Long-term outcomes of PCI vs. CABG for ostial/midshafl lesions in unprotected left main coronary artery

Background There are limited data on long-term （〉 5 years） outcomes of drug-eluting stent （DES） implantation compared with coro- nary artery bypass grafting （CABG） for ostial/rnidshaft left main coronary artery （LMCA） lesions. Methods Of the 259 consecutive pa- tients in beijing Anzhen Hospital with ostial/midshaff LMCA lesions, 149 were treated with percutaneous coronary intervention （PCI） with DES and 110 were with CABG. The endpoints of the study were death, repeat revascularization, myocardial infarction （MI）, stroke, the composite of cardiac death, and major adverse cardiac and cerebrovascular events （MACCE, the composite of cardiac death, MI, stroke or repeat revascularization）.The duration of follow-up is 7.1 years （interquartile range 5.3 to 8.2 years）. Results There is no significant differ- ence between the PCI and CABG group during the median follow-up of 7.1 years （interquartile range： 5.3-8.2 years） in the occurrence of death （HR： 0.727, 95% CI： 0.335-1.578; P = 0.421）, the composite endpoint of cardiac death, MI or stroke （HR： 0.730, 95% CI： 0.375-1.421; P = 0.354）, MACCE （HR： 1.066, 95% CI： 0.648-1.753; P = 0.801）, MI （HR： 1.112, 95% CI： 0.414-2.987; P = 0.833）, stroke （HR： 1.875, 95% CI： 0.528-6.659; P = 0.331）, and repeat revascularization （HR： 1.590, 95% CI： 0.800-3.161; P = 0.186）. These results remained after multivariable adjusting. Conclusion During a follow-up up to 8.2 years, we found that DES implantation had similar endpoint outcomes compared with CABG.

Evidence for association of D1S249 locus on human chromosome 1 with the susceptibility to essential hypertension in Han Chinese

Essential hypertension (EH) is thought to result from theinteraction of environmental and genetic factors. The molecular genetics of EH has witnessed considerable progress during the past few years. However, the number of genes involved, their chromosomal location and the magnitude of their effect on EH susceptibility are unknown. We conducted the present study to screen susceptibility genes to essential hypertension using a genome-wide scanning method in a group of Han people from Fangshan district located in the southwest of beijing. A case-control study and affected sibpair were performed. Genotyping was carried out using a fluorescence-based semiautomated technique on automated DNA sequencer (ABI 377, PE). The basis for the genome-screen was the ABI prism linkage mapping sets of 400 microsatellite markers (version 2, PE, Co.). PCR for amplification of markers was carried out as multiplex reactions with Ampli Taq gold (PE, Co.) following protocols developed in our laboratory. Data were exported as a text file from genotyper for subsequent two-point affected sibpair linkage analysis. The data from case-control association study showed a linkage disequilibrium between EH and marker D1S249 locus (X2 = 14.6, P = 0.002). There are 12 alleles in the D1S249 locus. The frequency of A9 allele in hypertension was higher than in normotensives, (13.6% v.s. 2.7%, X2 = 6.30, p = 0.01, OR = 4.57, 95%CI = 1.24-25.4). The data from two-point affected sibpair linkage analysis demonstrated a linkage between EH and A9 allele, P＜0.05. It suggested that microsatellite marker D1S249 locus might be associated with the genetic susceptibility to essential hypertension in Han Chinese.

Risk factors for acute kidney injury following coronary artery bypass graft surgery in a Chinese population and development of a prediction model

BACKGROUND Acute kidney injury(AKI)after coronary artery bypass graft(CABG)surgery is associated with significant morbidity and *** retrospective study aimed to establish a risk score for postoperative AKI in a Chinese *** A total of 1138 patients undergoing CABG were collected from September 2018 to May 2020 and divided into a derivation and validation *** was defined according to the Kidney Disease Improving Global Outcomes(KDIGO)*** logistic regression analysis was used to determine the independent predictors of AKI,and the predictive ability of the model was determined using a receiver operating characteristic(ROC)*** The incidence of cardiac surgery–associated acute kidney injury(CSA-AKI)was 24.17%,and 0.53%of AKI patients required dialysis(AKI-D).Among the derivation cohort,multivariable logistic regression showed that age≥70 years,body mass index(BMI)≥25 kg/m2,estimated glomerular filtration rate(eGFR)≤60 mL/min per 1.73 m2,ejection fraction(EF)≤45%,use of statins,red blood cell transfusion,use of adrenaline,intra-aortic balloon pump(IABP)implantation,postoperative low cardiac output syndrome(LCOS)and reoperation for bleeding were independent *** predictive model was scored from 0 to32 points with three risk *** AKI frequencies were as follows:0-8 points(15.9%),9-17 points(36.5%)and≥18 points(90.4%).The area under of the ROC curve was 0.730(95%CI:0.691-0.768)in the derivation *** predictive index had good discrimination in the validation cohort,with an area under the curve of 0.735(95%CI:0.655-0.815).The model was well calibrated according to the Hosmer-Lemeshow test(P=0.372).CONCLUSION The performance of the prediction model was valid and accurate in predicting KDIGO-AKI after CABG surgery in Chinese patients,and could improve the early prognosis and clinical interventions.

Corneal subbasal nerve fiber regeneration in myopic patients after laser in situ keratomileusis

A total of 26 myopic patients （52 eyes） underwent laser in situ keratomileusis. In vivo confocal microscopy revealed that most of the regenerated corneal subbasal nerve fibers in the comeal flap originated from the stump of corneal subbasal nerve fibers outside the ablation zone and extended towards the center of the cornea in all patients. Meanwhile, new fibers were also found to directly regenerate from deep in the stroma in some cases. Approximately 94% of regenerated corneal subbasal nerve fibers （73/78 eyes） regrew vertically into the peripheral central 6-mm circle area 1 month after surgery, 78% （28/36 eyes） grew into the central 3 6 mm area at 2 months, and 23% into the central 3-mm circle area at 3 months. In addition, there was no significant difference in corneal subbasal nerve fiber regenerative capacity between the basic fibroblast growth factor group and the 20% （vN） deproteinized extract of calf blood group. The majority of corneal subbasal nerve fiber regeneration occurred from the stump of corneal subbasal nerve fibers outside the corneal flap and the remaining growth occurred deep within the stroma.

Imbalance of matrix metalloproteinase-9 and matrix metalloproteinase tissue inhibitor-1 may contribute to hemorrhage in cerebellar arteriovenous malformations

In this study, we determined the expression levels of matrix metalloproteinase-2 and -9 and matrix metalloproteinase tissue inhibitor-1 and -2 in brain tissues and blood plasma of patients undergoing surgery for cerebellar arteriovenous malformations or primary epilepsy （control group）. Immunohistochemistry and enzyme-linked immunosorbent assay revealed that the expression of matrix metalloproteinase-9 and matrix metalloproteinase tissue inhibitor-1 was significantly higher in patients with cerebellar arteriovenous malformations than in patients with primary epilepsy. The ratio of matrix metalloproteinase-9 to matrix metalloproteinase tissue inhibitor-1 was significantly higher in patients with hemorrhagic cerebellar arteriovenous malformations compared with those with non-hemorrhagic malformations. Matrix metalloproteinase-2 and matrix metalloproteinase tissue inhibitor-2 levels were not significantly changed. These findings indicate that an imbalance of matrix metalloproteinase-9 and matrix metalloproteinase tissue inhibitor-I, resulting in a relative overabundance of matrix metalloproteinase-9, might be the underlying mechanism of hemorrhage of cerebellar arteriovenous malformations.

Haemostatic therapy in spontaneous intracerebral haemorrhage patients with high-risk of haematoma expansion by CT marker: a systematic review and meta- analysis of randomised trials

Cardioprotective effects of anesthetic preconditioning in rats with ischemia-reperfusion injury: propofol versus isoflurane

Objective： We compare the cardioprotective effects of anesthetic preconditioning by propofol and/or isoflurane in rats with ischemia-reperfusion injury. Methods： Male adult Wistar rats were subjected to 60 min of anterior descending coronary artery occlusion followed by 120 rain of reperfusion. Before the long ischemia, anesthetics were administered twice for 10 min followed by 5 min washout. Isoflurane was inhaled at 1 MAC （0.016） in I group, whereas propofol was inhaled intravenously at 37.5 mg/（kg.h） in P group. A combination ofisoflurane and propofol was administered simultaneously in I＋P group. Results： In control （without anesthetic preconditioning, C group）, remarkable myocardial infarction and apoptosis accompanied by an increased level of cardiac troponin T were noted 120 min aider ischemia-reperfusion. As compared to those of control group, I and P groups had comparable cardioprotection. In addition, I＋P group shares with I and P groups the comparable cardioprotective effects in terms of myocardial infarction and cardiac troponin T elevation. Conclusion： A combination of isoflurane and propofol produced no additional cardioprotection.
