Place-and Age-Responsive Disaster Risk Reduction for Hong Kong:Collaborative Place Audit and Social Vulnerability Index for Elders

This study reformulates the concept and contents of disaster risk reduction(DRR) in Hong Kong through an explorative study on collaborative place audit(CPA) and social vulnerability index(SVI) for elders.We believe that DRR should be place-and age-responsive.Accordingly,DRR needs to go beyond technical concerns and address vulnerability and risk encountered in the built environment where an individual is located.A place-centered DRR begins with an assessment of person–environment relations from an interdependent perspective.Community becomes a significant scale at which to address vulnerability and risks across a range of environmental,socioeconomic,and institutional factors.A CPA is a ground-level assessment tool that identifies vulnerability and risk in the built and social environment.The audit encourages collaboration in problem solving that uses social capital to effect decisionmaking change in hierarchies and policy networks.Ageresponsive DRR facilitates distinguishing living-alone elders from the general population.This perspective addresses varying degrees of vulnerability due to social and communicational isolation,poverty,disability,being sent to hospital and/or receiving institutional care,as well as lack of access to primary care.Accordingly,SVI,based oncompound indicators,is developed to assess the differentiation of vulnerability across the territory with particular reference to the elders.These two approaches,namely,CPA and SVI,build community capacity to develop a resilient city,as well as to provide evidence-based recommendations that improve government-led disaster preparedness and contingency plans.

Life skill-based training is essential for intellectual disabled individuals to regain autonomy and social inclusion.Virtual reality(VR)could deliver a nascent application that is more engaging and secure.The objective of this study was to evaluate the training effects of the VR-based multiple life skill training program on life skill perfor-mance,self-efficacy,memory,cognitive and behavioral functions via a multicenter randomized controlled trial.A total of 145 intellectual disabled participants were recruited for a randomized controlled trial with three in-tervention arms:VR(n=42);traditional(n=53);and control(n=50).The life skill tasks for the interventions included 1)grocery shopping,2)cooking,and 3)kitchen cleaning.Outcome measures were performance scores for the three tasks,self-efficacy scale,digit span score,and Frontal Assessment Battery score,graded by blinded assessors.Before-after effects of each group were evaluated using separated Wilcoxon Signed-rank tests.VR sig-nificantly improved cooking,cleaning performance,and memory span,while traditional training significantly improved shopping,cooking,and cleaning tasks.The generalized estimating equation evaluated effects between groups adjusted for age,gender and intelligence quotient(IQ).VR had significantly larger improvement effects in cooking and cleaning than the control group and memory span compared to traditional training and controls groups.IQ appeared to be a significant confounder on the training effect.Future work may consider developing artificial intelligence to customize programs depending on IQ level.

Ma'an Qiao Village, a Dai and Yi ethnic minority-based community in Sichuan Province, China sustained complete infrastructure devastation during the 2008 Panzhihua earthquake. Health emergency and disaster risk management(Health-EDRM) education intervention programs were implemented in 2010 and 2011. This serial cross-sectional survey study aimed to examine the immediate and long-term impacts of the Health-EDRM interventions in this remote rural community. The findings demonstrate knowledge improvement in areas of water and sanitation, food and nutrition, and disaster preparedness immediately after the Health-EDRM education interventions. Temporal stability of knowledge retention was observed in household hygiene and waste management and smoking beliefs in 2018, 7 years after the interventions.Other important findings include knowledge uptake pattern differences of oral rehydration solution(ORS) between earthquake-prone and flood-prone communities. Usage of Internet and mobile technology for accessing disaster-related information was found to be independent of gender and income. Overall, this study demonstrated the knowledge improvement through Health-EDRM education interventions in a remote rural community. Promoting behavioral changes through interventions to raise awareness has the potential to reduce health risks in transitional post-disaster settings. Future programs should aim to identify evidence-based practices and explore how technology can support Health-EDRM education among vulnerable subgroups.

Impact of climate change on dengue fever epidemics in South and Southeast Asian settings:A modelling study

The potential for dengue fever epidemic due to climate change remains uncertain in tropical areas.This study aims to assess the impact of climate change on dengue fever transmission in four South and Southeast Asian settings.We collected weekly data of dengue fever incidence,daily mean temperature and rainfall from 2012 to 2020 in Singapore,Colombo,Selangor,and Chiang Mai.Projections for temperature and rainfall were drawn for three Shared Socioeconomic Pathways(SSP126,SSP245,and SSP585)scenarios.Using a disease transmission model,we projected the dengue fever epidemics until 2090s and determined the changes in annual peak incidence,peak time,epidemic size,and outbreak duration.A total of 684,639 dengue fever cases were reported in the four locations between 2012 and 2020.The projected change in dengue fever transmission would be most significant under the SSP585 scenario.In comparison to the 2030s,the peak incidence would rise by 1.29 times in Singapore,2.25 times in Colombo,1.36 times in Selangor,and>10 times in Chiang Mai in the 2090s under SSP585.Additionally,the peak time was projected to be earlier in Singapore,Colombo,and Selangor,but be later in Chiang Mai under the SSP585 scenario.Even in a milder emission scenario of SSP126,the epidemic size was projected to increase by 5.94%,10.81%,12.95%,and 69.60%from the 2030se2090s in Singapore,Colombo,Selangor,and Chiang Mai,respectively.The outbreak durations in the four settings were projected to be prolonged over this century under SSP126 and SSP245,while a slight decrease is expected in 2090s under SSP585.The results indicate that climate change is expected to increase the risk of dengue fever transmission in tropical areas of South and Southeast Asia.Limiting greenhouse gas emissions could be crucial in reducing the transmission of dengue fever in the future.

The role of gut microbiota in modulating the durability of cOVID-19 vaccine immunity is yet to be characterised.In this cohort study,we collected blood and stool samples of 121 BNT162b2 and 40 CoronaVac vaccinees at baseline,1 month,and 6 months post vaccination(p.v).Neutralisation antibody,plasma cytokine and chemokines were measured and associated with the gut microbiota and metabolome composition.

The Relationship with Previous Use of Community-Based Services and Informal Caregiver Support and Elderly and Informal Caregivers’ Views of Living Arrangement: Analysis of Official Statistics from 2004 to 2014

Background and Objectives: Views on living arrangement from elderly and informal caregivers are crucial to “ageing in place”. They might be related to the experience in the use of elderly care services, which remains inconclusive in previous literature. This study aimed to explore the association of previous experience in formal and informal long-term care services with views of both elderly and their informal caregivers on living arrangement. Research Design and Methods: This study adopted a cross-sectional design. Assessment records of Minimum Data Set-Home Care for community-dwelling elderly who were eligible for subsidized long-term care services in Hong Kong from 2004 to 2014 were made available. Multivariate logistic regression was applied to examine associations between both views on elder’s living arrangement from the elderly, caregivers and their previous informal caregiver support, and experience in formal care services. Results: 82,306 dyads of elderly and informal caregivers were included in the analysis. The elderly with previous use of home and community-based services (OR = 0.84, 95% CI 0.80 - 0.88) and informal caregivers (OR = 0.78, 95% CI 0.76 - 0.81) believed that the elderly should live away from home. Temporal trends of fewer elderly and caregivers supporting the idea of living away from home were also observed. Discussion and Implications: The results highlighted the importance of informal caregiver’s support and previous utilization of formal home and community-based services. It was concluded that resources and information of community-based care have a significant association with views on living at home also proper support services and training of care for the elderly should be made available to informal caregivers to reduce their burden.

Objectives To identify clinical risk factors and retinal characteristics and evaluate a risk model that predicts 1-year recurrence after ischemic stroke.Method From June 2017 to January 2019, 332 patients with first-ever ischemic stroke were enrolled and followed in the Shenzhen Traditional Chinese Medicine hospital in the China. The primary end point was defined as fatal or nonfatal recurrent stroke after 1 year of the index stroke.Clinical risk factors and retinal characteristics were identified by a multivariate logistic model and further evaluated by latent class analysis. Results The multivariate logistic model with only clinical risk factors showedthat Cerebral Atherosclerosis(OR 1.657, 95%CI: 1.000–2.805), white matter lesions(OR 3.607, 95%CI:2.176–5.977), and Cardiac disease(OR 1,883, 95%CI: 1.024–3.463) were statistically significant associated with higher stroke recurrence risk. The sensitivity of the model was 69.1%, specificity was 68.4%, and overall accuracy percentage was 68.6%. The multivariate logistic model with only retinal characteristics showed that CRVE(OR 0.447, 95%CI: 0.230–0.871), hemorrhage(OR 0.004, 95%CI: 0.000–0.256), exudate(OR 22.978,95%CI: 2.552–206.884), CRAE(OR 2.190, 95%CI: 1.161–4.130), and Aangle(OR 0.889, 95%CI: 0.777–1.019)were associated with stroke recurrence. The sensitivity of the model was 62.0%, specificity was 64.4%, and overall accuracy percentage was 63.4%. The multivariate logistic model with both clinical risk factors and retinal characteristics showed that carotid artery stenosis or occlusion(OR 1.744, 95%CI: 1.020–2.981), white matter lesions(OR 3.648, 95%CI: 2.171–6.131), Cardiac disease(OR 1,990, 95%CI: 1.057-3.744), Hemorrhage(OR0.016, 95%CI: 0.000–0.649), exudate(OR 24.146, 95%CI: 2.514–231.899) were independent risk factors of stroke recurrence(table 4). The sensitivity of the model was 82.6%, specificity was 56.8%, and overall accuracy percentage was 64.2%.Conclusions Combining the traditional risk factors of stroke with the retinal vessels characteristics
