Objectives To evaluate the feasibility and safety of a second generation robotic percutaneous coronary intervention(R-PCI)system in *** Robotic PCI has been shown to be an effective method for conducting coronary *** has further benefits of more accurate lesion measurement,improved stent deployment,reduced incidence of geographic miss and reduction of operator radiation *** This single center evaluation enrolled 10 consecutive patients who had been selected for *** success was defined as residual stenosiscardiovascular *** curve effect was assessed by comparing efficiency metrics of early *** *** Eleven lesions were treated all successfully without manual interruption or MACE *** lesions(63%)were ACC/AHA class B2 and *** procedure time was 57.7±26.4 min,however two procedures were part of live *** the two live cases,the mean procedure time was 51.8±23.7 *** efficiency tended to improve from early cases to later cases based on PCI time(48.3±32.9 vs.25.5±13.0 min,P=0.27),fluoroscopy time(20.3±8.2 vs.12.5±4.6 min,P=0.16),contrast volume(145.0±28.9 vs.102.5±17.1 mL,P=0.05)and Air Kerma dose(1932±978 vs.1007±70 mGy,P=0.31).Conclusions Second generation robotic PCI was safe,effective and there were trends toward improvements in procedural efficiency during this early experience in China.

Objectives To evaluate the very long-term safety and effectiveness of drug-eluting stents （DES） compared to bare-metal stents （BMS） for patients with large coronary vessels. Methods From April 2004 to October 2006, 2407 consecutive patients undergoing de novo lesion percutaneous coronary intervention with reference vessel diameter greater than or equal to 3.5 mm at Fu Wai Hospital in Beijing, China, were prospectively enrolled into this study. We obtained 9-year clinical outcomes including death, myocardial infarction （MI）, thrombosis, target lesion revascularization （TLR）, target vessel revascularization （TVR）, and major adverse cardiac events （MACE, the composite of death, MI, and TVR）. We performed Cox＇s proportional-hazards models to assess relative risks of all the outcome measures after propensity match. Results After propensity scoring, 514 DES-treated patients were matched to 514 BMS-treated patients. The patients treated with BMS were associated with higher risk ofTLR （HR： 2.55, 95%CI： 1.520-4.277, P = 0.0004） and TVR （HR： 1.889, 95%CI： 1.185-3.011, P = 0.0075）, but the rates of death/MI and MACE were not statistically different. All Academic Research Consortium definition stent thrombosis at 9-year were comparable in the two groups. Conclusions During long-term follow-up through nine years, use of DES in patients with large coronary arteries was still associated with significant reductions in the risks of TLR and TVR.

BACKGROUND: This study aims to investigate whether small balloon aortic valvuloplasty(BAV) reduces the need for permanent pacemaker implantation (PPMI) after transcatheter aortic valveimplantation (TAVI).METHODS: This was a retrospective analysis using data from our local TAVI database. SmallBAV was defined as a small balloon size (=18 mm) pre-dilatation. Normal BAV was defined as aballoon size >18 mm. The primary endpoint was the incidence of new ***: Of 99 consecutive TAVI patients, five patients were excluded due to pre-existingpermanent pacemaker. Patients in the small BAV group (n=57) had a signifi cantly lower PPMI ratecompared with the normal BAV group (n=37) (3.5% vs. 18.9%, P=0.026). Moderate or severe aorticvalve regurgitation post-procedure was similar between the small BAV and normal BAV groups (5.3%vs. 8.1%, P=0.480);likewise, the mean aortic gradient post-procedure did not differ significantly(11.5±5.2 mmHg vs. 12.2±7.3 mmHg, 1 mmHg=0.133 kPa, P=0.580) between the groups. Devicesuccess rates were also similar (94.7% vs. 91.8%, P=0.680). In multivariable analysis, small BAV(P=0.027), the ratio of prosthesis diameter to annulus diameter (P=0.048), and mean aortic gradientby echo in the basement (P=0.021) were independent predictors of ***: The small BAV strategy is associated with a low rate of permanentpacemaker implantation after transcatheter self-expanding valve implantation in this single-centerobservational study.

Aspirin has been widely used in primary prevention of atherosclerotic cardiovascular disease(ASCVD)J141 Aspirin for primary prevention of ASCVD does not significantly reduce all-cause mortality or cardiovascular mortality,and the main benefit of aspirin for primary prevention of ASCVD is a significant reduction in non-fatal ischemic events including myocardial infarction,transient ischemic attack,ischemic stroke,and major cardiovascular events(cardiovascular death,non-fatal myocardial infarction,and non-fatal stroke),whereas the main risk is a significant increase in non-fatal major bleeding events including gastrointestinal bleeding and intracranial hemorrhage.1 Obviously,it makes sense to prescribe aspirin for primary prevention only if the benefits clearly outweigh the *** Europe and the United States,the benefit-risk ratio of aspirin for primary prevention has gradually decreased compared to the past due to the wide application of other primary prevention measures such as blood pressure reduction,smoking cessation,and the use of *** recent large-scale clinical trials have shown that aspirin has no net clinical benefit when used in low-risk populations.19'1^Therefore,aspirin must be used with caution in the primary prevention of ASCVD.

This study aims to construct a logistically derived additive score for predicting in-hospital mortality risk in Chinese patients undergoing coronary artery bypass surgery(CABG).Data from 9839 consecutive CABG patients in 43 Chinese centers were collected between 2007 and 2008 from the Chinese Coronary Artery Bypass Grafting *** database was randomly divided into developmental and validation subsets(9:1).The data in the developmental dataset were used to develop the model using logistic *** and discrimination characteristics were assessed using the validation *** were defined for each model to distinguish different risk *** excluding 275 patients with incomplete information,the overall mortality rate of the remaining 9564 patients was 2.5%.The SinoSCORE model was constructed based on 11 variables:age,preoperative NYHA stageⅢorⅣ,chronic renal failure,extracardiac arteriopathy,chronic obstructive pulmonary disease,preoperative atrial fibrillation or flutter(within 2 weeks),left ventricular ejection fraction,other elective surgery,combined valve procedures,preoperative critical state,and *** the developmental dataset,calibration using a Hosmer-Lemeshow(HL)test was at P=0.44 and discrimination based on the area under the receiver operating characteristic curve(ROC)was *** the validation dataset,the HL test was at P=0.34 and the area under the ROC(AUC)was 0.78.A logistically derived additive model for predicting in-hospital mortality among Chinese patients undergoing CABG was developed based on the most up-to-date multi-center data from China.

With the arrival of the era of global population aging, we strive for healthy aging and a healthy senior life rather than simple prolongation of the physical age. For the past 50 years, cardiovascular diseases （CVD） have been the most common cause of death among the elderly people globally. In China, there has been an exponential increase in the incidence of heart disease and stroke in the elderly population. Atherosclerosis is the pathological change in the coronary artery disease, stroke, and peripheral vascular disease. Despite the significant benefit demonstrated, control of classic risk factors alone, such as lifestyle change or drug therapy, was shown to have limitations in reducing the incidence of cardiovascular events. Vascular aging has been shown to be an important independent predictor of CVD events. Interventions targeting vascular aging have emerged as a new paradigm in conjunction with control of risk factors for the prevention of CVD. Vascular aging and atherosclerosis are two distinct pathological changes and difficult to distinguish clinically. Recent research with Chinese medicine （CM） has shown encouraging observations, linking the clinical benefit of delaying vascular aging and treating atherosclerosis. These results demonstrate great potential of CM in the prevention and treatment of CVD.
