An Innovative Concept of High-Dose-Rate (HDR) Intracavitary Brachytherapy with an Intrauterine Inflatable Balloon for Endometrial Carcinoma

Purpose: Curative radiation therapy is an established treatment option for non-surgical patients with early-stage endometrial carcinoma. Dosimetric analyses were performed using a single tandem, double tandem, Heyman capsules, and an inflatable intrauterine balloon to assess the dose homogeneity and conformality in the definitive treatment of inoperable endometrial cancer. Methods and Materials: Patients’ informed-consent was obtained. Dosimetric analyses were performed using four different after-loading applicators to assess the dose homogeneity and conformality of isodose to the three-dimensional (3-D) shape of the target volume (uterus) based on CT data in four patients (n = 4). The single tandem and double tandems were standard Fletcher-type (Nucletron Corporation, Columbia, MD). Heyman capsules were the disposable after-loading type (Radiation Products Design, Inc., Albertville, MN). The inflatable balloon with a central bi-lumen catheter was the Mammo Site Radiation therapy System (Proxima Therapeutics, Alpharetta, GA) that is currently used for local breast brachytherapy. Treatment planning and dosimetric analyses for all four techniques were done with HDR PLATO Brachytherapy (v14.2.3) Software (Nucletron Corporation). Results: The average dose gradient within the target (uterine wall) is highest with the tandem methods, followed by Heyman capsules. The intrauterine balloon method showed the least dose gradient across the uterine wall. The corresponding average homogeneity indices were 3.81, 3.83, 2.97, 2.50 for single tandem, double tandem, Heyman capsules, and intrauterine balloon respectively. Conclusions: The intra-uterine inflatable balloon appears to have the best overall dosimetric advantages for the treatment of the uterine wall. Furthermore, the potential ease of use, shorter time of applicator placement, and better patient comfort warrant further investigation and subsequent clinical implementation.

A survey on bug-report analysis

Bug reports are essential software artifacts that describe software bugs, especially in open-source software. Lately, due to the availability of a large number of bug reports, a considerable amount of research has been carried out on bug-report analysis, such as automatically checking duplication of bug reports and localizing bugs based on bug reports. To review the work on bug-report analysis, this paper presents an exhaustive survey on the existing work on bug-report analysis. In particular, this paper first presents some background for bug reports and gives a small empirical study on the bug reports on Bugzilla to motivate the necessity for work on bug-report analysis. Then this paper summaries the existing work on bug-report analysis and points out some possible problems in working with bug-report analysis.

Case Series of Methylene Blue Injections for the Treatment of Zygapophysial and Sacroiliac Joint Pain: Results of 5 Cases

Introduction: Shealy is credited with the use of fluoroscopy guided radiofrequency ablation (RFA) for denervation of the lumbar zygapophysial (L-Z) joint in the mid-1970’s. Peng and colleagues described the use of methylene blue (MB) injection for discogenic pain;MB may work through direct neurotoxicity or inhibition of nitric oxide synthesis. We present a retrospective case series of five patients with L-Z or SI joint pain where MB used as an alternative to RFA. Materials and Methods: This is a retrospective case series of five patients who received MB for the chemical neurolysis of either medial or/and lateral branch’s for zygapophysial or SI joint pain. All of the MB blocks were done under fluoroscopic guidance and performed in the same manner as diagnostic medial and lateral branch blocks but with care to consider the medication as neurotoxic in relation to spinal nerves. On average 1cc of 0.05% final concentration MB was injected per nerve or sacral foraminal level. Results: Four of the five patients had significant lasting pain relief. Discussion: Chemical neurolysis has potential to achieve technical success in these refractory cases of spine pain possibly due to spread over a broader area;for this same reason the procedure should be performed carefully under fluoroscopic guidance.

BACKGROUND Radiocarpal dislocations are rare but potentially devastating *** outcomes are associated with inadequate or lost reduction,such as ulnar translocation,but no consensus exists on the ideal fixation *** bridge plate fixation has been described for various settings in the treatment of complex distal radius fractures and can be fixed distally to the second or third metacarpal,but its application for radiocarpal dislocations has not been *** To determine whether distal fixation to the second or third metacarpal *** Using a cadaveric radiocarpal dislocation model,the effect of distal fixation was studied in two stages:(1)A pilot study that investigated the effect of distal fixation alone;and(2)a more refined study that investigated the effect of described techniques for distal and proximal *** were measured in various parameters to determine the quality of the reduction *** The pilot study found that focusing on distal fixation alone without changing proximal fixation results in ulnar translocation and volar subluxation when fixing distally to the second metacarpal compared with the *** second iteration demonstrated that anatomic alignment in coronal and sagittal planes could be achieved with each *** In a cadaveric radiocarpal dislocation model,anatomic alignment can be maintained with bridge plate fixation to the second metacarpal or the third metacarpal if the described technique is *** considering dorsal bridge plate fixation for radiocarpal dislocations,the surgeon is encouraged to understand the nuances of different fixation techniques and how implant design features may influence proximal placement.

Electronic transport and magnetic performance of Ni-Nb-Zr metallic glasses

The electronic transport and magnetic perfor- mances of Ni-Nb-Zr metallic glasses under low temperature were investigated. X-ray diffraction （XRD） analysis indicates that the Ni-Nb-Zr ribbons prepared by induction melting and melt spinning method have single amorphous phase. The electrical transport property results of these metallic glass alloys indicate that the Ni-Nb-Zr metallic glasses have negative resistivity temperature coefficients （RTC） and low- temperature drift. The negative RTC and corresponding pos- sible values of Fermi energy of Ni-Nb-Zr metallic glasses are interpreted by the extended Ziman-Faber diffraction theory. The magnetic measurements indicate that the magnetic in- tensities of the Ni-Nb-Zr metallic glasses are related to Ni content and atomic sizes while the low temperature has few influences on the magnetic properties.

A Comparison between Three-Dimensional Conformal Radiotherapy, Intensity-Modulated Radiotherapy, and Volumetric-Modulated Arc therapy Techniques for Stereotactic Body Radiotherapy of Lung Tumors

Introduction: The aim of this study is to dosimetrically compare 3D CRT, IMRT, and VMAT techniques that employ coplanar and non-coplanar beams for the SBRT of lung tumors. Methods and Materials: Nine (n = 9) consecutive SBRT lung patients with ten tumor sites who were previously treated at our institution were selected for this study. Six (n = 6) treatment plans were created for each PTV: 1 coplanar and 1 non-coplanar 3D CRT, IMRT, and VMAT such that 98% of PTV received 100% of prescription dose, Rx of 50 Gy in 5 fractions. The data collected from each plan included the conformity index, R50%, and homogeneity index of the target as well as the volume of normal tissue irradiated by 5 Gy, 25 Gy isodose lines, V5 and V20 of total lung, and maximum dose to organs at risk. Beam angles and arc lengths were chosen in order to achieve the lowest total lung V5, V20 and R50% values. Results: According to the observed data, the mean total lung V5 and V20 values were lowest for the non-coplanar VMAT plans, but were not statistically different from the other planning techniques. Conformity values were similar for the IMRT and VMAT plans, and significantly lower than the 3D CRT plans. R50% values were lowest for the VMAT plans and significantly lower than both IMRT and 3D CRT plans in both coplanar and non-coplanar beam arrangements. However, dose homogeneity in the PTV is significantly higher in the IMRT coplanar plans than the corresponding 3D CRT and SmartArc (SA) coplanar plans. Coplanar VMAT plans were able to produce a significant 35.5% reduction in the maximum cord dose than coplanar 3D CRT plans

Dear Editor,Tumor-associated macrophages(TAMs)are critical pro-tumor immunocytes and depletion of TAMs has been exploited for cancer therapy.1 However,the phenotypes and functions of TAMs are plastic,TAMs can also be effector cells by engulfing tumor cells and recruiting cytotoxic T cells,thus shaping the actions of TAMs is more scientific rational than depleting them indiscriminately.2 To promote the entry of reorienting TAMs into the clinical treatments of tumors,it is essential to explore the underline mechanisms controlling the anti-and pro-tumor activities of TAMs.

MRI-Based Patient-Specific Left Ventricular Model to Determine the Effects of Trabeculae Carneae on the Diastolic and Systolic Functions

Introduction Trabeculae carneae are irregular structures that cover the endocardial surfaces of both ventricles of human heart and account for a significant portion of the ventricular ***,the role of trabeculae carneae in left ventricular(LV)function is not well *** reports suggested that trabeculae help squeeze blood from the apical region during systole[1].Our recent study suggests that trabeculae carneae hypertrophy and fibrosis contribute to increased LV stiffness in patients with diastolic heart failure,and severing free-running trabeculae carneae may improve diastolic compliance of the LV[2].Objective To understand the role of trabeculae carneae in the left ventricular diastolic and systolic functions using anatomically detailed patient-specific finite element models of the human ***(1)Image acquisition An explanted human heart was collected from a 63 year old female donor with a history of stroke and congestive heart failure within 24 hours postmortem from South texas Blood and Tissue center(san antonio,TX).The heart was de-identified in accordance with Institutional Review Board(IRB)requirements and informed consent for research was obtained from the donor’s ***-dimensional MRI scanning was conducted on a 3T(128 MHz)MRI system(TIM Trio,Siemens Medical Solutions),comprised of a superconducting magnet with a 60 cm diameter accessible bore,when the heart was submerged in a saline filled plastic container.(2)Finite element analysis Three distinct LV models were derived from the MR *** first model was the intact trabeculated model(TM)which contained all trabeculae carneae and papillary *** high-resolution anatomically detailed 3D model of the LV was segmented from 2D MR images in DICOM format using Mimics(Materialise NV,Leuven,Belgium).The second model was the papillary model(PM),in which the papillary muscles remain intact but most of the trabeculae carneae were excluded in the smoothing *** third

Vaccination with the Live Attenuated FrancisellanovicidaMutant FTN0109Protects against Pulmonary Tularemia

Francisella tularensis is considered a potential bioterrorism agent due to its low infectious dose, high mortality rate, and ability to be spread via the aerosol route. We characterized the F. tularensis subspecies novicida mutant strain FTN0109 as a potential vaccine candidate against tularemia. This strain, which lacks an outer membrane lipoprotein, is attenuated in vitro and in vivo, as it exhibits reduced replication within murine J774 macrophages and has a pulmonary LD50 in BALB/c and C57BL/6 mice of >105 CFU (compared to WT parental strain U112, LD50 FTN0109 also conferred complete protection in BALB/c mice against subsequent pulmonary challenge with 10 LD50 (60,000 CFU) of the murine virulent Francisella strain LVS. We also have demonstrated partial protection (50%) against the highly human virulent subspecies tularensis strain SCHU S4 (25 LD50, 12,500 CFU) following intratracheal vaccination in the Fischer 344 rat, a second rodent model for tularemia. Overall, our results suggest that FTN0109 serves as a potential putative vaccine candidate against pulmonary tularemia.
