Lingguizhugan Decoction(LGZG)has been investigated in basic studies,with satisfactory effects on insulin resistance in non-alcoholic fatty liver disease(NAFLD).This translational approach aimed to explore the effect and underlying mechanism of LGZG in clinical setting.A randomized,double-blinded,placebo-controlled trial was performed.A total of 243 eligible participants with NAFLD were equally allocated to receive LGZG(two groups:standard dose and low dose)or placebo for 12 weeks on the basis of lifestyle modifications.The primary efficacy variable was homeostasis model assessment of insulin resistance(HOMA-IR).Analyses were performed in two populations in accordance with body mass index(BMI;overweight/obese,BMI 24 kg/m^(2);lean,BMI<24 kg/m^(2)).For overweight/obese participants,low-dose LGZG significantly decreased their HOMA-IR level compared with placebo(0.19(1.47)versus 0.08(1.99),P=0.038).For lean subjects,neither dose of LGZG showed a superior effect compared with placebo.Methylated DNA immunoprecipitation sequencing and real-time qPCR found that the DNA N6-methyladenine modification levels of protein phosphatase 1 regulatory subunit 3A(PPP1R3A)and autophagy related 3(ATG3)significantly increased after LGZG intervention in overweight/obese population.Low-dose LGZG effectively improved insulin resistance in overweight/obese subjects with NAFLD.The underlying mechanism may be related to the regulation of DNA N6-methyladenine modification of PPP1R3A and ATG3.Lean subjects may not be a targeted population for LGZG.

Progress in the design and quality control of placeboes for clinical trials of traditional Chinese medicine

In recent years,new preparations of traditional Chinese medicines(TCMs)have been developed,increasing the need for their clinical trials.Using placeboes rather than control drugs is increasingly popular in clinical trials of TCMs,as the therapeutic effects of the tested TCMs can be more properly judged.The basic attributes of TCM placeboes include similarity,safety,applicability and controllability.In particular,it is necessary to have similarities in appearance,color,smell and taste between the tested TCMs and placeboes.This is quite difficult for some TCMs due to their distinctive smell and taste.On the other hand,according to the TCM theory on homology of medicine and food,many foods also have certain bioactivities,potentially further complicating the selection of materials for TCM placeboes.In this review,firstly,studies on the special smell and taste of TCMs were introduced.Then,the preparation quality evaluation processes for TCM placeboes were summarized and discussed,based on the relevant literature published in recent years and the research results from our own lab.This review will facilitate the further research and development of TCM placeboes.

Gut microbiota controls the development of chronic pancreatitis:A critical role of short-chain fatty acids-producing Gram-positive bacteria

Chronic pancreatitis(CP)is a progressive and irreversible fibroinflammatory disorder,accompanied by pancreatic exocrine insufficiency and dysregulated gut microbiota.Recently,accumulating evidence has supported a correlation between gut dysbiosis and CP development.However,whether gut microbiota dysbiosis contributes to CP pathogenesis remains unclear.Herein,an experimental CP was induced by repeated high-dose caerulein injections.The broad-spectrum antibiotics(ABX)and ABX targeting Gram-positive(G+)or Gram-negative bacteria(G-)were applied to explore the specific roles of these bacteria.Gut dysbiosis was observed in both mice and in CP patients,which was accompanied by a sharply reduced abundance for short-chain fatty acids(SCFAs)-producers,especially G+bacteria.Broad-spectrum ABX exacerbated the severity of CP,as evidenced by aggravated pancreatic fibrosis and gut dysbiosis,especially the depletion of SCFAs-producing G+bacteria.Additionally,depletion of SCFAs-producing G+bacteria rather than G-bacteria intensified CP progression independent of TLR4,which was attenuated by supplementation with exogenous SCFAs.Finally,SCFAs modulated pancreatic fibrosis through inhibition of macrophage infiltration and M2 phenotype switching.The study supports a critical role for SCFAs-producing G+bacteria in CP.Therefore,modulation of dietary-derived SCFAs or G+SCFAs-producing bacteria may be considered a novel interventive approach for the management of CP.

Study of the toluene absorption capacity and mechanism of ionic liquids using COSMO-RS prediction and experimental verification

As green solvents,ionic liquids(ILs)are quite suitable for the absorption of volatile organic compounds(VOCs)such as benzene and its homologues.However,solvent selection is the key to the VOC absorption process.In the present study,a rapid solvent screening tool,Conductor-like Screening Model for Real Solvents(COSMO-RS),was used to predict the solubility of toluene in 816 ILs.The effects of four structure characters,namely,the type and alkyl chain length of the cations and anions on the solubility of toluene were discussed.The following conclusions were drawn from the results:(1)ILs with pyrrolidinium-based cations showed better solubility than pyridinium-and imidazoliumbased ones.(2)The solubility of toluene in PF6-based ILs increased with the increasing alkyl chain length,while its solubility in Ac-based ILs exhibited the opposite trend.(3)Toluene showed greater solubility in Cl-based ILs than those based on other anions.(4)The solubility of toluene increased with the anion alkyl chain length.Ac-based ILs were chosen as the most promising potential solvents,and further studied to determine the relationship between various interaction energy parameters and toluene solubility.The results showed that the misfit energy played a dominant role during the absorption process.Furthermore,several ILs were selected for experimental verification of the predicted solubility behavior using liquid and gaseous toluene.The results demonstrated that COSMO-RS could be used to semi-quantitatively and qualitatively predict the solubility of toluene,and this model had promising prospects in screening ILs for VOCs absorption.In summary,this study provided a fundamental basis and practical data for the control and treatment of VOCs.

Association between Ambient Air Pollution and Outpatient Visits for Acute Bronchitis in a Chinese City

Objective To investigate the short-term association between outdoor air pollution and outpatient visits for acute bronchitis,which is a rare subject of research in the mainland of China.Methods A time-series analysis was conducted to examine the association of outdoor air pollutants with hospital outpatient visits in Shanghai by using two-year daily data（2010-2011）.Results Outdoor air pollution was found to be associated with an increased risk of outpatient visits for acute bronchitis in Shanghai.The effect estimates of air pollutants varied with the lag structures of the concentrations of the pollutants.For lag06,a 10 μg/m3 increase in the concentrations of PM10,SO2,and NO2 corresponded to 0.94%（95% CI：0.83%,1.05%）,11.12%（95% CI：10.76%,11.48%）,and 4.84%（95% CI：4.49%,5.18%） increases in hospital visits for acute bronchitis,respectively.These associations appeared to be stronger in females（P〈0.05）.Between-age differences were significant for SO2（P〈0.05）,and between-season differences were also significant for SO2（P〈0.05）.Conclusion Our analyses have provided the first evidence that the current air pollution level in China has an effect on acute bronchitis and that the rationale for further limiting air pollution levels in Shanghai should be strengthened.

Objective To compare the efficacy and safety of medical abortion of different regimens for termination of pregnancy at 8-16 weeks of gestation. Methods healthy pregnant women requesting medical abortion at 8-16 weeks＇ gestation within 12 hospitals in Shanghai were randomly allocated to four treatment groups. Three intervention groups were given mifepristone 200 mg as a single dose then 24 h later misoprostol 600 #g at 3 h intervals vaginally, orally or vaginally followed by orally, respectively. Control group was given mifepristone 100 mg for 2 d, followed at 48 h by initiation of misoprostol 600 μg vaginally every 12 h. The primary outcome measures were the successful abortion rate, the induction-to-abortion interval, vaginal bleeding and side effects. Results Efficacy outcomes were analyzed for 1 112 women （92.67%）, excluding 88 protocol violations. Termination successful rates were similar among the four groups from 97.1% to 97.8%. The average dose of misoprostol and the incidence of side effects in control group were lower than those in three intervention groups. Stratified analysis showed that the interval of induction-to-abortion at gestation of 1116 weeks was decreased in control group. Conclusion The four regimens have the similar termination successful rates in spite of different administration intervals or routes. Control group was recommended for the advantages of reduced dose of misoprostol and fewer side effects.

First aid strategy for severe traumatic patients in hospital

Objective:To study the emergency management principles of severe trauma in hospital(injury severity score≥16). Methods:We used'ATP principle'to manage severe traumatic patients.The ATP principle is composed of:1)attending surgeons offering initial management(A); 2)teamwork commencement immediately after patients admitted to hospital(T);3)parallel principle,ie, emergency resuscitation,evaluation and laboratory test performed simultaneously(P).Clinical effects before and after applying ATP principle were retrospectively analyzed and compared. Results:During January 1,2002 to December 31, 2003,338 patients were treated without applying ATP principle,in which ISS was 25.9±6.4,152 cases died with the mortality being 39.2%,and the time stayed in emergency department and the time to operation room after admission were(102.8±16.7)min,(140.3±20.6)min, respectively.During January 1,2004 to December 31, 2005,438 patients were treated based on ATP principle,in which ISS was 28.6±7.8,87 cases died with the mortality being 19.9%,and the time in emergency department and the time to operation room after admission were(69.5±11.5)min,(89.6±9.3)min,respectively.ISS showed no significant difference between the two groups(P>0.05), but the mortality,the time stayed in emergency department and the time to operation room after admission were greatly reduced and showed significant difference between the two groups(P<0.05). Conclusions:Applying ATP principle to treat severe traumatic patients can shorten emergency treatment time in hospital and decrease mortality.

Inhibitory Effects of S-Nitroso-Glutathione on Sporulation Process of Eimeria tenella Oocysts

To study the inhibiting effects of S-Nitroso-Glutathione （GSNO） on sporulation process of Eimeria tenella （E. tenella） oocysts by detecting the sporulation rate, GSNO as nitric oxide donor was used to treat E. tenella oocysts sporulated for different times; Several kinds of antioxidants and oxidants were used to investigate the inhibitory effects of GSNO on sporogony process of E. tenella oocysts. The results showed that GSNO still inhibited the oocysts sporulated 10 h, but had few inhibiting effect on the oocysts sporulated for 14 h; the antioxidants, such as Vitamin C （VC）, mannitol and sodium salicylate, could not eliminate the inhibitory effects of GSNO on oocysts; neither did the ferrous sulfate （FeSO4） nor Dithiothreitol （DTT）. However, the oxidants, potassium dichromate （K2Cr2OT） and potassium permanganate （KMnO4） could obviously repress the effects of GSNO on oocysts. At present, the inhibitory mechanism of GSNO on unsporulated oocysts was still unclear.

Colourful treasure on Shanghai’s Bund:uncovering original paint fnishes in the Chinese Banking Hall of the former HSBC Building

The former HSBC Building,today occupied by Shanghai pudong Development Bank,is one of the largest iconic buildings on the Bund in Shanghai.Designed by Palmer&Turner in the neoclassic style,the building was completed in 1923.Restoration of the interior of the Chinese Banking Hall,located within the southwestern corner of the building,is now underway.This rectangular room features four columns in the centre and stucco decoration on parts of the walls;the ceiling originally had a distinctive,vibrantly colourful design in a bold mix of Western and Chinese styles.The only extant documentation of the banking hall’s original decoration includes black-and-white photographs and a short,written description.Today,all the plaster surfaces are covered over with several layers of white and cream-coloured paint fnishes.A preliminary analysis of the hall’s decorative fnishes was conducted to determine the colours,paint materials,and condition of the original paint layers.The investigation revealed that in some sections,the original paint was very brittle.Restoration by exposing the fnishes in the entire room(removing the modern layers covering the original one)did not appear feasible.This article investigates the results of the following:assessing the condition of the paint materials;identifying the signifcance of the decorations;the challenges facing the room’s conservation;and potential conservation options for the room’s decoration.

Abstract Porcine reproductive and respiratory syndrome(PRRS)is the severest disease of pigs worldwide,caused by a highly genetically diverse RNA virus,called Porcine reproductive and respiratory syndrome virus(PRRSV).The research summarized the genome characteristics of PRRSV particles and the most updated knowledge of structure protein function,and introduced the intellectual of PRRSV transmission and host immune response,which is very important for prevention and control for PRRS.A report showed that mass vaccination can stabilize the immunity of the entire herd,and this is the first required step for a PRRS eradication plan.However,the attenuated live vaccines may not achieve a valid prevention.The final goal of the EU project is to develop new generation,efficacious and safe maker vaccines that can be adapted to temporary changes and geographical differences.Robinson reported that broadly antibodies could neutralize all rapidly evolving typeⅠand typeⅡviruses,while further studies are expected to elucidate mechanisms of neutralizing antibody production and maturation and to investigate conserved epitope targets of cross-neutralization in this rapidly evolving virus.
