目的 :VEGF C基因的真核表达载体 ,以便进一步研究VEGF C基因在淋巴管生成中的作用。方法 :根据人VEGF C的cDNA序列 ,设计合成一对 5’端分别含有EcoRⅠ和BamHⅠ酶切位点的特异性引物 ,运用RT RCR方法扩增人乳腺癌细胞MDA MB 2 31中的VEGF CcDNA(1 2 6kb) ;回收PCR产物 (1 2 8kb) ,并将其连接至克隆载体pUMT 18中 ,重组的pUMT 18在大肠杆菌DH5α内扩增后 ,经质粒提取、EcoRⅠ和BamHⅠ酶切 ,筛选出阳性重组子 ,并进行基因测序鉴定 ;琼脂糖凝胶电泳回收含有VEGF CcDNA全长的酶切片断 (1 2 7kb) ,然后在DNA连接酶作用下将其与真核表达载体pcDNA3 1(- )连接 ,重组质粒经EcoRⅠ和BamHⅠ酶切予以鉴定。结果 :RT PCR产物含有VEGF CcDNA ,基因测序显示重组的pUMT 18中含有正确的人VEGF CcDNA全长序列 ,重组的pcDNA3.1(- )中含有人VEGF CcDNA全长序列。结论 :VEGF C pcDNA3.1(- )。

Objective. To investigate the role of cytoreductive surgery in patients with nongenital cancers metastatic to the ovaries. Patients and methods. One hundred and fifty-four patients with nongenital cancers metastatic to the ovaries treated in Hacettepe University Hospital, gynecologic Oncology Unit between 1982 and 2004 years were retrospectively evaluated. Data were obtained from patients’ records and pathology reports. Demographic characteristics, prognostic factors, 5- year and median survivals were analyzed in all patients. Results. During study period, nongenital cancers metastatic to the ovaries constituted 9% of all malignant ovarian neoplasms. Primary cancers were breast (35), stomach (35) and colorectal (33) cancers, lymphoma (17), undetermined origin (16), appendix (7), ileum (4), pancreas (3), gallbladder cancer (2) and mesothelioma (2). Of patients, 67% were premenopausal and 33% were postmenopausal. Although most common presenting symptoms were abdominal distension with dyspeptic complaints in 46% , abdominal mass in 22% , and pressure symptoms in 8.4% , 15 patients (10% ) were asymptomatic and were diagnosed in routine control examinations. Total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAH + BSO), omentectomy, and bilateral pelvic and para-aortic lymphadenectomy (BP-PALND) with cytoreduction were performed in 102 patients (66% ), TAH + BSO + omentectomy in 21 patients (14% ), TAH + BSO in 23 patients (15% ), minimal surgical effort including BSO or biopsy in 8 patients (5% ). Eighty-four percent of patients received adjuvant treatment according to the primary origins. Mean follow-up was 47.3 ± 5.9 months. Overall, 5 year survival was 36% and median survival was 42 months. The sentinel node(SN) technique has been established for the treatment of some types of solid cancers to avoid unnecessary lymphadenectomy. If node disease were diagnosed before surgery, minimal surgery with omission of lymph node dissection would be an option for patients with early gastric cancer. Although SN biopsy has been well ascertained in the treatment of breast cancer and melanoma, SN navigation surgery(SNNS) in gastric cancer has not been yet universal due to the complicated lymphatic flow from the stomach. Satisfactory establishment of SNNS will result in the possible indication of minimally invasive surgery of gastric cancer. However, the results reported in the literature on SN biopsy in gastric cancer are widely divergent and many issues are still to be resolved, such as the collection method of SN, detection of micrometastasis in SN, and clinical benefit. The difference in the procedural technique and learning phase of surgeons is also varied the accuracy of SN mapping. In this review, we outline the current status of application for SNNS in gastric cancer.

Objective: To evaluate feasibility and outcome of minilaparotomy hysterectomy in a consecutive series of patients. Study design: Cohort analytic study. From October 1995 to March 2001, 148 out of 228 (65%) consecutive hysterectomies for benign gynecologic disease were performed by an abdominal route. Minilaparotomy hysterectomy (transversal cutaneous incision surgery. Surgical parameters were prospectively assessed in terms of length of incision, operative time, estimated blood loss, duration of ileus, perioperative complications and length of postoperative stay. Results: A minilaparotomic approach was performed in 118 patients (80%). Conversion to Pfannenstiel was necessary in three cases. The minilaparotomy incision, 8 cm (range 6-10) of median length, was performed below the pubic hair line. The median operating time was 50 min (range 34-88). No intraoperative complications or perioperative blood transfusions were reported, while minor postoperative complications occurred in 16 patients (14%). The median postoperative stay was three days (range 2-5). Conclusions: The minilaparotomy hysterectomy is feasible in the majority of women undergoing hysterectomy for be nign disease. Because of the excellent outcome achieved, it should be considered a valid alternative to the classic abdominal hysterectomy.

Objective. At present, cervical cancer remains the only gynecologic tumor, which is staged by clinical examination according to FIGO. This is associated with a high percentage of over-and understaging of tumor extent. With the operative, especially laparoscopic staging, exact information about intraabdominal tumor spread, lymph node metastases, and involvement of adjacent organs is possible. However, the advantage of operative staging is still discussed controversially. The aim of this study is to describe the laparoscopic transperitoneal staging procedure in patients with cervical cancer and their oncologic outcome after primary chemoradiation. Methods. From November 1994 to October 2003, 456 consecutive patients with histologically confirmed primary cervical cancer were admitted to the department of Gynecology of the Friedrich-Schiller-Univer-sity Jena, Germany. Out of these, 84 patients with locally advanced tumor (tumor size ≥4 cm) and/or lymph node involvement and/or tumor infiltration to bladder or rectum were selected by a standardized laparoscopic staging procedure for primary chemoradiation. Data of surgery, chemoradiation, and follow-up were analyzed retrospectively for these patients. Results. The mean age of the patients was 54 years (26-80), and the mean body-mass-index was 24.8 (17.9-42.2). Preoperative clinical evaluation showed a stage distribution according to FIGO with stage IB1 in 15.5%, IB2 in 15.5%, IIA in 8.3%, IIB in 23.8%, IIIA in 8.3%, IIIB in 21.4%, IVA in 6%, and IVB in 1.2%. In 15 out of 84 (17.8%) patients, intraabdominal tumor spread was diagnosed by laparascopy. In 24 out of 84 (28.5%) patients, invasion of bladder and/or rectum was proven histologically after biopsy. In 60 out of 84 (71%) patients, lymph node metastases were confirmed histologically. In 2 out of 13 patients with FIGO-stage Ib1, skip metastases in infrarenal para-aortic lymph nodes were seen. Background: Postoperative nausea and vomiting (PONV) can lead to complications and increased healthcare costs. We investigated whether patient preoperative concern about PONV is associated with postoperative antiemetic use, independently of Apfel score. Methods: Patients eligible for study were English- or Spanish-speaking women with breast cancer undergoing mastectomy, lumpectomy or reconstructive surgery as outpatients during July 2014-July 2017, when the pre-anesthesia clinic routinely screened for preoperative concern via tablet computer-based survey. Excluded were patients who did not rate their concern or lacked Apfel score. Risk factors for concern were evaluated in a multinomial model adjusted for multiple hypotheses. Using generalized linear regression, preoperative concern was tested for association with number of antiemetics administered in the postanesthesia care unit. Results:?Of preoperative surveys, 7.1% (58/812) were excluded for missing data, leaving n = 754 surveys contributed by n = 706 subjects (age 26 - 80 years). Patient preoperative concern ranged from none (32.8%), mild (30.2%), moderate (22.9%), severe (7.8%), to very severe (6.2%). Adjusted for age, concern was increased by history of motion sickness (Odds Ratio 1.51, 95% Confidence Interval 1.11 - 2.06) and history of PONV (9.02, 6.30 - 12.90) and decreased by prior surgery without PONV (0.35, 0.23 - 0.53) and Spanish as primary language (0.42, 0.25 - 0.68). Number of postoperative antiemetics, usually 1 (41.2%) or 2 (33.4%) drugs, was unassociated with preoperative concern before or after adjustment for Apfel score. Conclusions: Among women undergoing breast cancer-related surgery, preoperative concern about PONV varies by prior history of PONV and motion sickness and by ethnicity. However, preoperative concern is not associated with postoperative antiemetic medications.

Objective To evaluate the results of abdominal myomectomy for intramu ral and s ubserosal fibroids and to identify factors that influence the reproductive outco me after surgery. Design Retrospective analysis of a case series. Setting An aca demic department specializing in gynecologic surgery. Patient(s) A total of 72 w omen with intramural and subserosal fibroids submitted to abdominal myomectomy w ho wished to conceive after surgery. Intervention(s) Data were collected on clin ical characteristics, surgical features, and obstetric history before and after surgery. Main outcome measure(s) Conception rate, pregnancy loss, and live birth rate before and aft er surgery, as well as postoperative probability of conception according to sele cted clinical and fibroid characteristics. Result(s) Conception rate was 28%bef ore myomectomy and 70%after surgery. The corresponding figures were 69%and 25 %for pregnancy loss and 30%and 75%for live birth rate, respectively. Age < 30 years and number of fibroids removed were the only significant and independent predictors of obstetric outcome by multivariate analysis. Conclusion(s) Our resu lts suggest that abdominal myomectomy might improve reproductive outcome in pati ents with intramural and subserosal fibroids. The reproductive performance was p articularly good when the patientswere younger than 30 years and had a single my oma to remove.
