Purpose: Androgen deprivation therapy (ADT) is a cornerstone in prostate cancer (PCa) management that prolongs PCa-free and overall survival, but effects of ADT on human cardiac function have not been investigated. We used echocardiography to examine cardiac structure and function in patients with prostate cancer receiving ADT and to determine whether an exercise intervention can elicit cardiac adaptations in these subjects. Methods: Forty-three patients with prostate cancer receiving ADT were randomized to 12 weeks football training (ST group;n = 20) or usual care (control [CO] group;n = 23). Cardiac function was assessed at baseline and after 12 weeks by comprehensive echocardiography. Peak oxygen consumption, blood pressure and peripheral microvascular function was also measured. Results: At baseline, no considerable echocardiographic abnormalities were observed. In the ST group, increases in left ventricular diastolic function variables including E/A ratio (P = 0.03), E’ (P = 0.016), E’TDIcolor (P = 0.040) and in left atrial diameter (P = 0.001) were observed after 12 weeks. In addition, diastolic blood pressure (P = 0.027) and resting heart rate (P 0.001) were reduced after ST. In the CO group, no significant changes were observed in the examined variables after 12 weeks. Despite within group changes in the ST group, no significant differences were observed after 12 weeks between groups in echocardiographic variables, peak oxygen consumption, blood pressure and peripheral microvascular function. Conclusion: In men with prostate cancer receiving ADT, echocardiography showed no abnormalities in cardiac structure and function. Twelve weeks of ST failed to elicit significant cardiovascular adaptations and ADT may blunt cardiovascular adaptations to short-term exercise training.

Background: Previous studies that evaluated the association of kidney function with incident heart failure may be limited by the insensitivity of serum creatinine concentration for detecting abnormal kidney function. Objective: To compare serum concentrations of cystatin C(a novel marker of kidney function) and creatinine as predictors of incident heart failure. Design: Observational study based on measurement of serum cystatin C from frozen sera obtained at the 1992-1993 visit of the cardiovascular Health Study. Follow-up occurred every 6 months. Setting: Adults 65 years of age or older from 4 communities in the United States. Participants: 4384 persons without previous heart failure who had measurements of serum cystatin C and serum creatinine. Measurements: Incident heart failure. Results: The mean(±SD) serum concentrations of cystatin C and creatinine were 82±25 nmol/L(1.10±0.33 mg/L) and 89±34 μmol/L(1.01±0.39 mg/dL), respectively. During a median follow-up of 8.3 years(maximum, 9.1 years), 763(17%) participants developed heart failure. After adjustment for demographic factors, traditional and novel cardiovascular risk factors, cardiovascular disease status, and medication use, sequential quintiles of cystatin C concentration were associated with a stepwise increased risk for heart failure in Cox proportional hazards models(hazard ratios, 1.0 reference , 1.30 95%CI, 0.96 to 1.75 , 1.44 CI, 1.07 to 1.94 , 1.58 CI, 1.18 to 2.12 , and 2.16 CI, 1.61 to 2.91 ). In contrast, quintiles of serum creatinine concentration were not associated with risk for heart failure in adjusted analysis(hazard ratios, 1.0 reference , 0.77 CI, 0.59 to 1.01 , 0.85 CI, 0.64 to 1.13 , 0.97 CI, 0.72 to 1.29 , and 1.14 CI, 0.87 to 1.49 ). Limitations: The mechanism by which cystatin C concentration predicts risk for heart failure remains unclear. Ultrasound of the brachial artery is widely used to assess endothelial function, but whether brachial artery flow-mediated vasodilation(FMD) differs between women and men who have coronary artery disease(CAD) has not been examined. To investigate gender-based differences in brachial artery FMD as an indicator of significant CAD, FMD was measured in women and men outpatients who had CAD(coronary stenosis >50% , n=64) and those who did not have significant CAD(n=145). FMD in women who had CAD(n=33, 9.1± 0.8% ) was higher than that in similarly aged men who had CAD(n=31, 6.4± 0.5% ; p=0.008). The FMD cutpoint that maximized sensitivity with least effect on specificity for screening CAD was 15% (91% sensitivity, 25% specificity) in women but 10% (90% sensitivity, 43% specificity) in men. If the cutpoint as defined in men were used to evaluate women, brachial artery ultrasound would fail to diagnose 42% of women who do not have significant CAD; thus, a higher FMD cutpoint is required to optimize the sensitivity of FMD for identifying women who have significant CAD compared with similarly aged men. In studies using FMD to evaluate cardiovascular risk, different standards should be applied for women and men.

BACKGROUND Chronic cocaine use is associated with stroke, coronary artery disease and myocardial infarction, resulting in severe impairments or sudden mortality. In the absence of clear cardiovascular symptoms, individuals with cocaine use disorder (iCUD) seeking addiction treatment receive mostly psychotherapy and psychiatric pharmacotherapy, with no attention to vascular disease (i.e., atherosclerosis). Little is known about the pre-clinical signs of cardiovascular risk in iCUD and early signs of vascular disease are undetected in this underserved population. AIM To assess inflammation, plaque burden and plaque composition in iCUD aiming to detect markers of atherosclerosis and vascular disease. METHODS The bilateral carotid arteries were imaged with positron emission tomography/magnetic resonance imaging (PET/MRI) in iCUD asymptomatic for cardiovascular disease, healthy controls, and individuals with cardiovascular risk. PET with 18F-fluorodeoxyglucose (18F-FDG) evaluated vascular inflammation and 3-D dark-blood MRI assessed plaque burden including wall area and thickness. Drug use and severity of addiction were assessed with standardized instruments. RESULTS The majority of iCUD and controls had carotid FDG-PET signal greater than 1.6 but lower than 3, indicating the presence of mild to moderate inflammation. However, the MRI measure of wall structure was thicker in iCUD as compared to the controls and cardiovascular risk group, indicating greater carotid plaque burden. iCUD had larger wall area as compared to the healthy controls but not as compared to the cardiovascular risk group, indicating structural wall similarities between the non-control study groups. In iCUD, wall area correlated with greater cocaine withdrawal and craving. CONCLUSION These preliminary results show markers of carotid artery disease burden in cardiovascular disease-asymptomatic iCUD.
