目的:建立和验证一个涉及多级临床场景的白内障协作通用的人工智能(artificial intelligence,AI)管理平台,探索基于AI的医疗转诊模式,以提高协作效率和资源覆盖率。方法:训练和验证的数据集来自中国AI医学联盟,涵盖多级医疗机构和采集模式。使用三步策略对数据集进行标记:1)识别采集模式;2)白内障诊断包括正常晶体眼、白内障眼或白内障术后眼;3)从病因和严重程度检测需转诊的白内障患者。此外,将白内障AI系统与真实世界中的居家自我监测、初级医疗保健机构和专科医院等多级转诊模式相结合。结果:通用AI平台和多级协作模式在三步任务中表现出可靠的诊断性能:1)识别采集模式的受试者操作特征(receiver operating characteristic curve,ROC)曲线下面积(area under the curve,AUC)为99.28%~99.71%);2)白内障诊断对正常晶体眼、白内障或术后眼,在散瞳-裂隙灯模式下的AUC分别为99.82%、99.96%和99.93%,其他采集模式的AUC均>99%;3)需转诊白内障的检测(在所有测试中AUC>91%)。在真实世界的三级转诊模式中,该系统建议30.3%的人转诊,与传统模式相比,眼科医生与人群服务比率大幅提高了10.2倍。结论:通用AI平台和多级协作模式显示了准确的白内障诊断性能和有效的白内障转诊服务。建议AI的医疗转诊模式扩展应用到其他常见疾病和资源密集型情景当中。

Background Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease （CKD） patients are *** aim of the present study was to investigate the prevalence,awareness,treatment,and control of hypertension in the non-dialysis CKD patients through a nationwide,multicenter study in *** The survey was performed in 61 tertiary hospitals in 31 provinces,municipalities,and autonomous regions in China （except Hong Kong,Macao,and Taiwan）.Trained physicians collected demographic and clinical data and measured blood pressure （BP） using a standardized *** was defned as systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg,and/or use of antihypertensive *** 〈140/90 mmHg and 〈130/80 mmHg were used as the 2 thresholds of hypertension *** multivariate logistic regression with adjustment for sex and age,we analyzed the association between CKD stages and uncontrolled hypertension in non-dialysis CKD *** The analysis included 8927 non-dialysis CKD *** prevalence,awareness,and treatment of hypertension in non-dialysis CKD patients were 67.3%,85.8%,and 81.0%,*** hypertensive CKD patients,33.1% and 14.1% had controlled BP to 〈140/90 mmHg and 〈130/80 mmHg,*** successive CKD stages,the prevalence of hypertension in non-dialysis CKD patients increased,but the control of hypertension decreased （P〈0.001）.When the threshold of BP 〈130/80 mmHg was considered,the risk of uncontrolled hypertension in CKD 2,3a,3b,4,and 5 stages increased 1.3,1.4,1.4,2.5,and 4.0 times compared with CKD 1 stage,respectively （P〈0.05）.Using the threshold of 〈140/90 mmHg,the risk of uncontrolled hypertension increased in advanced stages （P〈0.05）.Conclusions The prevalence of hypertension Chinese non-dialysis CKD patients was high,and the hypertension control was *** successive CKD stages,the risk of uncontrolled hypertension increased.

Background The nocturnal nondipping and elevated morning blood pressure （BP） in patients with obstructive sleep apnea syndrome （OSAS） have not yet been well investigated in Chinese patients. This study aimed to describe the BP profile, and to elucidate the relationships between daytime BP and nighttime BP, and between evening BP and morning BP in patients with OSAS. Methods Twenty teaching hospital sleep centers in China were organized by the Chinese Medical Association to participate in this study and 2297 patients were recruited between January 2004 and April 2006. BP assessments were made at four time points （daytime, evening, nighttime and morning） and polysomnography （PSG） was performed and subjects were classified into four groups by their apnea-hypopnea index （AHI）： control, n=213 with AHI 〈5; mild, n=420 with AHI -〉5 and 〈15; moderate, n=460 with AHI -〉15 and 〈30; and severe, n=1204 with AHI -〉30. SPSS 11.5 software package was used for statistical analysis and figure drawing. Results All the average daytime, nighttime, evening and morning BPs were positively correlated with AHI and negatively correlated with nadir nocturnal oxygen saturation. The ratios of nighttime/daytime and morning/evening average BP were positively correlated with AHI. The ratio of nighttime/daytime systolic BP became a ＂reversed BP dipping＂ pattern until the classification reached severe, while the ratio of nighttime/daytime diastolic BP became reversed at moderate. Similarly, the ratio of morning/evening diastolic BP becomes reversed even at mild. Conclusions OSAS may result in higher BP levels at all four time points. The ratios of nighttime/daytime and morning/evening BP increase with increased AHI. The increasing of diastolic BP, which is inclined to rise more quickly, is not parallel with increasing systolic BP.

With the digestive endoscopic tunnel technique(DETT), many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP). Through the tunnel, endoscopic diagnosis or treatment is performed for lesions in the mucosa, in the MP, and even outside the gastrointestinal(GI) *** present, the tunnel technique application range covers the following:(1)Treatment of lesions originating from the mucosal layer, e.g., endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2) treatment of lesions from the MP layer, per-oral endoscopic myotomy, submucosal tunnelling endoscopic resection, etc.; and(3) diagnosis and treatment of lesions outside the GI tract, such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity. With the increasing number of DETTs performed worldwide, endoscopic tunnel therapeutics, which is based on DETT, has been gradually developed and optimized. However, there is not yet an expert consensus on DETT to regulate its indications, contraindications, surgical procedure, and postoperative *** International DETT Alliance signed up this consensus to standardize the procedures of DETT. In this consensus, we describe the definition, mechanism,and significance of DETT, prevention of infection and concepts of DETTassociated complications, methods to establish a submucosal tunnel, and application of DETT for lesions in the mucosa, in the MP and outside the GI tract(indications and contraindications, procedures, pre-and postoperative treatments, effectiveness, complications and treatments, and a comparison between DETT and other operations).

BACKGROUND The treatment outcome of transarterial chemoembolization(TACE)in unresectable hepatocellular carcinoma(HCC)varies greatly due to the clinical heterogeneity of the ***,several prognostic systems have been proposed for risk stratification and candidate identification for first TACE and repeated TACE(re-TACE).AIM To investigate the correlations between prognostic systems and radiological response,compare the predictive abilities,and integrate them in sequence for outcome *** This nationwide multicenter retrospective cohort consisted of 1107 unresectable HCC patients in 15 Chinese tertiary hospitals from January 2010 to May *** Hepatoma Arterial-embolization Prognostic(HAP)score system and its modified versions(mHAP,mHAP2 and mHAP3),as well as the six-and-twelve criteria were compared in terms of their correlations with radiological response and overall survival(OS)prediction for first *** same analyses were conducted in 912 patients receiving re-TACE to evaluate the ART(assessment for re-treatment with TACE)and ABCR(alpha-fetoprotein,Barcelona Clinic Liver Cancer,Child-Pugh and Response)systems for post re-TACE survival(PRTS).RESULTS All the prognostic systems were correlated with radiological response achieved by first TACE,and the six-and-twelve criteria exhibited the highest correlation(Spearman R=0.39,P=0.026)and consistency(Kappa=0.14,P=0.019),with optimal performance by area under the receiver operating characteristic curve of 0.71[95%confidence interval(CI):0.68-0.74].With regard to the prediction of OS,the mHAP3 system identified patients with a favorable outcome with the highest concordance(C)-index of 0.60(95%CI:0.57-0.62)and the best area under the receiver operating characteristic curve at any time point during follow-up;whereas,PRTS was well-predicted by the ABCR system with a C-index of 0.61(95%CI:0.59-0.63),rather than ***,combining the mHAP3 and ABCR systems identified candidates suitable for TA

In a changing world with a high interest in new ecomaterials, natural fibers such as the steelwool or Luffa cylindrica appear in new presentations and mixing with other materials to develop better opportunities to replace synthetic fibers. This work presents a research on the use of steelwool fiber for composite materials, on the basis of the physical properties, that generate conditions of mixing with three binders (matrix) natural: the rosin, artificial hard: polyurethane resin and artificial flexible: flexible twin resin (epoxy) and finally a polyester resin to make a comparison with other major resins. A testing of compression and tension is carried out to the materials analyzed, obtaining three types of composite materials by the above mentioned binders and three proposed presentations of fiber (complete, tissue and ground). The test tube in tension with polyester resin presented a high rigidity and a percentage of deformation of 14%, resulting in less distortion than the woven with resin polyurethane with 12% of deformation. The presentation with resin polyurethane presented greater resistance to compression, because the resin acts as a sponge absorbing the energy of charge and the join of particles is larger than the other presentations generating greater cohesion among them and avoiding its rupture easily for a load of 2,000 kg. The presentation that is least resisted was the woven in two resins in the stage of energy absorption of load where polyurethane is 800 kg and the flexible twin is 850 kg, because the form of woven fiber distribution creates spaces where there are more resin than fiber.

目的:分析2019冠状病毒病(COVID-19)患者首次胸部CT影像表现及其与临床表现(包括发热症状、实验室检查)的相关性。创新点:初步探索COVID-19肺部病灶与临床数据的相关性,对评估COVID-19患者提供一定的依据。方法:纳入2020年1月16日至2020年2月6日入院的36例COVID-19患者,回顾性分析首次胸部CT的影像学表现,包括影像特征、分布模式、累及范围等,并进一步分析其与临床特征(包括发病时间、发热症状、白细胞计数等)的相关性。结论:结果发现,在自然病程中,COVID-19肺部病变与发病时间显著相关,特别是发热症状。COVID-19病变范围与淋巴细胞计数呈负相关,淋巴细胞减少可能是COVID-19进展的标志之一。

目的：就高血压、冠心病、充血性心力衰竭（CHF）患者测定＂中国心血管病人生活质量评定问卷＂（CQQC）的常模,并分析其影响因素。方法：采用整群随机抽样的方法,对地跨中国各大区的19个城市28所医院的7937名对象的生活质量进行调查,建立全国常模,并比较性别、年龄、运动、心理状态、教育、家庭以及医疗条件所致的差异。结果：1、正常人平均得分（国家常模）为（95.74±23.21）分;2、高血压患者平均得分为（73.75±23.62）,高血压3级患者生活质量（69.06±21.78）明显低于其他患者（P〈0.01）;3、冠心病患者的平均得分为（64.67±20.59）,其中不稳定型心绞痛患者的生活质量（55.37±18.62）最差（P〈0.01）;4、CHF患者的平均得分为（40.36±17.24）,显著低于高血压和冠心病患者（P〈0.01）;5、影响因素：（1）性别、年龄：所有人的总体得分为（71.41±27.8）分,男性得分非常显著高于女性（P=0.000）;正常人随着年龄增加,得分逐渐减少（P〈0.05）,除≥70岁组,其他各年龄组男性得分显著高于女性得分（P〈0.05）;（2）运动：参加运动者的得分（72.64±21.87）非常显著高于未参加运动者（66.91±23.13）,P=0.000;（3）心理：心理健康者的得分（70.13±25.66）非常显著高于心理有障碍者（68.91±20.44）,P=0.001;（4）其他：受教育程度越高,得分越高;家庭成员为1个人的得分明显低于2人组和3人及以上组的得分;自费患者的得分低于医保或公费患者的得分（P〈0.05~〈0.001）。结论：本研究样本量大,数据可靠;中国心血管病人生活质量评定问卷能反映年龄、性别、运动、心理、家庭、受教育程度、医疗条件和疾病对生活质量的影响,条目简短明了,可操作性好,值得推广。

Background:The 5-fluorouracil/leucovorin plus oxaliplatin(FOLFOX)regimen is the standard first-line treatment for metastatic colorectal cancer(mCRC),however,the optimal second-line regimen for KRAS wild-type mCRC patients is still *** this study,we aimed to determine the clinical efficacy and safety of CMAB009 plus irinotecan compared to irinotecan-only as a second-line regimen for treating KRAS wild-type mCRC ***:Patients with KRAS wild-type mCRC who had previously failed to respond to FOLFOX treatment were ran-domly assigned in a 2:1 ratio,to receive CMAB009 plus irinotecan or *** receiving irinotecan-only were permitted to switch to CMAB009 therapy on disease progression and were grouped as the sequential-CMAB009 *** primary endpoints were overall response rate(ORR)and median progression-free survival(PFS).The second-ary endpoints were median overall survival(OS),disease control rate(DCR),clinical benefit rate(CBR),and duration of response(DOR).Results:The CMAB009 plus irinotecan arm demonstrated significantly improved ORR(33.2%vs.12.8%;P<0.001)and longer median PFS(169 days vs.95 days;P<0.001)as compared to the irinotecan-only *** receiv-ing CMAB009 plus irinotecan also demonstrated improved DCR(80.1%vs.65.2%,P<0.001),CBR(30.0%vs.14.6%,P<0.001),and DOR(210 days vs.109 days;P<0.001)as compared to ***,patients treated with CMAB009 had an increased risk of skin rash(66.9%vs.5.5%,P<0.001)and paronychia(9.8%vs.0.0%,P<0.001).Anti-drug antibodies(ADA)were detected in 3.6%of patients,and only 0.9%of patients who received CMAB009 experienced hypersensitivity *** patients receiving sequential-CMAB009 therapy after failure with irinotecan,their median PFS was 84 days (95% CI 65 to 113 days).
