Adverse effects of oral antiviral therapy in chronic hepatitis B

Oral nucleoside/nucleotide analogues(NAs) are currently the backbone of chronic hepatitis B(CHB) infection treatment. They are generally well-tolerated by patients and safe to use. To date, a significant number of patients have been treated with NAs. Safety data has accumulated over the years. The aim of this article is to review and update the adverse effects of oral NAs. NAs can cause class adverse effects(i.e., myopathy, neuropathy, lactic acidosis) and dissimilar adverse effects. All NAs carry a "Black Box" warning because of the potential risk for mitochondrial dysfunction. However, these adverse effects are rarely reported. The majority of cases are associated with lamivudine and telbivudine. Adefovir can lead to dose- and time-dependent nephrotoxicity, even at low doses. Tenofovir has significant renal and bone toxicity in patients with human immunodeficiency virus(HIV) infection. However, bone and renal toxicity in patients with CHB are not as prominent as in HIV infection. Entecavir and lamivudine are not generally associated with renal adverse events. Entecavir has been claimed to increase the risk of lactic acidosis in decompensated liver disease and high Model for End-Stage Liver Disease scores. However, current studies reported that entecavir could be safely used in decompensated cirrhosis. An increase in fetal adverse events has not been reported with lamivudine, telbivudine and tenofovir use in pregnant women, while there is no adequate data regarding entecavir and adefovir. Further long-term experience is required to highlight the adverse effects of NAs, especially in special patient populations, including pregnant women, elderly and patients with renal impairment.

Chronic hepatitis B in children with or without malignancies:A 13-year follow-up

AIM:To evaluate the outcome of chronic hepatitis B(CHB)in children with or without ***:Twenty four children(15 boys and 9 girls)with malignancies,followed up by the pediatric gastroenterology outpatient clinic for CHB between January 2000 and December 2013,were enrolled in the study(Group 1).Group 2 was formed with twenty five children(11 girls and14 boys)diagnosed with CHB without *** data from the patients’records were compared between the two ***:Hepatitis B e antigen(HBe Ag)/anti HBe seroconversion was observed in 3 patients(12.5%)in group 1 and 15 patients(60%)in group 2,with annual seroconversion rates of 1.61%and 16.6%,respectively,and the difference was significant(P<0.01).One patient(6.6%)in Group 1 and 9 patients(53%)in Group 2 showed HBe Ag/anti HBe seroconversion after treatment and the difference between the two groups was significant(P<0.06)Loss of hepatitis B surface antigen was observed in one patient in each of group1 and *** clinical,laboratory and imaging findings of liver disease were observed in any of the patients at the end of the ***:HBe Ag/anti HBe seroconversion rate was lower in patients who had recovered from cancer.

Nerve regeneration following traumatic peripheral nerve injuries and neuropathies is a complex process modulated by diverse factors and intricate molecular *** studies have focused on factors that stimulate axonal outgrowth and myelin ***,recent studies have highlighted the pivotal role of autophagy in peripheral nerve regeneration,particularly in the context of traumatic ***,autophagy-targeting modulation has emerged as a promising therapeutic approach to enhancing peripheral nerve *** current understanding suggests that activating autophagy facilitates the rapid clearance of damaged axons and myelin sheaths,thereby enhancing neuronal survival and mitigating injury-induced oxidative stress and *** actions collectively contribute to creating a favorable microenvironment for structural and functional nerve regeneration.A range of autophagyinducing drugs and interventions have demonstrated beneficial effects in alleviating peripheral neuropathy and promoting nerve regeneration in preclinical models of traumatic peripheral nerve *** review delves into the regulation of autophagy in cell types involved in peripheral nerve regeneration,summarizing the potential drugs and interventions that can be harnessed to promote this *** hope that our review will offer novel insights and perspectives on the exploitation of autophagy pathways in the treatment of peripheral nerve injuries and neuropathies.

Central corneal thickness measurements in premature infants

AIM:To evaluate the central corneal thickness(CCT)measurements of premature infantsMETHODS:The premature infants who were born between 25-34wk of gestational age(GA)were retrieved for the *** CCT measurements were performed within the first 2d of their life under topical anesthesia by using an ultrasonic ***:The mean CCT of 200 eyes was 647.79±63.94μ*** mean CCT of the right and left eyes were similar(647.30±64.72μm and 648.29±63.47μm consecutively).The mean CCT of the girls was 642.08±54.89μm and the boys was 653.07±71.14μm,with no statistically significant difference(P>0.05).But there was a negative correlation of the CCT with GA and birth weight(BW)(P<0.01).CONCLUSION:The CCT values of premature babies were negatively correlated with GA and BW.

Corneal white-to-white distance and mesopic pupil diameter

·AIM:To study the relationship between corneal white-to- white (WTW) distance and mesopic pupil diameter. ·METHODS:This study is composed of 30 cases that underwent photorefractive keratotomy (PRK). Pupil size measurements were performed with Schwind ORK wavefront analyzer in mesopic conditions. WTW distance was measured with a measuring caliper. Also, A-scan ultrasound examination was performed in all patients. The relationship among the mesopic pupil diameter and age, sex, axial length, lens thickness, anterior chamber depth(ACD), horizontal WTW distance, vertical WTW distance, spherical equivalent, and average keratometry were analyzed with univariate and multivariate regression analysis. ·RESULTS:Mean pupil diameter was (6.39±0.80)mm (range:3.70mm to 7.73mm. Horizontal WTW distance measurements were between 11.00mm and 12.50mm and mean horizontal WTW distance was (11.79±0.43)mm. On the other hand, vertical WTW distances ranged between 10.00mm and 13.00mm, and their mean was(11.42±0.72)mm. Bivariate correlation between pupil diameter and other variables showed that the axial length, ACD, spherical equivalent, and horizontal WTW distance had a moderate correlation with mesopic pupil diameter. Multiple regression analysis revealed that spherical equivalent and horizontal WTW distances were significantly associated with mesopic pupil diameter (R =0.598, R 2=0.358 P =0.02). ·CONCLUSION:This study shows that mesopic pupil diameter is closely related to horizontal WTW distance. These two factors must be taken in consideration together in preoperative ablation zone planning.

Toxic keratopathy associated with abuse of topical anesthetics and amniotic membrane transplantation for treatment

AIMTo evaluate the clinical findings of toxic keratopathy associated with abuse of topical anesthetics and the treatment efficacy of amniotic membrane transplantation (AMT).METHODSFifteen eyes of 10 patients with abuse of topical anesthetics were included in this study. The data collected included patients' demographic information, initial best corrected visual acuity (BCVA), clinical findings, time to AMT, duration of epithelization, additional treatments, posttreatment BCVA, and the results of psychiatric *** patients were male, with the mean age of 37.9±5.4y. The patients had received 0.5% proparacaine hydrochloride due to pain resulting from foreign bodies, such as welding flash burns and car battery explosions, for a mean of 18.2±12.4d. The mean initial BCVA of the patients was 0.069±0.100. All patients had persistent epithelial defects, stromal infiltration, ring ulcer, and corneal edema. Initially, medical treatment was attempted in all cases. Because of severe pain, persistent epithelial defects and progressing stromal thinning did not improve with medical treatment, thus, the patients underwent AMT. The mean posttreatment BCVA of the patients was 0.33±0.27. All of them, except for two patients, showed improved visual acuity. One patient underwent evisceration for corneal melting and endophthalmitis in another center, and one patient underwent evisceration for severe pain of unknown origin. All 5 patients who consented to a psychiatric examination had depression, had personality disorder, and used *** appears to be an effective method for pain relief, rapid epithelial and stromal healing; however, visual prognosis is still poor despite medical and surgical interventions.

Novel markers in predicting Brucella sacroiliitis:The platelet large cell ratio and basal immature reticulocyte fraction

Objective:To present platelet large cell ratio(P-LCR),reticulocyte,and immature reticulocyte fraction(IRF)values as novel parameters in diagnosis and response to treatment in patients developing ***:Sixty-eight patients with clinical symptoms and Brucella standard tube agglutination(Wright)or Brucella Coombs agglutination test titers≥1:160 were included in the *** groups were established,one developing sacroiliitis and another with no sacroiliitis development.P-LCR,reticulocyte,and IRF levels were measured using a Sysmex XN-9000 device(Japan).These were then compared between the two ***:Reticulocyte(P=0.037)and IRF(P=0.026)levels were significantly lower among the patients developing sacroiliitis compared to the non-sacroiliitis group,while P-LCR(P=0.003)levels were significantly higher.P-LCR had the most powerful correlation with sacroiliitis *** negative correlation was observed between reticulocyte,IRF levels and ***:Elevated P-LCR levels were observed as a marker of persisting inflammation in patients developing sacroiliitis,while low reticulocyte and IRF levels secondary to bone marrow involvement were *** three parameters emerged as highly significant markers in terms of diagnosis and reflecting responses to treatment in organ involvement such as sacroiliitis in *** are presented as inexpensive,and easily accessible novel parameters.

Effect of long-term Helicobacter pylori infection on growth of children:a cohort study

Background:The effects of Helicobacter pylori(***)infection on growth are a controversial *** investigated the effects of long-term *** infection on height and weight in ***:A total of 200 children of 7-18 years old suffering from dyspeptic complaints were classified into two groups:*** positive and negative groups,*** the infection was impoved was followed up while performing urea breath test,and according to exposure time to the infection,the children were further divided into group 1(≤1.5 months),group 2(>1.5-≤6 months)and group 3(>6 months).Antropometric measurements were obtained and repeated every six ***:Mean growth velocity scores in the *** positive and negative groups were 0.49±3.85[95%confidence interval(CI):-0.21-1.18]and 1.98±4.42(95%CI:1-2.96),*** difference between both groups was statistically significant(P=0.012).Mean growth velocity scores in groups 1,2 and 3 were 0.96±3.84,0.16±4.51 and-0.85±3.09,*** growth velocity scores of group 3 were significantly lower than those of groups 0 and 1(P=0.005 and P=0.041).The mean weight scores in group 3 were similar to those in group 2,but the scores in group 3 were signifi cantly lower than those in group 1(-1.75±1.05,-1.21±1.37 and-0.88±1.49,respectively).Conclusion:As the duration of exposure is prolonged in children with *** infection,the negative effect of the infection on both height and weight is evident.
