The second edition of the expert consensus on pediatric nutrition was formed based on a global update of pedia-tric nutrition guidelines or consensus worldwide,the management of congenital heart disease,and the results of multi-center clinical nutrition research for congenital heart disease following theﬁrst Chinese consensus edition of *** consensus was also shaped by the results of three discussion sessions and two questionnaires con-ducted by the 13-member collaboration *** process was informed by both clinical guidelines and expert *** quality of literature,both in English and Chinese,and the level of recommendations were evaluated using the Grading of Recommendations Assessment,Development,and Evaluations(GRADE)system.

Short stature is among the most common endocrinological disease phenotypes of childhood and may occur as an isolated finding or in conjunction with other clinical *** the diagnostic utility of clinical genetic testing in short stature has been implicated,the genetic architecture and the utility of genomic studies such as exome sequencing(ES)in a sizable cohort of patients with short stature have not been investigated *** this study,we recruited 561 individuals with short stature from two centers in China during a 4-year *** performed ES for all patients and available *** patients were retrospectively divided into two groups:an isolated short stature group(group I,n=257)and an apparently syndromic short stature group(group II,n=304).Causal variants were identified in 135 of 561(24.1%)*** group I,29 of 257(11.3%)of the patients were solved by variants in 24 *** group II,106 of 304(34.9%)patients were solved by variants in 57 *** involved in fundamental cellularprocess played an important role in the genetic architecture of syndromic short *** genetic architectures and pathophysiological processes underlie isolated and syndromic short stature.

The global trend toward aging populations has resulted in an increase in the occurrence of Alzheimer's disease(AD)and associated socioeconomic *** metabolism of amyloid-β(Aβ)has been proposed as a significant pathomechanism in AD,supported by results of recent clinical trials using anti-Aβ***,the cognitive benefits of the current treatments are *** etiology of AD is multifactorial,encompassing Aβand tau accumulation,neuroinflammation,demyelination,vascular dysfunction,and comorbidities,which collectively lead to widespread neurodegeneration in the brain and cognitive ***,solely removing Aβfrom the brain may be insufficient to combat neurodegeneration and preserve *** attain effective treatment for AD,it is necessary to(1)conduct extensive research on various mechanisms that cause neurodegeneration,including advances in neuroimaging techniques for earlier detection and a more precise characterization of molecular events at scales ranging from cellular to the full system level;(2)identify neuroprotective intervention targets against different neurodegeneration mechanisms;and(3)discover novel and optimal combinations of neuroprotective intervention strategies to maintain cognitive function in AD *** Alzheimer's Disease Neuroprotection Research Initiative's objective is to facilitate coordinated,multidisciplinary efforts to develop systemic neuroprotective strategies to combat *** aim is to achieve mitigation of the full spectrum of pathological processes underlying AD,with the goal of halting or even reversing cognitive decline.

BACKGROUND Three-vessel disease(TVD)with a SYNergy between PCI with TAXus and cardiac surgery(SYNTAX)score of≥23 is one of the most severe types of coronary artery *** aimed to take advantage of machine learning to help in de-cision-making and prognostic evaluation in such *** We analyzed 3786 patients who had TVD with a SYNTAX score of≥23,had no history of previous revascularization,and underwent either coronary artery bypass grafting(CABG)or percutaneous coronary intervention(PCI)after *** patients were randomly assigned to a training group and testing *** C4.5 decision tree algorithm was applied in the training group,and all-cause death after a median follow-up of 6.6 years was regarded as the class *** The decision tree algorithm selected age and left ventricular end-diastolic diameter(LVEDD)as splitting features and divided the patients into three subgroups:subgroup 1(age of≤67 years and LVEDD of≤53 mm),subgroup 2(age of≤67 years and LVEDD of>53 mm),and subgroup 3(age of>67 years).PCI conferred a patient survival benefit over CABG in sub-group *** was no significant difference in the risk of all-cause death between PCI and CABG in subgroup 1 and subgroup 3 in both the training data and testing *** the total study population,the multivariable analysis revealed significant dif-ferences in the risk of all-cause death among patients in three *** The combination of age and LVEDD identified by machine learning can contribute to decision-making and risk assessment of death in patients with severe *** present results suggest that PCI is a better choice for young patients with severe TVD characterized by left ventricular dilation.

Background::Imprecise interpretation of coronary angiograms was reported and resulted in inappropriate *** Between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery(SYNTAX)score is a comprehensive system to evaluate the complexity of the overall *** hypothesized that a real-time SYNTAX score feedback from image analysts may rectify the mis-estimation and improve revascularization appropriateness in patients with stable coronary artery disease(CAD).Methods::In this single-center,historical control study,patients with stable CAD with coronary lesion stenosis≥50%were consecutively *** the control period,SYNTAX scores were calculated by treating *** the intervention period,SYNTAX scores were calculated by image analysts immediately after coronary angiography and were provided to cardiologists in real-time to aid *** primary outcome was revascularization deemed inappropriate by Chinese appropriate use criteria for coronary ***::A total of 3245 patients were enrolled and assigned to the control group(08/2016-03/2017,n=1525)or the intervention group(03/2017-09/2017,n=1720).For SYNTAX score tertiles,17.9%patients were overestimated and 4.3%were underestimated by cardiologists in the control *** adjustment,inappropriate revascularization significantly decreased in the intervention group compared with the control group(adjusted odds ratio[OR]:0.83;95%confidence interval[CI]:0.73-0.95;P=0.007).Both inappropriate percutaneous coronary intervention(adjusted OR:0.82;95%CI:0.74-0.92;P<0.001)and percutaneous coronary intervention utilization(adjusted OR:0.88;95%CI:0.79-0.98;P=0.016)decreased significantly in the intervention *** was no significant difference in 1-year adverse cardiac events between the control group and the intervention ***::Real-time SYNTAX score feedback significantly reduced inappropriate coronary revascularization in sta

尼可地尔是用于治疗缺血性心脏疾病的药物，具有双重性质。其作用机制（在细胞膜和线粒体水平选择性激活K＋ATP通道）是促使冠状动脉和外周血管扩张，随后还原前、后负荷。该药物已在欧洲上市多年，归为一类新的冠心病治疗药物。大规模、IONA随机试验评价尼可地尔与传统抗心绞痛药物治疗稳定型心绞痛的疗效，结果显示：主要疗效指标（心源性死亡、心肌梗死和计划外入院的胸痛）尼可地尔组（13.1%）较安慰剂组（15.5%，P=0.014）明显减少。虽然替代β-受体阻滞剂用于治疗心肌梗死患者还有待确定，但是尼可地尔作为β-受体阻滞剂仍被认为是治疗稳定型心绞痛患者缓解心绞痛的安全药物。
