Have patients with esophagitis got an increased risk of adenocarcinoma? Results from a population-based study

AIM: To examine an increased risk of esophageal adenocarcinoma is restricted to patients who develop barrett's esophagus or whether esophagitis per se is a risk factor for adenocarcinoma.METHODS: A population-based cohort of patients with histological evidence of esophagitis without barrett's esophagus was constructed using electronic pathology reports relating to all esophageal biopsies in Northern Ireland between 1993 and 1996. Person-years of followup and incident cases of esophageal cancer were calculated by linking the cohort to death files and the Northern Ireland Cancer Registry records. Standardized incidence ratios (SIR) were calculated for esophageal cancers (adenocarcinoma, squamous cell carcinoma (SCC), and histologically unspecified cancers).RESULTS: A total of 2 013 patients in the cohort provided 13 559 patient-years of follow-up (mean follow-up 6.7 years). None of the patients developed adenocarcinoma. Three patients developed SCC, and six developed histologically unspecified cancers. The SIR for all esophageal cancers and for SCC were 2.73 (95%CI 1.25-5.19) and 2.93 (95%CI 0.61-8.59), respectively. In a sensitivity analysis in which all unspecified esophageal cancers were treated as adenocarcinomas, the SIR for adenocarcinoma was 2.64 (0.97-5.75).CONCLUSION: The risk of adenocarcinoma is not elevated in patients with histological evidence of esophagitis without barrett's esophagus; however, these patients may have a moderately increased risk of SCC.Further studies are required to confirm these findings,which suggest that barrett's esophagus, not esophagitis,is the key precursor lesion in the development of adenocarcinoma.

This paper presents a comparative study on Magnetic-Dispersion based Energy Harvesting Systems (md-EHS) on electrical conductors that supply power for a three-phase AC motor. It introduces two md-EHS which are based on magnetic cores of different material, named, nanocrystalline, ferrite and iron powder. The first one consists of harvesting energy from magnetic flux through three symmetrical magnetic cores installed on each power conductors of a three-phase AC motor. The second one consists of a single magnetic core for harvesting energy from magnetic flux of only one of these conductors. Both ones have an AC/DC converter and a variable resistor based load. Experimental results have agreed with the theoretical analysis and show that the first proposed md-EHS is capable of supplying 14 times more energy than the second md-EHS, considering nanocrystalline cores and phase current of 3 A, and 7.5 times more energy, considering ferrite cores and phase current of 9 A. Such energy can be applied to various low-power devices, especially in wireless sensor network.

Chronic obstructive lung disease(COPD),predominantly emphysema,causes several thoracic anatomical and hemodynamic changes which may cause changes in various electrocardiographic parameters.A 12-lead electrocardiogram(ECG),which is often a part of routine evaluation in most clinical settings,may serve as a useful screening modality for diagnosis of COPD or emphysema.Our current article aims to provide a comprehensive review of the electrocardiographic changes encountered in COPD/emphysema utilizing published PubMed and Medline literature database.Several important ECG changes are present in COPD/emphysema and may serve as a good diagnostic tool.Verticalization of Pvector,changes in QRS duration,pattern recognition of precordial R-wave progression and axial shifts can be considered some of the most valuable markers among other changes.In conclusion,12-lead surface electrocardiogram can serve as a valuable tool for the diagnosis of COPD and/or emphysema.An appropriate knowledge of these ECG changes can not only help in the diagnosis but can also immensely help in an appropriate clinical management of these patients.

Stroke is the leading cause of death and disability.Currently,there is no effective pharmacological treatment for this disease,which can be partially attributed to the inability to efficiently deliver therapeutics to the brain.Here we report the development of natural compound-derived nanoparticles(NPs),which function both as a potent therapeutic agent for stroke treatment and as an efficient carrier for drug delivery to the ischemic brain.First,we screened a collection of natural nanomaterials and identified betulinic acid(ba)as one of the most potent antioxidants for stroke treatment.Next,we engineered ba NPs for preferential drug release in acidic ischemic tissue through chemically converting ba to betulinic amine(baM)and for targeted drug delivery through surface conjugation of Amd3100,a CXCR4 antagonist.The resulting Amd3100-conjugated baM NPs,or A-baM NPs,were then assessed as a therapeutic agent for stroke treatment and as a carrier for delivery of NA1,a neuroprotective peptide.We show that intravenous administration of A-baM NPs effectively improved recovery from stroke and its efficacy was further enhanced when NA1 was encapsulated.Due to their multifunctionality and significant efficacy,we anticipate that A-baM NPs have the potential to be translated both as a therapeutic agent and as a drug carrier to improve the treatment of stroke.

There is a need and opportunity for China to develop education and practice innovations given that advance practice nurses (APNs) improve health care and outcomes.The China Medical Board (CMB)China Nursing Network (CCNN) began planning for an Advanced Nursing Practice Program for education and career development that will facilitate CCNN's contributions to meeting national nursing policy priorities.This paper presents the discussion,recommendations and action plans developed at the inaugural planning meeting on June 26,2015 at Fudan University in Shanghai.The recommendations are:Develop standards for advanced nursing practice;Develop Master's level curricula based on the standards;Commence pilot projects across a number of University affiliated hospitals;and Prepare clinical tutors and faculty.The strategic directions and actions are: Develop a clinical career ladder system;Expand the nursing role from hospital to community;and Build a specialty nurse accreditation system.

In recent years,the ternary system baO-ZnO- TiO is of great interest in the production of microwave dielectric ceramics.The addition of ZnO to the barium polytitanates has been found to improve their dielectric property. Knowledge of crystallography of phases in the ba-Zn-Ti-0 system is important to understand the physical properties of these materials.

The development of a nation is deeply related to its energy consumption.2D nanomaterials have become a spotlight for energy harvesting applications from the small-scale of low-power electronics to a large-scale for industry-level applications,such as self-powered sensor devices,environmental monitoring,and large-scale power generation.Scientists from around the world are working to utilize their engrossing properties to overcome the challenges in material selection and fabrication technologies for compact energy scavenging devices to replace batteries and traditional power sources.In this review,the variety of techniques for scavenging energies from sustainable sources such as solar,air,waste heat,and surrounding mechanical forces are discussed that exploit the fascinating properties of 2D nanomaterials.In addition,practical applications of these fabricated power generating devices and their performance as an alternative to conventional power supplies are discussed with the future pertinence to solve the energy problems in various fields and applications.

Persistent slip band （PSB） is an important and typical microstructure generated during fatigue crack initiation. Intensive work has been done to investigate the mechanisms of the formation of persistent slip bands since the 1950s when Wadsworth[1] observed the fatigue fracture in copper. Simulations have indicated that PSBs formation during fatigue crack initiation is related to the dislocation driving force and interaction. In this paper, a molecular dynamics （md） simulation associated with embedded atom model （EAM） is applied to the PSBs formation in nickel-base superalloys with different microstructure and temperature under tensile- tensile loadings. Five md models with different microstructure （pure 5/ phase and γ/γ＇ phase）, grain orientation （[1 0 0][0 1 0][0 0 1] and [1 1 1][1 0 1][1 2 1]） and simulation temperature （300 K, 600 K, 900 K） were built up in these simulations. Our results indicated that within the γ phase by massive dislocations, pile-up and propagation which can penetrate the grain. Also, it is found that the temperature will affect the material fatigue performance and blur PSBs appearance. The simulation results are in strong agreement with published experimental test result. This simulation is based on the work[2]. The highlights of the article include： 1） investigation of the PSB formation via molecular dynamics simulation with three different parameters, 2） conduct of a new deformation and velocity combination controlled simulation for the PSB formation, 3） high-performance computing of PSB formation, and 4） systematic analysis of the PSB formation at the atomic scale in which the dislocation plays a critical role.

Hepatitis C virus (HCV) is a major cause of hepatitis world-wide. The majority of infected individuals develop chronic hepatitis which can then progress to liver cirrhosis and hepatocellular carcinoma. Spontaneous viral clearance occurs in about 20%-30% of acutely infected individuals and results in resolution of infection without sequaelae. Both viral and host factors appear to play an important role for resolution of acute infection. A large body of evidence suggests that a strong, multispecific and long-lasting cellular immune response appears to be important for control of viral infection in acute hepatitis C. Due too the lack of convenient neutralization assays, the impact of neutralizing responses for control of viral infection had been less defined. In recent years, the development of robust tissue culture model systems for HCV entry and infection has finally allowed study of antibody-mediated neutralization and to gain further insights into viral targets of host neutralizing responses. In addition, detailed analysis of antibody-mediated neutralization in individual patients as well as cohorts with well defined viral isolates has enabled the study of neutralizing responses in the course of HCV infection and characterization of the impact of neutralizing antibodiesfor control of viral infection. This review will summarize recent progress in the understanding of the molecular mechanisms of antibody-mediated neutralization and its impact for HCV pathogenesis.
