Colombia

During the 2012–2016 drought in La Guajira,Colombia, child mortality rates rose to 23.4 out of *** of these children belonged to the Wayuu indigenous community, the largest and one of the most vulnerable in Colombia. At the municipal level, this study found a significant positive correlation between the average child mortality rate and households with a monthly income of less than USD 100, the number of people without access to health insurance, being part of the indigenous population,being illiterate, lacking sewage systems, living in rural areas, and large households with members younger than5 years old and older than 65 years old. No correlation was found with households without access to a water *** stepwise regression analysis showed that households with a monthly income of less than USD 100, no members older than 65 years old, but several children younger than5 years old, account for 90.4% of the child mortality *** study concludes that, if inhabitants had had better incomes or assets, as well as an adequate infrastructure,they could have faced the drought without the observed increase in child mortality.

Reference-guided structure-aware deep sketch colorization for cartoons

Digital cartoon production requires extensive manual labor to colorize sketches with visually pleasant color composition and color *** colorization,the artist usually takes an existing cartoon image as color guidance,particularly when colorizing related characters or an animation ***-guided colorization is more intuitive than colorization with other hints,such as color points or scribbles,or text-based ***,reference-guided colorization is challenging since the style of the colorized image should match the style of the reference image in terms of both global color composition and local color *** this paper,we propose a novel learning-based framework which colorizes a sketch based on a color style feature extracted from a reference color *** framework contains a color style extractor to extract the color feature from a color image,a colorization network to generate multi-scale output images by combining a sketch and a color feature,and a multi-scale discriminator to improve the reality of the output *** qualitative and quantitative evaluations show that our method outperforms existing methods,providing both superior visual quality and style reference consistency in the task of reference-based colorization.

Prospective case series on trabecular-iris angle status after an acute episode of phacomorphic angle closure

AIM:To investigate the trabecular-iris angle with ultrasound biomicroscopy (UBM) post cataract extraction after an acute attack of phacomorphic angle closure. ·METHODS:This prospective study involved 10 cases of phacomorphic angle closure that underwent cataract extraction and intraocular lens insertion after intraocular pressure (IOP) lowering. Apart from visual acuity and IOP, the trabecular-iris angle was measured by gonioscopy and UBM at 3 months post attack. ·RESULTS:In 10 consecutive cases of acute phaco-morphic angle closure from December 2009 to December 2010, gonioscopic findings showed peripheral anterior synechiae (PAS) ≤90° in 30% of phacomorphic patients and a mean Shaffer grading of (3.1±1.0). UBM showed a mean angle of (37.1°±4.5°) in the phacomorphic eye with the temporal quadrant being the most opened and (37.1°±8.0°) in the contralateral uninvolved eye. The mean time from consultation to cataract extraction was (1.4 ± 0.7) days and the mean total duration of phacomorphic angle closure was (3.6 ±2.8) days but there was no correlation to the degree of angle closure on UBM (Spearman correlation P =0.7). The presenting mean IOP was (50.5±7.4) mmHg and the mean IOP at 3 months was (10.5±3.4) mmHg but there were no correlations with the degree of angle closure (Spearman correlations P =0.9). CONCLUSION:Anopentrabecular-irisangleandnormal IOP can be achieved after an acute attack of pha-comorphic angle closure if cataract extraction is performed within 1 day-2 days after IOP control. Gonioscopic findings were in agreement with UBM,which provided a more specific and object angle measurement. The superior angle is relatively more narrowed compared to the other quadrants. All contralateral eyes in this series had open angles.

Surgical outcomes for unilateral superior oblique palsy in Chinese population: a retrospective study

AIM: To evaluate the outcome after surgery for unilateral superior oblique(SO) palsy in ***: The medical records of 39 patients that underwent surgery for unilateral SO palsy between January 2003 and December 2012 at caritas Medical Centre, Hong Kong, were retrospectively reviewed. All surgeries were performed by a single surgeon. Pre-operative assessments for vertical deviation, cyclo-deviation, and Knapp’s classification were obtained to determine the nature and degree of surgical *** deviation was measured at 1wk; 1, 6mo and on last follow-up day post-operatively. Cyclo-deviation was measured on last follow-up day ***: During the 10 y period, 39 subjects were recruited. The most common etiology was congenital(94.9%). Knapp’s Type III(66.7%) and Type I(12.8%)classifications were the most common subtypes. To treat SO palsy, the most common surgical procedures were:isolated inferior oblique(IO) anteriorization(41.0%),isolated IO myectomy(10.3%), and isolated IO recession(10.3%). At 3.5 ±2.1y post-operatively, the vertical deviation was significantly reduced(15.1 ±6.2 PD versus0.5±1.4 PD, P <0.0001) without significant improvement in cyclo-deviation(P =0.5). Initial vertical deviation was correlated with cyclo-torsion(r =0.4, P =0.007). Those with over-correction had greater initial vertical deviation(19.4±7.2 PD versus 13.2±4.3 PD, P =0.003). After a single operation, 84.6% of subjects achieved a vertical deviation within ±3 PD.· CONCLUSION: The majority of subjects achieved corrected vertical deviation after a single surgery although there was no improvement in *** with over-correction of primary position deviation had greater preoperative vertical deviation and it may be related to simultaneous multiple muscle surgery.

Demand Learning and Cooperative Deployment of UAV Networks

Unmanned aerial vehicle(UAV) as a powerful tool has found its applicability in assisting mobile users to deal with computation-intensive and delaysensitive applications(e.g., edge computing, high-speed Internet access, and local caching). However, deployment of UAV-aided mobile services(UMS) faces challenges due to the UAV limitation in wireless coverage and energy storage. Aware of such physical limitations, a future UMS system should be intelligent enough to self-plan trajectories and best offer computational capabilities to mobile users. There are important issues regarding the UAV-user interaction, UAV-UAV cooperation for sustainable service provision, and dynamic UMS pricing. These networking and resource management issues are largely overlooked in the literature and this article presents intelligent solutions for cooperative UMS deployment and operation. Mobile users’ locations are generally private information and changing over time. How to learn on-demand users’ truthful location reporting is important for determining optimal UAV deployment in serving all the users fairly. After addressing the truthful UAV-user interaction issue via game theory, we further study the UAV network sustainability for UMS provision by minimizing the energy consumption cost during deployment and seeking UAV-UAV cooperation. Finally, for profit-maximizing purpose, we analyze the cooperative UAVs’ deployment, capacity allocation, and dynamic service pricing.

Artificial intelligence(AI), particularly deep learning algorithms, is gaining extensive attention for its excellent performance in image-recognition tasks. They can automatically make a quantitative assessment of complex medical image characteristics and achieve an increased accuracy for diagnosis with higher efficiency. AI is widely used and getting increasingly popular in the medical imaging of the liver, including radiology, ultrasound, and nuclear medicine. AI can assist physicians to make more accurate and reproductive imaging diagnosis and also reduce the physicians' workload. This article illustrates basic technical knowledge about AI, including traditional machine learning and deep learning algorithms, especially convolutional neural networks, and their clinical application in the medical imaging of liver diseases, such as detecting and evaluating focal liver lesions, facilitating treatment, and predicting liver treatment response. We conclude that machine-assisted medical services will be a promising solution for future liver medical care. Lastly, we discuss the challenges and future directions of clinical application of deep learning techniques.

Recurrent pulmonary embolism in a boy with antiphospholipid syndrome

We report a 14-year-old patient with recurrent pulmonary embolism due to catastrophic antiphospholipid syndrome (APS) with severe pulmonary inflammation. We considered elevated antibodies against cardiolipin and anti-beta2-glycoprotein-1, but no clinical nor laboratory manifestations of systemic lupus erythematosus (SLE). Pulmonary embolism had been the first manifestation of catastrophic APS in this patient. We prescribed warfarin and systemic corticosteroids. A second embolism appeared during anticoagulation with warfarin. This event led to a marked decrease of his physical performance due to his obstructive and restrictive lung disease. Anticoagulation was changed from enteral warfarin to subcutaneous enoxaparine. We also prescribed inhaled corticosteroid which led to an improvement of his respiratory symptoms and overall poor physical condition.

FROM WRITING TO CHATTING: PROMOTING REFLECTIVE PRATICE AMONGST INSERVICE ESL/EFL TEACHERS

It has been commonplace to hear of teacher educators promoting reflective practice. The feasibility of the ways of reflective practice suggested in the recent literature will be reviewed. This paper arises from the conceptualization of reflection and a review of the concrete problems facing Hong Kong practitioners. In revisiting reflective practice as it is limited and constrained by the heavy workload of English teachers in Hong Kong, it seems to be of utmost importance to provide feasible ways for these teachers to develop their professional competence in this area. The difficulties and the problems that constrain Hong Kong teachers’ reflective practice shall be highlighted and it is argued that what in-service teachers require is a viable and effective way of being reflective. I shall suggest "Chatin-private" (Ouyang, 1997) as an alternative for the overworked teachers to remain reflective in the case of Hong Kong with a particular emphasis on workload of secondary school teachers.
