Burnout among Caregivers in Four Maternities in brazzaville

Introduction: The professional exhaustion syndrome, also referred to as burnout, combines deep fatigue, disinvestment in professional activity, a feeling of failure and incompetence at work resulting from the chronic constraints suffered by the worker. The purpose of this work is to assess its prevalence among caregivers of the maternity wards in the district hospitals of brazzaville. Methodology: This is a descriptive cross-sectional study carried out in four maternities of respectively Bacongo, Makélékélé, Mfilou and Talangai hospitals in brazzaville between February 1st and February 28th, 2021. Socio-demographic and professional characteristics were collected from a sample of midwives and nurses. The level of exhaustion was evaluated using the “Maslach Burnout Inventory” scale defined as “high” when the 3 dimensions are reached, “moderate” when only 2 are reached, and low for any other result. Results: Among the 143 caregivers investigated (62.9%) of respondents declared being in burnout. According to the dimensions of the Maslach Burnout Inventory scale, 53.1% are caregivers are in high emotional exhaustion;53.1% have a high level of depersonalization and 75.5% have a low level of personal accomplishment. Workload (OR = 13.69;[4.28 - 61.27];p Conclusion: In our study, all dimensions of burnout were high. It is therefore a real phenomenon among caregivers in the four maternities of the district hospitals of brazzaville. The associated factors can be avoided through prevention.

Psychological Experience of Children and Adolescents with Homozygous Sickle Cell Disease in brazzaville

Introduction: Sickle cell disease, the most frequent hemoglobinopathy, is one of many causes of psychological repercussions. Objectives: To determine the prevalence of psychological disorders in children/adolescents living with sickle cell disease and to identify the associated factors. Patients and Method: An analytical cross-sectional study was conducted from June to September 2019 at the national sickle cell center and at the mother-child consultation of the University Hospital of Brazzaville. Children/adolescents aged six to 19 years old followed for sickle cell disease were included. Psychological disorders were assessed using the Diagnostic and Statistical Manual of Mental Disorders “DSM-5” which assesses depression and anxiety disorders, the Rosenberg Self-Esteem Scale, and the Brief Illness Perception Questionnaire which assesses representations of chronic diseases. SPSS 20.0 software was used for statistical analysis.

Etiologies of Chronic Renal Failure in Adolescent Patients at the University Hospital of brazzaville

Objectives:?The aim of our work was to identify the etiologies of chronic renal failure in adolescent patients.?Patients and Methods:?This is a descriptive cross-sectional retrospective study that included adolescents aged 10 to 19 years hospitalized between January 2014 and December 2017 in the department of nephrology at the?University Hospital of brazzaville. The CRF was defined according to the ANAES criteria by an estimated glomerular filtration rate less than 60 ml/min/?1.73?m2?of body surface.?Results:?Forty cases of CRF were collected, representing a prevalence of 4.9%. The average age was 16.32 ± 3.64 years and the sex ratio was 3. The most common mode of discovery was edematous syndrome 62.5%. The most common etiology was glomerular nephropathies 60%, hereditary nephropathies 25% and malformative uropathies in 15%. Ten patients (25%) had moderate CFR at admission, 62.5% had severe CRF, 12.5% had terminal CFR?and all of them died during the year.?Conclusion:?Epidemiological data CRF in adolescent patients in developing countries are scarce and limit treatment. Glomerular pathologies represent the most frequent etiology. Early treatment of chronic renal failure in childhood and multidisciplinary collaboration between pediatrician and nephrologist are necessary.

Anorectal Malformations Operated at University Hospital brazzaville

Aim: To appreciate the care of children operated for an anorectal malformation. Materials and methods: It was a retrospective study carried out from January 2014 to December 2018 (5 years) in the department of pediatric surgery of university hospital of Brazzaville. Results: We retained 35 files including 20 boys and 15 girls. The mean age of discovery of the malformation was 1.7 days (range 0 and 4 days). On physical examination, we found intestinal occlusion in 22 patients (62.9%): the absence of an anal opening (62.9%), abdominal bloating with tympanism were the most represented signs. (57.1%). There were 13 cases (37.1%) of anorectal malformations high, 12 cases (34.3%) of low and 10 cases (28.6%) of intermediate. There were 20 cases (57.1%) of anorectal malformations without fistula and 15 cases (42.9%) with fistula. The mean age at the time of the anal plasty was 12.7 months (range 2 days and 14 years). We performed the anorectoplasty according to the Peňa and De Vries technique in 26 cases (74.3%), perineal anoplasty in 5 cases (14.3%) and a transposition of the fistula in 4 cases (11.4%). Twenty-seven patients (77.1%) were reviewed with an average follow-up of 2.7 years (1 year and 5 years extremes). The anus had a normal aspect in 20 cases (74.1%). We evaluated anal continence according to the Krickenbeck criteria in 10 patients aged over 3 years, and six had good results.

Accidental Ingestion of Petroleum in Children at the University Hospital of brazzaville

Introduction: Petroleum ingestion is a health problem in Africa and can be responsible for significant mortality. Objectives: Improving the management of accidental petroleum ingestion in children, describe the socio-demographic characteristics, clinical, therapeutic and evolutionary profile of children hospitalized for accidental ingestion of petroleum and identify the factors associated with the occurrence of petroleum pneumopathy. Patients and Methods: We conducted a cross-sectional analytical study with retrospective collection from January 2016 to December 2015, i.e. in 24 months in the Pediatric Intensive Care and Infant Pediatrics departments at the University Hospital of Brazzaville. We included children who accidentally ingested petroleum. The variables studied were epidemiological, clinical, therapeutic, radiological and evolutionary. The statistical tests used were Pearson’s Chi2 and Odds ratio. The materiality threshold was set at 5%. Results: Out of 8013 children hospitalized, 78 (1%) had accidentally ingested petroleum, including 49 (62.8%) boys and 29 (37.3%) girls with an average age of 20 months. Ingestion of Caustics by Children in brazzaville (CONGO)

Ingestion of caustics by children is serious because of the increase in their frequency and the difficulties due to their management. Objective: To improve the management of caustic lesions in Pediatric Department at the University Hospital of brazzaville. Patients and Methods: Prospective study was conducted from January 2014 to December 2015 in the Pediatric Departments of the UHB and centers of digestive endoscopy in brazzaville. All children who ingested a caustic product and hospitalized were included after obtaining parental’s consent. The studied parameters were: Age, sex, nature and quantity of the caustic, the family’s attitude, clinical signs, endoscopic results according to Zagar’s classification, therapeutic modalities and evolution. Results: 8292 children were hospitalized and 68 because of the ingestion of caustic, a frequency of 0.8%. The average age was 23.6 months [2 months - 16 years]. The male sex was predominant (57.4%). Ingestion was accidental in 88.2% and voluntary in 11.8%. Factors favoring ingestion were storage methods (81.5%) and parental’s inattention or negligence (10.8%). Bleach was the most incriminated caustic (54.4%) followed by caustic soda (29.4%). The estimated quantity ingested was assumed to be minimal in 61.7%. Unsuitable acts were practiced by the family before the medical consultation in 66.1%. The dominant symptoms were digestive (47%) and neurological (25%). The average time to perform endoscopy was 48 hours in 88.2%. Endoscopy revealed lesions in 51.7%, the different stages were I (12 cases), II (16 cases) and III (3 cases). The mains factors of gravity were inappropriate gestures (85%) and the non-respect of the fasting (9%). Proton pump inhibitors were used in 50%. The evolution was favorable for in 73.5%. Conclusion: Ingestion of caustics by children is most often accidental favored by the inadequate packaging and storage. Prevention is about educating people in order to reduce their frequency.

Objective: Chronic end-stage renal failure is a major public health problem in developing countries and is poorly documented. The objective of this study was to describe the epidemiological, diagnostic, therapeutic, and evolutionary aspects of patients admitted for end-stage renal failure to the Department of Nephrology at the University Hospital of brazzaville. Patients and Methods: This was a descriptive cross-sectional study collecting prospective data that took place over 10 months. We included 128 patients who were exhaustively identified. Sociodemographic, clinical, therapeutic and developmental data were collected using a standardized and pretested pre-established survey form. The EPI info software enabled the analysis and processing of the data. Results: We reported a male predominance with a sex ratio of 2.5. The median age was 50 years. The first cause found was diabetes (55%) followed by hypertension (46%). The majority of patients had a dialysis emergency and half could not be dialyzed for financial reasons. Conclusion: Our results highlighted that certain epidemiological parameters of end-stage renal failure are constantly changing, such as frequency, while others remain static, including the main etiologies, gender, and age. However, mortality continues to increase and deserves more attention.

Prevalence of Children Vaccinated against Viral Hepatitis B in brazzaville

Introduction: Viral hepatitis B (VHL) is a public health problem, particularly in sub-Sahara Africa. The aim of this study was to assess vaccination coverage against HBV in children in brazzaville. Patients and Methods: This was a cross-sectional analytical study conducted in brazzaville health centres from January to September 2019. It involved children aged between six months and six years who received a vaccination against HBV. Sampling was exhaustive and based on stratified sampling. Results: The overall prevalence of children vaccinated against HBV in brazzaville was 96.2%. It was insufficient in the Talangai health district (79%). The pentavalent vaccine was administered to 97.7% of children, 85% of whom had received all three doses. The reasons for incomplete vaccination were parents’ ignorance of HVB (85.6%) and of vaccination (14.3%). Conclusion: Although the prevalence of vaccinated children is high in brazzaville, it is still insufficient in some health districts, particularly Talangai, because parents are unaware of the disease and of vaccination. Pentavalent is the only vaccine available in the national vaccination programme, which is why an effective national vaccination policy needs to be put in place. .

Computed Tomography Morphometry of the Lumbar Spine in Adults in brazzaville

Context and Justification: Reconstructive spine surgery and the design of implants depend on disc and vertebral measurements, which vary according to racial groups and geographic areas. This variability underscores the importance of studying the morphometric characteristics specific to each population. Research Objective: The primary aim of this research was to determine the measurements of spinal components in the Congolese population in order to establish references suitable for local clinical practice. Materials and Methods: Congolese individuals, free from any spinal disorders, participated in a study on vertebral and disc morphology of the lumbar spine after providing informed consent. They underwent computed tomography examinations at medical imaging centers in brazzaville. The parameters studied included: in axial sections, the transverse diameter (TDb) and sagittal diameter (SDb) of the vertebral bodies, the pedicular transverse diameter (TDp), and in sagittal reconstructions: the height of the vertebral bodies (Hb) and intervertebral discs (Hd). Results: The sample size consisted of 100 subjects (70 men and 30 women), with an average age of 24.47 ± 3.72 years (18 - 30 years). Cranio-caudally, the TDb, DSb, TDp, and Hd showed increasing values, while Hb values were decreasing. In L1, values for both sexes were: TDb = 39.3 ± 3 mm;DSb = 27.3 ± 2.9 mm;Hb = 26.6 ± 2.1 mm;TDp = 8.2 ± 1.8 mm;in L5: TDb = 50.3 ± 4 mm;DSb = 33.6 ± 2.7 mm;Hb = 25.7 ± 2.2 mm;TDp = 14.8 ± 2.1 mm. The Hd values were 8.1 ± 2.1 mm for L1 - L2 and 11.1 ± 2 mm for L5 - S1. Conclusion: Reconstructive spine surgery and the design of implants depend on disc and vertebral measurements, which vary according to racial groups and geographic areas. This variability underscores the importance of studying the morphometric characteristics specific to each population.

Post-Stroke Sexual Disorders and Its Real-Life Experience within Couples in brazzaville

Background: Post-stroke sequelae can have repercussions on the sexual life and relational life of patients and their partners. Objectives: To determine the frequencies of sexual disorders after a first stroke event;to identify the factors associated with sexual disorders;to explore the real-life experience of sexual disorders within couples. Methods: It was an analytical cross-sectional study, conducted on patients followed up for a first stroke and their partners. It was carried out at public and private hospitals in Brazzaville, Congo, during the period of patient follow-up, from May to September 2019 (5 months). The Female Sexual Function Index, International Index of Erectile Function 5 and interviews were used to describe sexual aspects. The real-life experience of couples was explored using standardized and pre-established questionnaires. Results: Of the 36 patients who had taken up again sexual intercourse after stroke, 29 (80.6%) were men. Among the post-stroke sexual disorders, erectile dysfunction (53.2%) and ejaculatory dysfunction (40.4%) were the most frequent in men. Decrease of sexual desire was predominant in women (100%). Factors associated with erectile dysfunction were sleep disorder (p = 0.007) and mean sleep duration per night (p = 0.01). Lack of verbal and non-verbal communication of sexual problems and psychological effects was noted in 77.8% of patients and 60% of their partners. The most frequent psychological effects were: in patients, frustration (52.8%), attitude of person decreased (50%) and sadness (41.7%);in partners, attitude of carer (86.7%).
