Modulation of splanchnic circulation:Role in perioperativemanagement of liver transplant patients

Splanchnic circulation is the primary mechanism thatregulates volumes of circulating blood and systemic blood pressure in patients with cirrhosis accompanied by portal hypertension. Recently, interest has been expressed in modulating splanchnic circulation in patients with liver cirrhosis, because this capability might produce beneficial effects in cirrhotic patients undergoing a liver transplant. Pharmacologic modulation of splanchnic circulation by use of vasoconstrictors might minimize venous congestion, replenish central blood flow, and thus optimize management of blood volume during a liver transplant operation. Moreover, splanchnic modulation minimizes any high portal blood flow that may occur following liver resection and the subsequent liver transplant. This effect is significant, because high portal flow impairs liver regeneration, and thus adversely affects the postoperative recovery of a transplant patient. An increase in portal blood flow can be minimized by either surgical methods(e.g., splenic artery ligation, splenectomy or portocaval shunting) or administration of splanchnic vasoconstrictor drugs such as Vasopressin or terlipressin. Finally, modulation of splanchnic circulation can help maintain perioperative renal function. Splanchnic vasoconstrictors such as terlipressin may help protect against acute kidney injury in patients undergoing liver transplantation by reducing portal pressure and the severity of a hyperdynamic state. These effects are especially important in patients who receive a too small for size graft. Terlipressin selectively stimulates V1 receptors, and thus causes arteriolar vasoconstriction in the splanchnic region, with a consequent shift of blood from splanchnic to systemic circulation. As a result, terlipressin enhances renal perfusion by increasing both effective blood volume and mean arterial pressure.

Groundwater fluoride contamination:A reappraisal

Dissolution of fluorite （CaF2） and/or fluorapatite （FAP） [Cas（PO4）3F], pulled by calcite precipitation, is thought to be the dominant mechanism responsible for groundwater fluoride （F） contamination. Here, one dimensional reactive-transport models are developed to test this mechanism using the published dissolution and precipitation rate kinetics for the mineral pair FAP and calcite. Simulation results correctly show positive correlation between the aqueous concentrations of F and CO2 and negative correlation between F- and Ca^2＋. Results also show that precipitation of calcite, contrary to the present understanding, slows down the FAP dissolution by 10G orders of magnitude compared to the FAP dissolution by hydrolysis. For appreciable amount of fluoride contamination rock-water interaction time must be long and of order 106 years.

A tunable adaptive detector for distributed targets when signal mismatch occurs

Aiming at the problem of detecting a distributed target when signal mismatch occurs,this paper proposes a tunable detector parameterized by an adjustable *** adjusting the parameter,the tunable detector can achieve robust or selective detection of mismatched ***,the proposed tunable detector,with a proper tunable parameter,can provide higher detection probability compared with existing detectors in the case of no signal *** addition,the proposed tunable detector possesses the constant false alarm rate property with the unknown noise covariance matrix.

Photoluminescence spectroscopy of defects in ZnO nano/microwires

Photoluminescence spectroscopy is used to study defects found in single ZnO nano/microwires at 90K. The defect, acting as binding site for bound exciton （BX） transition, is represented by BF, the fractional intensity of the BX peak in the whole near-band edge ultraviolet （UV） luminescence. The concentration of defects as origins of the visible emissions is proportional to the intensity fraction DF, i.e., the intensity fraction of visible emissions in the sum total of all UV and visible luminescences. By comparing BF and DF, it is concluded that the two defects are not correlated to each other. The former kind of defect is considered to be related to the blueshift of the near-band edge peak as the radius of the nano/microwires decreases at room temperature.

Hypersonic Vehicle Trajectory Prediction Algorithm Based on Hough Transform

Trajectory prediction is a prerequisite for missile and high-speed vehicle guidance interception. To address the trajectory problem for the near space hypersonic unpowered gliding vehicle. Firstly, the trajectory prediction mechanism is analyzed. Based on the existing NSHV trajectory prediction method, the key techniques of trajectory prediction are discussed from the perspectives of prediction parameter selection and prediction parameter description. Then, a trajectory prediction method based on Hough transform is proposed. The core of the method is to select the longitude latitude, and high direction positions as prediction parameters, and use the Hough transform to fit the prior basis functions. In the latitude and longitude direction, the rationality of predicting parameters is discussed from two aspects: position extreme point and lateral maneuverability. In the height direction,for the NSHV oscillation drop characteristic, two Hough transforms are used to separate the periodic term from the linear term. In addition, we give the approximate interval of the parameter values of the method, and design a parameter adaptive scheme. Finally, trajectory prediction is performed on two different control patterns. The error of the prediction time of 100 s is within 25 km, and the results show the effectiveness of the proposed method.

Is chronic hepatitis C virus infection a risk factor for breast cancer?

AIM:To evaluate the prevalence of breast tumors in adult females with chronic hepatitis C virus(HCV) ***:Prospective,single-center study,based on female outpatients consulting in a liver unit,for 1 *** study group included females with present and/or past history of chronic infection by *** with spontaneous recovery were *** hepatitis had been proved by liver biopsy in the majority of cases and/or biological markers of inflammation and *** control group included female patients with other well documented chronic liver diseases:chronic hepatitis B,alcoholic liver disease,autoimmune hepatitis,hemochromatosis,non alcoholic liver disease,chronic *** patients were prospectively questioned during consultation about past breast history and follow-up by ***:Breast carcinoma was recorded in 17/294 patients with HCV infection(5.8%,95% CI:3.1-8.4) vs 5/107 control patients(4.7%,95% CI:0.67-8.67).Benign tumors of the breast(mastosis,nodules,cysts) were recorded in 75/294 patients with HCV infection(25.5%,95% CI:20.5-30.5) vs 21/107(19.6%,95% CI:12.1-27.1) in the control *** lesion was noted in 202 patients with HCV(68.7%,95% CI:63.4-74) vs 81 control patients(75.7%,95% CI:67.6-83.8).Despite a trend to an increased prevalence in the group with HCV infection,the difference was not significant compared to the control group(P=NS).In patients over 40 years,the results were,respectively,as follows:breast cancer associated with HCV:17/266 patients(6.3%,95% CI:3.4-9.3) vs 5/95 patients(5.2%,95% CI:0.7-9.7) in the control group;benign breast tumors:72/266 patients with HCV infection(27%,95% CI:21.7-32.4) vs 18/95 patients(18.9%,95% CI:11-26.8) in the control group;no breast lesion 177/266(66.5%,95% CI:60.9-72.2) in patients with HCV infection vs 72/95(75.7%,95% CI:67.1-84.4) in the control *** differences were not significant(P=NS).CONCLUSION:These results suggest that chronic HCV infectio

Bile salts inhibit growth and induce apoptosis of culture human normal esophageal mucosal epithelial cells

AIM： To investigate the effect of six bile salts： glycocholate （GC）, glycochenodeoxycholate （GCDC）, glycodeoxycholate （GDC）, taurocholate （TC）, taurochenodeoxycholate （TCDC）, taurodeoxycholate （TDC）, and their mixture on cultured human normal esophageal rnucosal epithelial cells. METHODS： Human normal esophageal mucosal epithelial cells were cultured with serum-free keratinocyte medium. 3-[4,5-Dimethylthiaolyl]-2,5- diphenyl-tetrazolium bromide assay was applied to the detection of cell proliferation. Apoptotic morphology was observed by phase-contrast video microscopy and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling （TUNEL） assay. Sub-G1 DNA fragmentations and early apoptotic cells were assayed by flow cytometry （FCI） with propidium iodide （PI） staining and annexin V-FITC conjugated with PI staining. Apoptotic DNA ladders on agarose gel electrophoresis were observed. RESULTS： Except for GC, GCDC, GDC, TC, TCDC, TDC and their mixture could initiate growth inhibition of esophageal mucosal epithelial cells in a dose- and time-dependent manner. TUNEL and FCM assays demonstrated that the bile salts at 500 μmol/L and their mixture at 1 500 μmol/L induced apoptosis except for GC. The percentage of sub-G1 detected by FCM with PI staining was 83.5% in cells treated with 500 μmol/L TC for 2 h, and 19.8%, 20.4%, 25.6%, 13.5%, and 75.8% in cells treated with 500 μmol/L GCDC, TCDC, GDC, TDC, and 1 500 μmol/L mixture for 24 h, respectively, which were higher than that of the control （1.5%）. The percentage was 1.4% in cells with 500 μmol/L GC for 24 h. DNA ladders on agarose gel electrophoresis were seen in cells treated with 500 μmol/L TC for 2 h and i 500 μmnol/L mixture for 24 h. CONCLUSION： All GCDC, GDC, TC, TCDC, TDC and their mixture can inhibit growth and induce apoptosis of cultured human normal esophageal mucosal epithelial cells, but GC is well tolerated by the cells.

Determination of time-dependent strengths of salt pillars based on strain energy principle

The objective of this study is to determine the time-dependent strengths of salt mine pillars in the Maha Sarakham formation, northeast of Thailand. Strain rate-controlled triaxial compression tests have been performed on salt specimens under confining pressures from 0 MPa to 12 MPa. The strain rates are from 10^(-7) s^(-1) to 10^(-4) s^(-1). The axial stresses and lateral strains are monitored through the strain-softening region. The results indicate that the strengths and elastic moduli increase exponentially with the strain rates. The power creep law parameters are calibrated with the test results, and hence allows constructing series of strain-time curves for the salt pillars under different depths and extraction ratios. The strain energy density principle is applied to develop a strength criterion for the salt pillars. Combining this criterion with the series of the strain-time curves the time-dependent strengths of the salt pillars for different extraction ratios can be predicted.

A New Line Symmetry Distance and Its Application to Data Clustering

In this paper, at first a new line-symmetry-based distance is proposed. The properties of the proposed distance are then elaborately described. Kd-tree-based nearest neighbor search is used to reduce the complexity of computing the proposed line-symmetry-based distance. Thereafter an evolutionary clustering technique is developed that uses the new linesymmetry-based distance measure for assigning points to different clusters. Adaptive mutation and crossover probabilities are used to accelerate the proposed clustering technique. The proposed GA with line-symmetry-distance-based （GALSD） clustering technique is able to detect any type of clusters, irrespective of their geometrical shape and overlapping nature, as long as they possess the characteristics of line symmetry. GALSD is compared with the existing well-known K-means clustering algorithm and a newly developed genetic point-symmetry-distance-based clustering technique （GAPS） for three artificial and two real-life data sets. The efficacy of the proposed line-symmetry-based distance is then shown in recognizing human face from a given image.
