Computed tomography angiographic study of surgical anatomy of thyroid arteries:Clinical implications in neck dissection

BACKGROUND The course and variations of thyroid arteries must be understood by surgeons to prevent bleeding during operative procedures of the thyroid *** is limited scientific literature regarding the radiological anatomy of thyroid arteries in this geographical area,the Garhwal region of Sub-Himalayan belt,which is considered to be the endemic belt of *** tomography angiography provides a three-dimensional orientation of the vascular and surgical anatomy of the entire cervical *** To estimate the proportion of variation in origin of thyroid arteries using Computed Tomography *** Using Computed Tomography Angiography,the presence and origin of the superior thyroid artery,inferior thyroid artery,and thyroid ima artery were observed and *** Out of total 210 subjects,superior thyroid artery was seen to be emerging from external carotid artery in 77.1%*** artery was found to be originating at the level of bifurcation of common carotid artery in 14.3%cases,whereas in 8.6%cases,it emerged as a direct branch of the common carotid ***,the inferior thyroid artery was observed to be emerging from thyrocervical trunk,subclavian artery and vertebral artery in 95.7%cases,3.3%and 1%cases,*** ima artery was also reported in a subject,arising from the brachiocephalic *** To avoid vascular injuries,excessive and uncontrollable bleeding,intra-operative difficulties,and post-operative issues,it is imperative for surgeons to be aware of the course and variations of thyroid arteries。

The aberrant hepatic duct is classified by Huang et al.[1]into type A(right intrahepatic duct variant)and type B(left intrahepatic duct variant).Type A and B are further classified into 5(A1-5)and 6(B1-6)subtypes,*** variation of cystic duct involves only in type *** A1 or classic arrangement refers to the formation of the common hepatic duct(CHD)by the left hepatic duct(LHD)and the right hepatic duct(RHD).The RHD has two tributaries composed of the right anterior hepatic duct(RAHD)and the right posterior hepatic duct(RPHD).Then,the common bile duct(CBD)is formed when the cystic duct joins with the CHD(Fig.1).Type A2 refers to the RAHD and RPHD joining with LHD at hepatic hilum,and the absence of the *** A3 refers to the RAHD or RPHD directly draining to the *** A4 refers to the RAHD or RPHD directly draining to the *** A5 refers to the RAHD,RPHD or RHD directly draining to the cystic duct(or aberrant bile duct communicating with the cystic duct,ACC)(Fig.1)[1].

Identification of the surgical indication line for the Denonvilliers’fascia and its anatomy in patients with rectal cancer

Background:The high rate of urogenital dysfunction after traditional total mesorectal excision(TME)has caused doubts among scholars on the standard fashion of *** have proposed the necessity to preserve the Denonvilliers’fascia in patients with rectal ***,how to accurately locate the Denonvilliers’fascia is *** study aimed to explore anatomical features of the Denonvilliers’fascia by comparing autopsy findings and observations of surgical videos so as to propose a dissection method for the preservation of pelvic autonomic nerves during rectal cancer ***:Five adult male cadaver specimens were dissected,and surgical videos of 135 patients who underwent TME for mid-low rectal cancer between January 2009 and February 2019 were reviewed to identify and compare the structure of the Denonvilliers’***:The monolayer structure of the Denonvilliers’fascia was observed in 5 male cadaver specimens,and it was located between the rectum,the bottom of the bladder,the seminal vesicles,the vas deferens,and the *** Denonvilliers’fascia was originated from the rectovesical pouch(or rectum-uterus pouch),down to fuse caudally with the rectourethral muscle at the apex of the prostate,and fused to the lateral ligaments on both *** fascia was thinner on the midline with a thickness of 1.06±0.10 *** crown shape of the Denonvilliers’fascia was slightly triangular,with a height of approximately 5.42±0.16 cm at *** were more densely distributed in front of the Denonvilliers’fascia than behind,especially on both sides of *** laparoscopic view,the Denonvilliers’fascia was originated at the lowest point of the rectovesical pouch(or rectum-uterus pouch),with a thickened white line which was a good mark for identifying the Denonvilliers’***:Identification of the surgical indication line for the Denonvilliers’fascia could help us identify the Denonvilliers’fascia,and it would improve our ability to pr

Targeted muscle reinnervation(TMR)is a surgical procedure used to transfer residual peripheral nerves from amputated limbs to targeted muscles,which allows the target muscles to become sources of motor control information for function ***,the effect of TMR on injured motor neurons is still *** this study,we aimed to explore the effect of hind limb TMR surgery on injured motor neurons in the spinal cord of rats after tibial nerve *** found that the reduction in hind limb motor function and atrophy in mice caused by tibial nerve transection improved after *** enhanced nerve regeneration by increasing the number of axons and myelin sheath thickness in the tibial nerve,increasing the number of anterior horn motor neurons,and increasing the number of choline acetyltransferase-positive cells and immunofluorescence intensity of synaptophysin in rat spinal *** findings suggest that TMR may enable the reconnection of residual nerve fibers to target muscles,thus restoring hind limb motor function on the injured side.

While endoscopic retrograde cholangiopancreatography(ERCP)remains the primary treatment modality for common bile duct stones(CBDS)or choledocho-lithiasis due to advancements in instruments,surgical intervention,known as common bile duct exploration(CBDE),is still necessary in cases of difficult CBDS,failed endoscopic treatment,or altered *** evidence also supports CBDE in patients requesting single-step cholecystectomy and bile duct stone removal with comparable *** review elucidates relevant clinical anatomy,selection indications,and outcomes to enhance surgical *** selection between trans-cystic(TC)vs trans-choledochal(TD)approaches is described,along with stone removal techniques and ductal *** surgical techniques and strategies for both the TC and TD approaches,including instrument selection,is also ***,this review comprehensively addresses operation-specific complications such as bile leakage,stricture,and entrapment,and focuses on preventive measures and treatment *** review aims to optimize the management of CBDS through laparoscopic CBDE,with the goal of improving patient outcomes and minimizing risks.

surgical resection,chemotherapy,and radiation are the standard therapeutic modalities for treating *** approaches are intended to target the more mature and rapidly dividing cancer ***,they spare the relatively quiescent and intrinsically resistant cancer stem cells(CSCs)subpopulation residing within the tumor ***,a temporary eradication is achieved and the tumor bulk tends to revert supported by CSCs'resistant *** on their unique expression profile,the identification,isolation,and selective targeting of CSCs hold great promise for challenging treatment failure and reducing the risk of cancer ***,targeting CSCs is limited mainly by the irrelevance of the utilized cancer models.A new era of targeted and personalized anti-cancer therapies has been developed with cancer patient-derived organoids(PDOs)as a tool for establishing pre-clinical tumor ***,we discuss the updated and presently available tissue-specific CSC markers in five highly occurring solid ***,we highlight the advantage and relevance of the threedimensional PDOs culture model as a platform for modeling cancer,evaluating the efficacy of CSC-based therapeutics,and predicting drug response in cancer patients.
