Case Study: EIMS acupuncture Treatment for Posterior Tibialis Tendonitis

BACKGROUND AND OBJECTIVE: Posterior tibialis tendon dysfunction (PTTD) is a progressive ailment of a lower limb that, if left untreated, can lead to irreversible changes in the structure and function. The authors of the present study hypothesized the damages to the tibias tendon can be countered and reversed using electrical acupuncture and concurrent treatment of the associated muscle. INTERVENTION: The following study details a successful treatment of the early stage PTTD in a group of competitive athletes using a specific Electrical Intra Muscular Stimulation (EIMS) protocol developed and rooted in years of practice and the knowledge of the pathophysiology of the muscle and tendon tissues. Specifically, a combination the posterior and the original EIMS approach was used to stimulate the tendon and muscle concurrently at a frequency of 2 Hz. Analysis of the VAS scores showed a significant improvement following the treatment sessions. CONCLUSION: The success of this combined muscle/tendon stimulation protocol is given a strong correlation between the mechanism of tendon and muscle injury and recovery. The fast rate of healing and the simplicity of the materials required for the treatment offer a great potential for a wide-spread application. The authors extrapolate that the treatment method, with applicable modifications in needling locations, can be used for an array of common ailments involving tendons and muscles of the lower limb and foot, including the over-pronation, which is now common place due to the overweight problem in the general population.

通过阳极氧化法在Ti基体上垂直定向生长TiO2纳米管,其直径为60-80nm,长度为4μm。采用基体应变测试方法研究TiO2纳米管薄膜裂纹起源、扩展与饱和的情况。结果表明:退火可明显改善TiO2纳米管薄膜/Ti基体界面状况;随着拉伸应变的增加,裂纹扩展迅速加快,在很小的应变范围内达到饱和;TiO2纳米管薄膜室温样、250℃退火样和400℃退火样的界面剪切强度估算值分别为163.3、370.2和684.5MPa,临界能量释放率分别为49.6、102.6和392.7J/m2,断裂韧性分别为0.996、1.433和2.803MPa.m1/2。TiO2纳米管薄膜的界面结合机制为化学键结合。

采用基体应变测试方法研究TiO2纳米管薄膜的屈曲图样。结果表明:TiO2纳米管薄膜室温样、250℃退火样和 400℃退火样的屈曲应变依次为2.5%、8.9%和7.8%,说明退火改善了纳米管薄膜/Ti 基体界面状况。从 SEM 观察可以看出,TiO2纳米管薄膜室温样、250℃退火样和400℃退火样的临界屈曲应力分别为180.4、410.2 和 619.5MPa;从 AFM 观察可以看出,TiO2纳米管薄膜 250℃退火样的临界屈曲应力为470.2MPa,两者吻合得很好。采用AFM计算250℃退火样的真实应力和能量释放率分别为840.3MPa和77.2J/m2,此结果与从裂纹角度计算出的250℃退火样的能量释放率102.6 J/m2吻合得很好。

Effect of General Anaesthesia with Combination of acupuncture and Enflurane Applied in Radical Operation of Laryngocarcinoma

Objective： To observe the effect of general anaesthesia with combination of acupuncture [ conducted with Han＇s acupoint nerve stimulator （HANS） applied] and enflurane in radical operation of laryngocarcinoma （LC）. Methods： Sixty patients with LC of grade Ⅰ -Ⅱ, classified according to the standard of American society of Anesthesiologists （ASA）, were assigned by randomizing number table to the control group and the tested group, 30 patients in each group. The control group received anaesthesia with enflurane alone for inducing and maintaining; the tested group was anaesthetized with enflurane like that given to the control group but also received additionally needling stimulation conducted by HANS. The dosage of enflurane used, the minimum effective concentration of enflurane in alveolar air （MACEnf） and the changes of heart rate （HR） as well as blood pressure （BP） in patients at different time points in the operational process were observed. Results： As compared with those in the control group, in the tested group, both the MACEnf and the dosage of enflurane were reduced, with the difference between the two groups significant （ P〈0.01 ）. The changes of HR and BP among different time points in the tested group were slight, and as compared with those in the control group at the corresponding time points, the difference was significant （ P〈 0. 05 or P〈0.01）. Conclusion： General anaesthesia with combination of enflurane and needling conducted by HANS applied in radical operation of LC has definite effect with less complication. Needling could be cooperated with narcotics, and so it could be taken as an auxiliary measure of anaesthesia for radical operation of LC.

Objective:To systematically review the clinical practice guidelines(CPGs)for the treatment of patients with coronavirus disease 2019(COVID-19)using Chinese herbal medicine(CHM),assess the methodological quality as well as clinical credibility and implementability of specific recommendations,and summarize key ***:As of April 2022,we conducted a comprehensive search on major electronic databases,guideline websites,academic society websites,and government websites to assess the methodological quality and clinical applicability of the included CPGs using the Appraisal of Guidelines for Research and Evaluation(AGREE)II tool and Evaluation-Recommendations EXcellence(AGREE-REX)instructions,***:The search yielded 61 CPGs,which were mostly published in 2020;moreover,98.4%of the CPGs were published in *** five CPGs achieved a high-quality AGREE II rating;further,six CPGs could be directly recommended,with most of the CPGs still showing much room for *** had a low overall score in the AGREE-REX evaluation,with the domains of clinical applicability,values and preferences,and implementability being standardized in 21.80%±12.56%,16.00%±11.81%,and 31.33%±14.55%of the CPGs,*** high-quality CPGs mentioned 56 Chinese herbal *** of the recommendations had moderate or strong evidence level in the GRADE *** most frequently recommended herbal medicines were Lianhua Qingwen granule/capsule and Jinhua Qinggan granule;however,the strength of recommendation for each prescription varied across CPGs and ***:The overall quality of current CPGs for COVID-19 for CHM still needs to be improved;moreover,the strength of the evidence remains to be standardized across CPGs.
