Late corneal ectasia after penetrating and deep anterior lamellar keratoplasty for keratoconus

AIM:To investigate tomographic features of late corneal ectasia after keratoplasty for keratoconus and compare penetrating keratoplasty(PK)and deep anterior lamellar keratoplasty(DALK)in terms of incidence,time of onset and risk factors of corneal ***:Sixty eyes with PK and 30 eyes with DALK operated between 1999 and 2021 were *** Pentacam scans were evaluated together with vision and previous *** outcome measures were vision,K values,apparent thinning on graft-host cornea and the difference between opposing quadrants in the thinnest point *** segment optic coherence tomography was performed for further ***:Mean follow-up was 127.2mo(24–282mo)in PK,and 64.3mo(24–144mo)in DALK.K max was higher in DALK(60.6 vs 56.7 D,P=0.012).Inferior recipient was thinner(595.9μm)in PK than DALK(662.2μm,P=0.021),due to longer *** corneal ectasia rate was 20.0%within *** rate was the same(6.7%)in DALK 2/30 and in PK 4/60 in 10y and 13.3%in 12y(4/30 and 8/60,respectively).It increased to 23.3%(14/60)in PK over *** ectasia was not seen before 7y in PK,it could be seen in DALK starting from the 5^(th) *** intervals between keratoplasty and ectasia were 144.5mo in PK and 99mo in *** recipient was significantly thinner in 18 eyes with ectasia(502.7μm)compared to 76 non-ectasia(649.1μm,P=0.000).Inferior graft was thinner(561.0 vs 620.4μm,P=0.006),K max(63.3 vs 56.5 D,P=0.000),and anterior elevation was higher in ectasia(89.1 vs 48.6μm,P=0.002).Accelerated crosslinking was performed on 5 ***:Inferior-superior recipient and inferior graft thinning on tomography,with high K max and anterior elevation emerge as the most reliable criteria for the diagnosis of late *** incidence of corneal ectasia increases with the time.

Neuroimaging in atypical normal tension glaucoma:debating routine implementation in the absence of classic neurological findings

●AIM:To assess the necessity of neuroimaging in patients with neurological or atypical findings of normal tension glaucoma(NTG)who do not exhibit typical glaucoma manifestations.●METHODS:A retrospective analysis was conducted on 90 atypical NTG patients who underwent cranial magnetic resonance imaging(MRI)due to atypical *** demographic characteristics,clinical parameters,and radiological findings were recorded.●RESULTS:Among the patients,66.7%had abnormal radiology results,with the most common findings being gliosis(34.4%),sequelae of cerebrovascular events and vascular malformations(14.4%),and benign intracranial mass lesions(11%).Non-glaucomatous visual field defects were more frequently observed in patients with abnormal neuroimaging ***,there were no significant differences in intraocular pressure,optic disc parameters,retinal nerve fiber layer thickness,and visual field indices between patients with normal and abnormal radiological *** mean age of the patients was ***,there was a significant age difference,with the abnormal radiology group having a higher median age(P=0.021).●CONCLUSION:The study highlights the importance of cranial imaging in older NTG patients to detect underlying pathologies and prevent *** suggests that neuroimaging may be warranted in NTG patients with atypical visual field defects incompatible with ***,routine neuroimaging in all NTG patients without classic neurological signs may not be necessary.

Effect of a cervical collar on optic nerve sheath diameter in trauma patients

BACKGROUND:As advocated in advanced trauma life support and prehospital trauma life support protocols,cervical immobilization is applied until cervical spine injury is *** study aimed to show the difference in optic nerve sheath diameter(ONSD)between patients with and without a cervical collar using computed tomography(CT).METHODS:This was a single-center,retrospective study examining trauma patients who presented to the emergency department between January 1,2021,and December 31,*** ONSD on brain CT of the trauma patients was measured and analyzed to determine whether there was a difference between the ONSD with and without the cervical ***:The study population consisted of 169 *** CT imaging of patients with(n=66)and without(n=103)cervical collars,the mean ONSD in the axial plane were 5.43±0.50 mm and 5.04±0.46 mm respectively for the right eye and 5.50±0.52 mm and 5.11±0.46 mm respectively for the left *** results revealed an association between the presence of a cervical collar and the mean ONSD,which was statistically significant(P<0.001)for both the right and left ***:A cervical collar may be associated with increased *** effect of this increase in the ONSD on clinical outcomes needs to be investigated,and the actual need for cervical collar in the emergency department should be evaluated on a case-by-case basis.

Central venous catheterization(CVC) is an invasive procedure for administering fluids,nutrients,and drugs;monitoring central venous pressure;performing pulmonary artery catheterization;and placing transvenous pacemakers in intensive care units and all specialties,from anesthesia to emergency medicine,for the treatment of trauma and hemodynamically unstable pediatric and adult patients.[1,2]Complications have been observed in more than 15% of patients who underwent ***,infectious,and thrombotic complications have been reported in 5%–19%,5%–26%,and 2%–26% of patients,respectively.[3] Malposition,on the other hand,is common in subclavian catheter insertion and is usually associated with an initially misplaced guidewire.[4]

How has the COVID-19 pandemic changed treatment preferences of patients with proximal ureteral stones?

Background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:Retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.Results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[OR],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;OR,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;OR,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

Can serum tumor marker densities according to tumor volume and testicle size be used to predict progression in patients with testicular cancer?

Background The objective of this study is to determine the role of tumor marker density(TMD)values such as alpha-fetoprotein tumor volume ratio(ATVR),beta-human chorionic gonadotropin tumor volume ratio(βTVR),alpha-fetoprotein testicle size ratio(ATSR),beta-human chorionic gonadotropin testicle size ratio(βTSR),lactate dehydrogenase tumor volume ratio(LTVR),and lactate dehydrogenase testicle size ratio(LTSR)in the determination of progression-free survival(PFS)in patients with testicular *** and methods A retrospective study was conducted of 95 patients followed-up in our clinic with a diagnosis of testicular cancer between January 2015 and August *** were grouped according to clinical stage,as either early stage(n=50)or advanced stage(n=45).Clinical and pathological data and TMD values for all patients were *** The median age of patients was 35 years(21–63 years).All TMDs except LTVR in advanced stage patients were found to be significantly higher than those of early stage patients(p<0.05).Median ATVR(2.58 vs.0.0),ATSR(0.63 vs.0.03),βTVR(0.9 vs.0.009),andβTSR(0.18 vs.0.007)of the nonseminoma patients were found to be significantly higher than those of the seminoma patients,respectively(p<0.001).Progression-free survival(months)was decreased in seminoma patients with high values ofβTVR(11.3±1.9 vs.35.2±0.7),βTSR(16.2±3.4 vs.35.2±0.75),LTVR(17.7±3.4 vs.35.2±0.7),and LTSR(21.5±3.13 vs.35.09±0.8)(p<0.001).Decreased PFS(months)was associated with higher values of ATVR(5.37±0.7 vs.35.05±0.93),βTVR(7.4±1.5 vs.34.6±1.3),ATSR(5.37±0.75 vs.35.05±0.9),βTSR(7±1.5 vs.34.6±1.3),and LTSR(7.9±1.2 vs.34.3±1.5)in nonseminoma patients(p<0.001).Based on multivariate analysis,βTVR-LTVR and ATVR-ATSR were determined to be independent risk factors for reduced PFS in seminoma and nonseminoma patients,respectively(p<0.05).Conclusions The results of this study suggest that the calculation of TMDs could be a promising and simple method for prediction of PF
