Although some authors suggest that there is mitotic division in the heart,most cardiomyocytes do not have the capacity to regenerate after myocardial infarction and when this occurs there is a deterioration of contractile function,and if the area of infarction is extensive ventricular remodeling may occur,leading to the development of heart *** transplantation into the myocardium with the goal of recovery of cardiac function has been extensively studied in recent years. The effects of cell therapy are based directly on the cell type used and the type of cardiac *** myocardial ischemia in the hibernating myocardium, bone marrow cells have functional benefits,however these results in transmural fibrosis are not evident. In these cases there is a benefit of implantation with skeletal myoblasts,for treating the underlying cause of disease,the loss of cell contractility.

Objective:To describe a patient with presarcopenia and chronic cough secondary to chronic obstructive pulmonary disease(COPD)successfully treated with supplements and physical *** report:A 75-year-old female patient with a positive past medical history of systemic hypertension,dyslipidemia,heart arrhythmia,and smoking during 20 years evolved with chronic cough due to a chronic obstructive pulmonary disease diagnosed ten years *** came to our private clinic due to low weight,low *** weight was 44.8 kg,her height 1.57 m,body mass index of 18.18 kg/m^(2).Laboratory tests showed SDHEA 76.3,vitamin D of 15.6 ng/ml(nr:>30 ng/ml),C-reactive protein(CRP)of 55 mg/*** tomography showed *** had an skeletal muscle mass index of 5.0 kg/m^(2)(nr:>5.5 kg/m^(2))by DXA.A diagnosis of presarcopenia was determined based on DXA evaluation with low muscle mass but normal gait speed handgrip *** suggested to the patient to increase physical exercise and prescribed a supplement *** five months,she returned asymptomatic,without cough,marked improvement of fatigue,increased energy levels,and weight increased to 50 kg,BMI of 20.28 kg/m^(2).Laboratory tests showed SDHEA to 140,vitamin D3 to 64.5 ng/ml,reduced CRP reduced to 5 mg/dl,and amlodipine was excluded due to better blood pressure ***,two years later,the patient continues without cough and has dyspnea only with high efforts,without fatigue,and her weight is 52 kg and BMI 21.1 kg/m^(2).She also reduced her conventional treatment for COPD,using only a bronchodilator on-demand,without topical ***:This case illustrates an interesting case of a patient with presarcopenia and chronic cough,refractory to conventional approach,successfully treated with a combination of nutraceuticals and physical exercises.

The development of measurement geometry for medical X-ray computed tomography （CT） scanners was carried out from the first to the fourth-generation. This concept has also been applied for imaging of industrial processes such as pipe flows or for improving design, operation, optimization and troubleshooting. Nowadays, gamma CT permits to visualize failure equipment points in three-dimensional analysis and in sections of chemical and petrochemical industries. The aim of this work is the development of the mechanical system on a third-generation industrial CT scanner to analyze laboratorial process columns which perform highly efficient separation, turning the ^6oCo, ^75Se, ^137Cs and/or ^192Ir sealed gamma-ray source（s） and the NaI（Tl） multidetector array. It also has a translation movement along the column axis to obtain as many slices of the process flow as needed. The mechanical assembly for this third-generation industrial CT scanner is comprised by strength and rigidity structural frame in stainless and carbon steels, rotating table, source shield and collimator with pneumatic exposure system, spur gear system, translator, rotary stage, drives and stepper motors. The use of suitable spur gears has given a good repeatability and high accuracy in the degree of veracity. The data acquisition boards, mechanical control interfaces, software for movement control and image reconstruction were specially development. A multiphase phantom capable to be setting with solid, liquid and gas was testing. The scanner was setting for 90 views and 19 projections for each detector totalizing 11,970 projections. Experiments to determine the linear attenuation coefficients of the phantom were carried out which applied the Lambert-Beer principle. Results showed that it was possible to distinguish between the phases even the polymethylmethacrylate and the water have very similar density and linear attenuation coefficients. Altered Expression in Patients with Heart Failure of Circulating MicroRNAs Related to Lipoprotein Metabolism

＜正＞The Belo Monte Hydroelectric Complex （11, 181 MW）, in project stage, is planned to be built near the city of Altamira, on the lower course of the Xingu River, Para State, Brazil. Its reservoir presents uncommon characteristics, mainly as regards its layout. It can be said that the complex is composed of two reservoirs linked by high flow canals. Such a characteristic triggered three independent sedimentation studies, one for each reservoir. The reservoir reach upstream the canals consists, basically, of the Xingu River channel incorporating small bordering flooded areas and islands and stone sites existent on its bed. Such a reach of the reservoir, named "reservoir of the Xingu River Channel", though with more capacity, may present more significant silting up problems. To foresee the sedimentation phenomenon were used sediment discharge data dated since 1979. It was observed that the sediment yield is increasing in the basin at an annual rate of 2% due to problems triggered by deforestation, increase in the land use and climatic changes. Such a yield which may be increased by recent interference in the basin. Owing to the fact that the Xingu River Channel Dam is short in height, when compared with the flow through the turbines, as well as the water intake sills of the complementary power house are at very low elevation, such reservoir presents reduction of its useful life. The silting up prognostic, based on the computation of the time necessary for the deposits at the dam toe to reach the intake sills of its complementary power house, indicated the need to keep the previous upstream cofferdam （dike） separating the spillway catchment canals and the intake aiming hold back the sediments and therefore increase its useful life. The second reservoir may not present silting up problems within the economic life of the undertaking. Objective： This study aimed to investigate, for the first time, the expression of circulating miRNAs （microRNAs） related to lipoprotein metabolism in patients with HF （heart failure）. Medlods： Twenty patients with HF and 10 controls without HF were included. BNP （brain natxiuretic peptide）, a marker of HF severity, plasma lipid parameters and the expression of circulating miRNAs were determined. Key findings： Total, LDL-, non-HDL- and HDL-cholesterol, txiglycerides, and apo A-I did not differ between both groups, but apo B was lower in the HF group compared to controls （p = 0.007）. In respect to miRNAs, miR-33a, miR-144, miR-125, miR-30c, miR-122, miR-26a, miR-185, miR-758 and miR-106b were higher, from ten- to 25-fold, and miR-10b was lower about 4-fold, in HF group compared to controls. In HF patients a negative correlation between miR-26a and BNP, the marker of disease severity, was found （r = -0.552; p = 0.041）. Conclusions： Plasma levels of miRNAs involved in HDL and LDL metabolism regulation were strikingly changed in HF patients. The negative correlation between miR-26a and BNP values may suggest the possibility of the rise of a novel biomarker or therapeutic target in HF.

A significant portion of Landslide Early Warning Systems (LEWS) relies on the definition of operational thresholds and the monitoring of cumulative rainfall for alert issuance. These thresholds can be obtained in various ways, but most often they are based on previous landslide data. This approach introduces several limitations. For instance, there is a requirement for the location to have been previously monitored in some way to have this type of information recorded. Another significant limitation is the need for information regarding the location and timing of incidents. despite the current ease of obtaining location information (GPS, drone images, etc.), the timing of the event remains challenging to ascertain for a considerable portion of landslide data. Concerning rainfall monitoring, there are multiple ways to consider it, for instance, examining accumulations over various intervals (1 h, 6 h, 24 h, 72 h), as well as in the calculation of effective rainfall, which represents the precipitation that actually infiltrates the soil. However, in the vast majority of cases, both the thresholds and the rain monitoring approach are defined manually and subjectively, relying on the operators’ experience. This makes the process labor-intensive and time-consuming, hindering the establishment of a truly standardized and rapidly scalable methodology on a large scale. In this work, we propose a Landslides Early Warning System (LEWS) based on the concept of rainfall half-life and the determination of thresholds using Cluster Analysis and data inversion. The system is designed to be applied in extensive monitoring networks, such as the one utilized by Cemaden, Brazil’s National Center for Monitoring and Early Warning of Natural Disasters.

AIM:To study if impaired renal function is associated with increased risk of peri-infarct heart failure (HF) in patients with preserved ejection fraction (EF).METHODS:Patients with occluded infarct-related arteries (IRAs) between 1 to 28 d after myocardial infarction (MI) were grouped into chronic kidney disease (CKD) stages based on estimated glomerular filtration rate (eGFR).Rates of early post-MI HF were compared among eGFR *** regression was used to explore independent predictors of ***:Reduced eGFR was present in 71.1% of 2160 patients,with significant renal impairment (eGFR despite a small absolute difference in mean EF across eGFR groups:48.2 ± 10.0,47.9 ± 11.3 and 46.2 ± 12.1,respectively (P=0.02).The prevalence of HF was again higher with worsening renal function among patients with preserved EF:10.1%,13.6% and 23.6% (P depressed EF:27.1%,26.2% and 37.9% (P= 0.071).Moreover,eGFR was an independent correlate of HF in patients with preserved EF (P=0.003) but not in patients with depressed EF (P=0.181).CONCLUSION:A significant proportion of post-MI patients with occluded IRAs have impaired renal *** renal function was associated with an increased rate of early post-MI HF,the association being strongest in patients with preserved *** findings have implications for management of peri-infarct HF.
