Background:The Compendium of Physical Activities was published in 1993 to improve the comparability of energy expenditure values assigned to self-reported physical activity(PA)across studies.The original version was updated in 2000,and again in 2011,and has been widely used to support PA research,practice,and public health guidelines.Methods:This 2024 update was tailored for adults 19-59 years of age by removing data from those≥60 years.Using a systematic review and supplementary searches,we identified new activities and their associated measured metabolic equivalent(MET)values(using indirect calorimetry)published since 2011.We replaced estimated METs with measured values when possible.Results:We screened 32,173 abstracts and 1507 full-text papers and extracted 2356 PA energy expenditure values from 701 papers.We added303 new PAs and adjusted 176 existing MET values and descriptions to reflect the addition of new data and removal of METs for older adults.We added a Major Heading(Video Games).The 2024 Adult Compendium includes 1114 PAs(912 with measured and 202 with estimated values)across 22 Major Headings.Conclusion:This comprehensive update and refinement led to the creation of The 2024 Adult Compendium,which has utility across research,public health,education,and healthcare domains,as well as in the development of consumer health technologies.The new website with the complete lists of PAs and supporting resources is available at https://***.

The malaria transmission in Anhui province China

Plasmodium vivax and Plasmodium falciparum cases have opposite trends in Anhui China in the past decade.Long term and seasonal trends in the transmission rate of P.falciparum in Africa has been well studied,however that of P.vivax transmitted by Anopheles sinensis in China has not been investigated.There is a lot of work on the relationship between P.vivax cases and climatic factors in China,with sometimes contradicting results.However,how climatic factors affect transmission rate of P.vivax in China is unknown.We used Anhui province as an example to analyze the recent transmission dynamics where two types of malaria have been reported with differing etiologies.We examined breakpoints of the P.vivax and P.falciparum malaria long term dynamics in the recent decade.For locally transmitted P.vivax malaria,we analyzed the transmission rate and its seasonality using the combined human and mosquitos SIR-SI model with time-varied mosquito biting rate.We identified the effects of meteorological factors on the seasonality in transmission rate using a GAM model.For the imported P.falciparum ma-laria,we analyzed the potential reason for the observed increase in cases.The breakpoints of P.vivax and P.falciparum dynamics happened in a same year,2010.The seasonality in the transmission rate of P.vivax malaria was high(42.4%)and was linearly associated with temperature and nonlinearly with rainfall.The abrupt increase in im-ported P.falciparum cases after the breakpoint was significantly related to the increased annual Chinese investment in Africa.Under the conditions of the existing vectors of malaria,long-term trends in climatic fac-tors,and increasing trend in migration to/from endemic areas and imported malaria cases,we should be cautious of the possibility of the reestablishment of malaria in regions where it has been eliminated or the establishment of other vector-borne diseases.

The evolution of integrative medical education: the influence of the University of arizona center for Integrative Medicine

The University of arizona center for Integrative Medicine （AzCIM） was founded in 1994 with a primary focus of educating physicians in integrative medicine （IM）. Twenty years later, IM has become an internationally recognized movement in medicine. With 40% of United States＇ medical schools having membership in the Academic Consortium for Integrative Medicine and health it is foreseeable that all medical students and residents will soon receive training in the principles and practices of IM. The AzCIM has the broadest range and depth of IM educational programs and has had a major influence on integrative medical education in the United States. This review describes the fellowship, residency and medical student programs at AzCIM as well as other significant national drivers of IM education; it also points out the challenges faced in developing IM initiatives. The field of IM has matured with new nationa board certification in IM requiring fellowship training. Allied health professional IM educational courses, as well as integrative health coaching, assure that all members of the health care team can receive training. This review describes the evolution of IM education and will be helpful to academic centers, health care institutions, and countries seeking to introduce IM initiatives.

Background:Guidelines recommend that adolescents should accumulate an average of 60 min per day of moderate-to-vigorous physical activity(MVPA).However,using only this cut-off could hide important information.For instance,from a population-level point of view,increasing physical activity for those with no or low physical activity could provide more health benefits than increasing physical activity for those with intermediate levels.Also,including a more sensitive cut-point of≥1 days per week could be an additional strategy for identifying those with low access/opportunities for physical activity practice.Thus,the current study aims to estimate the prevalence of≥60 min of MVPA≥1 days per week among adolescents globally,and to describe any relevant gender inequalities.Methods:We used representative datasets from 146 countries/territories collected between 2003 and 2019.MVPA was self-reported.Participants were grouped into younger(≤14 years old)and older(>14 years old)adolescents.Crude Poisson regression models were used to identify the relative differences in≥60 min of MVPA≥1 days per week between boys and girls,and random-effects meta-analysis models were used to identify the pooled estimates.Analyses were stratified by country and region.Results:Approximately 80%of both younger and older adolescents reported≥60 min of MVPA≥1 days per week.This prevalence was≥94%in Europe and Central Asia and North America,while the estimates for the other regions were14y)=1.09(95%CI:1.08-1.10)).Conclusion:Approximately 8 out of 10 adolescents reported accumulating≥60 min of MVPA≥1 days per week,with notable differences between regions.Gender differences were observed in several countries,especially among the oldest adolescents.Priorities for physical activity promotion among adolescents

China’s biggest, most neglected health challenge: non-communicable diseases

Background:Over the past two decades,international health policies focusing on the fight against the human immunodeficiency virus/acquired immunodeficiency syndrome(HIV/AIDS),tuberculosis(TB),malaria,and those diseases that address maternal and child health problems,among others,have skewed disease control priorities in China and other Asian countries.Although these are important health problems,an epidemic of chronic,non-communicable diseases(NCDs)in China has accounted for a much greater burden of disease due to the ongoing rapid socioeconomic and demographic transition.Discussion:Although NCDs currently account for more than 80%of the overall disease burden in China,they remain very low on the nation’s disease control priorities,attracting marginal investment from central and local governments.This leaves the majority of patients with chronic conditions without effective treatment.International organizations and national governments have recognized the devastating social and economic consequences caused by NCDs in low-and middle-income countries,including China.Yet,few donor-funded projects that address NCDs have been implemented in these countries over the past decade.Due to a lack of strong support from international organizations and national governments for fighting against NCDs,affected persons in China,especially the poor and those who live in rural and less developed regions,continue to have limited access to the needed care.Costs associated with frequent health facility visits and regular treatment have become a major factor in medical impoverishment in China.This article argues that although China's ongoing health system reform would provide a unique opportunity to tackle current public health problems,it may not be sufficient to address the emerging threat of NCDs unless targeted steps are taken to assure that adequate financial and human resources are mapped for effective control and management of NCDs in the country.Summary:The Chinese government needs to develop a domestically-driven and evidence-

Factors associated with cervical cancer screening in a safety net population

AIM To identify factors associated with Papanicolaou-smear(Pap-smear) cervical cancer screening rates in a safety net population.METHODS From January 2012 to May 2013, the use of Pap-smear was determined for all patients seen at the breast clinic in a safety net hospital. health literacy assessment was performed using the validated Newest Vital Sign. The records of patients were reviewed to determine if they had undergone Pap-smears for cervical cancer screening. Sociodemographic information was collected included age, education, monthly income, race/ethnicity, employment, insurance status, and primary care provider of the patient. Logistic regression analysis was then performed to determine factors associated with utilization of Papsmears. Crude and adjusted odds ratios derived from multivariate logistic regression models were calculated as well as the associated 95% CIs and P-values.RESULTS Overall, 39% had Pap-smears in the prior 15 mo, 1377 consecutive women were seen during the study period and their records were reviewed. Significantly more patients with adequate health literacy underwent Papsmears as compared to those with limited health literacy(59% vs 34%, P health literacy, younger patients, and those with later age of first live birth were more likely to undergo Pap-smears. Patients whose primary care providers were gynecologists were also significantly more likely to have Pap-smears compared to other specialties(P health literacy and primary care physician were associated with Pap-smear utilization. Development of interventions to target low health literacy populations could improve cervical cancer screening.

Safety and Feasibility of Topical Application of Limonene as a Massage Oil to the Breast

Background: Limonene, a major component in citrus oil, has demonstrated anti-cancer effects in preclinical mammary cancer models. However, the effective oral dose translates to a human dose that may not be feasible for chronic dosing. We proposed to evaluate topical application of limonene to the breast as an alternative dosing strategy. Materials and Methods: We conducted a mouse disposition study to determine whether limonene would be bioavailable in the mammary tissue after topical application. SKH-1 mice received topical or oral administration of limonene in the form of orange oil every day for 4 weeks. Plasma and mammary pads were collected 4 hrs after the final dosing. We also conducted an exploratory clinical study to evaluate the safety and feasibility of topically applied limonene in the form of orange oil to the breast. healthy women were recruited to apply orange oil containing massage oil to their breasts daily for four weeks. Safety and feasibility were assessed by reported adverse events, clinical labs, and usage compliance. Pre and post-intervention nipple aspirate fluid (NAF) and plasma were collected for limonene concentration determination. Results: The mouse disposition study showed that topical and oral orange oil administration resulted in similar mammary tissue disposition of limonene with no clinical signs of toxicity. In the clinical study, the topical application of limonene containing massage oil to the breast was found to be safe with high levels of usage compliance for daily application, although NAF and plasma limonene concentrations were not significantly changed after the massage oil application. Conclusions: Our studies showed that limonene is bioavailable in mammary tissue after topical orange oil application in mice and this novel route of administration to the breast is safe and feasible in healthy women.

Background:Although TB health promotion directed at policy makers and healthcare workers(HCWs)is considered important to tuberculosis(TB)control,no indicators currently assess the impact of such promotional activities.This article is the second in a series of papers that seek to establish a framework of behavioral indicators for outcome evaluation of TB health promotion,using the Delphi method.In the first article,we sought to establish a framework of behavioral indicators for outcome evaluation of TB health promotion among TB suspects and patients.The objective of this second article is to present an indicator framework that can be used to assess behavioral outcomes of TB health promotion directed at policy makers and HCWs.Methods:A two-round,modified Delphi method was used to establish the indicators.Sixteen experts who were knowledgeable and experienced in the field of TB control were consulted in Delphi surveys.A questionnaire was developed following 4 steps,and involved ranking indicators on a five-point Likert scale.The consensus level was 70%.Median,mode,and Coefficient of variation(CV)were used to describe expert responses.An authority coefficient(Cr)was used to assess the degree of each expert’s authority.Results:Consensus was achieved following the two survey rounds and several iterations among the experts.For TB health-promotion activities directed at policymakers,the experts reached consensus on 2 domains(“Resource inputs”and“Policymaking and monitoring behaviors”),4 subdomains(“Human resources”among others),and 13 indicators(“Human resources per 100,000 person”among others).For TB health-promotion activities directed at HCWs,the experts reached consensus on 5 domains(“Self-protective behaviors”among others),6 sub-domains(“Preventing infection”among others),and 15 indicators(“Average hours of daily workplace disinfection by ultraviolet radiation”among others).Conclusions:This study identified a conceptual framework of core behavioral indicators to evaluate TB health-promotion activities directed at po

The healthy Children arizona (HCA) program was developed to promote early, positive exposure to cancer-preventive diet, physical activity and sun safety behaviors. Five lessons delivered weekly were evaluated by classroom teachers while pre- and post-curriculum surveys examined increases in students’ understanding of concepts and self-reported behavior. The first and second graders and their teachers in ethnically diverse elementary schools (including Title 1) in Tucson and Phoenix, arizona participated in the curriculum assessment. All 5 interactive lessons were highly rated by teachers in qualitative assessments. Aggregated analyses of pre- (n = 582) and post- (n = 588) comparison tests indicated that the HCA curriculum significantly increased students’ knowledge of each of six measured concepts (p cancer prevention knowledge among primary school children.

Barriers and facilitators to mexican-american participation in clinical trials: physician and patient focus group perspectives

Racial/ethnic minority populations are under- represented in clinical trials and Hispanic participation rates are particularly low. This study assessed barriers and facilitators to clinical trials participation by Medicaid eligible Mexican- Americans and their serving physicians. Qualitative data from two focus groups conducted among Mexican-American Medicaid eligible patients and four physician focus groups were analyzed. Mexican-American patients have a basic understanding of clinical trials. While most are open to participating in clinical research, not speaking English, time, and transportation were identified as barriers. Physicians believe that desperation and financial need are the primary patient motivators for participation. Barriers to physician recruitment and referral include: lack of information about clinical trials, concern that study participation may not be in the patient’s best interest, and lack of staffing and time to conduct trials. Ample opportunities exist to engage providers and patients in future efforts to increase Mexican-American patient recruitment into clinical trials.
