Pediatric Nd:YAG laser capsulotomy in th. operating room: review of 87 cases

AIM: To report a large series of children h.ving Nd:YAG laser capsulotomy in th. operating room using th. lateral decubitus position. METh.Ds: Medical records of children wh. underwent Nd:YAG laser capsulotomy in th. operating room at ann & robert h. lurie children's hospital of chicago between september 2008 and April 2017 were reviewed. Induction of general anesth.sia and intubation was performed in th. supine position after wh.ch.th. patient was placed in lateral decubitus position. Th. Nd:YAG laser capsulotomy was performed using a standard protocol. At th. completion of th. procedure, th. patient was turned back into th. supine position and extubated. REsULTs: Th.s study included 87 eyes of 60 patients. Patient's age ranged from 1 to 18 y(mean 6.4±4.1 y). In most cases(84/87, 97%), th. procedure was performed under general anesth.sia. In all cases, good focus on th. membrane was ach.eved, and th. procedure was performed successfully. Th.re were no intraoperative ocular or anesth.sia-related complications. CONCLUsION: Wh.n performing Nd:YAG laser capsulotomy in th. operating room, th. lateral decubitus position allowsan easy and safe approach.with.ut th. risk of potentially devastating complications th.t h.ve been associated with.th. previously described sitting and prone positions.

AIM To determine th. role of intravoxel incoh.rent motion(IVIM) diffusion-weigh.ed(DW) magnetic resonance imaging(MRI) using a bi-exponential model in ch.moth.rapy response evaluation in a gastric cancer mouse *** Mice bearing MKN-45 h.man gastric adenocarcinoma xenografts were divided into four treated groups(TG1, 2, 3 and 4, n = 5 in each.group) wh.ch.received Fluorouracil and Calcium Folinate and a control group(CG, n = 7). DW-MRI scans with.14 b-values(0-1500 s/mm2) were performed before and after treatment on days 3, 7, 14 and 21. Fast diffusion component(presumably pseudo-perfusion) parameters including th. fast diffusion coefficient(D*) and fraction volume(f p), slow diffusion coefficient(D) and th. conventional apparent diffusion coefficients(ADC) were calculated by fitting th. IVIM model to th. measured DW signals. Th. median ch.nges from th. baseline to each.posttreatment time point for each.measurement(ΔADC, ΔD* and Δf p) were calculated. Th. differences in th. median ch.nges between th. two groups were compared using th. mixed linear regression model by th. restricted maximum likelih.od meth.d sh.wn as z values. h.stopath.logical analyses including Ki-67, CD31, TUNEL and h.E were conducted in conjunction with.th. MRI scans. Th. median percentage ch.nges were compared with.th. h.stopath.logical analyses between th. pre-and post-treatment for each.*** Compared with.th. control group, D* in th. treated group decreased significantly(ΔD*treated% =-30%,-34% and-20%, with.z =-5.40,-4.18 and-1.95. P = 0.0001, 0.0001 and 0.0244) and f p increased significantly(Δfptreated% = 93%, 113% and 181%, with.z = 4.63, 5.52, and 2.12, P = 0.001, 0.0001 and 0.0336) on day 3, 7 and 14, respectively. Radiological imaging plays an important role in oncological trials to provide imaging biomarkers for disease staging, stratifying patients, defining dose setting, and evaluating th. safety and efficacy of new candidate drugs and innovative treatment. Th.s paper reviews th. tech.iques of most commonly used quantitative magnetic resonance imaging (qMRI) biomarkers (dynamic contrast enh.nced, dynamic susceptibility contrast, and diffusion weigh.ed imaging) and th.ir applications in oncological trials. Ch.llenges of incorporating qMRI biomarkers in oncological trials are discussed including understanding biological mech.nisms revealed by MRI biomarkers, consideration of rigorous trial design and standardized implementation of qMRI protocols.

Th.s study was to determine wh.th.r GM-CsF induced WT1 gene expression and to establish.an association with.markers of proliferation CD71+CD34+ using nPCR and flow cytometry respectively, in samples obtained from 5 newly diagnosed JMML patients. Overtime (day 0 to day 14) th.re was an insignificant difference in WT1 gene expression and CD71+CD34+ in JMML samples wh.n compared to periph.ral blood of normal volunteers (n = 3). Our study suggests th.t th.re is a correlation between WT1 gene expression and cellular proliferation and th.t GMCsF in vitro does not create a significant difference in JMML samples.
