A systematic review of clinical practice guidelines and best practice statements for the diagnosis and management of varicocele in children and adolescents

A systematic review was conducted to identify and qualitatively analyze the methods as well as recommendations of Clinical Practice Guidelines （CPG） and Best Practice Statements （BPS） concerning varicocele in the pediatric and adolescent population. An electronic search was performed with the MEDLINE, EMBASE, Science Direct, and Scielo databases, as well as guidelines＇ Web sites until September 2015. Four guidelines were included in the qualitative synthesis. In general, the recommendations provided by the CPG/BPS were consistent despite the existence of some gaps across the studies. The guidelines issued by the American Urological Association （AUA） and American Society for Reproductive Medicine （ASRM） did not provide evidence-based levels for the recommendations given. Most of the recommendations given by the European Association of Urology （EAU） and European Society of Pediatric Urology （ESPU） were derived from nonrandomized clinical trials, retrospective studies, and expert opinion. Among all CPG/BPS, only one was specifically designed for the pediatric population. The studied guidelines did not undertake independent cost-effectiveness and risk-benefit analysis. The main objectives of these guidelines were to translate the best evidence into practice and provide a framework of standardized care while maintaining clinical autonomy and physician judgment. However, the limitations identified in the CPG/BPS for the diagnosis and management of varicocele in children and adolescents indicate ample opportunities for research and future incorporation of higher quality standards in patient care.

The road to opportunities： landscape change promotes body-size divergence in a highly mobile species

Landscape change provides a suitable framework for investigating population-level responses to novel ecological pressures. However, relatively little attention has been paid to examine the poten- tial influence of landscape change on the geographic scale of population differentiation. Here, we tested for morphological differentiation of red-necked nightjars Caprimulgus ruficollis breeding in a managed property and a natural reserve situated less than 10 km apart. At both sites, we also esti- mated site fidelity over 5 years and quantified the potential foraging opportunities for nightjars. Breeding birds in the managed habitat were significantly larger in size--as indexed by keel length--than those in the natural one. However, there were no significant differences in wing or tail length. Immigration from neighboring areas was almost negligible and, furthermore, no individual （out of 1130 captures overall） exchanged habitats between years, indicating strong site fidelity. Food supply for nightjars was equally abundant in both habitats, but the av.ilability of foraging sites was remarkably higher in the managed property. As a result, nightjars--particularly fledg- lings-in the latter habitat benefited from increased foraging opportunities in relation to those in the natural site. It seems likely that the fine-scale variation in nightjar morphology reflects a pheno- typic response to unequal local conditions, since non-random dispersal or differential mortality had been determined not to be influential. High site fidelity appears to contribute to the mainten- ance of body-size differences between the two habitats. Results from this nightjar population high- light the potential of human-induced landscape change to promote population-level responses at exceedingly small geographic scales.

Convergence properties of trust-region methods for unconstrained nonconvex optimization is considered in the case where information on the objective function＇s local curvature is incomplete, in the sense that it may be restricted to a fixed set of ＂test directions＂ and may not be av.ilable at every iteration. It is shown that convergence to local ＂weak＂ minimizers can still be obtained under some additional but algorithmically realistic conditions. These theoretical results are then applied to recursive multigrid trust-region methods, which suggests a new class of algorithms with guaranteed second-order convergence properties.

The applications selected illustrate the relative advantage of Trefftz elements over conventional elements in the modeling of wav. propagation in unbounded saturated porous media and of the transient response of incompressible soft tissues.

Hybrid Equilibrium Finite Elements [1-2] provide statically admissible approximations of the stress distribution in linear elastic problems. Though the Finite Element Method has been dominated by formulations based on kinematically admissible approximations and the development of equilibrium based formulations has always been limited, it is unquestionable that there are advantages in also using alternative approaches, namely in terms of error analysis and in design.

Many-body Interatomic Potentials for Computer Simulation of Physical Processes in Metals and Alloys

A model of binding forces in metals and alloys, based on the density functional theory and accounted effects of the many-body interactions, was proposed. The developed method can be easily applied for study of metals and alloys by the molecular dynamics simulation. The potentials of interatomic interactions hav. been found by the proposed method and applied for calculations of equation of state, elastic moduli and phonon dispersion in metals Ni, Al, and alloys NiAl, Ni//3Al. Changing the treatment of heart failure with reduced ejection fraction： clinical use of sacubitril-valsartan combination

Despite significant therapeutic advances, patients with chronic heart failure （HF） remain at high risk of morbidity and mortality. Sacubitill valsartan （previously known as LCZ696） is a new oral agent approved for the treatment of symptomatic chronic heart failure in adults with reduced ejection fraction. It is described as the fast in class angiotensin receptor neprilysin inhibitor （ARNI） since it incorporates the neprilysin inhibitor, sacubitril and the angiotensin Ⅱ receptor antagonist, valsartan. Neprilysin is an endopeptidase that breaks down several vasoactive peptides including natriuretic peptides （NPs）, bradykinin, endothelin and angiotensin II （Ang-II）. Therefore, a natural consequence of its inhibition is an increase of plasmatic levels of both, NPs and Ang-Ⅱ （with opposite biological actions）. So, a combined inhibition of these both systems （Sacubitril / valsartan） may enhance the benefits of NPs effects in HF （natriuresis, diuresis, etc） while Ang-Ⅱ receptor is inhibited （reducing vasoconstriction and aldosterone release）. In a large clinical trial （PARADIGM-HF with 8442 patients）, this new agent was found to significantly reduce cardiovascular and all cause mortality as well as hospitalizations due to HF （compared to enalapril）. This manuscript reviews clinical evidence for sacubitril valsartan, dosing and cautions, future directions and its considered place in the therapy of HF with reduced ejection fraction.

Calculation of Surface Energy of Metals and Alloys by theElectron Density Functional Method

The surface energy of (100), (110), (111) surfaces in Ni, Al and ordered alloys NiAl and Ni//3Al has been calculated within the framework of methodology based upon the electron density functional method. The results of calculations are in good agreement with the known for pure metals experimental data and in case of Ni//3Al alloy. There is also a good agreement with the results obtained by calculation using the embedded atom method. The investigation has been carried out in this work to show that the surface energy sigma of alloys can not be interpreted as an av.raged concentration sigma of pure metals. For NiAl, the obtained results reveal considerable distinctions in anisotropy of sigma in comparison to the anisotropy of surface energy in pure metals. (Edited author abstract) 8 Refs.
