Dr. Jianxin Jiang joined the army and studied clinical medicine at the Third Military Medical University in 1980,which is now Army Medical University. After graduation, he devoted himself to military medical research. He traveled to the Ludwig Boltzmann trauma Institute in Austria as a visiting scholar from 1991 to 1993 and received a doctorate in field surgery.

Therapeutic Effectiveness of Leukocyte-and Platelet-rich Fibrin for Diabetic Foot Ulcers:A Retrospective Study

Objective Diabetic foot ulcer(DFU)is one of the most serious complications of ***-and platelet-rich fibrin(L-PRF)is a second-generation autologous platelet-rich *** study aims to investigate the clinical effects of L-PRF in patients with diabetes in real clinical *** Patients with DFU who received L-PRF treatment and standard of care(soc)from 2018 to 2019 in Tongji Hospital were *** clinical information including patient characteristics,wound evaluation(area,severity,infection,blood supply),soc of DFU,and images of ulcers was retrospectively extracted and analyzed.L-PRF treatment was performed every 7±2 days until the ulcer exhibited complete epithelialization or an overall percent volume reduction(PVR)greater than 80%.Therapeutic effectiveness,including overall PVR and the overall and weekly healing rates,was *** Totally,26 patients with DFU were enrolled,and they had an ulcer duration of 47.0(35.0,72.3)*** severity and infection of ulcers varied,as indicated by the Site,Ischemia,Neuropathy,Bacterial Infection,and Depth(SINBAD)scores of 2–6,Wagner grades of 1–4,and the Perfusion,Extent,Depth,Infection and Sensation(PEDIS)scores of 2–*** initial ulcer volume before L-PRF treatment was 4.94(1.50,13.83)cm3,and the final ulcer volume was 0.35(0.03,1.76)*** median number of L-PRF doses was 3(2,5).A total of 11 patients achieved complete epithelialization after the fifth week of treatment,and 19 patients achieved at least an 80%volume reduction after the seventh *** overall wound-healing rate was 1.47(0.63,3.29)cm3/week,and the healing rate was faster in the first 2 weeks than in the remaining *** treatment did not change the percentage of complete epithelialization or healing *** Adding L-PRF to soc significantly improved wound healing in patients with DFU independent of the ankle brachial index,SINBAD score,or Wagner grade,indicating that this method is appropriate for D

State of the art in enzymatic debridement

Surgical treatment of deep partial thickness to full thickness burn wounds by knife has been the undisputed standard of care and was one key point in surgical burn medicine for decades. Recently, it gets more and more challenged by Bromelain-based enzymatic burn wound debridement (ED) as technique for non-surgical, selective eschar removal. Although the literature on ED is increasing constantly it cannot comprise the rapid progress that is made in clinical application of ED. To outline the current state of art in ED, recent literature as well as clinical experience is summarized and the main steps in clinical application including indications, wound preparation, application of the enzyme, wound bed assessment and further treatment after ED are discussed. Initial indications and limitations in application of ED could be gradually extended to increase versatility of ED as tool in burn surgery. Several randomized controlled trials compared ED to standard of care (soc). They could show significant shorter time to complete burn wound debridement and wound closure, reduced need for surgery, reduced blood loss, reduced area of burns that needed surgical excision and need for autograft as well as an improved scar quality. Further research is necessary to justify an extensive use of ED as tool for burn eschar removal. Especially a robust comparison to surgical burn wound excision by knife as soc is required to facilitate evidence-based burn surgery.

[目的]本文对国际细胞治疗学会(International soc iety of Cell Therapy,ISCT)第10次年会的会议内容进行综述。本次年会中不同的学者报告了干细胞的分离、纯化和扩增方法,以及干细胞多分化潜能定向调控、免疫调节、归巢募集机理的最新进展。细胞治疗已在肌肉骨骼相关疾病、神经系统疾病、癌症或肿瘤、免疫系统疾病、糖尿病等多种临床学科得到应用,展现了广阔的基础与临床研究应用前景。细胞类型涉及胚胎干细胞、间充质干细胞、造血干细胞和神经干细胞等。这些干细胞可特异性归巢于病损部位,直接参与病损组织或器官的再生和修复,成为多种组织损伤修复的细胞源或基因治疗的细胞载体,其分泌的多种生长因子和细胞因子也直接参与损伤修复反应。有关细胞治疗的基础、临床与药物研发的多方面研究,将进一步提高临床细胞治疗的安全性和有效性。
