抢救 1例因摩托车事故急诊入院男病人。X线胸片发现病人右气管旁外侧纹理和左纵隔影异常增宽 ,并扩展至主动脉弓水平。经食管心脏超声检查发现围绕主动脉根部和升主动脉的壁间水肿 ,延伸至主动脉弓和左横膈下水平的全程降主动脉 ;左锁骨下动脉起始点远端降主动脉上端全层断裂。急诊手术行降主动脉上端端端吻合 ,终因心跳骤停未及手术死亡。尸检证实全程胸主动脉自破裂处向前和逆向延伸 ,引起双向主动脉夹层。

Objective Absence of significant epicardial coronary artery disease （CAD） in patients with acute onset of chest pain and elevation of myocardial necrosis markers is occasionally observed. The aim of this study was to analyse the clinical characteristics and outcome of such patients with advanced age. Methods We retrospectively analysed 4,311 patients with acute onset of chest pain plus necrosis marker elevation. Two hundred and seventy two patients without CAD on angiogram （6.3%） were identified. Out of them, 50 （1.2%） patients 〉 75 years （Group Ⅰ） were compared with （1） 222 acute coronary syndrome （ACS） patients without CAD on angiogram 〈 75 years （Group Ⅱ）, and （2） 610 consecutive patients ≥ 75 years with Non-ST-elevation Myocardial Infarction （NSTEMI） undergoing percutaneous coronary intervention （Group Ⅲ）. Results Group 1 compared to Group III patients made up for more females （64.0% vs. 49.2%; P 〈 0.0001）, and had more severe anginal symptoms on presentation [Canadian Cardiovascular Society （CCS） class Ⅰ/Ⅱ, 26.0% vs. 49.8%; P = 0.02]. Group I patients also had lower troponin levels （0.62 ± 0.8 ng/mL vs. 27 ± 74 ng/mL; P 〈 0.02）, lower leukocyte count （9.4 ± 3.13 × 10^9 vs. 12 ± 5.1 × 10^9; P = 0.001 ） and better preserved left ventricular function （56.7% ± 14.3 % vs. 45% ± 1 1%; P 〈 0.0001 ）. Event-free survival （cardiac death, myocardial infarction, recurrent angina, and re-hospitalisation） was more frequent in Group Ⅰ and Ⅱ patients compared to Group III patients （64.9%, 66.7%, and 41.6%, respectively; P 〈 0.0001）. Conclusions ACS in patients 〉 75 years without CAD is very infrequent, associated with a （1） similar outcome compared to ACS patients 〈 75 years without CAD, and （2） significant better outcome compared to NSTEMI patients 〉 75 years.

Background Endomyocardial biopsy （EMB） is an important tool when patients with inflammatoric cardiomyopathy （DCMi） are evaluated. We aimed to assess the clinical profile of elderly patients with DCMi on EMB. Methods Retrospective study of all consecutive patients hospitalized from January 2007 to December 2011 with clinical suspicion of DCMi undergoing EMB. Patients with evidence of DCMi on EMB （Group 1 〉 70 years, n = 85; Group 3 〈 70 years; n = 418） were compared to patients of the same age group without evi- dence of DCMi on EMB （Group 2 〉 70 years, n = 45; Group 4 〈 70 years; n = 147）. Results Among 24,275 patients treated at our institu- tion during the study period, 695 had clinical suspicion of DCMi and underwent EMB; 503 （2.1%） patients had DCMi on EMB. There were more male patients in Group 1, mean age was 74 ~ 2.8 years, mean ejection fraction was 38% q- 14%. On presentation, signs of hemody- namic compromise （NYHA functional class IIUIV, low cardiac output/index, and low cardiac power index） were more frequent in Group 1. EMB revealed viral genome in 78% of the patients, parvovirus B 19 （PVB） was frequently encountered in both age groups （Group 1： 69.4% vs. Group 2： 59.6%）; detection of more than one viral genome was more frequent in Group 1 （21.2% vs. 11.2%; P = 0.02） whereas the extent of immune response was significantly lower in individuals with advanced age. Conclusions In patients 〉 70 years with DCMi on EMB signs of hemodynamic compromise, detection of multiple viral genomes together with an overall lower extent of immune response were more frequently observed.

We report the case of an 81-year-old man with aortic coarctation and total occlusion who was otherwise asymptomatic. AIM： To investigate the sonographic features at time of diagnosis and follow-up in patients with neutropenic enterocolitis. METHODS： The sonographic findings in 14 patients with neutropenic enterocolitis were described and evaluated regarding symptoms and clinical outcome. RESULTS： In all patients with neutropenic enterocolitis, the ileocoecal region was involved with wall thickening 〉10 mm. A transmural inflammatory pattern, hypervascularity of the thickened bowel wall and free abdominal fluid were the common findings. The sonographically revealed thickness of the bowel wall was associated with lethal outcome （P〈0.03）. In the 11 surviving patients, the improvement of clinical symptoms was accompanied by progressive reduction of intestinal wall thickness. CONCLUSION： High-end sonography of the bowel is a helpful tool for diagnosis, assessment of prognosis and follow-up of patients with neutropenic enterocolitis. The ultrasonographically revealed bowel thickness reflects the severity and the course of the disease, and seems to be predictive for the clinical outcome.

Background The objective of this study was to identify prognostic indicators in patients with inflammatory cardiomyopathy （iCM） on endomyocardial biopsy （EMB）. Methods and results Between 2007 and 2011 all consecutive patients with diagnosed with iCM at EMB were retrospectively analyzed. The combined primary endpoint （EP）（1E°P） was cardiac death, aborted sudden cardiac death/appropriate implantable cardioverter defibrillator （ICD） shock, progressive heart failure requiring left venWicular assist device （LVAD） implantation and heart transplantation. 503 patients （mean age 58 ° 12 years, 73% male） were available for analysis. Genomes of cardiotrophic viruses were detected in 396 patients （79%） and immuno-histochemical signs of inflammation were present in 223 individuals （44%）. After 3.6 ° 2.4 years of follow-up, cardiac mortality was 3.0% （n = 14） and a total of 8.6% （n = 40） reached the primary endpoint. Independent predictors for the 1 °EP were： age 〉 50 years, presence and duration （〈 28 days） of symptomatic heart failure. A risk stratification approach based on the results of the multivariate analysis demonstrated that absence of signs and/or symptoms of congestive heart failure in younger （〈 50 years） patients with longer （〉 28 days） duration of disease appear to have an excellent prognosis with 100% survival and no events during follow-up The presence of all above mentioned independent risk factors results in an 1°EP occurrence of 35.9%. Conclusions Symptoms of heart fail- ure, short duration of disease, and older age are indicators of poor outcome in patients with iCM.

Congenital left ventricular aneurysm or diverticulum are rare cardiac malformations described in 809 cases since the first description in 1816, being associated with other cardiac, vascular or thoraco-abdominal abnormalities in about 70%. It appears to be a developmental anomaly, starting in the 4th embryonic week. In an experimental study, targeted knockdown of cardiac troponin T in the chick was performed at day 3, after the heart tube has formed. Morpholino treatment of gene TNNT2 at this stage led to the development of left ventricular diver- ticula （LVD） in the primitive left ventricular wall. Diagnosis of left ventricular aneurysms （LVA）/LVD can be made after exclusion of coro- nary artery disease, local or systemic inflammation or traumatic causes as well as cardiomyopathies. Clinically, most of LVA and LVD are asymptomatic or may cause systemic embolization, congestive heart failure, valvular regurgitation, ventricular wall rupture, ventricular tachycardia or sudden cardiac death. Diagnosis is established by imaging studies （echocardiography, magnetic resonance imaging or left ventricular angiography） visualizing the structural changes and accompanying abnormalities. Mode of treatment has to be individually tai- lored and depends on clinical presentation, accompanying abnormalities and possible complications, options include surgical resection （espe- cially in symptomatic patients）, anticoagulation after systemic embolization, radiofrequency ablation or implantation of an implantable car- dioverter defibrillator （ICD） in case of symptomatic ventricular tachycardias, and occasionally combined with class I- or III-antiarrhythmic drugs. Cardiac death occurs usually in childhood, is significantly more frequent in LVA patients and caused by congestive heart failure in most of the cases, whereas patients diagnosed with LVD died more frequently from rupture of the LVD.
