Background:The role of surgery in metastatic breast cancer(MBC)is currently *** novel statistical and deep learning(DL)methods promise to infer the suitability of surgery at the individual ***:The objective of this study was to identify the most applicable DL model for determining patients with MBC who could benefit from surgery and the type of surgery ***:We introduced the deep survival regression with mixture effects(DSME),a semi-parametric DL model integrating three causal inference *** models were trained to make individualized treatment *** who received treatments in line with the DL models'recommendations were compared with those who underwent treatments divergent from the *** probability weighting(IPW)was used to minimize *** effects of various features on surgery selection were visualized and quantified using multivariate linear regression and causal ***:In total,5269 female patients with MBC were *** was an independent protective factor,outperforming other models in recommend-ing surgery(IPW-adjusted hazard ratio[HR]=0.39,95%confidence interval[CI]:0.19–0.78)and type of surgery(IPW-adjusted HR=0.66,95%CI:0.48–0.93).DSME was superior to other models and traditional guidelines,suggesting a higher proportion of patients benefiting from surgery,especially breast-conserving *** debiased effect of patient characteristics,including age,tumor size,metastatic sites,lymph node status,and breast cancer subtypes,on surgery decision was also ***:Our findings suggested that DSME could effectively identify patients with MBC likely to benefit from surgery and the specific type of surgery *** method can facilitate the development of efficient,reliable treatment recommendation systems and provide quantifiable evidence for decision-making.

Background Domestic goose breeds are descended from either the Swan goose(Anser cygnoides)or the Greylag goose(Anser anser),exhibiting variations in body size,reproductive performance,egg production,feather color,and other phenotypic *** a pan-genome facilitates a thorough identification of genetic variations,thereby deepening our comprehension of the molecular mechanisms underlying genetic diversity and phenotypic *** To comprehensively facilitate population genomic and pan-genomic analyses in geese,we embarked on the task of 659 geese whole genome resequencing data and compiling a database of 155 RNA-seq *** constructing the pan-genome for geese,we generated non-reference contigs totaling 612 Mb,unveiling a collection of 2,813 novel genes and pinpointing 15,567 core genes,1,324 softcore genes,2,734 shell genes,and 878 cloud genes in goose ***,we detected an 81.97 Mb genomic region showing signs of genome selection,encompassing the TGFBR2 gene correlated with variations in body weight among ***-wide association studies utilizing single nucleotide polymorphisms(SNPs)and presence-absence variation revealed significant genomic associations with various goose meat quality,reproductive,and body composition *** instance,a gene encoding the SVEP1 protein was linked to carcass oblique length,and a distinct gene-CDS haplotype of the SVEP1 gene exhibited an association with carcass oblique ***,the pan-genome analysis revealed enrichment of variable genes in the“hair follicle maturation”Gene Ontology term,potentially linked to the selection of feather-related traits in geese.A gene presence-absence variation analysis suggested a reduced frequency of genes associated with“regulation of heart contraction”in domesticated geese compared to their wild *** study provided novel insights into gene expression features and functions by integrating gene expression patterns across multiple orga

Femoral Access with Ultrasound-Guided Puncture and Z-Stitch Hemostasis for Adults with Congenital Heart Diseases Undergoing Electrophysiological Procedures

Aims:Although the application of ultrasound-guided vascular puncture and Z-stitch hemostasis to manage femoral access has been widely utilized,there is limited data on this combined application in adult congenital heart disease(ACHD)patients undergoing electrophysiological(EP)*** sought to evaluate the safety and efﬁcacy of ultrasound-guided puncture and postprocedural Z-stitch hemostasis for ACHD patients under-going EP *** and Results:The population of ACHD patients undergoing transfemoral EP pro-cedures at the University of Zurich Heart Center between January 2019 and December 2022 was observed and *** the study period,femoral access(left/right,arterial/venous)was performed under real-time ultrasound *** the end of the procedure,a single Z-stitch was performed at the puncture *** eval-uated the incidence of in-hospital complications associated with femoral access puncture in this *** 101 patients who had a total of 147 previous ipsilateral vascular punctures(mean 1.5 per person),100 patients underwent successful femoral vascular access for EP *** median age of the patients was 47±15 years and 34(34%)were male.Z-stitches were performed after the procedure in 100 patients with 303 femoral vascular accesses(mean 3 punctures per person).No patient developed vascular puncture relevant inguinal hematoma,pseudo aneurysm,arteriovenousﬁstula,venous or arterial ***:In ACHD patients undergoing EP procedures,optimal femoral access management can be achieved with ultra-sound-guided puncture and postprocedural Z-stitch hemostasis.

Left bundle branch pacing set to outshine biventricular pacing for cardiac resynchronization therapy?

The deleterious effects of long-term right ventricular pacing necessitated the search for alternative pacing sites which could prevent or alleviate pacinginduced *** recently,biventricular pacing(BiVP)was the only modality which could mitigate or prevent pacing induced ***,BiVP could resynchronize the baseline electromechanical dssynchrony in heart failure and improve ***,the high non-response rate of around 20%-30%remains a major *** non-response has been largely attributable to the direct non-physiological stimulation of the left ventricular myocardium bypassing the conduction *** overcome this limitation,the concept of conduction system pacing(CSP)came *** initial success of the first CSP via His bundle pacing(HBP),certain drawbacks including lead instability and dislodgements,steep learning curve and rapid battery depletion on many occasions prevented its widespread use for cardiac resynchronization therapy(CRT).Subsequently,CSP via left bundle branch-area pacing(LBBP)was developed in 2018,which over the last few years has shown efficacy comparable to BiVP-CRT in small observational ***,its safety has also been well established and is largely free of the pitfalls of the *** the recent metanalysis by Yasmin et al,comprising of 6 studies with 389 participants,LBBPCRT was superior to BiVP-CRT in terms of QRS duration,left ventricular ejection fraction,cardiac chamber dimensions,lead thresholds,and functional status amongst heart failure patients with left bundle branch ***,there are important limitations of the study including the small overall numbers,inclusion of only a single small randomized controlled trial(RCT)and a small follow-up ***,the entire study population analyzed was from China which makes generalizability a *** the concerns,the meta-analysis adds to the growing body of evidence demonstrating the efficacy of *** this st

Cardiomyocyte death is one of the major mechanisms contributing to the development of myocardial infarction(MI)and myocardial ischemia/reperfusion(MI/R)*** to the limited regenerative ability of cardiomyocytes,understanding the mechanisms of cardiomyocyte death is ***,one of the regulated programmed cell death pathways,has recently been shown to play important roles in MI and MI/R *** is activated by damage-associated molecular patterns(DAMPs)that are released from damaged myocardial cells and activate the formation of an apoptosisassociated speck-like protein containing a CARD(ASC)interacting with NACHT,LRR,and PYD domains-containing protein 3(NLRP3),resulting in caspase-1 cleavage which promotes the activation of Gasdermin D(GSDMD).This pathway is known as the canonical *** has also been shown to be activated in a non-canonical pathway during MI and MI/R injury via caspase-4/5/*** of GSDMD has been shown to provide cardioprotection against MI and MI/R *** the effects of MI or MI/R injury on pyroptosis have previously been discussed,knowledge concerning the roles of GSDMD in these settings remains *** this review,the evidence from in vitro,in vivo,and clinical studies focusing on cardiac GSDMD activation during MI and MI/R injury is comprehensively summarized and *** from this review will help pave the way for a new therapeutic target in ischemic heart disease.

VERAPAMIL AND VENTRICULAR TACHYCARDIAS

In the absence of myocardial ischacmia, ventricular tachycardias occurring in patients with structural heart disease rarely respond to calcium antagonists like verapamil. In contrast, verapamil is markedly effective in the acute management of "supraventricular" tachycardias. Thus, intravenous verapamil has often been used for therapeutic and diagnostic purposes in patients with wide QRS complex tachycardias. This report details two unusual cases of vertricular tachycardia that are verapamil-seusitive. They highlight two major categories of "idiopathic" ventricular tachycardia, namely fascieular tachycardia and catecholamine-mediated ventricular tachycardia. The risks of verapamil administration for wide complex tachycardias are also reviewed.
