The main contribution of this paper is the development and demonstration of a novel methodology that can be followed to develop a simulation twin of a railway track switch system to test the functionality in a digital *** is important because,globally,railway track switches are used to allow trains to change routes;they are a key part of all railway ***,because track switches are single points of failure and safety-critical,their inability to operate correctly can cause significant delays and concomitant *** order to better understand the dynamic behaviour of switches during operation,this paper has developed a full simulation twin of a complete track switch *** approach fuses finite element for the rail bending and motion,with physics-based models of the electromechanical actuator system and the control ***,it provides researchers and engineers the opportunity to explore and understand the design space around the dynamic operation of new switches and switch machines before they are *** is useful for looking at the modification or monitoring of existing switches,and it becomes even more important when new switch concepts are being considered and *** simulation is capable of running in real time or faster meaning designs can be iterated and checked *** paper describes the modelling approach,demonstrates the methodology by developing the system model for a novel“REPOINT”switch system,and evaluates the system level performance against the dynamic performance requirements for the *** the context of that case study,it is found that the proposed new actuation system as designed can meet(and exceed)the system performance requirements,and that the fault tolerance built into the actuation ensures continued operation after a single actuator failure.

A Novel guide extension assisted stenting technique for coronary bifurcation lesions

A challenging technical scenario frequently encountered in a percutaneous coronary intervention of a coronary bifurcation lesion(CBL)is stent implantation of only the stenosed segment without compromising the other two normal segments in non-true bifurcation *** is precise stent implantation covering the side branch ostium without leaving excessive stent metal at the other two segments of a bifurcation lesion in complex true bifurcation *** aim of this study was to describe a novel stenting technique for both non-true and true CBLs by using a guide extension catheter(GuideLiner).With the assistance of a guide extension catheter mounted on both the main and the side-branch guidewires and with its intubation down to the bifurcation carina,a stent can be implanted in the side branch segment or distal main segment of the bifurcation lesion appropriately without compromising the other two segments of the coronary *** implantation is described in three bifurcation lesions in three cases and shown in detail with illustrative *** technique facilitates side-branch only stenting in side-branch mono-ostial(medina 0,0,1)CBL or only the distal main segment in distal mono-ostial(medina 0,1,0)CBL without compromising the other two remaining segments when using the onestent technique in non-true CBLs without leaving unnecessary excessive stent metal at the bifurcation site and when using a two-stent technique in complex true bifurcation lesions(tri-ostial or medina 1,1,1).Consequently,through optimizing stent deployment,the technique may have the potential to reduce the risk of subacute stent thrombosis and future in-stent *** most appropriate lesions suitable for the technique,and some other practical tips are also described.

The Application of Time Series Modelling and Monte Carlo Simulation: Forecasting Volatile Inventory Requirements

During the assembly of internal combustion engines, the specific size of crankshaft shell bearing is not known until the crankshaft is fitted to the engine block. Though the build requirements for the engine are consistent, the consumption profile of the different size shell bearings can follow a highly volatile trajectory due to minor variation in the dimensions of the crankshaft and engine block. The paper assesses the suitability of time series models including ARIMA and exponential smoothing as an appropriate method to forecast future requirements. Additionally, a Monte Carlo method is applied through building a VBA simulation tool in Microsoft Excel and comparing the output to the time series forecasts.

目的:对递增负荷的Bruce跑台测试方案给出的代谢当量(METs)与运动巾根据摄氧量(VO_2)计算的METs进行比较,旨在为心肌梗死后(PMI)患者提供不同康复时期METs的预测。方法:101名男性PMI患者参加了12周有氧多样化运动康复程序,程序前、后对其气体代谢、心肺机能、运动能力等指标进行测试。结果:康复程序前后受试者安静时的VO_2分别为3.9ml/kg·min和3.8ml/kg·min。运动中的METs与跑台测试方案给出的METs及以安静状态VO_2为3.5ml/kg·min(正常成人)时计算得出的METs均有差异。本研究为PMI患者提供的预测METs方法分别为:康复程序前,METs=3.2+1.07跑台等级,或METs=-7.4+0.12HR(心搏次数/min);康复程序后,METs=1.9+1.04跑台等级,或METs=-6.4+0.11HR(心搏次数/min).结论:本研究建立的HR与METs的相关关系,有助于指导患者确定运动的适宜强度。

background and purpose We present the long-term outcome after endovascular treatment of symptomatic intracranial posterior circulation *** 30 patients with symptomatic intracranial posterior circulation stenoses exceeding 70% underwent endovascular treatment between 2006 and *** regarding presentation,follow-up,procedure details,complications and imaging follow-up were *** surviving patients underwent a phone interview to establish their current Modified Ranking Scales(MRS).results Stenoses of the intracranial vertebral artery(24 patients)and basilar artery(6 patients)were treated with stents(10 patients),angioplasty alone(13 patients)or both(5 patients).Two procedures *** patient(3.3%)died after the procedure,two had stroke(6.6%)and one a subarachnoid haemorrhage without ensuing *** patients(6.7%)had asymptomatic complications(dissection and pseudoaneurysm).The median clinical follow-up time was 7 *** the 29 patients who survived the procedure,6 died due to unrelated *** patients(10%)had recurrent strokes and two(6.7%)a transient ischaemic attack in the posterior *** patients had subsequent middle cerebral artery ***(16.7%)patients had recurrent stenoses and three(10%)occlusions of the treated *** was performed in six patients,three(10%)with PTA and three(10%)with *** MRS scores were as follows:nine MRS 0,eight MRS 1,four MRS 2 and one MRS *** Long-term follow-up after endovascular treatment of high-risk symptomatic intracranial posterior circulation stenoses shows few stroke *** of intracranial vertebral artery stenosis may be beneficial in appropriately selected patients.

目的:测试服用和未服用β-受体阻断剂的心梗后患者(PMIP)在跑台运动中其功能、生理、临床及自我感觉方面的反应。方法:46名服用β-受体阻断剂的男性PMIP和55名未服用β-受体阻断剂的男性PMIP进行递增负荷运动实验,其间记录每级负荷时的摄氧量(VO2)、心率(HR)、血压和自我用力感觉(RPE),并持续监测12导联心电图(ECG)。结果:服用β-受体阻断剂的患者其安静时和运动中的HR、最大心率百分比(%HRmax)及心率—血压乘积(RPP)显著低于未服用β-受体阻断剂的患者(P<0.01)。运动中,服用β-受体阻断剂的患者有59%能够完成改良布鲁斯跑台方案的第级运动,而在未服用β-受体阻断剂的患者中仅有49%能完成这一负荷。两组在运动中各级别VO2、RPE及ST段下移无显著性差异。结论:由β受体阻断剂所致的HR降低并没有直接地影响到氧利用和代谢机能。

Introducing the Principles of Tendon Transfer for Surgical Trainees to Improve Anatomical Knowledge

This article reviewed the principles and outcomes of tendon transfer procedures described in the literature to restore function following injuries delivered in a workshop as a way of improving basic science and anatomical knowledge in surgical trainees preparing for surgical examinations. Post intervention surveys showed an improvement in trainees’ familiarity with musculoskeletal anatomy and engagement in learning with improved readiness for surgical examinations.

目的 :探讨预测心肌梗死患者 (PMIP)急性期后康复运动时心肌缺血的方法 ,并为他们参加康复锻炼提供适宜的运动强度。方法 :通过对 112名PMIP进行运动实验 ,将受试者分为 :服用β -阻断剂 (blockade)心电图显示ST段下降≥ 1mm组 ;服用 β -阻断剂 (blockade)ST段下降 <1mm组 ;未服用 β -阻断剂 (non -blockade)ST段下降≥ 1mm组 ;未服用 β -阻断剂 (non -blockade)ST段下降 <1mm组。并测试受试者运动前血胆固醇 (BC)、血乳酸 (BL)、心率 (HR)及运动中BL、HR、主观运动强度 (RPE)和ST段下降数值。结果 :( 1)运动心肌缺血阳性者安静时BC、BL显著高于阴性者 ,HR只在RPE为 13时二者出现显著性差异。 ( 2 )未服用 β -阻断剂与服用 β -阻断剂患者在每级运动强度的HR均存在显著差异。 ( 3)本研究确定安静时BC、BL、RPE为 13时HR作为预测运动心肌缺血的联合靶数值。 ( 4 )运动心肌缺血阳性者运动中在RPE为 15时出现血乳酸堆积 ,阴性者则在RPE为 17时才出现。结论 :本研究建立的联合靶数值预测PMIP发生运动心肌缺血的准确度评估方法的敏感性和特异性均超过 83% ,为国际认可的高敏感和高特异性。运动心肌缺血预测阳性的PMIP康复期的运动强度不宜超过RPE 13,预测阴性的PMIP的运动强度可控制在REP 15之内。
