Estimating Incubation Period and Predicting Risk of Developing Hepatocellular Carcinoma in Patients with HCV Infection from the Clinic of a County-Level City in China

Background: To estimate incubation period in patients infected with hepatitis C virus (HCV) and to predict patients’ risk of developing hepatocellular carcinoma (HCC) for indicating health policymaking. Methods: An outbreak of HCV infection from a clinic was occurred due to the injection treatment of varicose veins called minimally invasive thrombolysis and meridian activating therapy (MITMT) in a county-level city of China. Conventional method and bootstrapping approach were used to estimate HCV incubation period. Risk assessment model generated from the previous study was applied to predict the risk of developing HCC in the coming 5-, 10-, and 15-year for HCV infected patients from the above outbreak. Results: A total of 120 subjects were underwent MITMT between October 22, 2012 and January 28, 2013;of those, 99 subjects were diagnosed with HCV infection. Mean age of the subjects was 54.42 years old, and females were accounted for 51.5% in contracted subjects. Mean incubation period of hepatitis C (HC) was 45.76 days using first MITMT as infected date and 30.68 days using bootstrapping approach, their standard deviations were 23.01 and 15.05 days, respectively. A total of 1.3, 4.7, and 10.6 persons will be developed into HCC in the coming 5, 10, and 15 years in infected subjects based on a risk prediction model. Conclusion: Some patients with HCV infection from a nosocomial clinic in a county-level city will be suffered from more severe HCC in the coming decade;effective measures and controls were urgently needed to manage these patients with high risk of developing into end-stage liver disease.

The coronavirus disease 2019(COVID-19)pandemic poses a great threat to public *** who are immunocompromised because of the progression of the primary disease or receiving immunosuppressive medications are prone to severe COVID-19 complications and poor *** data have shown that many COVID-19 vaccines are safe and effective in large-scale populations;however,these clinical trials have excluded immunocompromised *** evidence indicates that immunocompromised populations have a blunted immune response to other vaccines,raising concerns regarding the efficacy of COVID-19 vaccination in these ***,there is an urgent need to delineate the efficacy of COVID-19 vaccines in these vulnerable ***,we review the characteristics of specific humoral and cellular responses to COVID-19 vaccination in immunocompromised populations,including HIV-infected patients and those receiving immunosuppressive treatment,especially solid organ transplant recipients and those undergoing anti-CD20 *** also addressed the challenges that immunocompromised populations will face in the future pandemic and the need for basic and clinical translational studies to highlight the best vaccination strategies for these populations.

Reoperation on aortic disease in patients with previous aortic valve surgery

Background Aortic valve replacement （AVR） is a safe and effective method in the treatment of aortic valve diseases. This study aimed to increase the understanding on re-treatment of aortic diseases after aortic valve surgery through a retrospective analysis of 47 related cases. Methods Forty-seven patients （38 males and 9 females） with previous aortic valve surgery have received reoperation on aorta from January 2003 to June 2012, and the mean interval time of re-intervention to aortic disease was 6 years （（6.0＋ 3.8） years）. The secondary aortic surgery included aortic root replacement （14 cases）, ascending aorta replacement （10 cases）, aortic root/ascending aorta plus total arch replacement with stented elephant trunk implantation （21 cases）, and total thoracoabdominal aorta replacement （2 cases）. All these patients have received outpatient re-exams or follow-up by phone calls, Results After the initial aortic valve replacement, patients suffered from aortic dissection （25 cases, 53%）, ascending aortic aneurysm （12 cases, 26%） or aortic root aneurysm （10 cases, 21%）, Diameter in ascending aorta increased （5.2＋7.1） mm per year and aortic sinus （3.3＋3.1） mm per year. The annual growth value of diameter in ascending aorta was higher in patients with rheumatic heart disease than that in Marfan syndrome （P〈0,05）. All 47 patients have received reoperation on aorta. One patient died in operating room because aortic dissection seriously involved right coronary artery. Seven patients had renal insufficiency after operation; neurological complications occurred in 14 patients including 7 patients with stroke and the others with transient brain dysfunction. All patients were followed up, the mean survival time was （97.25~17.63） months, 95% confidence interval was 55.24-73.33 months. The complete genome of methanol-utilizing Amycolatopsis methanolica strain 239T was generated,revealing a single 7,237,391 nucleotide circular chromosome with 7074 annotated protein-coding sequences(CDSs).Comparative analyses against the complete genome sequences of Amycolatopsis japonica strain MG417-CF17T,Amycolatopsis mediterranei strain U32 and Amycolatopsis orientalis strain HCCB10007 revealed a broad spectrum of genomic structures,including various genome sizes,core/quasi-core/non-core configurations and different kinds of *** polyketide synthase gene clusters were absent from the *** genome,12 gene clusters related to the biosynthesis of other specialized(secondary)metabolites were *** pathways attributable to the facultative methylotrophic physiology of *** strain 239T,including both the mdo/mscR encoded methanol oxidation and the hps/hpi encoded formaldehyde assimilation via the ribulose monophosphate cycle,were identified together with evidence that the latter might be the result of horizontal gene *** analyses based on 16S rDNA or orthologues of AMETH_3452,a novel actinobacterial class-specific conserved gene against 62 or 18 Amycolatopsis type strains,respectively,revealed three major phyletic lineages,namely the mesophilic or moderately thermophilic *** subclade(AOS),the mesophilic Amycolatopsis taiwanensis subclade(ATS)and the thermophilic *** subclade(AMS).The distinct growth temperatures of members of the subclades correlated with corresponding genetic variations in their encoded compatible *** study shows the value of integrating conventional taxonomic with whole genome sequence data.
