Background. The treatment of choice for an ingrowing nail has been su rgical r ather than nonsurgical. Yet, surgical treatments are far from successful, cause pain and patient apprehension, and leave disfigurement. Further, there is misund erstanding about the disease pathophysiology. Objective. To demonstrate the bene fits of a noninvasive method of treatment for an ingrowing nail using gutter spl int and formable acrylics and to present a current understanding of the disease pathophysiology. Methods. From a total of 541 cases of ingrowing nails treated, full follow-up data were obtained between January 1979 and March 2002. Formable acrylic treatments were carried out in 106 cases treated with acryli c-affixed gutter splint, 17 cases with sculptured nails, and 28 cases in which the two treatments were combined. These were then compared with 233 cases treat ed with adhesive tape-attached gutter splint and the remainder with other cons ervativemodalities. Results. Acrylic treatment with gutter splint and sculptured nail was found to be vastly superior to the other methods described, especially in the ability to firmly affix the gutter splint and sculptured nail for the ex tended period required for treatment. The treatment leads to a complete remissio n with almost instant alleviation of pain, with no disfigurement, while allowing for the resumption of daily activities. Conclusion. Conservative methods utiliz ing formable acrylics are highly beneficial in the treatment of an ingrowing nai l and should be viewed as the first treatment option.

Can endoscopic submucosal dissection be safely performed in a smaller specialized clinic?

AIM:To investigate whether endoscopic submucosal dissection(ESD) can be safely performed at small clinics,such as the Shirakawa ***:One thousand forty-seven ESDs to treat gastrointestinal tumors were performed at the Shirakawa clinic from April 2006 to March *** efficacy,technical feasibility and associated complications of the procedures were *** ESD procedures were performed by five *** was induced with propofol for esophagogastorduodenal ***:One thousand forty-seven ESDs were performed to treat 64 patients with esophageal cancer(E),850 patients with gastric tumors(G:764 patients with cancer,82 patients with adenomas and four others),four patients with duodenal cancer(D) and 129 patients with colorectal tumors(C:94 patients with cancer,21 patients with adenomas and 14 others).The en bloc resection rate was 94.3%(E:96.9%,G:95.8%,D:100%,C:79.8%).The median operation time was 46 min(range:4-360 min) and the mean size of the resected specimens was 18 mm(range:2-150 mm).No mortal complications were observed in association with the ESD *** occurred in 12 cases(1.1%,E:1 case,G:9 cases,D:1 case,C:1 case) and postoperative bleeding occurred in 53 cases(5.1%,G:51 cases,D:1 case,C:1 case);however,no case required either emergency surgery or blood *** of the perforations and postperative bleedings were resolved by endoscopic clipping or *** other problematic complication observed was pneumonia,which was treated with conservative ***:ESD can be safely performed in a clinic with established therapeutic methods and medical services to address potential complications.

Apoptosis inhibitor of macrophages （AIMs）, a homologue of human Spa, is a mouse soluble member of the scavenger receptor cysteine-rich superfamily （SRCR-SF）. This family integrates a group of proteins expressed by innate and adaptive immune cells for which no unifying function has yet been described. Pleiotropic functions have been ascribed to AIM, from viability support in lymphocytes during thymic selection to lipid metabolism and anti-inflammatory effects in autoimmune pathologies. In the present report, the pathogen binding properties of AIM have been explored. By using a recombinant form of AIM （rAIM） expressed in mammalian cells, it is shown that this protein is able to bind and aggregate Gram-positive and Gram-negative bacteria, as well as pathogenic and saprophytic fungal species. Importantly, endogenous AIM from mouse serum also binds to microorganisms and secretion of AIM was rapidly induced in mouse spleen macrophages following exposure to conserved microbial cell wall components. Cytokine release induced by well-known bacterial and fungal Toll-like receptor （TLR） ligands on mouse splenocytes was also inhibited in the presence of rAIM. Furthermore, mouse models of pathogen-associated molecular patterns （PAMPs）-induced septic shock of bacterial and fungal origin showed that serum AIM levels changed in a time-dependent manner. Altogether, these data suggest that AIM plays a general homeostatic role by supporting innate humoral defense during pathogen aggression.
