Asymptomatic Bacteriuria in Pregnant Women in the Antenatal Booking Clinic at aminu kano teaching hospital, kano, Nigeria

Asymptomatic bacteriuria is common in pregnant women and if untreated could lead to serious complications. Despite this, its prevalence and microbiological characteristics have not been well studied in kano and they vary among different populations. The objective of the study was to determine the prevalence, microbiological characteristics and pregnancy outcome of asymptomatic bacteriuria in pregnancy among women who came for antenatal booking clinic at the aminu kano teaching hospital (AKTH) kano. A cross sectional descriptive study was conducted among randomly selected 200 consenting pregnant women who came for antenatal booking from November 2010 to January 2011. Samples of 10 - 15 mls urine were examined for pus cells and bacteria and then cultured on cysteine lactose electrolyte deficient agar. Colony counts yielding bacterial growth of 105/ml or more of pure isolates were deemed significant. Isolates were identified to specie level. Antibiotic sensitivity test was done, treatment was given and pregnancy outcome was determined. Urinalysis and Full Blood Count were done. The prevalence of asymptomatic bacteriuria in pregnancy was 9%. Parity and Pyuria had statistically significant association with asymptomatic bacteriuria (P < 0.05). The commonest organisms isolated wereKlebsiella?spp. and?Staphylococcus saprophyticus. Following treatment of asymptomatic bacteriuria, there was no statistically significant difference in pregnancy outcome compared to patients who did not have the condition. The incidence of asymptomatic bacteriuria in pregnancy in AKTH is low. Screening of pregnant women using urine microscopy culture and sensitivity should be reserved for pregnant women with higher risk of developing asymptomatic bacteriuria.

Indications and Outcome of Gynaecological Hysterectomy at aminu kano teaching hospital, kano: A 5-Year Review

Background: Hysterectomy, is a common operation worldwide. It is a major surgery, which leads to significant morbidity and mortality, hence, there must be a justifiable indication before the procedure is undertaken. Symptomatic uterine fibroid and utero-vaginal prolapse are common indications in this environment. Objectives: To determine the prevalence, indications and outcome of hysterectomy at aminu kano teaching hospital, kano. Study Design: A retrospective study of all the hysterectomies that were done in aminu kano teaching hospital, kano between 1st of Jan 2009 and 31st of Dec 2013. Result: During the period of study, there were 3843 major gynaecological operations. Out of which hysterectomy was 196, giving hysterectomy rate of 5.1% of all major gynaecological operations. Hysterectomy was highest among patient aged 40 - 49 years accounting for 54.6% of cases and the commonest indication was uterine fibroid, constituting 66.4% of all hysterectomies. Total abdominal hysterectomy was the most frequently preformed, accounting for 78.1% while, vaginal hysterectomy accounts for 20.9% of cases and subtotal hysterectomy was 1%. Hysterectomy was mostly complicated with post-operative pyrexia constituting 24% of all the complications while ureteric injury was the least, constituting 1%. There was no mortality recorded. Most of the patients who had hysterectomy were discharge in less than 10th post-operative day accounting for 91.3% of cases. However, 80.5% of patients who had vagina hysterectomy were discharge in less than 5th post-operative day. Conclusion: The prevalence of hysterectomy was 5.1%. Uterine fibroid was the commonest indication while post-operative pyrexia was the commonest complication found.

Severe Cut Throat Injury in a Child: A Case Report and Review of Literature

Background: Cut-throat injury is a potentially fatal condition that may be associated with serious morbidity and mortality. The neck region is particularly at risk of serious injuries due to the location of vital structures, including nerves, viscera, and major vessels. Although cut-throat injury is said to be rare in children, its occurrence has been reported in some series. Management of this condition requires a multidisciplinary approach for a better outcome. Aim: The aim of this report is to highlight the successful management of an alarming case of severe cut-throat injury with associated laryngeal injury in a child. Case presentation: A 12-year-old boy presented with a cut-throat following an assault by an unknown person. Examination revealed an acutely ill child, conscious but pale. He sustained a deep transverse laceration that transected the thyroid cartilage, exposing the laryngeal cavity. An assessment of Zone II penetrating neck injury was made. He was resuscitated, and had emergency neck exploration, tracheostomy, and repair of the injuries. The patient was followed up for 6 months, and had a good voice outcome, with no significant complication. Conclusion: Cut-throat injury is rare in children. But it is a potentially life-threatening condition. It is therefore important to recognize this entity and develop effective protocol of management in our environment.

Outcome of percutaneous reconstruction of chronic lateral collateral ligament rupture

Purpose:Many techniques have been described for the reconstruction of chronic lateral collateral ligament(LCL)rupture with different autograft *** advantages of percutaneous LCL reconstruction include small incisions,minimal soft tissue disruption,less postoperative pain,and speedy rehabilitation and *** aim of this study was to report the functional outcome of percutaneous LCL reconstruction and overall patient satisfaction in ***:This prospective and interventional study involving 51 patients with chronic LCL rupture who had percutaneous LCL reconstruction using peroneus longus autograft was conducted between January 2021 and December 2022 in National Orthopaedic hospital,Dala-kano,*** inclusion criteria were patients between the ages of 18 and 45 years with chronic isolated LCL and not more than 1 injury of knee *** criteria were active infection,and multi-ligament knee injury requiring 2-staged *** knee functions were assessed preoperatively,3 months,6 months,and 12 months postoperatively using the Lysholm scoring *** satisfaction with the outcome of the treatment was assessed using a 5-point Likert *** information was recorded into Microsoft Excel sheet and data was analyzed using SPSS version 23.0 for *** paired samples t-test was used to compare the clinical outcomes as continuous *** significance was considered at p<***:The mean age of the patients was(30.10±5.90)*** median time from injury to surgery was 7 months(ranging from 3 to 28 months).The mean follow-up period was(14.07±3.13)*** mean preoperative and 1-year postoperative Lysholm scores were 44.33±12.97 and 97.96±1.23,***:Percutaneous LCL reconstruction using peroneus longus autograft significantly improves patient knee function and results in excellent patient satisfaction。

Background: Ethmoidal polyposis coexisting with adenoid enlargement and tympanic membrane perforation is a rare condition in otolaryngological practice. Evidences have shown that antrochoanal polyp often times coexists with adenoid enlargement especially in the young which was in sharp contrast to ethmoidal polyposis occurring in the presence of adenoid enlargement. Moreover, the hidden location of nasopharynx housing the adenoids, coupled with the masking effect of bilateral sinonasal polyps, leads to inadvertent misdiagnosis of this complex pathology with subsequent difficulty in providing effective management. Case Presentation: A 24 year-old lady presented with recurrent bilateral nasal obstruction that became persistent associated with nasal discharge and anosmia. She also had recurrent right otorrhoea with associated hearing loss. Diagnostic rigid nasal endoscopy revealed mucoid discharge with bilateral polypoid masses filling both nasal cavities. Otoendoscopic finding revealed a small (about 5%) central tympanic membrane perforation. A clinical assessment of chronic rhinosinusitis with nasal polyposis complicated by chronic suppurative otitis media (CSOM) was made. A non-contrast CT scan of the paranasal sinuses showed isodense lesions in the nasal cavities, all paranasal sinuses and the entire nasopharynx. She had endoscopic sinus surgery and a nasopharyngeal clearance biopsy. The nasal, paranasal and nasopharyngeal masses had histologic confirmation of inflammatory nasal polyps and lymphoid (adenoid) hyperplasia respectively. Her condition improved remarkably with subsequent medical treatment. She was followed up for 8 months and no recurrence was observed. Conclusion: Sinonasal polyposis can coexist with adenoid hypertrophy and middle ear disease as a single pathological condition. Hence, a high index of suspicion and thorough evaluation become necessary for making timely diagnoses and instituting effective management.

Outpatient prevention counseling for malaria in northwest Nigeria: A single-centre descriptive cross-sectional study

Is tumor necrosis factor-α monoclonal therapy with proactive therapeutic drug monitoring optimized for inflammatory bowel disease? Network meta-analysis

BACKGROUND The efficacy and safety of anti-tumor necrosis factor-α(TNF-α)monoclonal antibody therapy[adalimumab(ADA)and infliximab(IFX)]with therapeutic drug monitoring(TDM),which has been proposed for inflammatory bowel disease(IBD)patients,are still *** To determine the efficacy and safety of anti-TNF-αmonoclonal antibody therapy with proactive TDM in patients with IBD and to determine which subtype of IBD patients is most suitable for proactive TDM *** As of July 2023,we searched for randomized controlled trials(RCTs)and observa-tional studies in PubMed,Embase,and the Cochrane Library to compare anti-TNF-αmonoclonal antibody therapy with proactive TDM with therapy with reactive TDM or empiric *** and network meta-analyses were used to determine the IBD patient subtype that achieved clinical remission and to determine the need for *** This systematic review and meta-analysis yielded 13 studies after exclusion,and the baseline indicators were *** found a significant increase in the number of patients who achieved clinical remission in the ADA[odds ratio(OR)=1.416,95%confidence interval(CI):1.196-1.676]and RCT(OR=1.393,95%CI:1.182-1.641)subgroups and a significant decrease in the number of patients who needed surgery in the proactive vs reactive(OR=0.237,95%CI:0.101-0.558)and IFX+ADA(OR=0.137,95%CI:0.032-0.588)subgroups,and the overall risk of adverse events was reduced(OR=0.579,95%CI:0.391-0.858)according to the pairwise ***,the network meta-analysis results suggested that patients with IBD treated with ADA(OR=1.39,95%CI:1.19-1.63)were more likely to undergo TDM,especially in comparison with patients with reactive TDM(OR=1.38,95%CI:1.07-1.77).CONCLUSION Proactive TDM is more suitable for IBD patients treated with ADA and has obvious advantages over reactive *** recommend proactive TDM in IBD patients who are treated with ADA.

Optimized sequential therapy vs 10- and 14-d concomitant therapy for eradicating Helicobacter pylori: A randomized clinical trial

Liver transplantation as an alternative for the treatment of non-resectable liver colorectal cancer: Advancing the therapeutic algorithm
