We study the dynamics of short optical pulses in a real system with periodically distributed dispersion and the nonlinearity is governed by the higher order nonlinear Schr?dinger equation(HNLSE) with linear and nonlinear gain(loss). Under specific parametric circumstances, where the dark and bright solitary waves are combined, a set of entirely new types of solitary waves with nonlinear chirp have emerged. For a properly intense optical pulse in the combined solitary waves, the binding of the bright and dark solitary waves is very strong. It is seen that by numerical simulation, these pooled types of solitary-like solutions show a high degree of stability while propagating over an extremely long distance in the considering system, even in the presence of a high degree of perturbation of the amplitude and white noise. All constraint relations on the physical parameters are explicitly shown to be related to the development and to the dynamical study of the chirped solitary like solution in the considering medium.

The knowledge of the pathogenesis of type 1 diabetes mellitus(T1DM)continues to rapidly evolve.The natural course of the disease can be described in four clinical stages based on the autoimmune markers and glycemic status.Not all individuals of T1DM progress in that specific sequence.We hereby present a case of T1DM with a classical third phase(honeymoon phase)and discuss the intri-cacies of this interesting phase along with a possible future promise of“cure”with the use of immunotherapies.We now know that the course of T1DM may not be in only one direction towards further progression;rather the disease may have a waxing and waning course with even reversal of type 1 diabetes concept being discussed.The third phase popularly called the“honeymoon phase”,is of special interest as this phase is complex in its pathogenesis.The honeymoon phase of T1DM seems to provide the best window of opportunity for using targeted therapies using various immunomodulatory agents leading to the possibility of achieving the elusive“diabetes reversal”in T1DM.Identifying this phase is therefore the key,with a lot of varying criteria having been proposed.

Background: Postpartum hemorrhage (PPH) is the major contributor to maternal mortality and morbidity worldwide as well as in Tanzania. Studies have shown Pre-eclampsia as a risk indicator for Postpartum hemorrhage and D-dimer tends to rise in women with pre-eclampsia. Few studies that have shown the association between D-dimer and PPH have been controversial and differ according to ethnicity and lifestyle. Hence there is no suitable reference interval for D-dimer in predicting Postpartum hemorrhage among women with pre-eclampsia. Broad Objective: This study aimed to assess the association, sensitivity, and specificity of D-dimer as a laboratory predictor of postpartum hemorrhage among women with pre-eclampsia at KCMC hospital. Methodology: This was a hospital-based analytical cross-sectional study conducted at KCMC Hospital in Northern Tanzania from September 2022 to March 2023. A total of 195 women with pre-eclampsia were included in this study. Plasma D-dimer levels were taken from women with pre-eclampsia pre-delivery. Haematocrit was compared before and after delivery, and a fall of 10% was considered as Postpartum hemorrhage together with clinical assessment of the patient. Participants were divided among those who had severe features and those who did not have severe features and were further categorized into those who had PPH and those who did not have PPH. Logistic regression was used to determine the association between D-dimer and PPH adjusting for other factors. The Receiver Operating Curve (ROC) was used to evaluate the predictive value. Results: Higher median D-dimer levels were seen among women who had PPH compared to those who had no PPH. D-dimer was seen to be associated with PPH, thus for every unit increase of µg/ml of D-dimer among women who had pre-eclampsia without severe features there was a 14% significant increase in the odds of having postpartum hemorrhage and a 45% significant increase of having postpartum hemorrhage among those who had pre-eclampsia with severe features. Verticillium longisporum(Vl43)is a soilborne hemibiotrophic fungal pathogen causing stem striping on oilseed rape(OSR)and severe yield losses.Breeding for resistant varieties is the most promising approach to control this disease.Here,we report the identification of Hva22c as a novel susceptibility factor and its potential for improving OSR resistance.Hva22c is a member of the Hva22 gene family,originally described for barley(Hordeum vulgare).Several Hva22 members have been located at the endoplasmic reticulum.Hva22c is up-regulated in response to Vl43 in both Arabidopsis and OSR.We demonstrate that knock-out of Hva22c in OSR by CRISPR/Cas9 and its homolog in Arabidopsis by T-DNA insertion reduced plants’susceptibility to Vl43 infection and impaired the development of disease symptoms.To understand the underlying mechanism,we analysed transcriptomic data from infected and non-infected roots of hva22c knock-out and wild type plants.We identified a homozygous mutant with frame-shifts in all four BnHva22c loci displaying a vastly altered transcriptional landscape at 6 dpi.Significantly,a large set of genes was suppressed under mock conditions including genes related to the endomembrane systems.Among the up-regulated genes we found several defense-related and phytohormone-responsive genes when comparing mutant to the wild type.These results demonstrate that Hva22c is functionally required for a fully compatible plant-fungus interaction.Its loss of function reduces plant susceptibility,most likely due to endoplasmatic reticulum and Golgi dysfunction accompanied by additionally activated defense responses.These findings can help improve OSR resistance to V.longisporum infection.

Obesity and diabetes mellitus are common metabolic diseases prevalent worldwide.Mice are commonly used to study the pathogenesis of these two conditions.Obesity and diabetes mellitus are induced by administering a high-fat diet in many studies although other diet-induced models are also used.Several factors may influence the outcome of the studies done to study diet-induced obesity in mice.The immune system plays a crucial role in the susceptibility of mice to develop obesity and metabolic disease.In this article,the reasons for differences in susceptibility to develop obesity and diabetes mellitus in mice in response to high-fat-diet feeding and the influence of immunological bias of the mice strain used in studies are evaluated.Mice strains that induce proinflammatory and Th1-type immune responses are found to be susceptible to high-fat-diet-induced obesity.A few studies which directly compared the effect of a high-fat diet on obesity and diabetic phenotype in Th1-and Th2-biased mice strains were briefly analyzed.Based on the observations,it is proposed that the liver and adipose tissue may respond differently to high-fat-diet feeding regimens in Th1-and Th2-biased mice strains.For instance,in Th1-biased mice,adipose tissue fat content was high both in the baseline as well as in response to a high-fat diet whereas in the liver,it was found to be less.It can be inferred that the immune responses to diet-induced models may provide insights into the pathogenesis of obesity and diabetes mellitus.

Post-burn contractures are common entities seen in developing countries.There are multiple reasons for the development of contractures,most are preventable.In extensive contractures,a strategic plan is necessary to release all contractures and yet not antagonize post-operative positions.It is also necessary to be cost-effective and minimize the number of surgeries needed.Conventionally the release sequence in extensive burn contractures is proximal to distal.In this case report,we discuss an unusual sequence where we released distal contractures before the proximal to achieve optimum results.A 3-year-old child with post-burn contracture of hand,wrist,elbow,and axilla was treated in 2 stages,with the release of wrist contracture and cover with pedicled abdominal flap in the first stage and division of pedicled flap with the release of axilla and elbow contracture in the second stage.Thus,the release of all contractures was achieved without antagonizing post-operative positions and minimized the number of surgeries.A case-based approach may be crucial in making a strategic surgical plan to minimize the rehabilitation phase,rather than following known dictums.

BACKGROUND Burden due to intellectual disability(ID) is only third to the depressive disorders and anxiety disorders in India. This national burden significantly contributes to the global burden of ID and hence one has to think globally and act locally to reduce this burden. At its best the collective prevalence of ID is in the form of narrative reviews. There is an urgent need to document the summary prevalence of ID to enhance further policymaking, national programs and resource allocation.AIM To establish the summary prevalence of ID during the past 60 years in India.METHODS Two researchers independently and electronically searched Pub Med, Scopus, and the Cochrane library from January 1961 to December 2020 using appropriate search terms. Two other investigators extracted the study design, setting, participant characteristics, and measures used to identify ID. Two other researchers appraised the quality of the studies using the Joanna Briggs Institute critical appraisal format for Prevalence Studies. Funnel plot and Egger’s regression test were used to ascertain the publication and small study effect on the prevalence. To evaluate the summary prevalence of ID, we used the random effects model with arcsine square-root transformation. Heterogeneity of I^(2)≥ 50% was considered substantial and we determined the heterogeneity with meta-regression. The analyses were performed using STATA(version 16).RESULTS Nineteen studies were included in the meta-analysis. There was publication bias;the trim-and-fill method was used to further ascertain bias. Concerns with control of confounders and the reliable measure of outcome were noted in the critical appraisal. The summary prevalence of ID was 2% [(95%CI: 2%, 3%);I^(2)= 98%] and the adjusted summary prevalence was 1.4%. Meta-regression demonstrated that age of the participants was statistically significantly related to the prevalence;other factors did not influence the prevalence or heterogeneity.CONCLUSION The summary prevalence of ID in India was established to be 2% ta

BACKGROUND Emergence delirium(EmD)is a troublesome motoric,emotional,and cognitive disturbance associated with morbidity.It is often misdiagnosed despite being present in a substantial proportion of children and adolescents during emergence from anesthesia.AIM To evaluate the summary diagnostic accuracy of Pediatric Anesthesia Emergence Delirium Scale(PAEDS)for EmD among children and adolescents.METHODS Two researchers electronically and hand searched the published literature from May 2004 to February 2021 that evaluated the diagnostic accuracy of PAEDS for EmD among children and adolescents,using appropriate terms.Two independent researchers extracted the diagnostic parameters and appraised the study quality with QUADAS-2.Overall,the diagnostic accuracy of the measures was calculated with the summary receiver operating characteristic curve(SROC),the summary sensitivity and specificity,and diagnostic odds ratio(DOR)for EmD.Various diagnostic cut-off points were evaluated for their diagnostic accuracy.Heterogeneity was analyzed by meta-regression.RESULTS Nine diagnostic accuracy studies of EmD that conformed to our selection criteria and PRISMA guidelines were included in the final analysis.There was no publication bias.The area under the SROC was 0.97(95%confidence interval[CI]:95%-98%).Summary sensitivity and specificity were 0.91(95%CI:0.81-0.96;I2=92.93%)and 0.94(95%CI:0.89-0.97;I2=87.44%),respectively.The summary DOR was 148.33(95%CI:48.32-455.32).The effect size for the subgroup analysis of PAEDS cut-off scores of<10,≥10,and≥12 was 3.73,2.19,and 2.93,respectively;they were not statistically significantly different.The setting of the study and reference standard were statistically significantly related to the sensitivity of PAEDS but not specificity.CONCLUSION The PAEDS is an accurate diagnostic measure for the diagnosis of EmD among children and adolescents.Further studies should document its clinical utility.

BACKGROUND Ankylosing spondylitis at total hip arthroplasty(THA)has significant hip stiffness with flexion deformity,restricted mobility,and function.Range of movement(ROM)improvement with good functional outcome is seen following THA in these hips.The modified Hardinge approach without abductor compromise is helpful in these stiff hips with associated flexion deformity.AIM To assess improvement in ROM and functional outcomes with a modified lateral approach THA in ankylosing spondylitis with stiff hips.METHODS A total of 69 hips that underwent THA with a modified Hardinge approach in 40 patients were evaluated at a mean follow-up of 38.33 mo.All individuals ambulated with weight-bearing as tolerated and ROM exercises from the 1st postoperative day.Modified Harris hip score and ROM were assessed during follow-up.Quality of life assessments using the 36-item and 12-item short form health surveys were done along with clinical and functional outcomes at followup.SPSS 22.0 was used for statistical analysis.The correlation of ROM and functional score change was performed using Pearson’s correlation coefficient.RESULTS Sixty-nine hips with a significant decrease in ROM preoperatively with 32 clinically fused hips showed significant improvement in flexion range.The mean flexion in 69 hips improved from 29.35±31.38 degrees to 102.17±10.48 degrees.The mean difference of 72.82 with a P value<0.0001 was significant.In total,45 out of 69 hips had flexion deformity,with 13 hips having a deformity above 30 degrees.The flexion during the follow-up was below 90 degrees in 3 hips.Eleven hips had flexion of 90 degrees at follow-up,while the remaining 55 hips had flexion above 100 degrees.Modified Harris hip score improved from 17.03±6.02 to 90.66±7.23(P value<0.0001).The 36-item short form health survey at the follow-up indicated health status in 40 patients as excellent in 11,very good in 20,good in 5,fair in 3,and poor in 1.The mean mental health score was 84.10±11.58.Pain relief was good in all 69 hips.Altogether,28/40 patients
