Intestinal Cckbr-specific knockout mouse as a novel model of salt-sensitive hypertension via sodium over-absorption

OBJECTIVES To investigate the value of CCKBR^(fl/fl)villin-Cre mice as a mouse model of salt-sensitive hypertension(SSH).mETHODS In the first part,2-month-old CCKBR^(fl/fl)villin-Cre mice(CKO)and control CCKBR^(fl/fl)mice(WT)were fed with normal diet(0.4%NaCl)or high salt diet(4%NaCl),separately for 6 *** the rescue study,one week of hydrochlorothiazide or saline injection were treated with the CKO mice fed high salt *** blood pressure,biochemical indexes,and the expression of small intestinal sodium transporters(NHE3,NKCC1,eNaC)was *** organ injury markers(mmP2/mmP9)and the histopathological changes of kidneys were observed,whereas the changes of duodenal sodium absorption were detected by small intestinal perfusion in *** The CCKBR^(fl/fl)villin-Cre mice with high salt intake exhibited high blood pressure,increased duodenal sodium absorption and urinary sodium excretion,and with renal *** protein expression of NHE3,NKCC1 and eNaC were also significant increase in the intestine of CKO-HS *** with hydrochlorothiazide remarkably attenuated the elevated blood pressure by high salt absorption in the CCKBR^(fl/fl)villin-Cre mice,but no significant histopathological changes were *** These results support a crucial role of intestinal Cckbr deficiency on SSH development and the diuretic antihypertension effect in CCKBR^(fl/fl)villin-Cre *** CCKBR^(fl/fl)villin-Cre mice with the high salt intake may serve as a stable model of salt-sensitive hypertensive induced by sodium overloading.

Contrast-induced nephropathy after staged percutaneous coronary intervention in patients with ST-segment elevation myocardial infarction and multivessel coronary artery disease

Background Contrast-induced nephropathy（CIN） occurs frequently in patients undergoing primary percutaneous coronary intervention（PCI） for ST-segment elevation myocardial infarction（STEmI） and is associated with poor outcomes. multivessel coronary artery disease（mVCAD） is considered to be a potentially important risk factor for CIN. There are still no data on CIN in patients undergoing staged PCI for STEmI and mVCAD. Therefore, we explored the incidence, risk factors, in-hospital and follow-up outcomes of CIN in this special population. methods From 2011 to 2018, we enrolled 103 consecutive patients with STEmI who underwent staged PCI for mVCAD. CIN was defined as a relative increase of 〉 25% or an absolute increase of ≥ 0.5 mg/dL in SCr from the baseline value 72 h after exposure to the contrast medium. The incidence, risk factors, in-hospital and follow-up outcomes of CIN in this special population were studied. results We found1） the incidence of CIN after primary PCI and staged PCI was 16.50% and 25.20%, respectively. 2） patients with CIN had worse in-hospital and follow-up outcomes. 3） in multivariate logistic analysis, independent risk factors for CIN included：（1） lower creatinine clearance at baseline;（2） atrioventricular block requiring temporary cardiac pacemaker implantation;（3） use of IABP at baseline;（4） total contrast volume administered（ primary PCI ＋staged PCI）;（5） shorter time interval between primary PCI and staged PCI. Conclusions CIN is a frequent complication in patients with STEmI and mVCAD undergoing staged PCI. The development of CIN is associated with worse in-hospital and follow-up outcomes.

Comparison of Ventricular Electrophysiological Effects of Amiodarone in Canine models With Congestive Heart Failure and Normal Dogs

Objectives This study compared the effects of amiodarone on ventricular electrophysiological properties in normal dogs and CHF dogs. methods Dogs （n=44） were randomized into four groups： Group 1 （n=10） was the control. Group 2 （n=10） was given amiodarone orally 300 mg·d^-1 for 4 to 5 weeks. Group 3 （n=12） was the congestive heart failure （CHF） models induced by right ventricular rapid pacing （240 pulses·min^-1 for 4 to 5 weeks）. Group 4 （n=12） was the CHF models given amiodarone orally 300 mg·d^-1 for 4 to 5 weeks. The ventricular electrophysiological variables were evaluated by standard electric stimulation and monophasic action potential （mAP） recording. results Amiodarone prolonged sinus cycle length （SCL）, intra-ventricular conduction time （IVCT）, mAP duration （mAPD90）, ventricular effective period （VERP）, ventricular activation time （VAT） and ventricular recovery time （VRT） without significant effects on the ratio of VERP to mAPD90 （VERP/mAPD90）, ventricular fibrillation threshold （VFT）, the dispersion of VRT （VRT-D）, and ventricular late repolarization duration （VLRD） in normal dogs. However, amiodarone did not further prolong the prolonged SCL, mAPD90, VERP, VAT and VRT, but further prolonged IVCT in CHF dogs. Amiodarone normalized the abnormal ventricular electrophysiological properties in CHF dogs as manifested by increasing the decreased VERP/mAPD90 and VFT, shortening the prolonged VLRD, and decreasing the increased VRT-D. Amiodarone did not worsen the hemodynamic parameters in normal and CHF dogs. Conclusions Amiodarone had different effects on ventricular electrophysiological properties in normal and CHF dogs. The favorable effects of amiodarone in normalizing some abnormal cardiac electrophysiological properties in CHF models may have potential value on the prevention and treatment of ventricular arrhythmias and sudden cardiac death in CHF.

Epidemiological, Clinical, and Therapeutic Characteristics of Acute Appendicitis at the reference Health Center in Commune I of the District of Bamako mali

Acute appendicitis is the most common abdominal surgical emergency. This work is a prospective study from January 2012 to December to analyze the epidemiological, clinical and therapeutic aspects of acute appendicitis in the Cs ref of commune I of the district of Bamako and the results obtained were then compared with those reported in other countries. Our study included 72 patients. The appendectomy surgical specimens were the subject of an anatomical pathology study at Point G University Hospital in Bamako in mali, 1.4% of cases of which were healthy. All our operated patients received antibiotics (mono or dual antibiotics) with a post-operative hospitalization duration of 1.5 (36 hours) days on average. We recorded 6 complications such as parietal suppuration, i.e. 8.33% of cases. No 5th day syndrome or late complications were noted. The average length of hospitalization was 36 hours. Abdominal ultrasound;used as an additional examination was carried out in 55 of our patients or 76.39% of cases. We have not recorded any deaths. Raising awareness among patients about the dangers of complications motivated patients to undergo surgery as soon as possible.

Takotsubo cardiomyopathy:A case report

Takotsubo cardiomyopathy （TC） is a reversible non-ischemic cardiomyopathy characterized by transient left ventricular dysfunction, which is usually associated with a favorable prognosis. It is similar to an acute myocardial infarction, but coronary angiography shows no obstructive lesions and akinesia of the apex and the middle portion of the left ventricle. TC is often triggered by an intense physical or emotional stress event. Without appreciation of differentiating features, TC can easily be misdiagnosed as an acute coronary syndrome. misdiagnosis and the subsequent inappropriate and potentially harmful use of medication therapy can be avoided through detailed history-taking, laboratory examination and proper investigations. In order to raise the awareness of the importance of TC diagnosis and further to discuss the therapeutic strategies, we are going to present a case report regarding TC and provide a timely summary and update on current understand- ing.

Amiodarone Therapy for Cardiac Arrhythmias: Is It Associated with the Development of Cancers?

Amiodarone is used worldwide to treat cardiac arrhythmias, as well as highly symptomatic cases of atrial fibrillation. With this expanded use, especially following its 1985 United States Food and Drug Administration approval, and its use as a long-term therapy in common practice, reports of cancers temporarily related to amiodarone have begun to increase. Animal studies, several clinical trials, numerous case reports, and a population-based cohort study have suggested that cancers may be associated with amiodarone use. This review focuses on the ever increasing evidence in the literature that suggests amiodarone therapy, especially with long-term use, may increase the potential risk of cancer development. It also expresses the need for more definitive studies to be conducted to provide clinicians with a clear answer to this important question.

multi-Systemic Inflammatory Syndrome in Children

The objective of our work is to study the multi-systemic inflammatory syndrome (PImS) in children, to determine its frequency, by analyzing the epidemiological, clinical, paraclinical, therapeutic, and evolutionary profile of these patients. A retrospective study spanning a period of 2 years from April 2020 to march 2022. It concerns all children under the age of 16 admitted and cared for in the pediatric emergency department of the university hospital Hassan II of Fez for multi-system inflammatory syndrome (PImS). Twenty cases of PImS were collected over this period. multi-system inflammatory syndrome in children has been described in temporal association with COVID-19, usually within 2 to 6 weeks of illness or exposure. The age of the patients varies between 8 months and 15 years. All patients presented with fever and cutaneous signs, followed by digestive signs and neurological signs. The inflammatory syndrome is frankly positive in all patients who had a COVID-19 PCR and/or positive serology. The treatment is based on the administration of immunoglobulins in association with corticosteroid therapy and non-specific antibiotic therapy in the majority of cases (80%). The evolution was favorable. PImS should be considered in all children presenting with a clinical and/or biological inflammatory syndrome associated with COVID-19.

Impaired Arterial Elasticity Identified by Pulse Waveform Analysis as a marker for Vascular Wall Damage in Humans With Aging and Hypertension

Objectives Cardiovascular risk factors lead to pathogenesis of atherosclerosis and its clinical events by impairing vascular wall. Endothelial dysfunction is the earliest marker for vascular wall injuries. Development of new method to detect early vascular wall damage has an important clinical implication for the prevention and treatment of cardiovascular diseases. Therefore, the present study was performed to evaluate effect of aging and hypertension, two independent risk factors for cardiovascular diseases, on arterial elasticity by using pulse waveform analysis and investigate whether the changes in arterial elasticity can be used as a risk marker for vascular structural and functional abnormalities. methods Using modified Windkessel model of the circulation and pulse waveform analysis, C1 large artery and C2 small artery elasticity indices of 204 Chinese normal healthy subjects （ age 15-80 years） and 46 Chinese essential hypertensive patients （age 35-70 years） were measured. Age- and hypertension-related arterial elasticity changes were examined. results C1 large artery and C2 small artery elasticity indices were reduced with advancing age in healthy subjects. C1 large artery and C2 small artery elasticity indices were negatively correlated with age （r=-0.628, P〈0.001; r=-0.595, P〈0.001）. C1 large artery and C2 small artery elasticity indices in patients with essential hypertension compared with the agematched healthy subjects were （9.31±3.85 ml/mm Hg × 10 versus 15.13±4.14 ml/mmHg × 10, P〈0.001） and （3.57 ± 1.62 ml/mm Hg x 100 versus 7.89±2.91 ml/ mmHg ×　100 P 〈0.001）, respectively, and were significantly lower than the corresponding healthy subjects. There were negative association between C1 large artery and C2 small artery elasticity indices and systolic blood pressure （r=-0.37, P〈0.05; r=-0.39, P〈0.05） and pulse pressure （r=-0.39, P〈0.05; r=- 0.43,P〈0.05） in patients with essential hypertension.
