AIM:To evaluate(1) the association of the Helicobacter pylori(***) test and interleukin-8(IL-8) m RNA expression alone and the severity of gastric cancer(GC);(2) the association of both tests were added to patients' characteristics to identifli Thai suspected patients of gastric cancer who would receive the most benefit; and(3) diagnostic value of levels of IL-8 m RNA expression for gastric ***: A cross-sectional analytical study was completed with 220 patients with 86 GC patients who underwent endoscopy with gastric surgery divided into non-metastasis and metastasis groups,and 134 patients with benign lesions who underwent endoscopic examination,at the Gastrointestinal Surgery and Endoscopy Unit,Chiang mai University hospital between 2006 and 2010. Of 220 patients,86 cases of diagnosed gastric adenocarcinoma were in an advanced stage and 134 cases were non-cancer patients. RESULTS: The IL-8 m RNA expression showed predominant association with advanced GC when compared to H. pylori infection alone [OR(95%CI); 0.86(0.49-1.53) vs 5.44(3.08-9.62)] when including the patients' characteristics the highest of the area under the receiver operating characteristic curves(Au ROC) of the model were males older than 40 years of age [AuROC(95%CI); 0.81(0.75-0.86)]. However,preliminary testing for diagnostic indices of four cut-off points of IL-8 m RNA expression to predict the severity of GC cases found an increasing suboptimal trend from the likelihood ratio of positive to differentiate the severity in the GC group. The IL-8 mRNA expression showed a predominant association with GC when compared to H. pylori infection,especially in males older than 40 years of age who may benefit most from this test. CONCLUSION:The future research of IL-8 mRNA expression to predict severity in the gastric cancer group should be warranted.

Research objective: To study a clinical case that had a complication of subclavian artery bleeding during permanent pacemaker implantation process. Subject and Method: Clinical case report and literature review. Study of a patient had an indication for permanent pacemaker implantation who had complication of subclavian artery bleeding during the procedure. Result: An 84-year-old female patient was admitted to the hospital because of fainting spells with chest pain, on the background of dangerous complex arrhythmias and ischemic heart disease, and was prescribed a pacemaker for this patient. The patient had complication occurred during the procedure, which left subclavian artery bleeding was detected and treated promptly, a cardiac pacemaker was implanted on the right side of the chest, and the patient had a successful endovascular stent cover in the left subclavian artery. Conclude: Subclavian artery bleeding is a rare complication in the procedure of permanent pacemaker implantation, but it can still occur and requires quick detection and timely treatment, needs to be successful in cardiac pacemaker placement and subclavian artery drug-eluting stent cover placement intervention. Individualized patient risk assessment is needed;appropriate and standardized intervention techniques and experience can reduce the occurrence of this variable.

Objectives To investigate the rate of anticoagulant use,the reasons for not prescribing anticoagulant,and the factors associated with non-prescription of anticoagulant in older Thai adults with non-valvular atrial *** A multicenter registry of patients with non-valvular atrial fibrillation was conducted during 2014 to 2017 in ***,medical history,antithrombotic medication,non-antithrombotic medication,and laboratory data were collected and *** were compared between the older adult (≥ 65 years) and younger adult (main reasons for not prescribing anticoagulant were already taking antiplatelets,patient refusal,and bleeding *** reasons were more common in older adults as compared to younger *** analysis revealed current use of antiplatelets to be the most important factor that predict the non-prescription of anticoagulant in older *** The prevalence of anticoagulant prescription among older Thai adults with atrial fibrillation is 81.4%.Taking antiplatelet drugs was found to be the strongest reason that predicts the non-prescription of anticoagulant in this patient population.A guideline should be developed to optimize the use of anticoagulant and antiplatelet in older adults.

Background:Hepatocellular carcinoma(HCC)presenting with macroscopic bile duct tumor thrombus(BDTT)is an uncommon *** role of a curative hepatic resection and associated long-term outcomes remain *** addition the necessity for bile duct resection is still *** aim of this study was to evaluate outcomes of hepatectomy with a selective bile duct preservation approach for HCC with BDTT in comparison to outcomes without ***:A total of 22 HCC with BDTT patients who had undergone curative hepatic resection with a selective bile duct preservation approach at our institute were retrospectively *** were compared to group of 145 HCC without BDTT *** impact of curative surgical resection and BDTT on clinical outcomes and survival after surgical resection were ***:All HCC with BDTT cases underwent major hepatectomy vs.32.4%in the comparative *** duct preservation rate was 56.5%.The 1-,3-and 5-year survival rates of HCC with BDTT patients in comparison to the HCC without BDTT group were 81.8%,52.8%and 52.8%vs.73.6%,55.6%and 40.7%(P=0.804)*** resection margin,tumor size≥5 cm and AFP≥200 IU/mL were significant risk factors regarding overall ***,it is unclear whether presence of a bile duct tumor thrombus has an adverse impact on either recurrence free survival or overall ***:Bile duct obstruction from tumor thrombus did not necessarily indicate an advanced form of *** size and AFP had greater impact on long-term outcomes than bile duct tumor *** liver resection with a selective bile duct preserving approach in HCC with BDTT can achieve favorable outcomes comparable to those of HCC without BDTT in selected patients.

BACKGROUND Type B lactic acidosis and hypoglycemia can occur in various pediatric *** young children with a history of fasting preceding these metabolic derangements,inborn errors of metabolism should be primarily ***,the Warburg effect,a rare metabolic complication,can also manifest in children with hematologic *** a few reports of this condition in children have been published in the *** To identify the clinical course,treatment strategies,and outcomes of childhood hematologic malignancies with type B lactic *** We performed a comprehensive search of the PubMed,Scopus,and Cochrane databases without any time restriction but limited to English language *** databases were last accessed on July 1st,*** A total of 20 publications were included in the analysis,all of which were case reports or case *** higher quality evidence was *** children with hematologic malignancies and Warburg effect,there were 14 cases of acute lymphoblastic leukemia and 6 cases of non-Hodgkin’s lymphoma including our illustrative *** acidosis occurred in 55%of newly diagnosed cases and 45%of relapsed *** mean age was 10.3±4.5 years,and 80%of cases were *** mean serum lactate was 16.9±12.6 mmol/L,and 43.8%of the cases had concomitant *** acidosis initially subsided in 80%of patients receiving chemotherapy compared to 60%in the contrast *** mortality rate of newly diagnosed cases was 45.5%,while the relapsed cases represented a 100%mortality *** 8 patients reported before 2001 died from disease-related ***,patients described in reports published between 2003 and 2023 had a 54.5%rate of complete *** This complication has historically led to fatal outcome;however,patients who received chemotherapy showed a more favorable ***,it is crucial to promptly initiate specific treatment in this context.
