Coronary heart disease is among the most prevalent and costly of all global health problems. Coronary artery bypass grafting (CABG) has been increasingly used since the 1960s. Coronary heart disease is a leading cause of death worldwide, including Sudan. Self-efficacy and functional capacity is a critical factor for quality of life in patients who has undergone CABG as well as for their caregivers from their families. Aim of the Study: To explore the effects of an educational program on self-efficacy and functional capacity of patients after CABG. 1) There will be no significant difference between the intervention group and control group concerning knowledge regarding Self-efficacy and functional capacity post CABG surgery. 2) The application of our educational program for the patients will enhance the patients with CABG surgery and improve knowledge about Self-efficacy and functional capacity post CABG surgery. 3) There are some socio demographic factors that affect the response to the educational program. Methods: This was a quasi-experimental, interventional and hospital-based study, including pre-test, a post-test and follow-up test, with a control group, conducted during the period from September 2014 to June 2019. Patients were recruited consecutively to the cases and control group using convenience sampling. The sample consisted of 76 patients;41 as cases and 35 as control group. The sample included those who had undergone CABG in Khartoum State. Relevant data were collected using a checklist to monitor the effect of a health education program on self-efficacy and functional capacity of the patients. Result: The results showed marked improvement in patients’ self-efficacy and functional capacity between pretest and posttest for the study group;p-value = (0.001) in the health status of cases in comparison to the control group. Barriers to nurse-patient communication in primary healthcare centers in Bahrain:Patient perspective

Objectives:Effective and efficient communication is a core element in healthcare systems,especially between healthcare providers and *** study aimed to identify communication barriers be-tween nurses and patients in primary healthcare centers in ***:This is a cross-sectional study conducted across primary healthcare centers in *** hundred and two patients were recruited using convenience sampling.A self-administered question-naire comprising 29 items on communication barriers was ***:A total of 402 patients consented to *** majority of participants reported the following statements had large effects on communication:“shortage in the number of nurses compared to the large number of patients”(254/401,63.3%),“lack of desire of nurse to communicate with patients”(246/402,61.2%),and“negative attitude of the nurse toward the patient”(238/401,59.4%).Further,“difference in language between nurses and patients,”“lack of self-confidence by nurses,”and“nurses overwhelmed by work”were ranked as top three statements with a significant influence on commu-nication between nurses and ***:Communication between healthcare providers and patients is pivotal for an optimal healthcare *** on the findings of this study and the literature,we recommend formal training of health care workers in improving communication skills and including this not only in medical cur-riculum but also in the form of continuing medical education(CMEs)1.

Air pollutants and early origins of respiratory diseases

Air pollution is a global health threat and causes millions of human deaths annually. The late onset of respiratory diseases in children and adults due to prenatal or perinatal exposure to air pollutants is emerging as a critical concern in human health. Pregnancy and fetal development stages are highly susceptible to environmental exposure and tend to develop a long-term impact in later life. In this review, we briefly glance at the direct impact of outdoor and indoor air pollutants on lung diseases and pregnancy disorders. We further focus on lung complications in later life with early exposure to air pollutants. Epidemiological evidence is provided to show the association of prenatal or perinatal exposure to air pollutants with various adverse birth outcomes, such as preterm birth, lower birth weight, and lung developmental defects, which further associate with respiratory diseases and reduced lung function in children and adults. Mechanistic evidence is also discussed to support that air pollutants impact various cellular and molecular targets at early life, which link to the pathogenesis and altered immune responses related to abnormal respiratory functions and lung diseases in later life.

This study aimed to establish the core and functional competency model for the health care consultants,taking nine health care consultants from a health care center affiliated to a teaching hospital in Taiwan as the study *** study was conducted through three ***,it conducted behavior event interview to sort out the functional competency model for each group type,so as to understand the functional competencies of each functional ***,constructing the competency scale by questionnaire survey method aimed to verify the importance of the functional ***,the common competencies were found out based on the functional model of each group type,and the organizational vision was taken into account to analyze the core *** results of the study summarized nine core competencies and 7 functional competency items of the reporting group,1 functional competency item of the consulting room group,6 functional competency items of the endoscope group,2 functional competency items of the reception group and 5 functional competency items of the guiding group leader group and proposed suggestions for future practice.

Predictors associated with inappropriate transport of near shore spinal injuries

Purpose:Spinal injuries resulting in neurological damage cause significant *** neurosurgical intervention can mitigate negative ***,variable mechanisms of injury may be associated with inappropriate transport(IAT),which may delay necessary surgical *** with near shore spinal injuries(NSSI)presented with unique mechanisms,so we investigated factors associated with IAT in patients with ***:We performed a multicenter retrospective study of all adult patients transported from a beach resort to 3 hospitals for suspected NSSI between *** excluded patients transferred to other facilities,and those not injured in the *** outcome was IAT,defined as patients with NSSI requiring transfer to another trauma *** avoid heterogeneity in our analysis,we further excluded patients without NSSI who were inappropriately transported to a level I trauma *** used multivariable logistic regression to assess association of independent variables(such as demographic,environmental,and clinical factors)with ***:We analyzed 278 patients with suspected NSSI,and found 14(5.0%)had *** to appropriately transported patients,diving was associated with higher percentages of IAT(28.6%vs.3.9%,p=0.014)and more were transported by air(50.0%vs.20.6%,p=0.01).In multivariable regression,patients'oxygenation saturation(odds ratio[OR]=0.8,95%confidence intervals[Cl]:077-0.98)and diving(OR=7.5,95%Cl:1.2-46)were significantly associated with ***:Rate of IAT for patients with NSSI was ***,first responders and emergency medicine providers should be aware that diving is associated with a higher likelihood of IAT.

＂Besides an evaluation of an overseas supplier by using normal methods, some enterprises start their collaboration with him by assigning a small trial order and if supplier＇s performance is considered to be satisfactory then a full order capable of sufficing all the appeared demand comes from the enterprise＂[50]. This could be one of the practices followed by an organization/company that procures Military critical Items-MCIs, items with a major impact to the safety and the accomplishment of a mission. In our conceptual approach, we will try to propose a tool for a quick and simple supplier selection under the method mentioned above. Furthermore, in almost every procurement scenario, major critical success factors for supplier selection, such as Quality and Delivery, considered also as military criteria [46], play an extremely important role [5]. Based on this, we argue that final decision for the approved supplier could be based on what we call Coefficient of Total Trial Performance-CTTP, constituted by the following factors： P＆G Perfect Order concept of each trial order [50], Prediction Intervals/Confidence Intervals for achieved delivery times as a statistical estimator of future delivery times, A suggested factor, called the Management Trial Score-MTS.

Human retina development involves multiple well-studied signaling pathways that promote the genesis of a wide arrange of different cell types in a complex architectural *** embryonic stem cells(hESCs)-derived retinal organoids could recapitulate the human retinal *** performed single-cell RNA-seq of retinal organoids from 5 time points(D36,D66,D96,D126,D186)and identified 9 distinct populations of *** addition,we analyzed the molecular characteristics of each main population and followed them from genesis to maturity by pseudotime analysis and characterized the cell-cell interactions between different cell ***,we identified insulin receptor(INSR)as a specifically expressed receptor involved in the genesis of photoreceptors,and pleiothropin(PTN)-protein tyrosine phosphatase receptor type Z1(PTPRZ1)as a mediator of a previously unknown interaction between Müller and retinal progenitor *** together,these findings provide a rich transcriptome-based lineage map for studying human retinal development and modeling developmental disorders in retinal organoids.

Fluorescent glycan nanoparticle-based FACS assays for the identification of genuine drug-resistant cancer cells with differentiation potential

Herein we develop a unique differentiated-uptake strategy capable of efficient and high-purity isolation of genuine drug-resistant(DR)cells from three types of drug-surviving cancer cells,which include paclitaxel-surviving human ovarian OVCAR-3 cancer cells and human lung carcinoma A549/Taxol cells,and doxorubicin-surviving human immortalized myelogenous leukemia K562/ADR *** using this strategy which relies on fluorescent glycan nanoparticle(FGNP)-based fluorescence-activated cell sorting(FACS)assays,two subpopulations with distinct fluorescences existing in drug-surviving OVCAR-3 cells were separated,and we found that the lower fluorescence(LF)subpopulation consisted of DR cells,while the higher fluorescence(HF)subpopulation was comprised of non-DR ***,the DR cells and their progenies were found distinct in their increased expression of drug-resistant *** intriguingly,by using the FGNP-based FACS assay to detect DR/non-DR phenotypes,we found that the DR phenotype had a potential to differentiate into the non-DR progeny,which demonstrates the differentiation feature of stem-like cancer *** research disclosed that the assay can quantitatively detect the degree of drug resistance in DR cells,as well as the reversal of drug resistance that are tackled by various therapeutic *** strategy thus paves the way to develop theranostic approaches associated with chemotherapy-resistance and cancer stemness.
