Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

The causes and the problems of integration of scientific ＆ technical innovation （ISTI） and inevitability are proposed. The status of domestic scientific ＆ technical innovation is analysed. A series of emergent managerial problems caused by the system ISTI （SISTI） are pointed out. Because of Hall three dimensions structure＇s enlightenment, a three dimensional logical net diagram in which the system integration of innovation based on problem in real world but not on disciplinary logic is drawn. The intelligent group＇ s need span is greater under the conditions of relatively low material level than common groups who have lower degree of education. The SISTI is composed by multi-rule and intelligent multi-agent behavior. It is concluded that the logical relationships of integrated technology which based on the author＇s experience and observation must be considerded for a valid management of SISTI.

But the financial regulatory system is a large system,and the process of model building and index selection is very complicated. A model for China's financial regulation and supervision system was proposed and a system evaluation method was improved. A more exact result was drawn by choosing 2003- 2009 index data.

We demonstrate that there is significant skill in the GloSea5 operational seasonal forecasting system for predicting June mean rainfall in the middle/lower Yangtze River basin up to four months in *** of the rainfall in this region during June is contributed by the mei-yu rain *** find that similar skill exists for predicting the East Asian summer monsoon index(EASMI)on monthly time scales,and that the latter could be used as a proxy to predict the regional ***,there appears to be little to be gained from using the predicted EASMI as a proxy for regional rainfall on monthly time scales compared with predicting the rainfall *** interannual variability of the June mean rainfall is affected by synoptic and intraseasonal variations,which may be inherently unpredictable on the seasonal forecasting time scale,the major influence of equatorial Pacific sea surface temperatures from the preceding winter on the June mean rainfall is captured by the model through their influence on the western North Pacific subtropical *** ability to predict the June mean rainfall in the middle and lower Yangtze River basin at a lead time of up to 4 months suggests the potential for providing early information to contingency planners on the availability of water during the summer season.

China＇s FengYnn 3 （FY-3） polar orbiting satellites axe set to become an important sonrce of observational data for nu- merical weather prediction （NWP）, atmospheric reanalyses, and climate monitoring studies over the next two decades. As part of the Climate Science for Service Partnership China （CSSP China） prograln, FY-3B Microwave Humidity Sounder 1 （MWHS-1） and FY-3C MWHS-2 observations have been thoroughly assessed and prepared for operational assimilation. This represents the first time observations from China＇s polar orbiting satellites have been used in the UK＇s global NWP model. Since 2016, continuous data quality monitoring has shown occasional bias changes found to be correlated to changes in the energy supply scheme regulating the platform heating system and other transient anomalies. Nonetheless, MWHS-1 and MWHS-2 significantly contribute to the 24-h forecast error reduction by 0.3% and 0.6%, respectively, and the combination of both instruments is shown to improve the fit to the model background of independent sounders by up to 1%. The observations from the Microwave Radiation Imager （MWRI） also are a potentially significant source of benefits for NWP models, but a solar-dependent bias observed in the instrument half-orbits has prevented their assimilation. This paper presents the bases of a correction scheme developed at the Met office for the purpose of a future assimilation of MWRI data.
