Background: The objective of this pilot study was to describe clinical profile, electric, echocardiographic and angiographic caracteristics with procedural outcome of congolease patients undergoing coronarography in the first and only one cardiac catheterization center opened in Kinshasa. Methods: An analytical cross-sectional study was carried out over a period from October 2019 (date of establishment of the first coronary angiography unit in DR Congo) to March 2021. We proceeded to a serial sampling of the consecutive cases of all the patients who have an angiographic exploration of coronary arteries. Clinical, ECG and cardiac ultrasound data were collected in all patients. The indications for the coronary angiography examination were set by differents cardiologists on the basis of repolarization troubles in the electrocardiogram, cinetic troubles in echocardiography, positive stress test and chest pain in patients with cardiovascular risq factors. Results: The serie (47 patients) was predominantly male with a sex ratio M/W of 2.6. The average age was 59.8 ± 10.5 years. Arterial hypertension (HBP) was the main risk factor (89.4%);followed by diabetes mellitus (14.9%). Chest pain was the main functional sign with an atypical character in 44.7%. The ECG showed ST segment depression (17%) and T wave inversion (17%), the anterior region being the most affected. Hypokinesia was the most common echocardiographic abnormality (34%), followed by akinesia (10.6%). The anteroseptal and apical territories were affected in 12.8%. Dilated myocardiopathy (DMC) was significantly predominant in the male sex (29.4% vs 7.7%;p = 0.011). With radial puncture as the main approach, coronary angiography was pathological in 44% revealing mono-truncal lesions. The left coronary network was the most affected: the middle inter ventricular artery (12.8%), the proximal interventricular artery (10.6%) and the proximal circonflex artery (10.6%). Eyeball loss due to severe ocular trauma,intraocular malignancy or infection often requires surgical treatment called orbital implant reconstruction to rehabilitate the orbital volume and restore the aesthetic ***,it remains a challenge to minimize the postoperative exposure and infection complications due to the inert nature of conventional orbital ***,we developed a novel Ca-Zn-silicate bioceramic implant with multi-functions to achieve the expected *** porous hardystonite(Ca2ZnSi2O7)scaffolds with triply periodic minimal surfaces(TPMS)-based pore architecture and graded pore size distribution from center to periphery(from 500 to 800μm or vice versa)were fabricated through the digital light processing(DLP)technique,and the scaffolds with homogeneous pores(500 or 800μm)were fabricated as *** graded porous scaffolds exhibited a controlled bio-dissolving behavior and intermediate mechanical strength in comparison with the homogeneous counterparts,although all of porous implants presented significant antibacterial potential against *** and ***,the pore size-increasing scaffolds indicated more substantial cell adhesion,cell viability and angiogenesis-related gene expression in ***,the gradually increasing pore feature exhibited a stronger blood vessel infiltrating potential in the dorsal muscle embedding model,and the spherical implants with such pore structure could achieve complete vascularization within 4 weeks in the eyeball enucleation rabbit ***,our results suggested that the novel antibacterial hardystonite bioceramic with graded pore design has excellent potential as a next-generation orbital implant,and the pore topological features offer an opportunity for the improvement of biological performances in orbital reconstruction.

Background/Aims: Foreign body (FB) ingestion is a common clinical situation. In some cases, it could be life-threatening, requiring interventional digestive endoscopy. Knowing the main FBs observed could help to prevent their ingestion or to improve management. The aim of this study is to report the results of upper digestive endoscopies performed for ingestion of FBs in Yaoundé (Cameroon). Methods: We recorded all patients who did a gastroscopy for FB ingestion from January 2000 to April 2020 in three medical centers of Yaoundé. We collected data concerning the socio-demographic characteristics of patients, foreign body type, endoscopic management and outcome. Results: A total of 9380 upper digestive endoscopies were performed, with 51 FBs ingestion (0.54%). Male were 27 (52.9%). The mean age was 25.8 ± 22.3 years (8 months to 75 years). Coins were the most frequent FB (23.5%), only observed in children, followed by fishbones (17.6%), only observed in adults. We also observed dental wears (11.8%), metallic objects (11.8%), non-metallic objects (3.9%), batteries (3.9%), toothpick (2%), packet of tablets (2%), and bezoars (2%). The FB was unknown at 21.6%. The most frequent localization was the esophagus in 29/36 patients (80.5%). Endoscopic removal was a success in 35/36 patients (97.2%). A surgery has been performed

This study was carried out in the Health districts of Kayanza and Gahombo. Its aim was to assess the nutritional status and associated factors of infants aged between 6 and 23 months. This descriptive and cross-sectional study was carried out in the health centers of the action zone during medical consultations in community medicine from 13 to 22 February 2023. For this purpose, 53.0% of a sample of 398 mothers of infants aged 6 - 23 months were male, and the most represented age group was 12 - 23 months with 53.8%. Mothers were asked about the diet of their children. All the children involved in this study were vaccinated, whereas 99.2% were given vitamin A supplements. The most frequent illnesses in this age group were acute respiratory infection (ARI) (73.4%), fever (66.6%), diarrhea (53.8%), and vomiting (40.5%). Analysis of nutritional status showed that infants had severe (8.3%) and moderate (70.4%) acute malnutrition. Factors associated with infant nutritional status comprised the occupation of mother (P = 0.009), level of education of mother (P = 0.0625), and prolonged diarrhea (P = 0.004). To remedy this problem, concrete nutritional and educational interventions are needed to promote optimal infant nutrition during the first two years of life.

The use of body mass index(BMI)to determine eligibility for gender-affirming surgery in transgender and nonbinary individuals has been *** BMI thresholds are often meant to be protective,restricting patients from access to surgery can also cause *** is a rationale for the continued use of BMI,but the inherent problems with it must also be recognized,including how weight stigma impacts patients’access to gender-affirming surgery and influences clinical *** article uses a narrative review of current literature to discuss how high BMI affects surgical outcomes in gender-affirming genital surgeries,as well as analogous procedures,existing de facto BMI thresholds,and how to both minimize the harms of proceeding with surgery in patients with a high BMI or the harms of delaying for weight *** factors into surgical decision-making based on the existing literature,which demonstrates that high BMI is associated with increased surgical risk,including higher incidences of surgical site infections and poor wound healing,as well as the possibility of free flap complications,which are a component of certain genital *** patient population is at higher risk for eating disorders,and it is prudent to find alternatives to requiring patient self-monitored weight *** impacts of weight stigma should be considered when treating gender-affirming surgery patients,and further data and research are needed to augment shared decision-making and lead to practice change.

目的探讨下颌骨牵引成骨同期正畸治疗的意义和原则。方法采用方丝托槽固定矫治器颌间牵引正畸。对正在下颌骨牵引成骨中和牵引后巩固阶段的患者进行正畸治疗。同期正畸时间为3—4个月。结果收治11例小颌畸形患者，在获得预期下颌骨延长的同时，开骀、牙齿错位等畸形程度明显减轻，患者的咬合关系改善。结论下颌骨牵引成骨同期进行正畸治疗能明显减轻牙骀畸形程度和缩短术后正畸时间，并能辅助引导下颌骨牵引成骨方向。

Factors Prognosis of Ebola Virus Disease at the Wonkifong Ebola Treatment center, Coyah, Guinea

Background: The Ebola Virus Epidemic epidemic in Guinea was marked by its rapid spread in the capital, forcing the authorities to build several Ebola treatment centers including Wonkifong (Coyah), Conakry. Objective: To evaluate the case fatality rate of Ebola Virus Disease in this center and to identify associated factors. Patients and Method: There is a prospective study at wonkifong Ebola treatment center from January 1st, 2014 to April 30th, 2015. All patients diagnosed with an Ebola Virus Disease from January 1st, 2014 to April 30th, 2015 were enrolled in this study. Kaplan-Meier curves and multivariate regression model were used to analyze factors associated with death. Results: 216 patients were included, the mean age was 30.52 ± 17 years and 53.2% were female. The average time between admission to the Ebola treatment center and the onset of signs was 5.13 ± 3.8 days. The average period of stay in the Ebola Treatment center was 12.64 ± 11.31 days and duration median of death after admission in Wonkifong ETC was 11 days. According to the source, more than half of the patients were from Forécariah, i.e. 60.2% (n = 134). The case fatality rate was 64.4%. This rate was 72.0% in Dubréka, 65.1% in Coyah and 64.6% in Forécariah. The factors independently associated with Ebola Virus Disease death were signs of diarrhea [OR = 2.22 (1.17 - 4.20)], anorexia [OR= 0.40 (0.19 - 0.81)] and period of patients stay in ETC [OR = 0.96 (0.93 - 0.99)]. Conclusion: These results show that the fatality rate linked to the Ebola virus disease remains high. The factors independently associated with death were Diarrhea, anorexia and period between beginning of illness and date of latest news. Biological monitoring and resuscitation seem interesting for the reduction of this mortality.

Evaluation of bone remodeling in regard to the age of scaphoid non-unions

AIM: To analyse bone remodeling in regard to the age of scaphoid non-unions(SNU) with ***: Thirty-six patients with symptomatic SNU underwent surgery with resection of the pseudarthrosis. The resected material was evaluated histologically after staining with hematoxylin-eosin(HE), tartrate resistant acid phosphatase(TRAP), CD 68, osteocalcin(OC) and osteopontin(OP). Histological examination was performed in a blinded ***: The number of multinuclear osteoclasts in the TRAP-staining correlated with the age of the SNU and was significantly higher in younger SNU(P = 0.034; r = 0.75). A higher number of OP-immunoreactive osteoblasts significantly correlated with a higher number of OC-immunoreactive osteoblasts(P = 0.001; r = 0.55). Furthermore, a greater number of OP-immunoreactive osteoblasts correlated significantly with a higher number of OP-immunoreactive multinuclear osteoclasts(P = 0.008; r = 0.43). SNU older than 6 mo showed a signifi-cant decrease of the number of fibroblasts(P = 0.04). Smoking and the age of the patients had no influence on bone remodeling in ***: Multinuclear osteoclasts showed a significant decrease in relation to the age of SNU. However, most of the immunhistochemical findings of bone remodeling do not correlate with the age of the SNU. This indicates a permanent imbalance of bone formation and resorption as indicated by a concurrent increase in both osteoblast and osteoclast numbers. A clear histological differentiation into phases of bone remodeling in SNU is not possible.

Removal of phosphate ions from aqueous solution using Tunisian clays minerals and synthetic zeolite

Phosphate ions are usually considered to be responsible for the algal bloom in receiving water bodies and aesthetic problems in water. From the environmental point of view, the management of such contaminant and valuable resource is very important. The present work deals with the removal of phosphate ions from aqueous solutions using kaolinitic and smectic clay minerals and synthetic zeolite as adsorbent. The pH effect and adsorption kinetic were studied. It was found that phosphate could be efficiently removed at acidic pH （between 4 and 6） and the second order model of kinetics is more adopted for all samples. The isotherms of adsorption of phosphate ions by the two clays and the zeolite samples show that the zeolite has the highest rate of uptake （52.9 mg P/g）. Equilibrium data were well fitted with Langmuir and Freundlich isotherm.

Diabetes Complications and Associated Factors in Type 2 Diabetic Patients in Cotonou

Long-term complications are the main sources of morbidity and mortality in diabetic patients. Aims: The aims of the study were to determine the rate of long-term complications in type 2 diabetic patients and to identify factors associated to these complications. Patients and method: Successive type 2 diabetic patients attending the diabetic center were submitted to a questionnaire and to clinical examination. Data were completed by consulting their medical reports. Chi square test was used for statistical analysis. Results: In 150 diabetic patients included in the study, the global rate of complications was 78.0%. Specific rate for itch complication investigated was 57.7% for peripheral neuropathy, 75.0% for erectile dysfunction, 20.0% for nephropathy, 36.6% for retinopathy, 40% for macroangiopathy and 8.0% for foot ulcer. Factors significantly associated with high rate of complications were age above or equal to 50 years (p = 0.001), the male gender (p = 0.000), high blood pressure (p = 0.0001), the absence of familial history of diabetes (p = 0.02), the duration of the disease above 5 years (p = 0.001) and high HbA1c level (p = 0.001). Conclusion: This study revealed that type 2 diabetic patients followed up in the diabetic center in Cotonou showed a high rate of chronic complications which often occurred in a younger age than in developed countries. Numerous socio-demographic and biological factors were significantly associated with the high rate of complications.
